RESUMO

This study aimed to produces and characterize bovine hydroxyapatite (HA) bioceramic with 3Y-TZP addition and analyze different sintering curves. HA was extracted from bovine bones and nanoparticulated. HA discs (0, 1, 5 and 10 wt% 3Y-TZP) were subjected to uniaxial and isostatic pressing. Dilatometry analysis was performed and the groups were sintered using 3 different firing curves (conventional, 1300 °C; 2-step, 1292 °C; 2-step, 1420 °C). The samples were analyzed by X-ray diffraction (XRD), biaxial flexural strength (BFS), Vickers microhardness (VH) and Field emission scanning electron microscopy (FE-SEM). The dilatometry results signaled the need for sintering optimization in groups added with 3Y-TZP. XRD demonstrated the characteristic crystallographic peaks of HA in the pure groups and with 1% 3Y-TZP, and decomposition of HA into ß-TCP and formation of calcium zirconate in the groups with 5 and 10% 3Y-TZP. Considering each composition, the groups of pure HA (131.3 ± 13.5 MPa; 401 ± 12.7 GPa) sintered by the conventional curve and HA+1%3Y-TZP (145 ± 8.6 MPa; 507 ± 47.9 GPa), HA+5%3Y-TZP (68.1 ± 14.2 MPa; 183 ± 9.8 GPa) and HA+10%3Y-TZP (55.6 ± 5.1 MPa; 96.1 ± 7.64 GPa) sintered by the 2-step curve at 1420 °C, combined the best BFS and VH results. The addition of 1 wt% 3Y-TZP and optimization in the sintering process improved the mechanical and microstructural properties of HA bioceramics and maintenance of its crystalline characteristics. Refinement in material processing is necessary for the future use of this bioceramic in dentistry.

OBJECTIVES: This study evaluated the effect of TiO2 nanoparticles + dense hydroxyapatite (HA) on human osteoblast cells (SAOS-2). METHODS: Particulate bovine HA powder with or without the addition of either 5 or 8 % TiO2 (HA, HA/TiO2Np5 % or HA/TiO2Np8 %) were pressed into disks (Ø = 12.5 mm; thickness = 1.3 mm) uniaxially (100 MPa) and isostatically (200 MPa/1 min) and sintered at 1300 °C. Y-TZP disks were used as control. The following tests were performed: Scanning Electron Microscopy and Dispersive Energy Spectroscopy (SEM/EDS), Atomic Force Microscopy (AFM), cell viability assay (Alamar Blue-AB) and mineralized matrix deposition (Alizarin Red-AR). AB and AR data were submitted to 2-way ANOVA/Tukey tests and ANOVA/Tukey tests, respectively. RESULTS: SEM revealed that the surface of HA/TiO2Np5% resembles DPBHA surface, but also contains smaller granules. HA/TiO2Np8% characteristics resembles HA/TiO2Np5% surface, but with irregular topography. Y-TZP showed a typical oxide ceramic surface pattern. EDS revealed Ca, O, and P in all samples. C, O, and Zr appeared in Y-TZP samples. AFM data corroborates SEM analysis. AB test revealed excellent cellular viability for HA/TiO2Np5% group. AR test showed that all groups containing TiO2np had more mineralized matrix deposition than all other groups, with statistically differences between HA/TiO2Np8% and HA cultivated in non-osteogenic medium. Culture in osteogenic medium exhibited much more mineralized matrix deposition by TiO2np groups. SIGNIFICANCE: In conclusion, the addition of TiO2np showed chemical, superficial, and biological changes in the reinforced materials. HA/TiO2Np5% showed the best results for cell viability and HA/TiO2Np8% for mineralized matrix deposition.

OBJECTIVES: To evaluate the marginal adaptation and internal space of crowns produced by 4 CAD/CAM systems using microcomputed tomography (µCT) and replica technique (RT). MATERIALS AND METHODS: Monolithic lithium disilicate crowns were milled (Ceramill, Cerec, EDG, and Zirkonzahn) (n = 10). The cement film obtained with low viscosity silicone was scanned by the µCT system and captured by a stereomicroscope, according to RT. Two-way ANOVA followed by Tukey's test were used for statistical analysis (α = 0.05). A uniformity index (UI) was idealized to describe the distribution of crowns' internal space and submitted to the Kruskal-Wallis and Tukey's test (α = 0.05). The correlation between µCT and RT was performed by Pearson's Correlation Coeficient (α = 0.05). RESULTS: Marginal adaptation and internal space were statistically significant different between the experimental groups for the µCT and RT (p < 0.05). The medians of the 4 systems tested were within clinically acceptable range and the mean (± SD) highest marginal discrepancy was recorded in the Ceramill group at 133.0 ± 71.5 µm (µCT) and 90.6 ± 38.5 µm (RT). For internal fit, the UI disclosed a better distribution of the internal space for the Zirkonzahn group (p < 0.001). There was a strong correlation between the methods (p = 0.01 and r = 0.69). CLINICAL RELEVANCE: Because of the variability of the CAD/CAM systems available, evaluating their accuracy is of clinical interest. The 4 systems are capable to produce restorations adaptated within clinically appropriate levels. The µCT and RT are efficient adaptation methodologies.

