The Child Eating Behaviour Questionnaire for toddlers (CEBQ-T-Mex) and the Adult Eating Behaviour Questionnaire (AEBQ-Esp) measure appetitive traits (ATs) in children and adults, respectively, both validated for use in Spanish. ATs are inherited variations in appetite, present from birth, that are reasonably stable throughout childhood and can explain why some infants over- or undereat in response to environmental exposures. "Food approach" traits predispose to overweight while "food avoidance" traits provide protection, but little is known about the relationships between parents' and their toddler's ATs. The aim was to examine the associations between maternal and toddler appetitive traits, using the AEBQ-Esp and CEBQ-T-Mex, and to examine the associations between ATs and Body Mass Index z-scores (BMIz). Sociodemographic data and the weights and heights of mothers and toddlers (aged 12-36 months) were collected from a teaching hospital in Guadalajara, Mexico. Mothers completed both the AEBQ-Esp and the CEBQ-T-Mex. Direct correlations were found between the ATs of toddlers and their mother (p < 0.005), except for "Slowness in Eating" (SE), and only an inverse correlation was found between the "Satiety Responsiveness" (SR) of toddlers and their BMIz (r = -0.147; p = 0.007). These results suggest that ATs could potentially run in families. These may be useful targets for family-wide interventions to support the development and maintenance of healthy eating behaviours in childhood.

The objective was to evaluate the anthropometric indicators and dietary intake of toddlers attending private clinics in Guadalajara. In a cross-sectional study, 101 toddlers aged 12 to 24 months were included. They were born full term, had an adequate weight for gestational age, and attended private clinics in Guadalajara. Two 24 h dietary recalls were administered. Anthropometric measurements were taken, and anthropometric indices were estimated. Student's t test or the Mann-Whitney U test, chi-square test, and odds ratio were used for quantitative or qualitative variables. Males had lower Z scores for the weight/age index than females. During the week, energy intake was excessive in males [OR = 5.5 (95% CI 1.4, 20.8)], and cholesterol intake was insufficient in females [OR = 3.03 (95% CI 1.2, 7.1)]. On weekends, energy [OR = 2.5 (95% CI 1.1, 5.7)] and fiber intake [OR = 3.1 (95% CI 1.2, 7.8)] were insufficient in females. Most of the toddlers who attend the private clinics in the upper-middle socioeconomic stratum of the Guadalajara Metropolitan Area had excessive protein intake, excessive consumption of added sugars was frequent, and there was insufficient intake of vitamin D and calcium. Sex was shown to be a factor influencing nutrient intake in these toddlers aged 12-24 months. Males had a Z-score of weight/age lower than females, suggesting nutritional risk.

The main objective was to explore the relationship between the microbiota of human milk and adiposity in Mexican mothers during the first lactation stage. METHODS: Seventy lactating women were included. Adiposity by anthropometric measurements and by bioelectric impedance was obtained. The donation of human milk was requested, from which bacterial DNA was extracted and qPCR of the 16S region was performed. The Mann-Whitney U test, Spearman and Pearson correlations, and multiple linear regressions models were also calculated. RESULTS: The median percentage of Bacteroidetes had a direct and significant correlation with normal adiposity, current BMI, waist circumference, and body fat percentage. The correlation with current BMI became significantly inverse in women with BMI ≥ 25. In women with normal BMI, the percentage of Actinobacteria showed a direct and significant correlation with current BMI, waist circumference, and percentage of body fat. Multiple linear regressions showed that pre-pregnancy BMI was the variable with the highest predictive value with the Bacteroidetes phyla in normal BMI and in BMI ≥ 25. CONCLUSIONS: the adiposity of the woman before pregnancy and during lactation would have an important effect on the abundance of Bacteroidetes and Actinobacteria in human milk.

