RESUMO

This paper is based on a survey on 1,044 medical students during the 3rd, 4th and, 5th year of the career, internship and social service in four different medical schools in Mexico City's metropolitan area. Their motivations for studying medicine and their perspectives for professional development are analyzed. More than 90% decided on their own to study medicine, about 60% decided to study medicine for personal satisfaction, 50% have relatives in the field, 90% wish to become specialists and the same percentage would like to work in a public institution. About 60% of the students believe they will find a job immediately after finishing their training.

RESUMO

This paper discusses the right to health in Mexico. The authors present some ideas regarding equity and data which shows the existence of a deep inequity in the field of health in the country. The efforts of the social security agencies and of the Ministry of Health aimed at diminishing inequity in health are also described. The authors conclude that it is time to avoid retorical positions and attitudes of administrative solidarity and start to implement actions to improve the health conditions of those living in extreme poverty. These actions should include providing comprehensive health services to all the population, reinforcing specific preventive programs, improving certain basic health indicators and prompting intersectoral collaboration.

This paper discusses the relationship between health problems and the definition of priorities in health research in Mexico. The author stresses the importance of increasing research and training in public health, and of relating these activities to the most urgent health needs of the country.

RESUMO

In this work, different aspects of the ambivalence presented by the university will be analyzed. On the one hand, there exists the difficult job of breaking away from the administrative structures within, plus, the fact that the university itself must stimulate educational advancement; on the other hand, the University has an obligation to society to adequately handle the resources assigned to it, and, at the same time, maintain its academic and administrative autonomy. The challenges which confront the university are: to ensure the continuation of superior education and to establish a more reliable relationship with the community with respect to health, above all. The current goal is to better qualify professionals and, at the same time, have them feel an obligation to society to establish a spirit of public service; in this sense, the university should be considered neither protagonist nor antagonist, since it does not function in an isolated way. The various health institutions together with the university should define their objectives as a whole in order to establish an efficient system. Finally, with regard to the subject of health, the university as well as the state and the community must unite in order to accomplish the changes which have stimulated the World Health Organization.

It will be necessary to coordinate the University role with health institutional policies concerning the definition of health goals, and personnel functions as the latter should be trained if and when they are offered the opportunities of grasping both theory and practice in an environment closely related to everyday problems and realities. Accordingly, health care changes must precede health educational changes. There are two factors that interfere with a proper integration between the education care system: 1) an excess number of medical (or health students that overcome teaching facilities and 2) the correct trend in health care, based on specialists, attached to large hospital centers in urban environments. No attention is paid in this way to the importance of the health team, to health promotion, health education, preventive measures, etc. In several Mexican schools new curricula have been developed in which students face actual health problems from the beginning of the studies and they are trained as another resource of the health system being involved in all type of health activities in health centers, schools, nurseries, out-patient community clinics, etc.
