To gain insight into the functional relationship between the nucleocapsid (NC) domains of the Gag polyproteins of feline and simian immunodeficiency viruses, FIV and SIV, respectively, we generated two FIV Gag chimeric proteins containing different SIV NC and gag sequences. A chimeric FIV Gag protein (NC1) containing the SIV two zinc fingers motifs was incapable of assembling into virus-like particles. By contrast, another Gag chimera (NC2) differing from NC1 by the replacement of the C-terminal region of the FIV NC with SIV SP2 produced particles as efficiently as wild-type FIV Gag. Of note, when the chimeric NC2 Gag polyprotein was expressed in the context of the proviral DNA in feline CrFK cells, wild-type levels of virions were produced which encapsidated 50% of genomic RNA when compared to the wild-type virus.

This work investigated the safety of extracts obtained from plants growing in Colombia, which have previously shown UV-filter/antigenotoxic properties. The compounds in plant extracts obtained by the supercritical fluid (CO2) extraction method were identified using gas chromatography coupled to mass spectrometry (GC/MS) analysis. Cytotoxicity measured as cytotoxic concentration 50% (CC50) and genotoxicity of the plant extracts and some compounds were studied in human fibroblasts using the trypan blue exclusion assay and the Comet assay, respectively. The extracts from Pipper eriopodon and Salvia aratocensis species and the compound trans-ß-caryophyllene were clearly cytotoxic to human fibroblasts. Conversely, Achyrocline satureioides, Chromolaena pellia, and Lippia origanoides extracts were relatively less cytotoxic with CC50 values of 173, 184, and 89 µg/mL, respectively. The C. pellia and L. origanoides extracts produced some degree of DNA breaks at cytotoxic concentrations. The cytotoxicity of the studied compounds was as follows, with lower CC50 values representing the most cytotoxic compounds: resveratrol (91 µM) > pinocembrin (144 µM) > quercetin (222 µM) > titanium dioxide (704 µM). Quercetin was unique among the compounds assayed in being genotoxic to human fibroblasts. Our work indicates that phytochemicals can be cytotoxic and genotoxic, demonstrating the need to establish safe concentrations of these extracts for their potential use in cosmetics.

The community restrictions during the coronavirus disease 2019 pandemic adversely impacted older adults' physical activity levels. This convergent mixed-method study assessed the adaptation of the Recreovía, a community-based physical activity program in Bogotá, and characterized physical activity levels among older adult participants. Our results showed how the Recreovía adapted during the pandemic to continue promoting physical activity, through indoor and outdoor strategies, including virtual physical activity sessions and safety protocols. During this time, 72%-79% of the older adults attending the adapted program were physically active. A greater proportion of park users (84.2%) and more people involved in vigorous physical activity were observed during Recreovía days. Older adults had positive experiences and perceptions of the Recreovía program related to their health and social well-being. Even though the older adults prefer being outdoors, the adapted program allowed participants to continue with their physical activity routines as much as possible during the pandemic.

Introducción: las medidas auditivo-perceptuales subjetivas GRABS (Grade [grado de gravedad], Roughness [aspereza], Asthenia [astenia], Breathiness [soplosidad] y Strain [tensión] y CAPE-V (Consensus Auditory-Perceptual Evaluation of Voice) han sido el estándar de oro en evaluar la voz. La prominencia del pico cepstral suavizado (CPPS) se impone como la medida acústica objetiva más robusta para evaluar la gravedad de la disfonía. Se evaluó el análisis cepstral del programa PRAAT en pacientes con voz patológica y no patológica en hablantes de español colombiano y los valores estándar para el CPPS en lenguaje conectado (CPPSc), comparado con el análisis perceptual de la voz (GRBAS). Métodos: estudio observacional y analítico de prueba diagnóstica, de corte transversal y multicéntrico, con 120 individuos. Se grabó la voz y se analizó el CPPSc con el programa PRAAT. Se aplicó la valoración auditiva-perceptual por dos evaluadores expertos para la escala GRBAS. Resultados: el promedio de edad fue 43,3 años, 64,2 % mujeres. La media del CPPSc en el grupo disfónico fue de 6,40 dB y en el grupo de normofónicos fue 8,54 dB con p=0,000. Sensibilidad de 83,6% y especificidad de 96,2 % para la voz patológica en los pacientes con valores de CPPSc menores a 7,38 dB. Hubo alta correlación del valor de CPPSc con la gravedad de la disfonía. Conclusiones: el valor del CPPSc mediante el programa PRAAT es una medida eficiente por su alta validez, bajo costo y fácil aplicación para evaluar objetivamente la disfonía y su gravedad.


