Acanthamoeba genus can affect humans with diseases such as granulomatous amebic encephalitis (GAE), a highly lethal neuroinfection. Several aspects of the disease still need to be elucidated. Animal models of GAE have advanced our knowledge of the disease. This work tested Wistar rats (Rattus norvegicus albinus) as an animal model of GAE. For this, 32 animals were infected with 1 × 106A. castellanii trophozoites of the T4 genotype. Ameba recovery tests were carried out using agar plates, vascular extravasation assays, behavioral tests, and histopathological technique with H/E staining. Data were subjected to linear regression analysis, one-way ANOVA, and Tukey's test, performed in the GraphPad Prism® 8.0 program, with a significance level of p < 0.05. The results revealed the efficiency of the model. Amebae were recovered from the liver, lungs, and brain of infected animals, and there were significant encephalic vascular extravasations and behavioral changes in these animals, but not in the control animals. However, not all infected animals showed positive histopathology for the analyzed organs. Nervous tissues were the least affected, demonstrating the role of the BBB in the defense of the CNS. Supported by the demonstrated evidence, we confirm the difficulties and the feasibilities of using rats as an animal model of GAE.

Mycoplasma spp. are wall-less bacteria able to infect mammals and are classified as hemotropic (hemoplasma) and nonhemotropic. In aquatic mammals, hemoplasma have been reported in California sea lions (Zalophus californianus) and river dolphins (Inia spp.). We investigated Mycoplasma spp. in blood samples of West Indian manatees (Trichechus manatus), pinnipeds (5 species), and marine cetaceans (18 species) that stranded or were undergoing rehabilitation in Brazil during 2002-2022. We detected Mycoplasma in blood of 18/130 (14.8%) cetaceans and 3/18 (16.6%) pinnipeds. All tested manatees were PCR-negative for Mycoplasma. Our findings indicate that >2 different hemoplasma species are circulating in cetaceans. The sequences from pinnipeds were similar to previously described sequences. We also detected a nonhemotropic Mycoplasma in 2 Franciscana dolphins (Pontoporia blainvillei) that might be associated with microscopic lesions. Because certain hemoplasmas can cause disease and death in immunosuppressed mammals, the bacteria could have conservation implications for already endangered aquatic mammals.

BACKGROUND: The literature indicates that the enteric nervous system is affected in inflammatory bowel diseases (IBDs) and that the P2X7 receptor triggers neuronal death. However, the mechanism by which enteric neurons are lost in IBDs is unknown. AIM: To study the role of the caspase-3 and nuclear factor kappa B (NF-κB) pathways in myenteric neurons in a P2X7 receptor knockout (KO) mouse model of IBDs. METHODS: Forty male wild-type (WT) C57BL/6 and P2X7 receptor KO mice were euthanized 24 h or 4 d after colitis induction by 2,4,6-trinitrobenzene sulfonic acid (colitis group). Mice in the sham groups were injected with vehicle. The mice were divided into eight groups (n = 5): The WT sham 24 h and 4 d groups, the WT colitis 24 h and 4 d groups, the KO sham 24 h and 4 d groups, and the KO colitis 24 h and 4 d groups. The disease activity index (DAI) was analyzed, the distal colon was collected for immunohistochemistry analyses, and immunofluorescence was performed to identify neurons immunoreactive (ir) for calretinin, P2X7 receptor, cleaved caspase-3, total caspase-3, phospho-NF-κB, and total NF-κB. We analyzed the number of calretinin-ir and P2X7 receptor-ir neurons per ganglion, the neuronal profile area (µm²), and corrected total cell fluorescence (CTCF). RESULTS: Cells double labeled for calretinin and P2X7 receptor, cleaved caspase-3, total caspase-3, phospho-NF-κB, or total NF-κB were observed in the WT colitis 24 h and 4 d groups. The number of calretinin-ir neurons per ganglion was decreased in the WT colitis 24 h and 4 d groups compared to the WT sham 24 h and 4 d groups, respectively (2.10 ± 0.13 vs 3.33 ± 0.17, P < 0.001; 2.92 ± 0.12 vs 3.70 ± 0.11, P < 0.05), but was not significantly different between the KO groups. The calretinin-ir neuronal profile area was increased in the WT colitis 24 h group compared to the WT sham 24 h group (312.60 ± 7.85 vs 278.41 ± 6.65, P < 0.05), and the nuclear profile area was decreased in the WT colitis 4 d group compared to the WT sham 4 d group (104.63 ± 2.49 vs 117.41 ± 1.14, P < 0.01). The number of P2X7 receptor-ir neurons per ganglion was decreased in the WT colitis 24 h and 4 d groups compared to the WT sham 24 h and 4 d groups, respectively (19.49 ± 0.35 vs 22.21 ± 0.18, P < 0.001; 20.35 ± 0.14 vs 22.75 ± 0.51, P < 0.001), and no P2X7 receptor-ir neurons were observed in the KO groups. Myenteric neurons showed ultrastructural changes in the WT colitis 24 h and 4 d groups and in the KO colitis 24 h group. The cleaved caspase-3 CTCF was increased in the WT colitis 24 h and 4 d groups compared to the WT sham 24 h and 4 d groups, respectively (485949 ± 14140 vs 371371 ± 16426, P < 0.001; 480381 ± 11336 vs 378365 ± 4053, P < 0.001), but was not significantly different between the KO groups. The total caspase-3 CTCF, phospho-NF-κB CTCF, and total NF-κB CTCF were not significantly different among the groups. The DAI was recovered in the KO groups. Furthermore, we demonstrated that the absence of the P2X7 receptor attenuated inflammatory infiltration, tissue damage, collagen deposition, and the decrease in the number of goblet cells in the distal colon. CONCLUSION: Ulcerative colitis affects myenteric neurons in WT mice but has a weaker effect in P2X7 receptor KO mice, and neuronal death may be associated with P2X7 receptor-mediated caspase-3 activation. The P2X7 receptor can be a therapeutic target for IBDs.