OBJECTIVE: To compare and correlate anthropometric measurements of the left arm versus the right arm and to establish their relation to laterality, sex, and nutritional status in children and adolescents from 6 to 12 years of age. METHODS: A cross-sectional study included 605 children and adolescents (285 males; 320 females), with a middle socioeconomic status, that attended an urban elementary school. Anthropometric measurements were taken in both arms, along with height and weight, to assess nutritional status. Self-reported hand preference was recorded. RESULTS: A total of 49.3% were females and 93% of the subjects were right-handed (n = 563). The mean comparison of right arm and left arm measurements and areas, according to laterality, showed differences in arm fat measurements, with higher values in the right arm. However, the mean difference in triceps skinfold and upper arm measurements between the right arm and left arm was smaller than the technical error of measurement. Significant differences were observed between arm measurements and areas. Upper arm and total upper arm areas of both arms had the highest regression coefficients, with an adjusted R2 of .98, whereas the arm fat index had the lowest (adjusted R2 of .88). Adjusting the previous differences between the right arm and left arm by age, sex, BMI, and laterality produced similar results. CONCLUSIONS: Statistical but not clinically significant differences were found between some measurements and areas of both arms, discretely modified in models adjusted for age and BMI, so the measurements can be made on either arm.

We aimed to conduct an updated review on the pathophysiology, diagnosis, and nutritional intervention of CCLD and secondary malnutrition in infants. Protein-energy malnutrition, impaired linear growth, fat-soluble vitamin deficiencies, and hepatic osteodystrophy can occur in up to 80% of cases. The proposed pathophysiological mechanisms include insufficient energy intake, lipid- and fat-soluble vitamin malabsorption, increased energy expenditure, altered intermediate metabolism, hormonal dysregulation, and systemic inflammation. The current approach to diagnosis is the identification of the deviation of growth parameters, body composition, and serum concentration of micronutrients, which determines the type and magnitude of malnutrition. Currently, liver transplantation is the best therapeutic alternative for the reversal of nutritional impairment. Early and effective portoenteroanatomosis can extend survival in patients with biliary atresia. Medical and dietary interventions in some storage and metabolic diseases can improve liver damage and thus the nutritional status. A proportion of patients with biliary atresia have fat-soluble vitamin deficiencies despite receiving these vitamins in a water-soluble form. With aggressive enteral nutrition, it may be possible to increase fat stores and preserve muscle mass and growth. The nutritional issues identified in the pre- and post-transplantation stages include muscle mass loss, bone demineralization, growth retardation, and obesity, which seems to correspond to the natural history of CCLD. Due to the implications for the growth and development of infants with CCLD with this complex malnutrition syndrome, innovative projects are required, such as the generation of prediction and risk models, biomarkers of growth and body composition, and effective strategies for nutritional prevention and intervention.

The objective of this study was to validate and measure the internal reliability of the Baby and Child Eating Behavior Questionnaires for Toddlers (BEBQ-Mex and CEBQ-T-Mex), that evaluate appetitive trait (ATs). Mothers recruited from a public hospital in Guadalajara, Mexico, completed the BEBQ-Mex or CEBQ-T-Mex along with information on sociodemographic characteristics. Internal reliability of the BEBQ-Mex was sufficient for Food Responsiveness (FR) (Cronbach α = 0.82), while Enjoyment of Food (EF) and Satiety Responsiveness (SR) showed poor reliability (α = 0.56) and Slowness in Eating (SE) had unacceptable reliability (a = 0.36). All reliability values for the CEBQ-T-Mex were acceptable (>0.70), except for SE (α = 0.64). Confirmatory factor analysis (CFA) revealed an adequate model fit for the BEBQ-Mex, except the SE subscale. CFA for the CEBQ-T-Mex confirmed the six-factor structure. Mothers of a low sociodemographic background were unable to recognize their infants' ATs; the BEBQ-Mex partly met the criteria for validity and reliability. Mothers from similar sociodemographic characteristics were more able to recognize the ATs of their toddlers than their infants; the CEBQ-T-Mex was found to be a valid and reliable tool. Findings support the need to help mothers' ability to recognize their infants' ATs, which have been previously associated with weight and growth.

The polyamine content of human breast milk, which is the first exogenous source of polyamines for the newborn, can be affected by several factors associated with the mother, the infant, or breastfeeding itself. The aim of this study was to evaluate the influence of different breastfeeding factors on the polyamines found in human milk. For this study, a cohort of 83 mothers was considered for up to 4 months, and a subgroup of 33 mothers were followed during the first six months of breastfeeding. Two breast milk samples were collected at each sampling point (foremilk and hindmilk) and the polyamine content was determined by UHPLC-FL. Polyamine levels varied considerably between the mothers and tended to decrease over time. Putrescine was the minor polyamine, whereas spermidine and spermine contents were very similar. The concentrations of the three polyamines were significantly higher in hindmilk than foremilk (p < 0.001). Spermidine and spermine levels decreased significantly through the lactation progress (p < 0.05). Finally, slightly higher levels of polyamines were observed in the milk of mothers providing partial, rather than full, breastfeeding, although the differences were not significant. The polyamine content in human milk was found to change during a single feed (foremilk versus hindmilk) and as lactation progressed, mainly in response to the specific circumstances of the newborn.

