Objectives. To compare the frequency of another primary malignancy in patients with differentiated thyroid carcinoma (DTC) who received radioiodine therapy or not ((131)I). Material and Methods. 168 cases of DTC patients were retrospectively evaluated as to the frequency of another neoplasia by comparing patients with and without it, taking into account clinical, laboratory, and therapeutic parameters. Results. Another primary malignancy occurred in 8.9% of patients. Of these, 53.3% showed the malignancy before (131)I and 46.7% after it. By comparing both groups, the age at the moment of diagnosis of another neoplasia was 46.1 ± 20.2 years for the group before (131)I therapy and of 69.4 ± 11.4 years for the group after it (P = 0.02). Of the 148 patients treated with (131)I, 4.7% developed another malignancy. The latter were older (61 ± 17 years) than those who did not show another cancer type (44.1 ± 14.2 years) (P < 0.05). Conclusion. The frequency of another neoplasia found after (131)I was similar to that found before (131)I.
RESUMO
Pituitary tumor apoplexy is a rare neuroendocrine syndrome resulting, in most cases, from hemorrhage or infarctation of a pre-existing pituitary adenoma. Treatment recommendations vary; some authors advocate urgent surgical decompression of the tumor, whereas others suggest that conservative management can lead to recovery of neuro-ophthalmologic function. We describe two patients with pituitary tumor apoplexy who had clinically non-functioning macroadenomas and hypopituitarism, including hypogonadism. They were treated conservatively without surgery, and achieved tumor remission.

Chordomas are tumors derived from cells that are remnants of the notochord, particularly from its proximal and distal extremes, they are mainly midline and represent approximately 1% of all malignant bone tumors and 0.1 to 0.2% of intracranial neoplasms. Chordomas involving the sellar region are rare. Herein, we describe a 57-year-old male patient presenting with a history of retro-orbital headache, progressive loss of vision, and clinical features of hypopituitarism, for over 2 months. During evaluation, the CT scan revealed a large contrast-enhancing intrasellar tumor with a 3.6-cm largest diameter. The patient underwent transsphenoidal partial resection of the tumor, and histological examination was consistent with the diagnosis of chondroid chordoma. Although chordomas are rare, they may be considered to constitute a differential diagnostic of pituitary adenomas, especially if a calcified intrasellar tumor with bone erosion is diagnosed.

Objetivos: Determinar o perfil clínico dos pacientes com emergência hipertensiva e identificar variáveis associadas ao óbito. Métodos: Análise dos prontuários de pacientes com diagnóstico de emergência hipertensiva. Emergência foi caracterizada com evidência de comprometimento agudo de órgão alvo. Utilizou-se a regressão logística para identificar os possíveis fatores de predição para mortalidade. Resultados: Foram estudados 93 pacientes com diagnóstico de emergência hipertensiva. A maioria (94%) tinha história prévia de hipertensão, sendo que parcela significativa (31 %) apresentara episódio prévio de crise hipertensiva. Em nossos pacientes, as apresentações clínicas mais frequentes foram edema agudo de pulmão (29%), angina pectoris (26%), acidente vascular encefálico (21 %) e encefalopatia hipertensiva (10%). Em relação ao tratamento, 68% dos pacientes com emergência hipertensiva receberam tratamento endovenoso. Não foram encontradas diferenças na evolução dos pacientes tratados com medicações endovenosas ou orais (p igual 0,159). Do total dos casos estudados, 10 pacientes foram a óbito. O acidente vascular encefálico (p igual 0,009) e o uso prévio de inibidores da enzima conversora da angiotensina (IECA) (p igual 0,017) foram fatores de predição de mortalidade. Conclusão: As formas de apresentação mais frequentes da emergência hipertensiva foram o edema agudo de pulmão e a angina; acidente vascular encefálico e uso prévio de IECA foram fatores de predição de mortalidade.