Introduction: Subjective auditory-perceptual measurements, GRABS (Grade, Roughness, Asthenia, Breathiness y Strain) and CAPE-V (Consensus Auditory Perceptual Evaluation of Voice) have been the gold standard in voice evaluation. Recently, the smoothed cepstral peak prominence (CPPS) has been imposed as the acoustic measurement, objective and more robust for dysphonia severity. Objectives: The cepstral analysis of the PRAAT program was evaluated in patients with a patho-logical and non-pathological voice in Colombian Spanish speakers and the standard values for the CPPS in connected language (CPPSc), compared with the perceptual analysis of the voice (GRBAS). Methods: An observational, analytical, cross-sec-tional, multicenter diagnostic test was performed with 120 individuals. The voice was recorded and the CPPSc was analyzed with the PRAAT program. The auditory-perceptual assessment was applied by two expert evaluators for the GRBAS scale. Results: The average age was 43.3 ± 18.1 years, 64.2% were women. The mean CPPSc in the dysphonic group was 6.40 dB, while in the group of normophonic indi-viduals it was 8.54 dB, with significant differences between both groups (p=0.000). Sensitivity of 83.6% and specificity of 96.2%; for pathological voice was established in patients with CPPSc values less than 7.38 dB. A high correlation was found in the CPPSc value with the severity of dysphonia. Conclusions: The value of CPPSc through the Praat program is an efficient measure, due to its high validity, low cost and easy application to objectively evaluate dysphonia and its severity.

Resumen Introducción : En los inicios de la pandemia por SARS-CoV-2 la insuficiencia respiratoria aguda ha sido la causa más importante de hospitalización inmediata en los pacientes con COVID-19 que acudían a los servicios de urgencias, siendo mayor la gravedad en pacientes con comorbilidades y factores de riesgo preexistentes; en estos escenarios la hipoxemia ha sido asociada a mortalidad. Nuestro objetivo fue identificar parámetros obtenidos de los gases en sangre arterial (GSA) asocia dos a mortalidad en pacientes con COVID-19 al ingreso hospitalario. Métodos : Se estudiaron retrospectivamente muestras de GSA obtenidos respirando aire ambiente (FiO2 21%) procesadas en el laboratorio clínico en un analizador ABL90 flex (Radiometer). Resultados : La alcalosis respiratoria aguda fue el dis turbio ácido base predominante. Considerando aquellos pacientes con insuficiencia respiratoria (paO2 < 60 mmHg) se observó hipoxemia "silenciosa" en 11/176 (6%) de los pacientes estudiados. En un análisis multivariado tres parámetros gasométricos al ingreso mostraron asocia ción positiva a mortalidad hospitalaria: paO2 (p = 0.053), índice paO2/pO2e (que expresa la paO2 ajustada a la paO2 esperada para la edad) (p = 0.047) y saturación fraccional de hemoglobina (OxiHb%) (p = 0.028). Discusión : Los GSA generan un aporte clave en la comprensión de la fisiopatología del paciente COVID-19; en la evaluación inicial, seguimiento y pronóstico de esta enfermedad.

