RESUMO

Follicle-stimulating hormone (FSH) and luteinizing hormone (LH) control antral follicular growth by regulating several processes, such as the synthesis of hormones and signaling molecules, proliferation, survival, apoptosis, luteinization, and ovulation. To exert these effects, gonadotropins bind to their respective Gs protein-coupled receptors, activating the protein kinase A (PKA) pathway or recruiting Gq proteins to activate protein kinase C (PKC) signaling. Although the action mechanism of FSH and LH is clear, recently, it has been shown that both gonadotropins promote the synthesis of sphingosine-1-phosphate (S1P) in granulosa and theca cells through the activation of sphingosine kinase 1. Moreover, the inhibition of SPHKs reduces S1P synthesis, cell viability, and the proliferation of follicular cells in response to gonadotropins, and the addition of S1P to the culture medium increases the proliferation of granulosa and theca cells without apparent effects on sexual steroid synthesis. Therefore, we consider that S1P is a crucial signaling molecule that complements the canonical gonadotropin pathway to promote the proliferation and viability of granulosa and theca cells.

Resumen: Las lesiones multiligamentarias de la articulación de la rodilla se caracterizan por la afección de dos o más ligamentos principales, ligamento cruzado anterior (LCA), ligamento cruzado posterior (LCP), ligamento colateral medial (LCM), ligamento colateral lateral (LCL), esquina posteromedial (EPM) y esquina posterolateral (EPL). Estadísticamente, las lesiones multiligamentarias son poco frecuentes, ya que se encuentran en menos de 0.02% de todas las lesiones traumáticas de rodilla; sin embargo, diferentes lesiones agregadas son las que hacen de esta patología un estado grave para salud y funcionalidad. Tomando en cuenta que la mayoría de los pacientes son personas jóvenes en edad altamente productiva, es de vital importancia observar la evolución de los mismos a corto y largo plazo, así como su reincorporación a su vida cotidiana. Se ha descrito que las lesiones vasculares se presentan aproximadamente en 32% de los casos, lesiones meniscales en 35% y hasta 60% con algún tipo de lesión ósea. Estas lesiones afectan principalmente al sexo masculino con más frecuencia entre la tercera y cuarta década de la vida, lo cual hace a este tipo de lesiones de gran importancia, ya que este grupo de pacientes se encuentran en su mayor período de producción laboral. El objetivo principal del tratamiento de estas lesiones, además de resolver los daños agregados que suelen ser los agravantes del estado de salud, es lograr una pronta recuperación y posterior reincorporación a sus actividades laborales y en algunos casos deportivas.

RESUMO

A 34-year-old male was admitted with presumed acute, severe aortic regurgitation. Multimodal imaging was performed and showed a ruptured right coronary sinus of Valsalva aneurysm into the right atrium. He underwent a percutaneous closure of the ruptured sinus of Valsalva aneurysm. The patient had major clinical improvement.

BACKGROUND: High-density lipoprotein (HDL) is considered a complex plasma-circulating particle with subfractions that vary in function, size, and chemical composition. We sought to test the effects of HDL, and HDL subfractions on insulin secretion and cholesterol efflux in the ß-cell line MIN-6. METHODS: We used total HDL and HDL subfractions 2a, 2b, 3a, 3b, and 3c, isolated from human plasma, to test insulin secretion under different glucose concentrations as well as insulin content and cholesterol efflux in the insulinoma MIN-6 cell line. RESULTS: Incubation of MIN-6 cells with low glucose and total HDL increased insulin release two-fold. Meanwhile, when high glucose and HDL were used, insulin release increased more than five times. HDL subfractions 2a, 2b, 3a, 3b, and 3c elicited higher insulin secretion and cholesterol efflux than their respective controls, at both low and high glucose concentrations. The insulin content of the MIN-6 cells incubated with low glucose and any of the five HDL subclasses had a modest reduction compared with their controls. However, there were no statistically significant differences between each HDL subfraction on their capacity of eliciting insulin secretion, insulin content, or cholesterol efflux. CONCLUSIONS: HDL can trigger insulin secretion under low, normal, and high glucose conditions. We found that all HDL subfractions exhibit very similar capacity to increase insulin secretion and cholesterol efflux. This is the first report demonstrating that HDL subfractions act both as insulin secretagogues (under low glucose) and insulin secretion enhancers (under high glucose) in the MIN-6 cell line.