This study aimed to investigate the distal colon myenteric plexus and enteric glial cells (EGCs) in P2X7 receptor-deficient (P2X7-/-) animals after the induction of experimental ulcerative colitis. 2,4,6-Trinitrobenzene sulfonic acid (TNBS) was injected into the distal colon of C57BL/6 (WT) and P2X7 receptor gene-deficient (P2X7-/-, KO) animals. Distal colon tissues in the WT and KO groups were analyzed 24 h and 4 days after administration. The tissues were analyzed by double immunofluorescence of the P2X7 receptor with neuronal nitric oxide synthase (nNOS)-immunoreactive (ir), choline acetyltransferase (ChAT)-ir, and PGP9.5 (pan neuronal)-ir, and their morphology was assessed by histology. The quantitative analysis revealed 13.9% and 7.1% decreases in the number of P2X7 receptor-immunoreactive (ir) per ganglion in the 24 h-WT/colitis and 4 day-WT/colitis groups, respectively. No reduction in the number of nNOS-ir, choline ChAT-ir, and PGP9.5-ir neurons per ganglion was observed in the 4 day-KO/colitis group. In addition, a reduction of 19.3% in the number of GFAP (glial fibrillary acidic protein)-expressing cells per ganglion was found in the 24 h-WT/colitis group, and a 19% increase in the number of these cells was detected in the 4 day-WT/colitis group. No profile area changes in neurons were observed in the 24 h-WT and 24 h-KO groups. The 4 day-WT/colitis and 4 day-KO/colitis groups showed increases in the profile neuronal areas of nNOS, ChAT, and PGP9.5. The histological analysis showed hyperemia, edema, or cellular infiltration in the 24 h-WT/colitis and 4 day-WT/colitis groups. Edema was observed in the 4 day-KO/colitis group, which showed no histological changes compared with the 24 h-KO/colitis group. We concluded that ulcerative colitis differentially affected the neuronal classes in the WT and KO animals, demonstrating the potential participation and neuroprotective effect of the P2X7 receptor in enteric neurons in inflammatory bowel disease.