Neck circumference was studied for the first time in a pediatric population in 2010. Since then, various countries have proposed cutoff values to identify overweight, obesity, and metabolic syndrome. However, no reference values have been established for the Mexican child population. The aim of this study is to provide percentile reference values for the neck circumference of Mexican schoolchildren. Only normal-weight schoolchildren aged 6-11 years were included. Percentiles and growth charts were constructed based on the "Generalized Additive Model for Location, Scale and Shape" (GAMLSS). A total of 1059 schoolchildren (52.9% female) was evaluated. Weight, height, and BMI values were higher for males; however, this difference was not statistically significant. The 50th percentile for females was 24.6 cm at six years old and 28.25 cm at 11 years old, and for males, it was 25.75 cm and 28.76 cm, respectively. Both males and females displayed a pronounced increase in neck circumference between 10 and 11 years of age. The greatest variability was found in the 11-year-old group, with an increase of 5.5 cm for males and 5.4 cm for females. This study presents the first reference values for neck circumference for a Mexican child population.

Background: It has been observed that breastfeeding mobilizes the deposits of fat that accumulate during pregnancy and promotes weight loss through energy expenditure. The purpose of this study was to demonstrate that full breastfeeding (FBF) reduces anthropometric and body composition indicators in women between the 8th and the 16th week postpartum. Methods: In a nonrandom cohort study, 170 mothers at the Hospital Civil de Guadalajara, Guadalajara, México, were enrolled: FBF 74, partial breastfeeding (PBF) 57, and human milk substitutes (HMS) 39. Anthropometric indicators and body composition were measured at the 8th and 16th week postpartum. We performed analysis of variance to compare body composition according to the type of feeding and paired Student's t-test to compare the changes from the 8th to 16th week postpartum. Results: We found that FBF mothers had a trend to lower arm fat area and triceps skinfold than PBF and HMS mothers at 8 and 16 weeks postpartum. We observed a decrease in weight (p = 0.004), weight/age index (p = 0.003), body mass index (p = 0.003), hip circumference (p = 0.037), and lean mass (p = 0.003) from 8 to 16 weeks postpartum in mothers who offered FBF. The mid-upper arm circumference, the arm total area, and their z-score increased in the three feeding groups. Conclusions: Our results show that FBF mothers had some lower adiposity from 8 to 16 weeks postpartum compared with the result observed among PBF mothers and those who utilized HMS.

INTRODUCTION: IIntroduction: maternal employment after childbirth is associated with abandonment of breastfeeding; however, lactation rooms in the workplace increase the practice of breastfeeding. Objective: to compare the frequency and duration of breastfeeding among working mothers based on the presence of lactation rooms in their workplaces. Methods: we included mothers from different institutions whose infants were between 6 and 35 months (n = 158), and an ad hoc questionnaire was applied to assess breastfeeding, exclusive breastfeeding (EBF), partial breastfeeding (PBF), and use of human milk substitutes (HMS). Two groups were compared: working mothers with a lactation room at their workplace (n = 76) versus working mothers without this resource (n = 82). Results: breastfeeding duration (7.5 vs. 5.0 months, p < 0.001) and EBF (3.0 vs. 1.2 months, p = 0.005) were higher in mothers who had a lactation room. HMS use was shorter in mothers who had a lactation room (2.5 vs. 10.0 months, p = 0.001). There were more working mothers who breastfed for more than six months (75.0 % vs. 48.8 %) [OR = 3.15 (95 % CI, 1.60-6.19), p = 0.001] and 12 months (31.6 % vs. 14.6 %) [OR = 2.69 (95 % CI, 1.23-5.87), p = 0.014] when lactation rooms were available in their workplaces. Conclusion: the presence of a lactation room in the workplace was associated with a higher frequency and duration of breastfeeding.