PURPOSE: Both type 2 diabetes (T2D) and low levels of high-density lipoprotein cholesterol (HDL-C) are very prevalent conditions among Mexicans. Genetic variants in the LIPC gene have been associated with both conditions. This study aimed to evaluate the association of the -514C < T (rs1800588) LIPC gene polymorphism with different metabolic traits, particularly the effects of this polymorphism on HDL-C plasma levels and T2D risk. METHODS: Mediation analysis was used to assess the direct and indirect effects of the -514C>T LIPC gene variant on HDL-C levels, T2D risk, and body mass index (BMI), in 2105 Mexican mestizo participants. We also assessed the functional effect of the -514C>T LIPC variant on the promoter activity of a reporter gene in the HepG2 cell line. RESULTS: Direct effects show that the -514C>T LIPC polymorphism is significantly associated with increased HDL-C plasma levels (ß = 0.03; p < 0.001). The -514C>T variant resulted in an indirect protective effect on T2D risk through increasing HDL-C levels (ß = - 0.03; p < 0.001). Marginal direct association between -514C>T and T2D was found (ß = 0.08; p = 0.06). Variables directly influencing T2D status were European ethnicity (ß = - 7.20; p < 0.001), age (ß = 0.04; p < 0.001), gender (ß = - 0.15; p = 0.017) and HDL-C (ß = - 1.07; p < 0.001). In addition, we found that the -514C>T variant decreases the activity of LIPC promoter by 90% (p < 0.001). CONCLUSIONS: The -514C>T polymorphism was not directly associated with T2D risk. HDL-C acts as a mediator between -514C>T LIPC gene variant and T2D risk in the Mexican population.

Resumen La calcifilaxis es una enfermedad poco frecuente, aunque presenta una elevada tasa de mortalidad debido sobre todo a complicaciones como sepsis o gangrena. Generalmente se asocia a insuficiencia renal severa. Se define como la calcificación de la capa media de vasos de pequeño y mediano tamaño de la dermis y tejido celular subcutáneo. Clínicamente se manifiesta como un síndrome de livedoracemosa que progresa a púrpura retiforme y necrosis cutánea. La primera línea de tratamiento es el tiosulfato sódico.


Abstract Calciphylaxis is a rare disease, although it has a high mortality rate due mainly to complications such as sepsis or gangrene. It is usually associated with severe renal failure. It is defined as the calcification of the middle layer of small and medium-sized vessels of the dermis and subcutaneous cellular tissue. Clinically it manifests as a livedoracemosa syndrome that progresses to retinal purpura and cutaneous necrosis. The first line of treatment is sodium thiosulfate.

RESUMO

PURPOSE: Type 2 diabetes (T2D) and low serum concentration of high-density lipoprotein cholesterol (HDL-c) are common coexisting metabolic disorders. ABCA1 variants have been shown to be associated to these conditions. We sought to test the combined effect of two ABCA1 gene common variants, rs2422493 (- 565C > T) and rs9282541 (R230C) on HDL-c levels and T2D risk. METHODS: Path analysis was conducted in 3,303 Mexican-mestizos to assess the specific contributions of rs2422493 and rs9282541 ABCA1 variants, insulin resistance, waist-to-height ratio (WHtR), and age on HDL-c levels and T2D risk. Participants were classified into four groups according to their ABCA1 variants carrier status: (i) the reference group carried wild type alleles for both ABCA1 variants (-/-), (ii) +/- were carriers of rs2422493 but non-carriers of rs9282541, (iii) -/+ for carriers of rs9282541 but not carriers of rs2422493 and (iv) carriers of minor alleles for both SNPs (+/+). Principal components from two previous genome-wide association studies were used to control for ethnicity. RESULTS: We identified significant indirect effects on T2D risk mediated by HDL-c in groups -/+ and +/+ (ß = 0.04; p = 0.03 and ß = 0.06; p < 0.01, respectively) in comparison to the -/- reference group. Low concentrations of HDL-c were directly and significantly associated with increased T2D risk (ß = -0.70; p < 0.01). WHtR, male gender, age, and insulin resistance were also associated with T2D risk (p < 0.05). There was no significant direct effect for any of the ABCA1 groups on T2D risk: p = 0.99, p = 0.58, and p = 0.91 for groups +/-, -/+, and +/+ respectively. CONCLUSIONS: The ABCA1 rs9282541 (R230C) allele is associated with T2D in Mexicans through its effect on lowering HDL-c levels. This is the first report demonstrating that HDL-c levels act as an intermediate factor between an ABCA1 variant and T2D.