Shrimp farming is blooming worldwide, posing a severe threat to mangroves and its multiple goods and ecosystem services. Several studies reported the impacts of aquaculture on mangrove biotic communities, including microbiomes. However, little is known about how mangrove soil microbiomes would change in response to mangrove forest recolonization. Using genome-resolved metagenomics, we compared the soil microbiome of mangrove forests (both with and without the direct influence of shrimp farming effluents) with active shrimp farms and mangroves under a recolonization process. We found that the structure and composition of active shrimp farms microbial communities differ from the control mangrove forests, mangroves under the impact of the shrimp farming effluents, and mangroves under recolonization. Shrimp farming ponds microbiomes have lower microbial diversity and are dominated by halophilic microorganisms, presenting high abundance of multiple antibiotic resistance genes. On the other hand, control mangrove forests, impacted mangroves (exposed to the shrimp farming effluents), and recolonization ponds were more diverse, with a higher abundance of genes related to carbon mobilization. Our data also indicated that the microbiome is recovering in the mangrove recolonization ponds, performing vital metabolic functions and functionally resembling microbiomes found in those soils of neighboring control mangrove forests. Despite highlighting the damage caused by the habitat changes in mangrove soil microbiome community and functioning, our study sheds light on these systems incredible recovery capacity. Our study shows the importance of natural mangrove forest recovery, enhancing ecosystem services by the soil microbial communities even in a very early development stage of mangrove forest, thus encouraging mangrove conservation and restoration efforts worldwide.

OBJECTIVE: The objective of our study was to evaluate if functional training with the Functional Movement Screen (FMS) can reduce the risk of a new injury for patients that underwent an anterior cruciate ligament reconstruction (ACLR). Our hypothesis was that the functional training might reduce the risk of a new injury. METHODS: Our training protocol consisted of six phases, each one lasting six weeks. It began two months after surgery. The study group was composed of 10 individuals that completed our protocol after ACLR. The control group consisted of 10 people that completed a regular ACLR rehabilitation protocol. The FMS was used to compare the study and control group performance. Patients with a score of 14 or less on the FMS were considered more likely to suffer an injury than those with a score higher than 14. RESULTS: The study group average FMS score was 16.6 compared to the control group at 12.3. Functional training for ACLR rehabilitation added a statistically significant benefit (p < 0.0002) to reduce the risk of a new injury compared to regular protocol. CONCLUSION: Functional training may be considered an alternative to the regular ACLR rehabilitation to reduce the risk of a new injury before returning to sports. OBJETIVO: Nosso objetivo foi avaliar se o treinamento funcional pode reduzir o risco de nova lesão, após a reconstrução do ligamento cruzado anterior (RLCA), pelo Functional Movement Screen (FMS). Nossa hipótese foi que o treinamento funcional pode diminuir o risco de nova lesão. MÉTODOS: O treinamento consistiu de seis fases de seis semanas cada uma. Começou dois meses após a reconstrução do ligamento. O grupo estudo foi composto por 10 indivíduos que completaram o treinamento, após a RLCA. O grupo controle consistiu em 10 pessoas que fizeram o protocolo regular de reabilitação da RLCA. O FMS foi utilizado para comparar o desempenho dos dois grupos. Pacientes com pontuação igual ou inferior a 14 foram considerados mais propensos a sofrer nova lesão em comparação àqueles com pontuação maior que 14. RESULTADOS: A pontuação média do grupo estudo foi de 16,6 e a do grupo controle, 12,3. O treinamento funcional adicionou um benefício estatisticamente significativo (p < 0,0002) para diminuir o risco de nova lesão, em comparação com o protocolo regular. CONCLUSÃO: O treinamento funcional pode ser mais uma estratégia a ser incluida na reabilitação regular da RLCA, para diminuir o risco de uma nova lesão, antes de retornar ao esporte. Nível de Evidência III, Estudo de Caso controle.

A combination of immunosuppressants may improve outcomes due to the synergistic effect of their different action mechanisms. Currently, there is no consensus regarding the best immunosuppressive protocol after liver transplantation. This review aimed to evaluate the effectiveness and safety of tacrolimus associated with mycophenolate mofetil (MMF) in patients undergoing liver transplantation. We performed a systematic review and meta-analysis of randomized clinical trials. Eight randomized trials were included. The proportion of patients with at least one adverse event related to the immunosuppression scheme with tacrolimus associated with MMF was 39.9%. The tacrolimus with MMF immunosuppression regimen was superior in preventing acute cellular rejection compared with that of tacrolimus alone (risk difference [RD]=-0.11; p =0.001). The tacrolimus plus MMF regimen showed no difference in the risk of adverse events compared to that of tacrolimus alone (RD=0.7; p=0.66) and cyclosporine plus MMF (RD=-0.7; p=0.37). Patients undergoing liver transplantation who received tacrolimus plus MMF had similar adverse events when compared to patients receiving other evaluated immunosuppressive regimens and had a lower risk of acute rejection than those receiving in the monodrug tacrolimus regimen.

