RESUMO

Background: Hailey-Hailey disease is a rare autosomal dominant genodermatosis whose cause is the ATP2C1 gene mutation. A prevalence of 1 in 50,000 cases is estimated and it manifests as grouped flaccid vesicles that break easily. The diagnosis is confirmed with the histopathological study creating an appearance called "dilapidated brick wall", identifying dyskeratosis in the form of round bodies and pimples. Treatment ranges from general measures to multiple pharmacological options, with topical corticosteroids being the most commonly used. Clinical case: Male patient diagnosed with Hailey-Hailey disease. On physical examination we observed a dermatosis disseminated to the neck, trunk, axillary and inguinal folds, and intergluteal region, unilateral, asymmetric with a polymorphous appearance, constitution due to exulceration, erythema, some pustules and flaccid vesicles that coalesced to form eczematous and hypertrophic plaques with the presence of fine scales on their surface, with a chronic evolution accompanied by pruritus. We also took the opportunity to review the most relevant information in the literature regarding Hailey-Hailey disease, especially focused on the therapeutic aspect. Conclusions: It is important to take into account that Hailey-Hailey disease is a rare pathology, in order to make a differential diagnosis in daily clinical practice.

This study is aimed at the analysis of the pyrolysis kinetics of Nanche stone BSC (Byrsonima crassifolia) as an agro-industrial waste using non-isothermal thermogravimetric experiments by determination of triplet kinetics; apparent activation energy, pre-exponential factor, and reaction model, as well as thermodynamic parameters to gather the required fundamental information for the design, construction, and operation of a pilot-scale reactor for the pyrolysis this lignocellulosic residue. Results indicate a biomass of low moisture and ash content and a high volatile matter content (≥70%), making BCS a potential candidate for obtaining various bioenergy products. Average apparent activation energies obtained from different methods (KAS, FWO and SK) were consistent in value (~123.8 kJ/mol). The pre-exponential factor from the Kissinger method ranged from 105 to 1014 min-1 for the highest pyrolytic activity stage, indicating a high-temperature reactive system. The thermodynamic parameters revealed a small difference between EA and ∆H (5.2 kJ/mol), which favors the pyrolysis reaction and indicates the feasibility of the energetic process. According to the analysis of the reaction models (master plot method), the pyrolytic degradation was dominated by a decreasing reaction order as a function of the degree of conversion. Moreover, BCS has a relatively high calorific value (14.9 MJ/kg) and a relatively low average apparent activation energy (122.7 kJ/mol) from the Starink method, which makes this biomass very suitable to be exploited for value-added energy production.

RESUMO

Background: The acquired cutaneous pigmentation represents a little recognized clinical manifestation in liver disorders, both acute and chronic, and can occur in the exacerbation processes of preexisting hepatopathies, as in the context of acute-on-chronic liver failure. Several hypotheses about the increase in pigment at skin and mucous membranes have been developed; some try to explain it as a defect in the degradation of melanin with secondary accumulation at tissues; others, on the other hand, describe it as a consequence of the release of fibroblast growth factors like endothelial growth factor and hepatocyte growth factor, which are produced under the stimulation of liver regeneration and cause a melanogenesis stimulation. The aim of this article is to study pigmentary skin changes in the background of liver diseases. Clinical cases: We described two clinical cases of patients with acuteon chronic liver failure secondary to different clinical scenarios are presented, who have in common the development of acquired pigmentary skin changes. Conclusion: In hepatopathies, the cutaneous hyperpigmentation is a sign with unknown etiology, so further studies are required to know the accurate pathophysiology. Reporting this finding is useful for physicians, since timely identification can help in the early diagnosis of underlying liver diseases.

Bowel preparation has been used in colon and rectal surgery for a variety of reasons. A clean colon is thought to facilitate bowel manipulation, enable passage and firing of surgical staplers, and allow for intraoperative colonoscopy, if needed. The most studied and debated aspect of bowel preparation, however, is its role in reducing surgical morbidity, namely surgical site infections(SSIs). Colon and rectal surgery has among the highest rates of SSIs reported for all types of elective surgery, with recent reviews demonstrating rates varying from 5.4% to 23.2%, with a weighted mean of 11.4%(AU)