Introducción: Las lesiones en la mucosa bucal constituyen un grupo heterogéneo de alteraciones, de cuyo diagnóstico temprano depende el pronóstico y el tratamiento. Objetivo: Caracterizar a pacientes con lesiones en la mucosa bucal según variables clínicas, epidemiológicas e histopatológicas. Métodos: Se realizó un estudio descriptivo, longitudinal y prospectivo de 23 pacientes con lesiones en la mucosa bucal pertenecientes a los consultorios médicos de la familia del reparto Vista Alegre, quienes fueron asistidos en la consulta de estomatología del Hospital General Docente Dr. Juan Bruno Zayas Alfonso de Santiago de Cuba desde septiembre del 2021 hasta junio del 2022. Resultados: En la serie predominaron el sexo femenino, el grupo etario de 60 y más años, la estomatitis subprótesis como lesión más frecuente (39,1 %) y los traumatismos crónicos como factores de riesgo (65,2 %); asimismo, en los pacientes remitidos al nivel secundario de atención hubo coincidencia entre los diagnósticos clínico e histopatológico (76,9 % ). Conclusiones: De la valoración clínica, epidemiológica e histopatológica de estas lesiones y de la preparación de los especialistas sobre el tema dependerá el diagnóstico definitivo.


Introduction: Lesions in the oral mucosa constitute a heterogeneous group of disorders which prognosis and treatment depend on the early diagnosis. Objective: To characterize patients with lesions in the oral mucosa according to clinical, epidemiological, histological and pathological variables. Methods: A descriptive, longitudinal and prospective study of 23 patients with lesions in the oral mucosa was carried out. They belonged to the family doctor offices from Vista Alegre neighborhood and were assisted in the Stomatology Service of Dr. Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba from September, 2021 to June, 2022. Results: In the series there was a prevalence of the female sex, the 60 years and over age group, subprosthesis stomatitis as more frequent lesion (39.1%) and chronic traumatisms as risk factors (65.2%); also, in patients referred to the secondary care level there was coincidence between the clinical, histological and pathological diagnoses (76.9%). Conclusions: The final diagnosis will depend on the clinical, epidemiological, histological and pathological valuation of these lesions and on the preparation of the specialists on the topic.

The kinematic assessment of the upper limbs in breast cancer (BC) survivors is one of the most common procedures to determine the recovery process after BC surgery. However, the methodology used is heterogeneous, finding various evaluation methods, which makes it difficult to compare results between studies. The objective of this review was to identify the technical features of the kinematic evaluation methods used in patients with mastectomy for BC. A literature review was conducted to search in electronic databases, such as PubMed, ScienceDirect, Clinical Key, Google Scholar, and Scopus. A total of 641 articles were obtained. After screening the title and the summary of the investigations, 20 manuscripts were kept for a deeper analysis. Different methodologies were found for the analysis of the kinematics of the upper limbs. Eight (40%) articles used the optoelectronic system, nine (45%) used the electromagnetic system, and three (15%) used other optoelectronic systems to assess shoulder kinematics. Each investigation studied different variables such as the type of surgery, the evaluation time, the age of the patients, the rehabilitation protocol, and so on. This makes the comparison among studies difficult, and the recovery process of the patients cannot be easily determined. In conclusion, the interpretation of the movement of the upper limbs should be easy to understand for oncologists, physiotherapists, clinicians, and researchers.