Resumen Objetivo: Presentar un algoritmo estable que determina, a partir de mediciones electroencefalográficas, los parámetros de fuentes de tipo dipolar asociadas a focos epilépticos ubicados sobre la superficie de la corteza cerebral. Metodología: Se utiliza un problema de contorno para establecer correlaciones entre la fuente y la medición. El problema se divide en dos subproblemas lineales y en cada uno de ellos, se utilizan el método de mínimos cuadrados y la regularización de Tikhonov para encontrar soluciones estables. Estos subproblemas son problemas mal planteados en el sentido de Hadamard, debido a la inestabilidad numérica que presentan, es decir, pequeños cambios en las mediciones pueden producir grandes variaciones en la solución de cada problema. El parámetro de regularización de Tikhonov fue elegido usando el método de la curva L. Para hallar la solución del problema de contorno se utiliza el método de las series de Fourier y el Método del Elemento Finito. Resultados: Se propuso un tipo de fuente para representar a los focos epilépticos en la corteza cerebral y un algoritmo estable para el problema de identificación de los parámetros de dichas fuentes. Se desarrollaron ejemplos sintéticos y programas en MATLAB para el caso de geometría simple bidimensional. Originalidad: La separación del problema original en dos subproblemas así como los ejemplos sintéticos son producto de esta investigación. Conclusión general: Se propuso un algoritmo estable que determina a los parámetros de fuentes de corriente dipolar definidas en la corteza cerebral.


Abstract Objective: To present a stable algorithm that determines, from electroencephalographic measurements, the parameters of dipolar sources associated with epileptic foci located on the cerebral cortex. Methodology: A boundary value problem is used to establish correlations between the sources and the measurements. The problem is divided into two linear subproblems and in each one, the method of Minimum Square and the Tikhonov regularization are used for finding stables solutions. These subproblems are an ill-posed problem in the Hadamard sense, which is due to the numerical instability, that is, small changes in the data can produce substantial variations in the solution of each problem. The Tikhonov regularization parameter was chosen using the L curve method. To find the solution of the boundary value problem are used the Fourier series method and the Finite Element Method. Results: A type of source that represents the epileptic foci on the cerebral cortex and a stable algorithm for finding the parameter of these sources were proposed. Synthetics examples and MATLAB programs were developed for the case of bidimensional geometry. Originality: The separation of the original problem into two subproblems and the synthetics examples are a product of this research. Conclusion: A stable algorithm was proposed for determining the parameters of the dipolar current defined on the cerebral cortex.

Resumen El presente trabajo describe un prototipo de una silla de ruedas que es dirigido hacia enfrente y hacia atrás usando 2 o 3 parpadeos, respectivamente, y es detenido cuando se alcanzan ciertos niveles de atención. El objetivo principal es que las personas que tienen discapacidad motora en sus extremidades puedan usarlo para desplazarse y les brinde autonomía. Para captar la señal de los parpadeos, se utilizó la diadema MindWave Mobile de Neurosky. Se implementó un circuito electrónico en conjunto con Arduino que permite complementar la ejecución del accionamiento del prototipo. El prototipo se probó con 10 personas cuyas edades oscilan entre 20 y 35 años. Los resultados muestran que, en un 80% de los casos, el prototipo se mueve correctamente. La gran ventaja del presente trabajo es que la interfaz cerebro-computadora con la que cuenta este prototipo no requiere entrenamiento previo del sistema, por lo cual, puede ser usado por cualquier persona. Además, su costo es más accesible comparado con otros dispositivos para el mismo fin.