INTRODUCTION: Aberrant Crypt (AC) and Aberrant Crypt Focus (ACF) are considered pre-neoplasic lesions, ranging from hyperplasia to different degrees of dysplasia in the colon. This work aimed to evaluate and quantify the chemopreventive activity of Zingiber officinale essential oil in the colorectal region of Wistar rats. MATERIALS AND METHODS: We extracted the essential oil from ginger rhizomes and carried out ACF induction, in rats, with 1.2 Dimethylhydrazine (DMH) at a 20 mg/kg dose. The experimental groups were GI (negative control); GII (positive induction control); GIII (DMH + essential oil); GIV (DMH +5-Florouracil) and GV (essential oil). The histological techniques used were methylene blue, hematoxylin-eosin (HE) dyeing, and immunohistochemistry (IHQ). RESULTS: The major essential oil compounds were citral (17.25%), δ-citral (10.25%), camphene (9.55%), α-zingiberene (7.57%), nerol (6.37%) and plelandrene (6.83%). For the presence of AC or ACF, we did not observe them in GI and GV, while in GII and GIII, they were observed, in high values, in both regions, but only in the distal region, there was a significant difference between them. For GIV, for both regions, there were significant lower numbers of AC when compared to GIII. As observed, with HE, there were hyperplastic and dysplastic ACF in the proximal and distal portions of the colon. For IHQ analyses, there were positively PCNA antibody marked cells in all experimental groups. Yet, there was no significant correlation of mitotic index among them. Moreover, the results of GIII compared to GIV were very similar. CONCLUSION: In this sense, the Zingiber officinale essential oil has good antioxidant potential because it presents a mixture of monoterpene and sesquiterpene compounds. Thus, it is able to develop a chemoprotective effect, as it presented similar results to the standard drug, showing cell proliferation control.

Aim: The aim of this study was to explore the impact of oral health-related quality of life (OHRQoL) on the child's family structure. Methods: A representative sample of 613 children from public preschools, in a city in southeastern Brazil, were enrolled in this cross-sectional study. The sample was determined through probabilistic sampling in two stages (preschools and children). The outcome variable (Early Childhood Oral Health Impact Scale [ECOHIS]) was multi-categorized in children and family. Independent individual variables were sex, race, family income, parents'/caregivers' level of education, family income, dental caries, and malocclusion. Initially, individual analyses were performed, relating the study variables to the outcome variables, estimating the raw odds ratio with the respective confidence intervals of 95%. The variables with p < 0.20 in the individual analyses were tested in the multiple logistic regression models, and those with p < 0.10 remained in the model. Results: Impact on OHRQoL was reported by 40.9% of the children and 17% of their families. Children with low family income and caries experience had, respectively, 1.53 (95% CI: 1.00-2.32) (p = 0.0465) and 2.96 (95% CI: 1.81-4.84) (p < 0.0001) more chance of presenting negative impact on OHRQoL. Conclusions: The aspects that most affected the OHRQoL of child's family structure were low income and dental caries experience

This study aimed to evaluate the effects of increasing levels of chitosan (CHI) on sugarcane fermentation profile and losses, chemical composition, and in situ degradation. Treatments were: 0, 1, 2, 4, and 8 g of CHI/kg of dry matter (DM). Twenty experimental silos (PVC tubing with diameter 28 cm and height 25 cm) were used. Sand (2 kg) was placed at the bottom of each silo to evaluate effluent losses, and silos were weighed 60 d after ensiling to calculate gas losses. Samples were collected from the center of the silo mass to evaluate silage chemical composition, in situdegradation, fermentation profile, and mold and yeast count. Data were analyzed as a completely randomized design, and the treatment effect was decomposed using polynomial regression. Chitosan linearly increased acetic acid and NH3-N concentration, while yeast and mold count, and ethanol concentration decreased. Intermediary levels of CHI (from 4.47 to 6.34 g/kg DM) showed the lower values of effluent, gas, and total losses. There was a quadratic effect of CHI on the content of non-fiber carbohydrates, neutral and acid detergent, and in situ DM degradation. The lowest fiber content was observed with levels between 7.01 and 7.47 g/kg DM, whereas the highest non-fiber carbohydrate content and in situ DM degradation were found with 6.30 and 7.17 g/kg DM of CHI, respectively. Chitosan linearly increased acetic acid and NH3-N concentration, whereas it linearly reduced ethanol concentration and count of yeast and mold. Thus, intermediary levels of CHI, between 4.47 and 7.47 g/kg of DM, decrease fermentation losses and improve the nutritional value of sugarcane silage.(AU)