Throughout the entire coronavirus disease 19 (COVID-19) pandemic, there were disruptions in the supply chain of test materials around the world, primarily in poor- and middle-income countries. The use of 3D prints is an alternative to address swab supply shortages. In this study, the feasibility of the clinical use of 3D-printed swabs for oropharyngeal and nasopharyngeal sampling for the detection of SARS-CoV-2 infection was evaluated. For that purpose, paired samples with the 3D printed and the control swabs were taken from 42 adult patients and 10 pediatric patients, and the results obtained in the detection of SARS-CoV-2 by reverse transcription and quantitative polymerase chain reaction (RT-qPCR) were compared. Additionally, in those cases where the result was positive for SARS-CoV-2, the viral load was calculated by means of a mathematical algorithm proposed by us. For both adults and children, satisfactory results were obtained in the detection of SARS-CoV-2 by RT-qPCR; no significant differences were found in the quantification cycle values between the 3D-printed swab samples and the control samples. Furthermore, we corroborated that the 3D-printed swabs caused less discomfort and pain at the time of sampling. In conclusion, this study shows the feasibility of routinely using 3D-printed swabs for both adults and children. In this way, it is possible to maintain local and cheaper consumption along with fewer distribution difficulties.

Industrial, agricultural, and urban areas can be sources of pollution and a cause of habitat fragmentation. The Conlara River located in the northeast of San Luis Province suffers different environmental pressures along its course from urban to agro-industrial areas. The present study aims to assess the water quality of the Conlara basin by evaluating how metals and pesticide contamination as well as physicochemical parameters relate to physiological stress in Jenynsia multidentata. Samplings were carried out in four sites characterized by a growing gradient of anthropic impact from the springs to the final sections of the river, starting with tourism passing through urban areas and ending with large agricultural areas (from S1 to S4) during both the dry and wet seasons. A total of 27 parameters were determined (11 physicochemical, 9 heavy metals, and 7 pesticides) in surface waters. Biomarkers (CAT, TBARS, ChE, and MN) showed significant physiological and cytological alterations in J. multidentata depending on the hydrology season. The combination of physicochemical parameters, metals, and pesticide levels allowed typification and differentiation of the sites. Some metal (Cr, Mn, Pb, and Zn) and pesticide (α-BHC, chlorpyrifos, permethrin and cypermethrin, and endosulfan α) levels recorded exceeded the recommended Argentinian legislation values. A principal component analysis (PCA) allowed detection of differences between both seasons and across sites. Furthermore, the differences in distances showed by PCA between the sites were due to differences in the presence of physicochemical parameters, metals, and pesticides correlated with several biomarkers' responses depending on type of environmental stressor. Water quality evaluation along the Conlara River shows deterioration and different types of environmental stressors, identifying zones, and specific sources of pollution. Furthermore, the biomarkers suggest that the native species could be sensitive to anthropogenic environmental pressures.

BACKGROUND: The aim of this study was to explore the associations between the 10 key indicators of the Global Matrix 4.0 project and human development index (HDI) at a national level according to sex, age, area of residence, and ability levels. METHODS: Information from the 57 countries/localities included in the Global Matrix 4.0 project was compiled and presented according to the HDI of each country/locality for each of the 10 key indicators. Grades were assigned based on the benchmarks of the Global Matrix 4.0 project ranged between "A+" (best performance) and "F" (worst performance). RESULTS: The population subgroups of females, children, rural residents, with/without disabilities from countries/localities with higher HDI performed better in the organized sport and physical activity indicator than their peers from countries/localities with lower HDI. Children and adolescents living in rural areas of countries/localities with higher HDI showed better performance for active play, and children and adolescents living in urban areas of countries/localities with lower HDI showed better performance for the active transportation. Countries/localities with higher HDI showed better grades for sources of influence than the countries/localities with lower HDI. CONCLUSIONS: Physical activity patterns in some population subgroups of children and adolescents differed according to the development level of countries/localities.