INTRODUCCIÓN: RESUMEN Introducción: el empleo materno después del parto se asocia con el abandono de la lactancia materna, mientras que las salas de lactancia en el lugar de trabajo aumentan la práctica de la lactancia. Objetivo: comparar la frecuencia y duración de la lactancia materna entre madres trabajadoras en función de la presencia o no de salas de lactancia en sus lugares de trabajo. Métodos: incluimos madres de diferentes instituciones cuyos bebés tenían entre 6 y 35 meses (n = 158) y se aplicó un cuestionario ad hoc para evaluar la lactancia materna, la lactancia materna exclusiva (LME), la lactancia materna parcial (LMP) y el uso de sucedáneos de la leche humana (SLH). Se compararon dos grupos: madres trabajadoras con una sala de lactancia en su lugar de trabajo (n = 76) y madres trabajadoras sin este recurso (n = 82). Resultados: la duración de la lactancia (7,5 vs. 5,0 meses, p < 0,001) y LME (3,0 vs. 1,2 meses, p = 0,005) fueron mayores en las madres que tenían sala de lactancia. El uso de SLH fue más corto en las madres que tenían sala de lactancia (2,5 vs. 10,0 meses, p = 0,001). Hubo más madres trabajadoras que amamantaron más de seis meses (75,0 % vs. 48,8 %) [OR = 3,15 (IC 95 %: 1,60-6,19), p = 0,001] y 12 meses (31,6 % vs. 14,6 %) [OR = 2,69 (IC 95 %: 1,23-5,87), p = 0,014] cuando había salas de lactancia disponibles en sus lugares de trabajo. Lactose is a unique component of breast milk, many infant formulas and dairy products, and is widely used in pharmaceutical products. In spite of that, its role in human nutrition or lactose intolerance is generally not well-understood. For that reason, a 2-day-long lactose consensus meeting with health care professionals was organized in Mexico to come to a set of statements for which consensus could be gathered. Topics ranging from lactase expression to potential health benefits of lactose were introduced by experts, and that was followed by a discussion on concept statements. Interestingly, lactose does not seem to induce a neurological reward response when consumed. Although lactose digestion is optimal, it supplies galactose for liver glycogen synthesis. In infants, it cannot be ignored that lactose-derived galactose is needed for the synthesis of glycosylated macromolecules. At least beyond infancy, the low glycemic index of lactose might be metabolically beneficial. When lactase expression decreases, lactose maldigestion may lead to lactose intolerance symptoms. In infancy, the temporary replacing of lactose by other carbohydrates is only justified in case of severe intolerance symptoms. In those who show an (epi)genetic decrease or absence of lactase expression, a certain amount (for adults mostly up to 12 g per portion) of lactose can still be consumed. In these cases, lactose shows beneficial intestinal-microbiota-shaping effects. Avoiding lactose-containing products may imply a lower intake of other important nutrients, such as calcium and vitamin B12 from dairy products, as well as an increased intake of less beneficial carbohydrates.

OBJECTIVE: We aimed to study the correlation between neck circumference (NC) and anthropometric adiposity indicators, and to determine cut-off points of NC for both sexes to identify elevated central adiposity in schoolchildren in western Mexico. DESIGN: Cross-sectional study. SETTING: Rural settings in western México.ParticipantsChildren from a convenience sample of six schools in Acatlán, Jalisco, Mexico (n 1802). RESULTS: NC showed a strong positive correlation with all anthropometric adiposity indicators in both sexes, which were notably higher in boys regardless of age. Noteworthy, waist circumference displayed the highest significant correlation when analysed by both age and sex. As age increased, NC cut-off points to identify elevated central adiposity ranged from 25·7 to 30·1 cm for girls and from 27·5 to 31·7 cm for boys. CONCLUSIONS: NC could be used as a simple, inexpensive and non-invasive indicator for central obesity assessment in Mexican schoolchildren.