Foram avaliados os efeitos do aumento dos níveis de quitosana (CHI) sobre o perfil e as perdas fermentativas, a composição química e degradação in situ da silagem de cana-de-açúcar. Os tratamentos foram: 0, 1, 2, 4 e 8 g de CHI / kg de matéria seca (MS). Foram utilizados vinte silos experimentais (tubos de PVC com 28 cm de diâmetro e 25 cm de altura). Areia (2 kg) foi adicionada na porção inferior de cada silo para avaliar as perdas por efluentes e os silos foram pesados 60 dias após a ensilagem para calcular as perdas por gases. Amostras foram coletadas do centro da massa do silo para avaliar a composição química, degradação in situ, perfil fermentativo e a contagem de fungos e leveduras da silagem. Os dados foram analisados como um delineamento inteiramente casualizado e o efeito do tratamento foi decomposto usando regressão polinomial. A CHI aumentou linearmente a concentração de ácido acético e N-NH3, enquanto diminuiu a contagem de leveduras e bolores e a concentração de etanol. Os níveis intermediários de CHI (de 4,47 a 6,34 g/kg MS) mostraram os menores valores de perdas por efluentes, gases e totais. Houve efeito quadrático da CHI sobre o teor de carboidratos não fibrosos, fibra em detergente neutro e ácido e sobre a degradação in situ da MS. Os menores teores de fibras foram observados com níveis de CHI entre 7,01 e 7,47 g/kg MS, enquanto que os maiores teores de carboidratos não fibrosos e degradação in situ da MS foram encontrados com 6,30 e 7,17 g/kg MS de CHI, repectivamente. A CHI aumentou linearmente as concentrações de ácido acético e N-NH3, enquanto reduziu linearmente a concentração de etanol e a contagem de fungos e leveduras. This study aimed to evaluate the effects of increasing levels of chitosan (CHI) on sugarcane fermentation profile and losses, chemical composition, and in situ degradation. Treatments were: 0, 1, 2, 4, and 8 g of CHI/kg of dry matter (DM). Twenty experimental silos (PVC tubing with diameter 28 cm and height 25 cm) were used. Sand (2 kg) was placed at the bottom of each silo to evaluate effluent losses, and silos were weighed 60 d after ensiling to calculate gas losses. Samples were collected from the center of the silo mass to evaluate silage chemical composition, in situdegradation, fermentation profile, and mold and yeast count. Data were analyzed as a completely randomized design, and the treatment effect was decomposed using polynomial regression. Chitosan linearly increased acetic acid and NH3-N concentration, while yeast and mold count, and ethanol concentration decreased. Intermediary levels of CHI (from 4.47 to 6.34 g/kg DM) showed the lower values of effluent, gas, and total losses. Abstract Introduction Elastomers are considered important allies to orthodontic treatment and due to the aesthetic demand, aesthetic elastomers are increasingly used in adult patients. Objective The aim of this experimental in vitro study was to evaluate the degradation of force of esthetic elastomeric chains, of three different sizes: short, medium and long. Material and method For evaluating the degradation of force, 90 elastomeric chain segments were used, divided into 9 Groups (n=10), as follows: Morelli short elastomer, Morelli medium, Morelli long, Orthometric short, Orthometric medium, Orthometric long, American Orthodontic short, American Orthodontic medium and American Orthodontic long. Acrylic plates with pins were used to provide stretching of the elastomeric chains. The initial stretching force of the elastomeric chain was 150 grams, measured by a tensiometer. After this, they were prestretched to 50% of their original length, measured in an Instron 4411 Universal test machine, and placed on the seating pin on the plate. The plate was immersed in artificial saliva at 37 °C in a plastic receptacle, and removed for measurement after a time interval of 21 days. For statistical analysis, the methodology of mixed models for repeated measures in time and Tukey-Kramer test were used. Degradation of the forces was analyzed by 2-way analysis of variance (ANOVA) and the Tukey test. Result All the elastomeric chains showed statistically significant reduction in force (p<0.05). The American Orthodontic elastomeric chains showed higher reduction in force values, irrespective of the elastic size (p<0.05). Conclusion The three commercial brands studied underwent significant reduction in the quantity of force released; the American Orthodontic brand of elastomer showed the highest value of degradation in force.