Plants are sources of sunscreen ingredients that prevent cellular mutations involved in skin cancer and aging. This study investigated the sunscreen properties of the extracts from some ornamental plants growing in Colombia. The UV filter capability of the flower extracts obtained from Rosa centifolia L., Posoqueria latifolia (Rudge) Schult, and Ipomoea horsfalliae Hook. was examined. Photoprotection efficacies were evaluated using in vitro indices such as sun protection factor and critical wavelength. UVB antigenotoxicity estimates measured with the SOS Chromotest were also obtained. Extract cytotoxicity and genotoxicity were studied in human fibroblasts using the trypan blue exclusion and Comet assays, respectively. Major compounds of the promising flower extracts were identified by UHPLC-ESI+-Orbitrap-MS. The studied extracts showed high photoprotection efficacy and antigenotoxicity against UVB radiation, but only the P. latifolia extract showed broad-spectrum photoprotection at non-cytotoxic concentrations. The P. latifolia extract appeared to be safer for human fibroblast cells and the R. centifolia extract was shown to be moderately cytotoxic and genotoxic at the highest assayed concentrations. The I. horsfalliae extract was unequivocally cytotoxic and genotoxic. The major constituents of the promising extracts were as follows: chlorogenic acid, ecdysterone 20E, rhamnetin-rutinoside, cis-resveratrol-diglucoside, trans-resveratrol-diglucoside in P. latifolia; quercetin, quercetin-glucoside, quercetin-3-rhamnoside, kaempferol, kaempferol-3-glucoside, and kaempferol-rhamnoside in R. centifolia. The potential of the ornamental plants as sources of sunscreen ingredients was discussed.

Polychlorinated dibenzo-p-dioxins (PCDDs) are known to be a group of compounds of high toxicity for animals and, particularly, for humans. Given that the most common method to destroy these compounds is by high-temperature combustion, finding other routes to render them less toxic is of paramount importance. Taking advantage of the physisorption properties of nanotubes, we studied the reactions of atomic hydrogen on physisorbed PCDDs using DFT; likewise, we investigated the reaction of molecular hydrogen on PCDDs aided by Ni10 and Ni13 clusters adsorbed on single-wall carbon nanotubes. Because dihydrogen is an easily accessible reactant, we found these reactions to be quite relevant as dehydrohalogenation methods to address PCDD toxicity.

Two series of novel amphiphilic compounds were synthesized based on carbamates and ureas structures, using a modification of the synthesis methods reported by bibliography. The compounds were tested for organic solvent removal in a model wastewater. The lipophilic group of all compounds was a hexadecyl chain, while the hydrophilic substituent was changed with the same modifications in both series. The structures were confirmed by FT-IR, NMR, molecular dynamic simulation and HR-MS and their ability to gel organic solvents were compared. The SEM images showed the ureas had a greater ability to gel organic solvents than the carbamates and formed robust supramolecular networks, with surfaces of highly interwoven fibrillar spheres. The carbamates produced corrugated and smooth surfaces. The determination of the minimum gelation concentration demonstrated that a smaller quantity of the ureas (compared to the carbamates, measured as the weight percentage) was required to gel each solvent. This advantage of the ureas was attributed to their additional N-H bond, which is the only structural difference between the two types of compounds, and their structures were corroborated by molecular dynamic simulation. The formation of weak gels was demonstrated by rheological characterization, and they demonstrated to be good candidates for the removal organic solvents.

Currently, the design of highly efficient materials for photochemical applications remains a challenge. In this study, an efficient semiconductor was prepared, based on a coordination complex (Cu-TTC) of Cu(I) and trithiocyanuric acid on ZnTiO3/TiO2 (ZTO/TO). The Cu-TTC/ZTO/TO composite was prepared by the solvothermal method at room temperature. The structural, optical, and electrochemical characteristics, as well as the photocatalytic performance of the composite, were experimentally and computationally studied. The results show that the Cu-TTC/ZTO/TO composite efficiently extended its photoresponse in the visible region of the electromagnetic spectrum. The electrochemistry of the proposed tautomeric architecture (s-Cu-TTC) clearly reveals the presence of metal-ligand charge-transfer (MLCT) and π → π* excitations. The maximum methylene blue (MB) dye photodegradation efficiency of 95% in aqueous solutions was achieved under the illumination of simulated solar light. Finally, computational calculations based on the Density Functional Theory (DFT) method were performed to determine the electronic properties of the s-Cu-TTC tautomeric structure and clarify the adsorption mechanism of this complex on the surface (101) of both ZnTiO3 and TiO2 oxides. The results obtained allow us to suggest that the Cu-TTC complex is an effective charge carrier and that the Cu-TTC/ZTO/TO composite can be used efficiently for photochemical applications.