INTRODUCTION: Objective: differences have been shown between males and females in terms of the prevalence of malnutrition in different parts of the world, which point to discrimination against females, including with respect to full breastfeeding. Therefore, the objective was to show that exclusive breastfeeding is less common for females in a population of medium-low and low socioeconomic strata. Methods: this was a cross-sectional analysis of a sample of 170 mother-infant dyads according to type of feeding (74 full breastfeeding, 57 partial breastfeeding and 39 human milk substitutes) at the Nuevo Hospital Civil de Guadalajara. Dependent variables according to type of feeding: full breastfeeding (exclusive and/or predominant), partial breastfeeding, and human milk substitutes. Independent variables: demographic data, schooling, occupation of mothers and/or parents, and family income. Kruskal-Wallis, Mann-Whitney U and Chi-square tests and odds ratio were used. Results: the probability of full breastfeeding was 3.8 times lower in females than in males. In a non-significant way, the likelihood of full breastfeeding was lower than that of partial breastfeeding, and full breastfeeding was lower than the combination of partial breastfeeding and human milk substitutes in females. Full breastfeeding and partial breastfeeding were lower than human milk substitutes, and partial breastfeeding was lower than human milk substitutes in females. Conclusion: there is a differentiated character in the privilege of full breastfeeding; it is four times lower in females than in males.


INTRODUCCIÓN: Objetivo: se han observado diferencias entre niñas y varones en la prevalencia de desnutrición en diferentes partes del mundo, lo que apunta a la discriminación contra las niñas, incluso con respecto a la lactancia materna completa. El objetivo fue mostrar que la lactancia completa es menos común en las niñas en una población de estratos socioeconómicos medio-bajo y bajo. Métodos: se realizó un análisis transversal de una muestra de 170 díadas madre-lactante según el tipo de alimentación (74 de lactancia completa, 57 de lactancia parcial y 39 sucedáneos de la leche humana) en el Nuevo Hospital Civil de Guadalajara. Variables dependientes según el tipo de alimentación: lactancia completa (exclusiva y/o predominante), lactancia parcial y sucedáneos de leche humana. Variables independientes: datos demográficos, escolaridad, ocupación de madres y/o padres e ingresos familiares. Se utilizaron pruebas de Kruskal-Wallis, Mann-Whitney U y Chi-cuadrado y razón de momios. Resultados: la probabilidad de lactancia materna completa fue 3,8 veces menor en niñas que en varones. De manera no significativa, la probabilidad de lactancia completa fue menor que la de lactancia parcial y la frecuencia de lactancia completa fue menor que la combinación de lactancia parcial y sucedáneos de leche humana en niñas. La frecuencia de lactancia completa y lactancia parcial fue menor que los sucedáneos de leche humana y la lactancia parcial fue menos frecuente que los sucedáneos de leche humana en niñas. Conclusión: hay un carácter diferenciado en el privilegio de ofrecer lactancia materna completa. Es cuatro veces menor en las niñas que en varones.

INTRODUCTION: Introduction: the purpose of this study was to evaluate the relationship of arm anthropometric indicators with direct indicators of nutritional status in hospitalized pediatric patients. Methods: an analytical cross-sectional study with 760 patients hospitalized in the Pediatric Division of the Nuevo Hospital Civil de Guadalajara during 2014 was used. The anthropometric indices were weight/length, weight/height, weight/age, length/age, height/age, head circumference/age and body mass index (BMI)/age. The arm indicators were mid-upper arm circumference (MUAC), total arm area (TAA), arm muscle area (AMA), arm fat area (AFA) and fat percentage (FP). The ANOVA, Kruskall-Wallis, Mann-Whitney U and Pearson's correlation tests and also odds ratios were used to identify the probability of nutritional status impairment. Results: the prevalence of acute and chronic malnutrition was higher in infants (31% and 30%, respectively). With arm areas (TAA, AMA, AFA), the risk of deficit (≤ -2DE) was higher in infants and early preschoolers (p < 0.001). The correlation between the anthropometric indexes and the arm areas was direct and significant (p < 0.001). The BMI variability was explained in 68% by the AMA, AFA, and FP (p < 0.001); the variability of the height/age index was also explained in 34% by the AMA and AFA (p < 0.001). Conclusion: it is possible to diagnose both a chronic and acute deficit using the indirect indicators of the arm, while the body mass index only reflects an acute deficit. Therefore, arm areas would be more useful indicators in the assessment of nutritional status and the diagnosis of chronic-acute malnutrition in hospitalized pediatric patients.


