OBJECTIVE: The aim of the study was to investigate the caries experience, dental care level, and oral hygiene in children and adolescents with Fanconi anemia. METHODS: Decay-missing-filled teeth index, restorative index and simplified oral hygiene index were examined in two groups of children and adolescents: FA, diagnosed with Fanconi anemia (n = 35) and a healthy control group, non-FA (n = 35). Oral hygiene habits were assessed through questionnaires completed by parents. RESULTS: FA group presents higher decay-missing-filled teeth index values, dental care index, oral hygiene index. Nevertheless, no statistical difference was observed between the groups. Frequency of visits to the dentist was higher in the non-FA group. Frequency of tooth brushing was higher in FA group and it was performed by the subjects without the help of their parents. CONCLUSIONS: No difference was observed in caries experience, dental care level, and oral hygiene in children and adolescents with FA when compare with non-FA.

BACKGROUND: Fanconi anaemia is a genetic disease characterized by congenital abnormalities, progressive bone marrow failure, and a higher predisposition of oral squamous cell carcinoma. The purpose of this study was to evaluate the prevalence of oral mucosa lesions in patients with Fanconi anaemia without hematopoietic stem cell transplantation (HSCT). PROCEDURE: Patients with Fanconi anaemia who had not undergone HSCT was cross-sectional evaluated for the presence of oral lesions. RESULTS: The sample was composed of 78 male and 60 female patients, with a median age of 9 years. Of the 138 patients, approximately 45% manifested at least one oral mucosa abnormality: 35 patients (25%) presented with traumatic injuries, and 16 (12%) exhibited leukoplakia. The following lesions were observed in low prevalence: aphthous ulcers, atrophic tongue, petechiae and hematomas, gingival hyperplasia, mucoceles, herpes, hyperpigmentation, haemangioma, non-neoplastic proliferative lesions, neutropenic ulcers, papilloma, and candidiasis. CONCLUSION: There was a high prevalence of oral leukoplakias in patients with Fanconi anaemia who had not undergone HSCT. It highlights the need of regular oral screenings in this cohort of concern for head and neck malignancies and suggests that oral leukoplakias should be further investigated as part of the syndrome phenotype.

Fraturas de dentes artificiais de bases de próteses dentárias são problemas comuns. Objetivo: O propósito deste estudo foi avaliar a resistência da união ao cisalhamento de três marcas comerciais de dentes artificiais (Biolux® [B]; Vip Dent® [V]; Trybite® [T]) com e sem retenção mecânica a uma resina acrílica termopolimerizável (Vip Cril). Métodos: Dentes artificiais foram fixados com cera em 60 cilindros de PVC. A seguir, foram incluídos em mufla odontológica com gesso especial e, após a eliminação da cera os 60 cilindros, foram divididos em dois grupos experimentais com (CR=30) e sem (SR=30) retenção mecânica, destes ainda foram criados subgrupos (n=10) para cada marca de dente. Os corpos de prova, depois de preenchidos com a resina acrílica, polimerização, desinclusão e acabamento, foram armazenados em água destilada a 37°C por 24 horas e submetidos aos ensaios mecânicos de cisalhamento (0,5 mm/min). Falhas foram analisadas em lupa (40 X). Resultados: A análise de variância não evidenciou diferença estatisticamente significante (P > 0,05) entre os grupos com ou sem retenção mecânica. O teste de Tukey (α = 0,05) evidenciou diferença significante (P < 0,001) entre as marcas: (T = 20,3 MPa; V = 17,8 MPa e B = 15,5 MPa). As falhas foram predominantemente mistas. Conclusão: A retenção mecânica não aumentou a resistência de união e as diferentes marcas de dentes podem influenciar na união...

Long-term survivors of hematopoietic stem cell transplantation are recognized as a risk group for malignization. Malignant oral neoplasms are increasingly being reported in the literature as a consequence of lesions of chronic graft-versus-host disease, and prolonged multidrug treatment to control its manifestations. This report describes a 43-year-old patient who, after allogeneic bone marrow transplantation, developed an oral squamous cell carcinoma secondary to the use of azathioprine, cyclosporine, prednisone, and tacrolimus, associated with multiorgan chronic graft-versus-host disease involving the oral mucosa, skin, eyes, and liver. This report aims to discuss the possible role of immunosuppressant therapy for chronic graft-versus-host disease on the development of oral squamous cell carcinoma, and the relevance of a close oral follow-up of patients to detect dysplastic or malignant alterations at an early stage.