PURPOSE: To evaluate the fatigue survival, fracture loads and failure modes of monolithic lithium disilicate screw-retained crowns, attached to titanium insert, and cement-retained crowns. MATERIALS AND METHODS: Internal tapered connection implants, embedded in acrylic resin at 30° inclination, were restored with lithium disilicate restorations, simulating a maxillary premolar (n = 20), with different designs: screw-retained titanium base abutment-crowns, and cement-retained crowns. The specimens were submitted to cyclic mechanical loading (1.2 × 106 cycles with a load of 0-250 N at 2 Hz). Surviving specimens were subjected to single load to fracture in a universal testing machine and failure modes were determined with the aid of an optical microscope. Maximum load values were analyzed statistically using the t-test and differences in failure modes were analyzed using the chi-squared test (α = 0.05). RESULTS: All specimens survived the cyclic mechanical loading. Fracture load was significantly higher for screw-retained crowns (821.69 ±196.71 N) than the cement-retained crowns (577.03 ± 137.75 N) (p = 0.005). Ceramic failure was the predominant mode, with no statistical difference between groups. CONCLUSIONS: Screw-retained and cement-retained lithium disilicate crowns survived the cyclic mechanical loading. The use of titanium inserts to support a monolithic restoration enhances the fracture strength of the crown/abutment system.

This study aimed to produce a new SiO2+Y-TZP ceramic via uniaxial/isostatic compression that was structurally and chemically characterized relating to its translucency and flexural strength. SiO2 and Y-TZP were mixed using a ball mill, pressed and sintered at 1150 °C. The optical and mechanical properties of the specimens were compared to lithium disilicate (LD) and zirconia-reinforced lithium silicate (ZLS) (Kruskal-Wallis, α = 0.05). The Fourier Transform Infrared Spectroscopy bands suggested an interaction between Si, O and Zr. Contrast ratio and translucency parameter of the experimental ceramic were higher and lower, (p = 0.000001) respectively, than those of the LD and ZLS. The experimental ceramic presented similar flexural strength to ZLS, but lower than LD (p < 0.0001). It can be concluded that this processing method is efficient to obtain a SiO2+Y-TZP ceramic and 1150 °C crystallizes SiO2 without inducing t-m transformation. The SiO2+Y-TZP ceramic presented lower translucency and higher masking ability than the commercially available glass-ceramics, but similar flexural strength to one glass-ceramics.

AIM: The aim of this meta analysis was to evaluate the influence of the processing method on the marginal and internal gaps of lithium disilicate inlays/onlays. SETTINGS AND DESIGN: A systematic literature review was conducted using the PubMed/Medline, Embase, Scopus, and Cochrane Library databases. This review was registered on the PROSPERO platform. MATERIALS AND METHODS: The studies were selected according to the marginal and internal gaps of two different fabrication methods for lithium disilicate (milled and pressed). STATISTICAL ANALYSIS USED: The meta analysis was performed based on the Mantel-Haenszel and inverse variance methods, using the random effects model and a 95% confidence interval. RESULTS: From all databases, 127 studies were identified. Four in vitro studies were included in the qualitative analysis and three in the meta analysis. Moreover, 197 restorations were evaluated (103 pressed and 94 milled). During the evaluation of only the internal gap, there was a statistically significant difference favoring the pressed technique (P = 0.002). There was no statistically significant difference in the analyses of the marginal gap (P = 0.530) and the total gap (P = 0.450). CONCLUSION: Both the techniques provided acceptable marginal and total gaps, although the pressed technique revealed a more favorable internal adaptation than the milled onlays/inlays.