Abstract The present work describes a prototype of a wheel chair directed by means of eye blinks, which can be moved forwards, and backwards using 2 or 3 eye blinks, respectively, and stopped when a certain attention level is met. The main objective of this work is to help people, who have motor disabilities on their arms and legs, move and have autonomy. In order to register the eye blinking signals, the MindWave Mobile device from Neurosky was used. Moreover, an electronic circuit in combination with Arduino has been used to make the prototype work. This prototype has been tested in 10 healthy people from 20 to 35 years old. According to the results, in 80% of the cases the prototype worked correctly. The main advantage of the present work is that the brain-computer interface, which is part of the prototype, does not require training, and hence, it could be used by most of the people. Moreover, its cost is less than similar devices.

BACKGROUND: Cryopreservation induces spermatic cryo capacitation, which can decrease thawed sperm fertilizing capability. OBJECTIVE: To evaluate the effect of uterus-vaginal union protein factors to inhibit sperm cryo capacitation and maintain viability and fertilizing capability of rooster spermatozoa. MATERIALS AND METHODS: Rooster spermatozoa was cryopreserved using Lake extender supplemented with different hen's uterus-vaginal junction protein concentrations, to determine spermatic viability, sperm physiological condition and fertilizing capability in vivo. RESULTS: It was possible to induce spermatic decapacitation in vitro, inhibiting cryo capacitation and allowing fertility results comparable to those obtained with fresh semen. CONCLUSION: Uterus-vaginal protein extracts induce spermatic decapacitation in vitro.

INTRODUCTION AND AIMS: Gastric cancer is one of the main causes of cancer worldwide, but there is currently no global screening strategy for the disease. Endoscopy is the screening method of choice in some Asian countries, but no standardized technique has been recognized. Systematic alphanumeric-coded endoscopy can increase gastric lesion detection. The aim of the present article was to compare the usefulness of systematic alphanumeric-coded endoscopy with conventional endoscopy for the detection of premalignant lesions and early gastric cancer in subjects at average risk for gastric cancer. MATERIALS AND METHODS: A cross-sectional, comparative, prospective, randomized study was conducted on patients at average risk for gastric cancer (40-50 years of age, no history of H. pylori infection, intestinal metaplasia, gastric atrophy, or gastrointestinal surgery). Before undergoing endoscopy, the patients had gastric preparation (200mg of oral acetylcysteine or 50mg of oral dimethicone). Conventional chromoendoscopy was performed with indigo carmine dye for contrast enhancement. RESULTS: Fifty consecutive cases (mean age 44.4 ± 3.34 years, 60% women, BMI 27.6 ± 5.82 kg/m2) were evaluated. Endoscopic imaging quality was satisfactory in all the cases, with no differences between methods (p = 0.817). The detection rate of premalignant lesions and early gastric cancer was 14% (6 cases of intestinal metaplasia and one case of gastric adenocarcinoma). Sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic accuracy were 100, 95, 80, 100 and 96%, respectively, for systematic alphanumeric-coded endoscopy, and 100, 45, 20, 100, and 52%, respectively, for conventional endoscopy. Lesion detection through systematic alphanumeric-coded endoscopy was superior to that of conventional endoscopy (p = 0.003; OR = 12). CONCLUSION: Both techniques were effective, but systematic alphanumeric-coded endoscopy significantly reduced the false positive rate.

A compact, highly sensitive optical fiber displacement and curvature radius sensor is presented. The device consists of an adiabatic bi-conical fused fiber taper spliced to a single-mode fiber (SMF) segment with a flat face end. The bi-conical taper structure acts as a modal coupling device between core and cladding modes for the SMF segment. When the bi-conical taper is bent by an axial displacement, the symmetrical bi-conical shape of the tapered structure is stressed, causing a change in the refractive index profile which becomes asymmetric. As a result, the taper adiabaticity is lost, and interference between modes appears. As the bending increases, a small change in the fringe visibility and a wavelength shift on the periodical reflection spectrum of the in-fiber interferometer is produced. The displacement sensitivity and the spectral periodicity of the device can be adjusted by the proper selection of the SMF length. Sensitivities from around 1.93 to 3.4 nm/mm were obtained for SMF length between 7.5 and 12.5 cm. Both sensor interrogations, wavelength shift and visibility contrast, can be used to measure displacement and curvature radius magnitudes.