INTRODUCCIÓN: Introducción: el objetivo de este estudio fue evaluar la relación de los indicadores antropométricos de brazo con los indicadores directos del estado de nutrición en pacientes pediátricos hospitalizados. Métodos: se utilizó un estudio transversal analítico con 760 pacientes ingresados en la División de Pediatría del Nuevo Hospital Civil de Guadalajara durante 2014. Los índices antropométricos fueron peso/longitud, peso/altura, peso/edad, longitud/edad, altura/edad, circunferencia cefálica e IMC. Los indicadores del brazo fueron circunferencia media del brazo (CMB), área total del brazo (ATB), área muscular del brazo (AMB), área grasa del brazo (AGB) y porcentaje de grasa. Se utilizaron las pruebas de ANOVA, Kruskall-Wallis, U de Mann-Whitney, correlación de Pearson y razón de momios para identificar la probabilidad de deterioro del estado nutricional. Resultados: la prevalencia de desnutrición aguda y crónica fue mayor en lactantes (31% y 30%, respectivamente). Con las áreas del brazo (ATB, AMB, AFA), el riesgo de déficit (≤ -2 DE) fue mayor en lactantes y preescolares tempranos (p < 0,001). La correlación entre los índices antropométricos y las áreas del brazo fue directa y significativa (p < 0,001). La variabilidad del IMC fue explicada en un 68% por AMB, AGB y porcentaje de grasa (p < 0,001); la variabilidad del índice de talla/edad también fue explicada en un 34% por AMB y AGB (p < 0,001). Conclusión: es posible diagnosticar el déficit crónico y agudo utilizando los indicadores indirectos del brazo, mientras que el IMC solo refleja un déficit agudo. Las áreas de brazo serían indicadores más útiles en el diagnóstico de desnutrición crónica-aguda en pacientes pediátricos hospitalizados.

El lactante de 12 a 24 meses es un foco de atención debido a que se encuentra en un periodo de edad de gran vulnerabilidad a la mala nutrición y/o al establecimiento de malos hábitos de alimentación; en consecuencia, es crucial identificar las prácticas de alimentación de los infantes durante los primeros 24 meses de edad. En este estudio, transversal analítico, se reportan las prácticas de lactancia materna y alimentación complementaria de 211 lactantes, de 15 a 24 meses de edad, separados por género. Se obtuvieron las características del lactante, las prácticas de lactancia materna y de alimentación complementaria, el consumo de bebidas azucaradas y de alimentos ultraprocesados. Se utilizaron las pruebas t de Student no pareada, chi cuadrada y se estimó la razón de momios (IC95%). Las madres prolongaron más la lactancia materna en varones. En 99% de los lactantes se observó una ingestión excesiva de alimentos ultraprocesados, y los mismos se consumían con mayor frecuencia (seis veces/semana). La frecuencia de consumo de bebidas azucaradas fue de cinco veces por semana en ambos géneros. La cantidad de bebidas azucaradas consumidas al día fue significativamente mayor en varones y tenían cuatro veces mayor probabilidad de consumir las mismas [RM= 4.9 (IC95% 1.22, 19.5), p=0.02] y mayor probabilidad de consumir jugos procesados [RM=1.94 (IC 95% 0.92, 4.09), p=0.078] que las niñas. En la población estudiada se observó un consumo excesivo de alimentos ultra-procesados y bebidas azucaradas, especialmente entre los varones(AU)