RESUMO

BACKGROUND: Male infertility is associated with altered characteristics of the sperm within the ejaculate. It is possible to find molecular explanations for the observed phenotypes and their consequences. This study aimed to analyze, using a specialized software, a gene set of transcriptomic data from different types of ejaculates. METHODS: Data from ejaculate samples categorized as normal, oligospermia, and teratozoospermia were obtained from Gene Expression Omnibus (GEO). After normalization, the data average for each sample category was calculated and analyzed independently using Ingenuity Pathway Analysis (IPA). RESULTS: Five important canonical pathways are involved in normal and altered semen samples (Oligospermia and teratozoospermia) except sirtuin signaling and mitochondrial dysfunction pathways. The five most important biological processes are identified in all semen phenotypes, but the only difference is the genes connected with initiation of RNA transcription in oligospermic and asthenospermic samples. CONCLUSION: Surprisingly, different types of ejaculates share many pathways and biological processes; sperm proteomics as a new global approach gives clues for the development of strategies to explain the reason for observed phenotypes of ejaculated spermatozoa, their possible effect on fertility, and for implementing research strategies in the context of infertility diagnosis and treatment.

RESUMO

Veterinary drugs are used for disease control in bovines Their presence at acceptable levels is a cause of concern for consumers and control agencies, as well as being a limitation for accessing international markets. The objective of this research was to evaluate the presence of residues of veterinary drugs and heavy metals in meat cuts from the Urabá region in Colombia. From a total of 80 samples of meat cuts from the loin and neck, we determined the presence of 29 veterinary drug residues and of cadmium and lead. The drug residues were quantified by liquid chromatography coupled to mass spectrometry. Heavy metals were measured by electrothermal atomic absorption. As a result, all evaluated samples complied with Colombian and European regulations for drug residues and heavy metals. These results demonstrate good veterinary practices used for bovines raised in this part of Colombia, and they represent an export opportunity and an opening for new markets that can be enhanced by the implementation of a meat quality seal from this region.

RESUMO

Agaves resist extreme heat and drought. In A. tequilana var. azul, the central spike of the rosette -containing the shoot apical meristem and folded leaves in early stages of development- is remarkably heat tolerant. We found that the most abundant protein in this organ is a 27 kDa protein. This protein was named mayahuelin to honor Mayáhuel, the agave goddess in the Aztec pantheon. LC-MS/MS analyses identified mayahuelin as a type I RIP (Ribosome Inactivating Protein). In addition to the spike, mayahuelin was expressed in the peduncle and in seeds, whereas in mature leaves, anthers, filaments, pistils, and tepals was absent. Anti-mayahuelin antibody raised against the A. tequilana var. azul protein revealed strong signals in spike leaves of A. angustifolia, A. bracteosa, A. rhodacantha, and A. vilmoriniana, and moderate signals in A. isthmensis, A. kerchovei, A. striata ssp. falcata, and A. titanota, indicating conservation at the protein level throughout the Agave genus. As in charybdin, a type I RIP characterized in Drimia maritima, mayahuelin from A. tequilana var. azul contains a natural aa substitution (Y76D) in one out of four aa comprising the active site. The RIP gene family in A. tequilana var. azul consists of at least 12 genes and Mayahuelin is the only member encoding active site substitutions. Unlike canonical plant RIPs, expression of Mayahuelin gene in S. cerevisiae did not compromise growth. The inhibitory activity of the purified protein on a wheat germ in vitro translation system was moderate. Mayahuelin orthologs from other Agave species displayed one of six alleles at Y76: (Y/Y, D/D, S/S, Y/D, Y/S, D/S) and proved to be useful markers for phylogenetic analysis. Homozygous alleles were more frequent in wild accessions whereas heterozygous alleles were more frequent in cultivars. Mayahuelin sequences from different wild populations of A. angustifolia and A. rhodacantha allowed the identification of accessions closely related to azul, manso, sigüín, mano larga, and bermejo varieties of A. tequilana and var. espadín of A. angustifolia. Four A. rhodacantha accessions and A. angustifolia var. espadín were closer relatives of A. tequilana var. azul than A. angustifolia wild accessions or other A. tequilana varieties.

Intestinal failure (FI) is defined as the decrease in intestinal function below the minimum necessary for the absorption of macronutrients and / or water and electrolytes, in such a way that intravenous supplementation (IVS) is required to maintain health and growth. From a functional point of view, it is classified into three types. FI type I: acute condition, of short duration and generally self-limited, FI type II: prolonged acute state, often in metabolically unstable patients, requiring multidisciplinary care and SIV for periods of a week or months, accompanied by septic, metabolic and nutrition and FI type III: chronic condition, in metabolically stable patients, who require SIV for months or years. Its management requires nutritional therapy and in selected cases autologous reconstruction surgery(AU)