Objetivo: Comparar la prevalencia de cuatro bacterias periodontopáticas incluyendo Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Fusobacterium nucleatum y Treponema denticola en muestras de placa supragingival de niños con y sin anemia de Fanconi. Material y métodos: Muestras de placa supragingival fueron colectadas en 71 personas con edades entre 6-18 años de edad. Las muestras se dividieron en tres grupos: anemia de Fanconi pre-trasplante (n= 25), anemia de Fanconi post-trasplante (n=23) y control (n=24). Las bacterias se identifi on mediante la reacción en cadena de la polimerasa para amplifi in vitro al gen codifi 16S rRNA. Resultados: El A. actinomycetemcomytans sólo fue encontrado en una muestra del grupo pretrasplante. El microorganismo P. gingivalis se identificó en una muestra del grupo pre-trasplante y en una del grupo post-trasplante. El T. denticola se encontró únicamente en dos muestras del grupo pre-trasplante. El microorganismo F. nucleatum se observó en todos los grupos. La presencia de los microorganismos varió del 30% en el grupo control al 58% en el grupo Prétrasplante. No fueron encontradas diferencias estadísticas entre los grupos. Conclusión: Los resultados del estudio sugieren que las alteraciones sistémicas encontradas en los individuos con AF no afectan la prevalencia de las cuatro bacterias analizadas.

As bactérias se identificaram por meio da reação em cadeia da polimerase para amplificar in vitro o gene codificante 16S rRNA. Resultados: O A. actinomycetemcomytans foi encontrado unicamente em uma amostra do grupo pré-transplante. O microorganismo P. gingivalis foi identificado em uma amostra do grupo pré-transplante e uma do grupo póstransplante. O T. denticola foi observado em duas amostras do grupo pré-transplante. O microorganismo F. nucleatum foi observado em todos os grupos. A presença dos microorganismos variou de 30% no grupo controle a 58% no grupo pré-transplante. Não foram encontradas diferenças estatísticas entre os grupos. Conclusão: Os resultados do estudo sugerem que as alterações sistêmicas encontradas nos indivíduos com AF não afetam a prevalência das quatro bactérias analisadas.

Material and methods: Dental plaque samples were collected from 71 children and adolescents, aged 6-18 years old. The samples were divided in three groups: Fanconi anemia before transplant (n=25), Fanconi anemia after transplant (n=23) and control (n=24). The test bacteria were identified using a 16S rRNA-based PCR analysis. Results: A. actinomycetemcomytans was found only in one sample of Fanconi anemia before transplant group. P. gingivalis was identified in one sample of Fanconi anemia before transplant group and Fanconi anemia after transplant group. T. denticola was found in two samples of Fanconi anemia before transplant group. F. nucleatum was observed in all groups. The presence of microorganisms ranged from 30% in the control group to 58% in the Fanconi anemia before transplant group. No statistical differences were found between groups. Conclusion: Systemic alterations found in FA subjects did not affect the prevalence of the four bacteria analyzed.

Fanconi's anemia (FA) is characterized by bone marrow failure and can lead to infections such as periodontal disease. The aim of this study was to compare the prevalence of four periodontopathic bacteria including Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Fusobacterium nucleatum and Treponema denticola in saliva samples from children with and without FA. Paraffin-stimulated saliva samples were collected from 71 children and adolescents, aged 6-18 years old. The samples were divided in three groups: FA without HSCT (n = 25), FA with HSCT (n = 23) and Non-FA (n = 24). The test bacteria were identified using a 16S rRNA-based PCR analysis. P. gingivalis was the most frequent species in all groups and T. denticola the less frequent. P. gingivalis was also common species in combinations in all groups. No statistically difference was observed between the groups for single bacteria, pair or triple combination. The combination of the four species was detected in one saliva sample of the FA without HSCT group and in five samples of the Non-FA group, with statistical difference between them (p < 0.05, Fisher exact test). Systemic alterations found in FA subjects did not affect the salivary distribution of the four bacteria analyzed.