Introducción: La adolescencia es una etapa de importantes cambios biopsicosociales: imagen corporal, ideales, proyecto vital, roles intrafamiliares y sociales sufren redefiniciones tendientes a la autonomía y el fortalecimiento de la identidad. El proceso en su conjunto implica desafíos y conquistas, pero también un grado importante de vulnerabilidad psicológica. La enfermedad oncológica tiene un alto impacto socio emocional, afectando todos los procesos adolescentes. Objetivos: Explorar las problemáticas que enfrentan los adolescentes durante el tratamiento oncológico y conocer los recursos psicológicos, sociales e institucionales que utilizan para transitar los procesos de la adolescencia en dicha situación. Material y métodos: estudio observacional, transversal y prospectivo. Para la recolección de datos se utilizó un cuestionario para adolescentes, un cuestionario para cuidadores, y la Escala de Afrontamiento para adolescentes (ACS) de Frydenberg y Lewis. Los cuestionarios fueron elaborados para este estudio y contenían preguntas con respuestas abiertas y cerradas. Para el procesamiento de datos, se registraron todas las respuestas cerradas y con las respuestas abiertas se formaron categorías por consenso siempre que una misma idea fuera referida por al menos el 10% de los entrevistados. Resultados: 42 adolescentes y 32 cuidadores participaron del estudio. Mediana de edad: 15,04 años (13,08-18,09). Tiempo desde el diagnóstico: mediana 6 meses (1-24). El 81% de los adolescentes contaba con comprensión adecuada de la enfermedad y el 19% con comprensión parcial. El 57% refirió preocupaciones vinculadas con la enfermedad y/o el tratamiento. El síntoma físico más reportado fue el cansancio; respecto al estado emocional fue la alternancia de momentos buenos y momentos malos. Los principales recursos de ayuda fueron la familia y los amigos. Presentaron variabilidad en el uso de estrategias de afrontamiento, mayor uso de estrategias eficaces y menor uso de disfuncionales. Conclusiones: Los adolescentes en tratamiento oncológico requieren del apoyo y aliento de la familia, amigos y equipo de salud. Mejorar la infraestructura y el equipamiento electrónico del hospital facilitaría que se mantengan activos y comunicados. Crear espacios asistenciales y recreativos diferenciales para adolescentes favorecería el encuentro e intercambio entre ellos. Estimular la participación activa de los adolescentes en el diálogo con los médicos y en el tratamiento permitiría dar lugar a sus preocupaciones y que recobren algo de la autonomía perdida (AU)


Introduction: Adolescence is a stage of important biopsychosocial changes: Body image, ideals, project of life, intra-family and social roles are subject to redefinitions related to autonomy and identity strengthening. Taken together, this process implies challenges and achievements, but also a considerable degree of psychological vulnerability. Oncological disease has a strong socio-emotional impact, affecting all processes of adolescence. Aims: To assess problems that affect adolescents during oncology treatment and determine psychological, social, and institutional resources they use to go through the processes of adolescence in this situation. Material and methods: Prospective, observational, and cross-sectional study. For data collection a survey for adolescents, a survey for caregivers, and the Adolescent Coping Scale (ACS) by Frydenberg and Lewis were used. The surveys were developed for this study and had open-ended and closedended questions. For data processing all closed-ended questions were recorded and categories were developed for the openended questions based on consensus when the same idea was expressed by at least 10% of the interviewees. Results: 42 adolescents and 32 caregivers participated in the study. Median age was 15.04 years (13,08-18,09). Median time from diagnosis was 6 months (1-24). Overall, 81% of the adolescents had an adequate understanding of the disease and 19% a partial understanding. Of the adolescents, 57% reported worries related to the disease and/or the treatment. The most commonly reported physical symptom was fatigue; emotional state was alternating between good and bad moments. The main support resources were the family and friends. The adolescents used a variability of coping strategies, with more use of efficacious than dysfunctional strategies. Conclusions: Adolescents receiving oncology treatment need support and encouragement from the family, friends, and the health-care team. Improving the infrastructure and electronic equipment of the hospital may be helpful in keeping them active and connected. Creating differential care and recreational spaces for adolescents will favor meetings and exchange among them. Stimulating active participation of the adolescents in the dialogue with physicians and in the treatment will allow them to reveal their worries and recover some of their lost autonomy (AU)