Acute pancreatitis (AP) is defined as an acute inflammatory process of the pancreas which can compromise other organs and tissues. The diagnosis requires at least 2 of the following characteristics: moderate to severe abdominal pain, accompanied by nausea and vomiting; biochemical evidence of pancreatitis and/or imaging evidence through dynamic computed axial tomography (TACD) and/or magnetic resonance imaging (MRI) of the pancreas. It is the most common acute gastrointestinal disease that requires hospital admission, with a favorable evolution in most cases (80%). However, necrotizing pancreatitis can develop in up to 20% of patients and is associated with significant rates of early organ failure (38%). Metabolic disorders and fasting compromise the nutritional status which could aggravate the course of the disease, therefore the route of administration of nutritional therapy has been shown to have an impact on the evolution of patients. There is now a better definition of which AP patients need aggressive nutritional therapy and which are likely to benefit from early enteral nutrition(AU)

RESUMO

PURPOSE: The study's purpose was to assess population demographics and resource utilization of the Medical Student Run Clinic, which provides primary care services to patients in El Paso, Texas along the Texas-Mexico border. METHODS: A retrospective cross-sectional chart review was performed on 760 patients evaluated at the medical student-run clinic between 2013 and 2016. Data included demographic characteristics, chief complaints, diagnoses, and interventions, which were analyzed with calculations of means, standard deviations, and percentages. RESULTS: Most (79.7%) patients were female; average age was 38.43 years; 91% of patients were Hispanic, and 66.8% spoke Spanish. Average BMI was 30.9 kg/m2. Less than 1% of patients presented with a psychiatric complaint; however, 17.9% screened positive for anxiety, and 16.5% screened positive for depression. CONCLUSIONS: This study shows that diabetes, hypertension, obesity, anxiety, and depression represent avenues for future patient-centered interventions and provide insight into challenges patients face along the border.

RESUMO

AIM: The objective of this study is to explore the use of volumetric arc therapy (VMAT) to perform total marrow irradiation (TMI) and compare its results to the standard TBI technique in the Mexican public health system. BACKGROUND: The standard total body irradiation (TBI) technique is used with chemotherapy as a method of a pre-transplant conditioning of the bone marrow. In this technique, the whole body of the patient is considered to be PTV and irradiated generating toxicities and raising concerns about possible development of radio-induced tumors. MATERIALS AND METHODS: Through the use of simulation tomography of 12 patients previously treated with TBI, twelve different treatment plans were created with the proposed TMI technique and compared with the conventional protocol, the treatment plans were evaluated with a dose volume histogram analysis and quality assurance was evaluated with a portal dosimetry system using the gamma index criteria 3%/3 mm. RESULTS: Experimental results show an increasing dose to 99% of PTV of up to 41.1% by using TMI with the VMAT technique. The mean average dose to PTV was increased up to 19.3%. The use of the new TMI technique caused an improvement in the mean average dose to 99% of the PTV as well the homogeneity of the dose distribution prescribed at the PTV while leading to a better reproducibility of the treatment. The Qa of all the plans met the criterion of gamma index 3 mm-3%. CONCLUSION: The results analysis shows that the proposed TMI technique is feasible and applicable in the Mexican public health system.

Objective: To determine the effect of ethanolic extract of Chuquiraga spinosa (huamanpinta) on the metabolic syndrome and hypercholesterolemia in rats induced with fructose and cholesterol. Material and methods: SM was induced in rats with oral administration of 60% fructose for 90 days and the induction of hypercholesterolemia was by oral administration of cholesterol solution at a daily dose of 120 mg/kg suspended in gum tragacanth 2 %. Six groups of 15 animals were formed in each group, divided into negative white group (physiological saline solution) and positive (fructose plus cholesterol), ethanolic extract at doses of 50, 250 and 500 mg/kg and one atorvastatin plus enalapril group at 20 mg/Kg. The respective treatments were initiated on day 31 postinduction and continued induction along with treatments up to 90 days. The lipid profile, glycemia and blood pressure levels were determined. In the determination of chronic toxicity, hematological and biochemical evaluation of all the groups that were induced and treated were performed. Results: There was a decrease in total cholesterol, triglycerides, glucose, blood pressure and body weight, as well as a rise in HDL-cholesterol levels, which were statistically significant (p <0.05). In the determination of chronic toxicity, it was observed that there was no toxicity and no anatomopathological alterations were evidenced. Conclusions: The ethanolic extract of Chuquiraga spinosa has a positive effect on the metabolic syndrome and induced hypercholesterolemia in rats, without toxic effects.