After mastectomy, women might lose mobility and develop kinematic changes in the shoulder. The objective of this research was to compare the kinematics of the glenohumeral joint in women, before and after unilateral breast cancer surgery. This was a longitudinal study with a pre- and post-evaluation design; in total, 15 Mexican women who had a mastectomy for breast cancer and who received a physical therapy program after surgery were evaluated. Flexion-extension and abduction-adduction movements of the glenohumeral joint were evaluated (15 days before and 60 days after mastectomy). For the kinematic analysis of the glenohumeral joint, an optoelectronic motion capture system was used to monitor 41 reflective markers located in anatomical landmarks. There was no significant difference in the range of motion of the glenohumeral joint when comparing pre- and post-mastectomy, flexion-extension (p = 0.138), and abduction-adduction (p = 0.058). Furthermore, patients who received chemotherapy (53%) before mastectomy were more affected (lower range of motion) than those who did not receive it. There were no significant differences in the kinematics of the glenohumeral joint after mastectomy in this group of patients who received a physical therapy program after surgery. Moreover, patients who received chemotherapy treatment before breast cancer surgery tended to have a lower range of motion than those who did not receive it. Therefore, it is necessary for the physical rehabilitation team to attend to these patients even before the mastectomy.

ABSTRACT After breast cancer surgery, women might develop musculoskeletal impairments that affect movements of the upper limbs and reduce the quality of life. The objective of this research was to analyze the effect of the unilateral breast cancer surgery on the kinematics of the shoulder and the electrical activity of the upper trapezius, middle deltoid and pectoralis major muscles. Eight right-handed female participants, mean age 46.5 ± 5.45 years and mean body mass 71.21 ± 13.33 kg, with unilateral breast cancer surgery, without breast reconstruction and without lymphedema symptoms were included in the research. Flexion-extension and abduction-adduction movements of the shoulder were evaluated with infrared cameras and the electrical activity was measured using surface electromyography. The statistical analysis of the direction angles showed a significant reduction of the flexion-extension and abduction-adduction movements in the affected side in most of the participants (p<0.05). The muscle electrical activity did not present a significant difference between the two sides for the flexion-extension and abduction-adduction movements (p>0.05). The results suggest that the surgical procedure could compromise the range of motion of the affected side. Furthermore, this research contributes to clarify the effect of the surgical procedure in the range of motion of the upper limbs.


RESUMEN Después de la cirugía de cáncer de mama, las mujeres pueden desarrollar deficiencias musculoesqueléticas que afectan los movimientos de las extremidades superiores y reducen la calidad de vida. El objetivo de esta investigación fue analizar el efecto de la cirugía unilateral del cáncer de mama sobre la cinemática del hombro y la actividad eléctrica de los músculos trapecio superior, deltoides medio y pectoral mayor. Se incluyeron ocho participantes diestras, edad promedio 46.5 ± 5.45 años y masa promedio 71.21 ± 13.33 kg, con cirugía de cáncer de mama unilateral, sin reconstrucción mamaria y sin síntomas de linfedema. Los movimientos de flexión-extensión y abducción-aducción del hombro se evaluaron con cámaras infrarrojas y se midió la actividad eléctrica mediante electromiografía de superficie. El análisis estadístico de los ángulos de dirección mostró una reducción significativa del movimiento de flexión-extensión y abducción-aducción en el lado afectado en la mayoría de las participantes (p<0.05). La actividad eléctrica muscular no presentó diferencia significativa entre los dos lados para los movimientos de flexión-extensión y abducción-aducción (p>0.05). Los resultados sugieren que el procedimiento quirúrgico podría comprometer el rango de movimiento del lado afectado. Además, esta investigación contribuye a esclarecer el efecto del procedimiento quirúrgico en el rango de movimiento de los miembros superiores.