RESUMO

Most people with diabetes suffer some deterioration of the feet. Diabetic foot syndrome causes ulceration in about 15% of cases and such deterioration leads to amputation in about 2.5% of diabetic patients, diminishing their quality of life and generating extraordinary costs for patients and public health systems. Currently, there is no objective method for the detection of diabetic foot syndrome in its early stages. We propose terahertz imaging as a method for the evaluation of such deterioration. This screening method could aid the prevention and medical treatment of this condition in the future.

Laparoscopic Nissen fundoplication is currently considered the surgical treatment of choice for gastroesophageal reflux disease (GERD) and its long-term effectiveness is above 90%. Adequate patient selection and the experience of the surgeon are among the predictive factors of good clinical response. However, there can be new, persistent, and recurrent symptoms after the antireflux procedure in up to 30% of the cases. There are numerous causes, but in general, they are due to one or more anatomic abnormalities and esophageal and gastric function alterations. When there are persistent symptoms after the surgical procedure, the surgery should be described as "failed". In the case of a patient that initially manifests symptom control, but the symptoms then reappear, the term "dysfunction" could be used. When symptoms worsen, or when symptoms or clinical situations appear that did not exist before the surgery, this should be considered a "complication". Postoperative dysphagia and dyspeptic symptoms are very frequent and require an integrated approach to determine the best possible treatment. This review details the pathophysiologic aspects, diagnostic approach, and treatment of the symptoms and complications after fundoplication for the management of GERD.

Textile manufacturing is the one responsible for water bodies' contamination through the discharge of colored wastes. This work presents the study of reactive yellow HF (RYHF) dye degradation under two different electrochemical advanced oxidation processes (EAOP), namely anodic oxidation (AO) and electro-Fenton (EF)/boron-doped diamond (BDD) process. For the AO, 100 and 300 mg/L solutions using Pt and BDD as anodes in a 100 mL stirred tank cell were used, with a supporting electrolyte of 0.05 mol/L of Na2SO4 at pH 3 under 30 and 50 mA/cm2 current density. The EF/BDD process was carried out in a flow reactor at 4 and 7 L/min to degrade 100, 200, and 300 mg/L RYHF solutions under 50 and 80 mA/cm2. UV-Vis determinations were used for decolorization evaluation, while high-performance liquid chromatography (HPLC) method provided information on dye degradation rate.

The objective of this study was to evaluate if short-term dietary concentrate supplementation increased IGF-I serum concentration and resulted in a reproductive response during estrus synchronization treatment in non-lactating beef cows. Thirty non-lactating beef cows (Bos indicus × Bos taurus) were allocated to the same pastureland and fed native tropical grasses as a basal diet. Cows were synchronized using a 7-day CO-Synch plus controlled internal drug release (CIDR) protocol and received fixed time artificial insemination (FTAI). Cows were divided into two groups; the control group (n = 16) received 0.5 kg of concentrate/cow/day, whereas the supplemented group (n = 14) received 4.0 kg of concentrate/cow/day. The period of supplementation was 10 days from the day of CIDR insert to FTAI. The concentration of IGF-I increased (P < 0.05) in the supplemented group, while no significant changes were observed in the control group. Moreover, at the time of insemination, IGF-I serum concentrations were higher in supplemented cows compared with control cows (P < 0.05). Notably, metabolite and insulin concentrations did not differ (P > 0.05) between treatment groups or sampling day. The response to estrus induction, measured as estrus presentation, ovulation rate, and pregnancy rate, was similar between experimental groups (P > 0.05). In conclusion, our results indicated that supplementation with dietary concentrate for 10 days in non-lactating beef cows changed the endocrine milieu, specifically increasing IGF-I serum concentration. However, these endocrine changes did not affect response to estrous induction treatment.
