RESUMO

This paper describes psychocardiology studies conducted from 2002 through 2018 in the Cardiology Department of the Dr Gustavo Aldereguía Lima University General Hospital in Cienfuegos Province, Cuba. Given the strong association between cardiovascular diseases and lifestyle, negative emotions and personality traits, psychology and medicine are equally necessary components of prevention and patient care, indispensable to primary and secondary prevention and to rehabilitation. When its therapeutic principles are appropriately applied, psychology can have a positive impact on the course of the disease and on patients' adaptation to new habits and lifestyles. The psychocardiologist's job is fundamental in achieving conscious participation by patients in their rehabilitation. Theoretical and practical contributions include a gender-based approach, addressing patients' sexual needs, neuropsychological assessment of damage associated with cardiovascular diseases, the particularities of pediatric patient care, family involvement in rehabilitation, and services for families at risk for cardiovascular events due to genetic factors. Results of these studies are included in published methodology for intensive psychological treatment for patients and health care workers.

RESUMO

RESUMEN La problemática del muy bajo peso al nacer (MBP) ha sido abordada desde hace varios años, sin embargo, las investigaciones están enfocadas desde diferentes perspectivas y contextos, no siempre se ha considerado su repercusión a corto y a largo plazo, así como la interacción de los diferentes factores que se relacionan con este riesgo biológico. Por ello en esta revisión bibliográfica se realiza una valoración de estudios ejecutados a nivel internacional y en Cuba dedicados al desarrollo neuropsicológico de niños con muy bajo peso al nacer. Se concluye que el muy bajo peso al nacer repercute en el desarrollo neuropsicológicos desde edades tempranas, hasta la adolescencia; por ello es necesario una visión integradora que tenga en cuenta el riesgo biológico, las particularidades individuales y el contexto social en que se desenvuelve el niño desde los primeros momentos, lo que posibilitará, perfeccionar la atención a corto y largo plazo al grupo de riesgo.


ABSTRACT The problems of very low birth weight (MBP) have been dealt with for many years, however, research on the matter is approached from different perspectives and contexts, not always have its short and long term repercussions being considered as well as the interaction of different factors related to this biological risk. Therefore, this bibliographical review makes an assessment of international and Cuban studies devoted to very low birth weight children's neuropsychological development. It was reached the conclusion that very low birth weight affects neuropsychological development from early ages to adolescence; for that reason, it is necessary to have an integrated approach of the problem that takes into account the biologicals risk, individual characteristics and the social context in which the child develops from the beginning, which will make the improvement of short and long term care to this risk group possible.

RESUMO

Fundamento: Existe una gran variabilidad del funcionamiento cognitivo en el curso de la esclerosis múltiple, incluso entre pacientes que tienen similares patrones de disfunción cerebral. Objetivo: Sintetizar los resultados científicos más representativos sobre las particularidades de la reserva cognitiva en esta patología. Desarrollo: Se sintetiza sobre los referentes teóricos y científicos relacionados con el modelo de reserva cognitiva y cerebral en la esclerosis múltiple, bases neurobiológicas de la reserva y el valor de las actividades cognitivas de ocio como proxies de la reserva cognitiva. Además, se analizan los resultados relacionados con la importancia de la intervención temprana en la prevención del deterioro cognitivo en estos pacientes. Conclusiones: La teoría de la reserva cognitiva puede resultar de gran utilidad para la práctica de la evaluación e intervención neuropsicológica en los pacientes con esclerosis múltiple; en este sentido, la reserva cognitiva puede actuar como un factor modulador (riesgo o protector) del declive cognitivo y marcador de eficiencia cerebral en este tipo de pacientes.


Background: A great variability of the cognitive functioning exists in the course of the multiple sclerosis, even among patients that have similar patterns of cerebral dysfunction. Objective: To synthesize the most representative scientific results about the particularities of the cognitive reservation in this pathology. Development: It is synthesized on the theoretical and scientific referents related with the pattern of cognitive and cerebral reservation in the multiple sclerosis, base neurobiological of the reservation and the value of the leisure cognitive activities like proxies of the cognitive reservation. Also, the results related with the importance of the early intervention are analyzed in the prevention of the cognitive deterioration in these patients. Conclusions: The theory of the cognitive reservation can be of great utility for the practice of the neuropsychological evaluation and intervention in the patients with multiple sclerosis; in this sense, the cognitive reservation can act as a modulator factor (risk or protective) of the cognitive decline and marker of cerebral efficiency in this type of patient.

RESUMO

Los factores psicológicos en el proceso de salud-enfermedad constituyen un área controvertida aunque son vitales para propiciar programas de intervención preventivos o rehabilitatorios. En este sentido, la controlabilidad constituye uno de los procesos que más se ha investigado,aunque subsisten algunas insuficiencias teóricas para entenderlo como un factor de riesgo de muchas enfermedades. Analizando la controlabilidad como un proceso multideterminado, producto de la ruptura del equilibrio gratificador y la expresión de la personalidad del sujetoen la relación que establece con su medio, se expone, en el presente artículo, una teoría integradora acerca de los mecanismos que subyacen a la misma tomando ejemplificaciones deun estudio realizado en sujetos con accidente cerebrovascular. Así, se establece que la controlabilidad de carácter potencialmente patógeno se presenta en aquellas personas que presentan cierta predisposición a patrones de comportamiento estrictos. Además, se encuentra estrechamente vinculada a las estrategias de afrontamiento y las creencias de salud; propiciando, de manera indirecta, estados negativos como la ansiedad, la depresión, las rumiaciones yel efecto rebote.


Psychological factors in the health-disease process are a controversial topic but at the same time are vital points to develop any kind of program focused on prevention or rehabilitation.Controllability is a process that concentrate a lot of investigations although subsist some theory deficiencies to understand it as a factor of risk to suffer several diseases. That is why we propose to analyze control as a multidetermined process, a product of the rupture of the reward balanceand a personality expression in the relation between subject and environment. Analyzing somepatients with brain stroke we can conclude that control become into a factor of risk when it is formed as a strong personality characteristic in persons with strict behavior patterns. Besides, control is closely related with confrontation strategies and health beliefs inducing anxiety,depression, rumination and bounce effect.