Minimally invasive approach lead composite resin veneers restorations as a conservative and alternative treatment option, with the aim to improve the aesthetics anterior dentition. Clinically, this treatment reducing the clinical time and has a lower cost than indirect technique. In addition, new resin composite was developed to support the clinician to make more natural restorations. The present case report aim to evaluated the clinical performance of direct composite veneer technique was restored with 100% supra-nano spherical filler composite. A 39-year-old female, searched for restorative treatment of anterior teeth, which were unaesthetically altered. In the clinical examination, it was observed color alterations in maxillary left and right incisors and canines due to use of medicines, and bruxism. The treatment plan included an in-office dental bleaching, removal of inadequate anterior restorations and confection of composite veneers restorations. The dental bleaching was performed with hydrogen peroxide 35% activated by LED/laser in two sessions. Those restorations were performed using supra-nano spherical resin-based composite, color A1 for dentin and enamel, with direct composite laminate veneer technique. After six-month follow-up, the restorations presented good clinical aspect, absence of biofilm accumulation and abrasive wear. Therefore, direct veneer restorations may be alternative treatment for anterior teeth esthetic problems, suggesting a promising performance of these new composites(AU)


A abordagem minimamente invasiva torna as facetas de resina composta conservadoras e opção de tratamento alternativo para melhorar a estética da dentição anterior. Clinicamente, esse tratamento reduz o tempo operatório e tem um custo menor que a técnica indireta. Além disso, uma nova resina composta foi desenvolvida para ajudar o clínico a fazer restaurações mais naturais. O presente relato de caso tem como objetivo avaliar o desempenho clínico da técnica direta de faceta de resina composta de preenchimento 100% supra-nano-esférico. Paciente de 39 anos, sexo feminino procurou o tratamento restaurador dos dentes anteriores, que foram alterados inesteticamente. No exame clínico foram observadas alterações de cor nos incisivos superiores, esquerdo e direito, e caninos devido ao uso de medicamentos e bruxismo. O plano de tratamento incluiu clareamento dentário em consultório, remoção de restaurações anteriores inadequadas e confecção de facetas de resina composta. O clareamento dental foi realizado com peróxido de hidrogênio a 35% ativado por LED / laser em duas sessões. Essas restaurações foram realizadas utilizando resina composta supra-nano-esférica, cor A1 para dentina e esmalte, com técnica direta de laminado composto. Após seis meses de acompanhamento as restaurações apresentaram bom aspecto clínico, ausência de acúmulo de biofilme e desgaste abrasivo. Portanto, facetas diretas de resina composta podem ser um tratamento alternativo para problemas estéticos de dentes anteriores, sugerindo um desempenho promissor desses novos compósitos(AU)


El abordaje mínimamente invasivo hace que las facetas de resina compuesta conservadoras y opción de tratamiento alternativo para mejorar la dentición anterior estética. Clínicamente, este tratamiento reduce el tiempo clínico y tiene un costo menor que la técnica indirecta. Además, un nuevo compuesto de resina fue desarrollado para ayudar al clínico a hacer restauraciones más naturales. El presente relato de caso tiene como objetivo evaluar el desempeño clínico de la técnica directa de faceta de resina compuesta de relleno 100% supra-nano-esférico. Paciente de 39 años, sexo femenino buscó el tratamiento restaurador de los dientes anteriores, que fueron alterados intemporal. En el examen clínico se observaron alteraciones de color en los incisivos superiores, izquierdo y derecho, y caninos debido al uso de medicamentos y bruxismo. El plan de tratamiento incluyó blanqueamiento dental en consultorio, remoción de restauraciones anteriores inadecuadas y confección de facetas de resina compuesta. El blanqueamiento dental fue realizado con peróxido de hidrógeno al 35% activado por LED / láser en dos sesiones. Estas restauraciones se realizaron utilizando resina compuesta supra-nano esférica, color A1 para dentina y esmalte, con técnica directa de laminado compuesto. Después de seis meses de seguimiento las restauraciones presentaron buen aspecto clínico, ausencia de acumulación de placa dental y desgaste abrasivo. Por lo tanto, las facetas directas de resina compuesta pueden ser un tratamiento alternativo para problemas estéticos de dientes anteriores, sugiriendo un desempeño prometedor de estos nuevos compuestos(AU)

This in vitro study evaluated the impact of initial erosion on the susceptibility of enamel to further erosive challenge. Thirty bovine enamel blocks were selected by surface hardness and randomized into two groups (n = 15): GC- group composed by enamel blocks without erosion lesion and GT- group composed by enamel blocks with initial erosion lesion. The baseline profile of each block was determined using the profilometer. The initial erosion was produced by immersing the blocks into HCl 0.01 M, pH 2.3 for 30 seconds, under stirring. The erosive cycling consisted of blocks immersion in hydrochloric acid (0.01 M, pH 2.3) for 2 minutes, followed by immersion in artificial saliva for 120 minutes. This procedure was repeated 4 times a day for 5 days, and the blocks were kept in artificial saliva overnight. After erosive cycling, final profile measurement was performed. Profilometry measured the enamel loss by the superposition of initial and final profiles. Data were analyzed by t-test (p<0.05). The result showed no statistically significant difference between groups (GS = 14.60±2.86 and GE = .14.69±2.21 µm). The presence of initial erosion on bovine dental enamel does not enhance its susceptibility to new erosive challenges.