Resumo Introdução Os elastômeros são considerados aliados importantes do tratamento ortodôntico e, devido à demanda estética, os elastômeros estéticos são cada vez mais utilizados em pacientes adultos. Objetivo O objetivo desse estudo experimental in vitro foi avaliar a degradação de forças geradas pelas cadeias elastoméricas estéticas, em três diferentes tamanhos curto, médio e longo. Material e método Para a avaliação da degradação da força foram utilizados 90 segmentos de cadeias elastoméricas, divididos em 9 grupos (n=10), sendo: elastômero Morelli curto, Morelli médio, Morelli longo, Orthometric curto, Orthometric médio, Orthometric longo, American Orthodontic curto, American Orthodontic médio e American Orthodontic longo. Foram utilizadas placas de acrílico com pinos que proporcionaram o estiramento das cadeias elastoméricas. A força inicial de estiramento da cadeia elastomérica foi de 150 gramas, medidas por um tensiômetro. Em seguida foram pré-estiradas em 50% do seu comprimento original, medidas em uma máquina Universal Instron 4411 e levadas ao pino de assentamento na placa. A placa foi imersa em um recipiente plástico com saliva artificial a 37 °C e removida após 21 dias para aferição. Foi aplicada metodologia de modelos mistos para medidas repetidas no tempo e teste de Tukey-Kramer. A degradação das forças foi analisada por análise de variância (ANOVA) "2 fatores" e teste de Tukey. Resultado Todas as correntes elastoméricas apresentaram redução estatisticamente significativa da força (p<0,05). As correntes elastoméricas da American Orthodontic apresentaram maior redução de força, independentemente do tamanho do elástico (p<0,05). Conclusão As três marcas comerciais estudadas sofreram uma redução significativa na quantidade de força liberada, sendo o elastômero da marca American Orthodontic o que apresentou maior degradação de força.

Embora a criopreservação ainda seja um grande desafio devido a queda na qualidade espermática, essa tecnologia possibilita a utilização dos gametas masculinos por um período relativamente longo ou indeterminado, reduz riscos e custos com aquisição e transporte de reprodutores, além de favorecer rápida distribuição do material genético entre locais distantes.(AU)

BACKGROUND: The prevalence of coronary artery disease (CAD) in valvular patients is similar to that of the general population, with the usual association with traditional risk factors. Nevertheless, the search for obstructive CAD is more aggressive in the preoperative period of patients with valvular heart disease, resulting in the indication of invasive coronary angiography (ICA) to almost all adult patients, because it is believed that coronary artery bypass surgery should be associated with valve replacement. OBJECTIVES: To evaluate the prevalence of obstructive CAD and factors associated with it in adult candidates for primary heart valve surgery between 2001 and 2014 at the National Institute of Cardiology (INC) and, thus, derive and validate a predictive obstructive CAD score. METHODS: Cross-sectional study evaluating 2898 patients with indication for heart surgery of any etiology. Of those, 712 patients, who had valvular heart disease and underwent ICA in the 12 months prior to surgery, were included. The P value < 0.05 was adopted as statistical significance. RESULTS: The prevalence of obstructive CAD was 20%. A predictive model of obstructive CAD was created from multivariate logistic regression, using the variables age, chest pain, family history of CAD, systemic arterial hypertension, diabetes mellitus, dyslipidemia, smoking, and male gender. The model showed excellent correlation and calibration (R² = 0.98), as well as excellent accuracy (ROC of 0.848; 95%CI: 0.817-0.879) and validation (ROC of 0.877; 95%CI: 0.830 - 0.923) in different valve populations. CONCLUSIONS: Obstructive CAD can be estimated from clinical data of adult candidates for valve repair surgery, using a simple, accurate and validated score, easy to apply in clinical practice, which may contribute to changes in the preoperative strategy of acquired heart valve surgery in patients with a lower probability of obstructive disease.