Infants aged 12 to 24 months are a focus of attention because they are in an age period of great vulnerability to poor nutrition and/or the establishment of bad eating habits; consequently, it is crucial to identify infant feeding practices during the first 24 months of age. In this analytical, cross-sectional study, breastfeeding and complementary feeding practices of 211 infants, aged 15 to 24 months, separated by gender, are reported. Infant characteristics, breastfeeding and complementary feeding practices, consumption of sugary drinks and ultra-processed foods were obtained. The unpaired Student t-tests, chi-square were used and the odds ratio (95% CI) was estimated. Mothers prolonged breastfeeding in boys. Excessive ingestion of ultra-processed foods was observed in 99% of infants, and they were consumed more frequently (six times / week). The frequency of consumption of sugary drinks was five times a week in both genders. The amount of sugary drinks consumed per day was, not only significantly higher in boys, but also they were four times more likely to consume this kind of beverages [RM = 4.9 (95% CI 1.22, 19.5), p = 0.02], as well as processed juices [RM = 1.94 (95% CI 0.92, 4.09), p = 0.078] than girls. OBJECTIVE: the purpose was to evaluate the nutritional status in hospitalized children according to their socio-demographic variables, underlying pathology and based on hospital length of stay. METHODS: seven-hundred and sixty-three hospitalized patients of the Civil Hospital of Guadalajara Dr. Juan I. Menchaca were included in a cross-sectional study. The following anthropometric indices were taken: weight/length, weight/height, weight/age, length/age, height/age, head circumference/age and BMI/age. Data of socio-demographic variables and underlying pathology were also obtained. Student's t-test, Chi-squared test, ANOVA with post-hoc tests and logistic regression were used to identify the likelihood of risk of malnutrition. RESULTS: subjects were divided according to their hospital length of stay in lower than seven days and seven days or more. At the time of admission, children with stay of seven days or more showed a greater deficit in the height/age index (OR = 1.77 [95% CI 1.16-2.7], p = 0.007), head circumference (OR = 2.68 [95% CI 1.5-4.7], p ≤ 0.001) and BMI (OR = 2.9 [95% CI 1.9-4.4], p ≤ 0.001). Males had higher risk of moderate/severe malnutrition (OR = 1.87 [95% CI 1.22-2.85], p = 0.003) and higher deficit in head circumference (OR = 1.89 [95% CI 1.06-3.37], p = 0.029). Infants with gastrointestinal and respiratory problems had higher frequency of malnutrition at the time of admission than did those with other pathologies (p < 0.001). CONCLUSION: the pathological disease at time of admission and longer hospital length of stay increased the likelihood of malnutrition in male infants and preschool-aged children. There were demographic and educational risk factors in the home environment that could influence acute and chronic malnutrition.

This study demonstrated the relationship of low bone mineral density (BMD) with the degree of motor impairment, method of feeding, anthropometric indicators, and malnutrition in children with quadriplegic cerebral palsy (CP). The control of these factors could optimize adequate bone mineralization, avoid the risk of osteoporosis, and would improve the quality of life. PURPOSE: The purpose of the study is to explore the relationship between low BMD and nutritional status in children with quadriplegic CP. METHODOLOGY: A cross-sectional analytical study included 59 participants aged 6 to 18 years with quadriplegic CP. Weight and height were obtained with alternative measurements, and weight/age, height/age, and BMI/age indexes were estimated. The BMD measurement obtained from the lumbar spine was expressed in grams per square centimeter and Z score (Z). Unpaired Student's t tests, chi-square tests, odds ratios, Pearson's correlations, and linear regressions were performed. RESULTS: The mean of BMD Z score was lower in adolescents than in school-aged children (p = 0.002). Patients with low BMD were at the most affected levels of the Gross Motor Function Classification System (GMFCS). Participants at level V of the GMFCS were more likely to have low BMD than levels III and IV [odds ratio (OR) = 5.8 (confidence interval [CI] 95% 1.4, 24.8), p = 0.010]. There was a higher probability of low BMD in tube-feeding patients [OR = 8.6 (CI 95% 1.0, 73.4), p = 0.023]. The probability of low BMD was higher in malnourished children with weight/age and BMI indices [OR = 11.4 (1.3, 94), p = 0.009] and [OR = 9.4 (CI 95% 1.1, 79.7), p = 0.017], respectively. CONCLUSION: There was a significant relationship between low BMD, degree of motor impairment, method of feeding, and malnutrition. Optimizing these factors could reduce the risk of osteopenia and osteoporosis and attain a significant improvement of quality of life in children with quadriplegic CP.

BACKGROUND: The aim of this paper was to estimate the adiposity trend in children from a public elementary school in Guadalajara, Mexico, between 2007 and 2011. METHODS: Comparative study of two samples taken transversely in 1432 schoolchildren aged 6-12 years. Z-score of BMI and anthropometric indicators of arm adiposity (triceps skinfold, fat area and arm fat index) were calculated. Reference standards of the 2007-WHO and Frisancho were used. Analysis: Chi-square and Student t test. RESULTS: Adiposity decreased significantly in children with normal weight (arm fat area Z-score -0.3 SD) overweight and obesity (arm fat area and index -0.4 and -0.3 z-scores respectively). An increase in the prevalence of subjects with normal weight and thinness and a decrease of overweight (4.6, -2.0 and -2.9 percentage points respectively) was observed. Obesity prevalence remained unchanged. CONCLUSIONS: Decrease of adiposity in overweight and obese children, reduction in the prevalence of thinness and overweight and the increase in the proportion of subjects with normal weight was observed during the study period. These data probably reflect the nutritional transition in Mexico.


