Surprisingly, little data are available for the use of disease-modifying antirheumatic drugs in ankylosing spondylitis. Sulfasalazine has been the best studied. Efficacy data for individual agents (including pamidronate) and combinations of agents are detailed in this review. Intriguingly, these agents continue to be used with some frequency, even in the absence of efficacy data. To answer these questions, additional systematic studies of these agents in ankylosing spondylitis are needed and will likely need to be done by interested collaborative groups such as SPARTAN.

The Spondyloarthritis Research and Therapy Network (SPARTAN) is a network of health care professionals who are dedicated to research, awareness and treatment of ankylosing spondylitis and related forms of spondyloarthritis (SpA). Members meet yearly to promote research, education and treatment of SpA. The SPARTAN 2010 meeting was held on July 23 to 24 in Houston, Texas. Major presentations were given on structural bone changes in SpA, the sensitivity and specificity of magnetic resonance imaging in early SpA, mycobacterial infections in this era of modern biologic therapies and findings from the U.S. National Health and Nutrition Examination Surveys for ankylosing spondylitis, SpA and inflammatory back pain. Additional country-specific presentations on the epidemiology of SpA were provided by representatives of North America, Central America, Brazil, Argentina, Colombia and Mexico. These topics were all developed into separate contributions contained in this in-issue supplement and summarized herein.

The Spondyloarthritis Research and Therapy Network (SPARTAN), founded in 2003 to promote research, education, and treatment of ankylosing spondylitis (AS) and related forms of spondyloarthritis (SpA), held its 7th Annual Research and Education Meeting in July 2009 in Houston, Texas. Current controversies in SpA discussed during the meeting included an update on the epidemiology of AS, axial SpA, and inflammatory back pain; the adequacy of the mSASS to assess radiographic involvement; the helpfulness of magnetic resonance imaging in assessing disease progression; the reliability of metrology in assessing damage; and whether biologic agents alter the course of AS. Presentations also were made on psoriasis in the SCID mouse model; the challenges and opportunities of SpA in China; a discussion of the special needs in managing SpA in Ibero-America, and the SPARK Survey in Europe and North America.
