RESUMO

The objective of this study was to assess and compare the frequency of some joint disorders in 130 CMD + bruxing behavior patients, 66 CMD/nonbruxing behaviors patients referred for diagnosis and treatment to the Center for the Study of Craniomandibular Disorders and 130 control subjects seeking routine dental care. Both patients and controls were consecutive referrals to the clinic occurring over a five year period. The mean age of the CMD + bruxing behavior group was about 35.48 years (range 14-54, SD = 8.45), and 36.84 years (range 17-60, SD = 9.30) in the 66 CMD nonbruxing behavior group, and 34.34 years (range 14-62, SD = 9.92) in the control group of 130 subjects. Information gathered included a questionnaire, history of signs and symptoms, and a clinical examination. Different types of joint disorders, muscle signs and symptoms and bruxing behavior were assessed in the CMD groups and in the corresponding control group. The study concluded that capsulitis/synovitis, retrodiskal pain and disk-attachment pain predominated in CMD + bruxing behavior patients. The data reinforces the need to assess CMD + bruxing behavior patients to evaluate signs and symptoms of such disorders in order to obtain additional information about the true source of pain and the need for proper management.

RESUMO

The purpose of this investigation was to evaluate under experimental conditions the influence of six different brands of dental amalgam, exclusively in proximal cavity preparations (proposed by Elderton and Almquist and associates) on failure characteristics of these "restored dice" teeth. The volume of amalgam contained in each cavity was also calculated. The static "masticatory force" was applied through a small steel-ball (2.5 mm) in order to cause the failure of the amalgam condensed in a cavity on prepared Ni-Cr dice. No statistical significance at 0.05 level was observed for the variables (two types of cavity and six amalgam alloys), considering that the cavity design proposed by Elderton has a volume of material about 18.35% smaller than the Almquist one.

The authors, as a sequence of a previous research, dealt with a morphological study, by means of level curves, of the lower first molar (mesial surface). This study based on dental amalgam reconstructions used two different types of interproximal cavity preparations; one type was related to the modern or conservative preparation and the other was related to the conventional cavity preparation. Two types of matrix holders were utilized as well as copper rings. In this project the contacting interproximal areas and adjacent structures were observed stereophotogrammetrically. As in the previous study, the models used were obtained from patients with good occlusal anatomical functional conditions and mounted in a semiadjustable articulator. The procedures for restorations followed the prescribed technique for these cases as far as the selection of the material, placement of the matrices, condensation pressure, etc, were concerned. The reconstruction of the serial slices, through level curves, were able to show the location of the contacting area for each matrix used, although it is necessary to stress that the original tooth contour has never been restored.

This study dealt with laboratory tests (oblique force application of 45 degrees to check the fracture strength), in order to compare two types of dentin pins: threaded metallic TMS Whaledent pins and amalgapins. It was used multiple number of these elements, with or without thermal cycling. The statistical analysis has indicated a highly significance difference between these pins, where the amalgapins provide to be more efficient than the steel one. As the resistance with the use of 3 or 4 amalgapins are similar, is up to the professional to choose the number of pins. It was also observed that the use of 2 or 3 steel pins had the same results. In this study it was observed that almost all of the specimens were able to resist to oblique load of more than 70 kgf. This value is higher than any force occurring in the mouth (HELKINO & INGERVALL 10). Hence, the results in vitro indicate that extensive amalgam restorations may well work in the oral cavity. Clinical studies must be conducted in order to check if really these mastication, since it is practically impossible to duplicate these natural condition in a laboratory environment.