Introducción: el objetivo de este estudio fue estimar la tendencia de la adiposidad de alumnos de una escuela primaria pública entre 2007 y 2011. Métodos: estudio comparativo de dos muestras obtenidas transversalmente en 1432 escolares de 6 a 12 años de edad. Se calculó la puntuación z del índice de masa corporal y de indicadores antropométricos de adiposidad (pliegue cutáneo tricipital, área grasa e índice de grasa del brazo). Se utilizaron los patrones de referencia de la OMS-2007 y de Frisancho. El análisis se llevó a cabo con Chi cuadrada y t de Student. Resultados: la adiposidad disminuyó significativamente tanto en niños con peso normal (z área grasa del brazo -0.3) como en los que tenían sobrepeso y obesidad (z área grasa del brazo -0.4 y -0.3, respectivamente). Se observó un incremento en la prevalencia de niños con peso normal (4.6 puntos porcentuales), así como disminución de casos con delgadez y sobrepeso (-2.0 y -2.9 puntos porcentuales, respectivamente). La prevalencia de obesidad no se modificó. Conclusiones: se identificó disminución de la adiposidad en niños con sobrepeso y obesidad, disminución en la prevalencia de delgadez y sobrepeso, así como aumento en la proporción de sujetos con peso normal. Estas observaciones probablemente reflejan la transición nutricia en México.

BACKGROUND: There are few articles that document the association between growth and obesity. The objective of this study was to compare height between obese schoolchildren and healthy weight schoolchildren. METHODS: Cross-sectional study performed in 369 healthy weight schoolchildren and 162 obese schoolchildren of an elementary school; 49.4 % were females. Subjects were classified by body mass index percentiles in healthy weight (5-84), and obese (> 95), and grouped by gender and one-year class intervals. It was used Mann-Whitney U test for statistical analysis. RESULTS: In all class intervals, height was higher in obese schoolgirls when we compared them with healthy weight schoolgirls; we did not identify an increasing or decreasing trend related to age. No height difference was observed in 6 and 7 years old obese/healthy weight schoolboys; however, from 8 to 11 years, height increased progressively in obese schoolchildren with a difference of 8.8 cm at the age of 10. CONCLUSIONS: The association of obesity with periods of accelerated growth suggests the existence of phenotypic variants related to metabolic and hormonal factors. Significant higher height values were identified in obese schoolchildren when they were compared with their healthy weight peers.


IBTRODUCCIÓN: la asociación entre crecimiento y obesidad ha sido poco estudiada en la población de escolares obesos mexicanos. El objetivo de este estudio fue comparar la talla para la edad entre escolares obesos y con peso saludable. MÉTODOS: estudio transversal realizado en 369 escolares con peso saludable y 162 obesos en una escuela primaria; 49.4 % era de sexo femenino. Se utilizó la clasificación percentilar del Índice de masa corporal: peso saludable: del percentil 5 al 84, obesidad: valor > percentil 95; se agruparon por sexo e intervalos de clase de un año. El análisis estadístico se hizo con U de Mann-Whitney. RESULTADOS: en las niñas obesas la talla fue mayor a la del grupo con peso saludable en todos los intervalos de clase sin que se identificara una tendencia de incremento o decremento en relación con la edad. En los niños no se observaron diferencias de talla en los 6 y 7 años; sin embargo, de los 8 a los 11 años la talla se incrementó en forma progresiva en los obesos, con diferencia máxima de 8.8 cm a los 10 años. CONCLUSIÓN: la asociación de obesidad con periodos de crecimiento acelerado sugiere la existencia de variantes fenotípicas relacionadas con factores metabólicos y hormonales. Se identificaron valores de talla que fueron significativamente mayores en los niños con obesidad al ser comparados con sus pares con peso saludable.