Objectives: Excessive recreational screen time (RST) is associated with detrimental effects for physical, psychological and cognitive development. This article aims to describe the prevalence of excessive RST among Colombian preschoolers, children and adolescents and explore its factors associated. Methods: We analyzed data from the National Survey of Nutrition 2015. The sample included 4,503 preschoolers, 5,333 school-aged children and 6,623 adolescents. Poisson regression models with robust variance were conducted to estimate prevalence ratios and determine associated factors of excessive RST. Results: Fifty percent of preschoolers, 61% of school-aged children and 73% of adolescents in Colombia had excessive RST. Positive associations were observed with the availability of TV in the child's bedroom, the availability of video games at home, and eating while using screens. A negative association with rural area was observed for all age groups. Conclusion: The majority of Colombian children and adolescents have excessive RST. Younger preschoolers, older school-aged children, wealthiest children and those from urban areas should be targeted by interventions to decrease RST. These interventions should promote limiting the availability of electronic devices in children's bedrooms and not eating in front of screens.

RESUMEN Introducción: El volumen sistólico (VS) del ventrículo izquierdo (VI) es eyectado por una combinación de acortamiento longitudinal (AL), circunferencial (AC) y giro ventricular. El AL se produce por el movimiento del anillo mitral hacia el ápex, lo que ocasiona engrosamiento de la pared (AC y radial), reducción de la cavidad ventricular y eyección del VS. No está definido el rol del AL en la estenosis aórtica (EAo). Objetivo: Analizar la contribución del AL y AC al VS y la fracción de eyección ventricular izquierda (FEVI) en la EAo grave. Materiales y métodos: Se estudiaron 152 pacientes (70 ± 12 años, 64 mujeres) con EAo grave (área valvular <1 cm2) con ecoDoppler cardíaco. La contribución del AL al VS se evalúo considerando a la base del corazón como un cilindro., con volumen = base × altura; la base, un círculo con radio igual a (espesor septal en sístole + espesor pared posterior en sístole + diámetro de fin de sístole)/2; y la altura, el promedio de la excursión sistólica del anillo lateral, septal, anterior e inferior. La contribución del AC al VS se estimó como: VS - contribución del AL al VS. Ambas contribuciones se expresaron en forma absoluta y como porcentajes del VS (% VS-AL y % VS-AC). Resultados: El AL contribuyó con 2 tercios aproximadamente al VS (68 ± 18 %) y el AC con el resto. El VS-AL correlacionó inversamente con el VS (r = - 0,45 p <0,001) y el VS-AC tuvo correlación directa (r = 0,45 p <0,001). La contribución del % VS-AL fue mayor en los pacientes con FEVI <50%. El % VS-AL correlacionó inversamente con el EPR (r = - 0,32 , p <0,01). Conclusión: La contribución del AL al VS es mayor que la del AC, y correlaciona inversamente con el VS. Es mayor el % VS-AL en los pacientes con FEVI <50%. Los hallazgos mencionados podrían tener implicancias al considerar puntos de corte para los índices de función longitudinal (strain) sin considerar la FEVI y/o geometría ventricular.


ABSTRACT Background: Left ventricular (LV) stroke volume (SV) is ejected by a combination of longitudinal shortening (LS), circumferential shortening (CS) and ventricular twist. Longitudinal shortening is caused by the motion of the mitral annulus towards the apex, causing wall thickening (CS and radial thickening), reduction of the ventricular cavity and LV ejection. The role of LS in aortic stenosis (AS) is not defined. Objective: The aim of this study was to analyze the contribution of LS and CS to SV and left ventricular ejection fraction (LVEF) in severe AS. Methods: The study included 152 patients (70 ± 12 years, 64 women) with severe AS (valve area <1 cm2) studied by cardiac Doppler echocardiography. The contribution of LS to SV was evaluated by considering the base of the heart as a cylinder, with volume = base x height; the base was assumed as a circle with radius equal to (systolic septal thickness + systolic posterior wall thickness + end-systolic diameter)/2; and height, as an average of the mitral lateral, septal, anterior and inferior annulus systolic excursion. The contribution of CS to SV was estimated as: SV - contribution of LS to SV. Both contributions were expressed in absolute form and as SV percentages (%SV-LS and %SV-CS). Results: Longitudinal shortening contributed with approximately 2 thirds of SV (68 ± 18 %) and CS with the rest. SV-LS correlated inversely with SV (r = - 0.45 p <0.001) and SV-CS had direct correlation (r = 0.45 p <0.001). The contribution of %SV-LS was greater in patients with LVEF <50%. Percent SV-LS correlated inversely with relative wall thickness (RWT) (r = 0.32, p <0.01). Conclusion: The contribution of LS to SV is greater than that of CS, and correlates inversely with SV. Percent SV-LS is higher in patients with EF <50%. The aforementioned findings could have implications when considering cut-off points for longitudinal function indices (strain) without considering LVEF and/or ventricular geometry.

Introduction: In patients with severe aortic stenosis, low-gradient, low-flow and preserved ejection fraction (≥ 50%) (LG/LFp) the decrease in stroke volume index (Svi) could be due to diminished contractility. The aim of this study was to analyze if low Svi in patients with LG/LFp is related to a decrease in contractility. Methods: Thirty patients with LF/LGp and 63 normal subjects were studied with Doppler echocardiography. The level of contractility (LC) was assessed by mean of midwall shortening fraction (mSF) ­ end-systolic stress (ESS) relationship. Relative wall thickness (RWT) and aortic valve area (AVA) were calculated. Results: LC was diminished in 40% of LF/LGp patients. By bivariate analysis LC correlated with RWT and mSF. Svi correlated with AVA but not with LC. In multiple linear regression RWT and mSF were predictors independents of LC. About Svi, AVA was the only predictor independent variable. Conclusion: Decreased contractility was present in 40% of LF/LGp patients despite preserved ejection fraction. LC had correlation with mSF and EPR. Low Svi appears to be related to aortic stenosis severity rather than low contractility.


Introducción: En la estenosis aórtica grave con gradiente bajo, fracción de expulsión preservada (≥ 50%) y flujo bajo (GB/FBp), la disminución del índice de volumen sistólico (IVS) podría deberse a una reducción de la contractilidad. El objetivo de este estudio fue analizar si la disminución del IVS se relaciona con la de la contractilidad. Métodos: Se estudió a 30 pacientes con GB/FBp y 63 individuos normales con Doppler cardíaco. El nivel de contractilidad (NC) se valoró mediante la relación entre la fracción de acortamiento mesoparietal (FAm) y el estrés de fin de sístole (EFS). Se calculó el espesor parietal relativo (EPR) y el área valvular aórtica (AVA). Resultados: El NC estuvo disminuido en el 40% de los pacientes con GB/FBp. En el análisis bivariado, el NC se correlacionó con el EPR y la FAm, pero no con el IVS, el cual mantuvo correlación con el AVA. En el análisis de regresión lineal múltiple, el EPR y la FAm fueron variables predictoras independientes del NC y el AVA del IVS. Conclusión: La contractilidad estuvo deprimida en el 40% de los pacientes con GB/FBp a pesar de tener fracción de expulsión preservada. El NC se correlacionó con el acortamiento circunferencial (FAm) y la geometría ventricular (EPR). La disminución del IVS que caracteriza a los pacientes con GB/FBp parece vincularse más con la gravedad de la estenosis aórtica y menos con la disminución de la contractilidad.
