Abstract Introduction Foreign body accidents (FBAs) are frequent in children and can be severe, being a common cause of morbidity and mortality and a public health problem. As these accidents are multifactorial, their cause can only be determined by analyzing the clinical details and characteristics of the object. Knowing the associated mechanisms and factors is essential to determine a risk profile and have a preventive, therapeutic, and diagnostic purpose. Objective The present study aimed to describe the incidence of FBAs in otorhinolaryngology according to their anatomical location, focusing on the aerodigestive system. Methods This is a retrospective study performed by reviewing the medical records of 668 cases seen in the emergency department of a tertiary pediatric hospital in São Paulo, state of São Paulo, Brazil, between 2014 and 2017. Results Foreign bodies (FBs) were found in the digestive system (238/668), in the nasal cavities (206/668), in the ears (182/668), in the oropharynx (34/668), and in the respiratory system (8/668). A total of 91.77% of the patients were treated in the emergency room. The main age group affected was < 5 years old, with no difference between genders. The most frequent complications affected the digestive system and the most serious occurred in cases involving the respiratory system. Conclusion Multidisciplinary teams should be ready in the emergency room to provide adequate care in FBAs. Early diagnosis, FB removal in the emergency room or the surgical center and follow-up are essential. Developing prevention campaigns including a risk profile for certain products and/or materials, helping to ensure safety for consumers, is necessary. For this, a national database with compulsory notification containing relevant information on FBAs in the pediatric population should be created.

BACKGROUND Tracheomalacia, a disorder of the large airways where the trachea is deformed or malformed during respiration, is commonly seen in tertiary paediatric practice. It is associated with a wide spectrum of respiratory symptoms from life-threatening recurrent apnoea to common respiratory symptoms such as chronic cough and wheeze. Current practice following diagnosis of tracheomalacia includes medical approaches aimed at reducing associated symptoms of tracheomalacia, ventilation modalities of continuous positive airway pressure (CPAP) and bi-level positive airway pressure (BiPAP), and surgical approaches aimed at improving the calibre of the airway (airway stenting, aortopexy, tracheopexy). OBJECTIVES To evaluate the efficacy of medical and surgical therapies for children with intrinsic (primary) tracheomalacia. METHODS Search The Cochrane Airways Group searched the Cochrane Central Register of Controlled Trials (CENTRAL), the Cochrane Airways Group's Specialized Register, Medline and Embase databases. The Cochrane Airways Group performed the latest searches in March 2012. Selection criteria All randomized controlled trials (RCTs) of therapies related to symptoms associated with primary or intrinsic tracheomalacia. Data collection and analysis Two reviewers extracted data from the included study independently and resolved disagreements by consensus. MAIN RESULTS We included one RCT that compared nebulized recombinant human deoxyribonuclease (rhDNase) with placebo in 40 children with airway malacia and a respiratory tract infection. We assessed it to be a RCT with overall low risk of bias. Data analyzed in this review showed that there was no significant difference between groups for the primary outcome of proportion cough-free at two weeks (odds ratio (OR) 1.38; 95% confidence interval (CI) 0.37 to 5.14). However, the mean change in night time cough diary scores significantly favoured the placebo group (mean difference (MD) 1.00; 95% CI 0.17 to 1.83, P = 0.02). The mean change in daytime cough diary scores from baseline was also better in the placebo group compared to those on nebulized rhDNase, but the difference between groups was not statistically significant (MD 0.70; 95% CI -0.19 to 1.59). Other outcomes (dyspnoea, and difficulty in expectorating sputum scores, and lung function tests at two weeks also favoured placebo over nebulized rhDNase but did not reach levels of significance. AUTHORS' CONCLUSIONS There is currently an absence of evidence to support any of the therapies currently utilised for management of intrinsic tracheomalacia. It remains inconclusive whether the use of nebulized rhDNase in children with airway malacia and a respiratory tract infection worsens recovery. It is unlikely that any RCT on surgically based management will ever be available for children with severe life-threatening illness associated with tracheomalacia. For those with less severe disease, RCTs on interventions such as antibiotics and chest physiotherapy are clearly needed. Outcomes of these RCTs should include measurements of the trachea and physiological outcomes in addition to clinical outcomes.

A greater difficulty in exposing the arytenoid cartilage during rotation surgeries has been observed for men. The objective of the present study was to describe the position of the cricothyroid articulation and the distance between the right and left articulations, and to compare these findings between genders. The following measurements were obtained for 16 cadavers (9 men and 7 women): angulation of the cricothyroid articulation in the cricoid ring, dimensions of the thyroid articular surface of the cricoid cartilage, and distance between the cricothyroid and cricoarytenoid articulations. The cricothyroid articulation angle was narrower in men than in women (p = .04). The major diameter of the articular facet of the thyroid cartilage was wider in men (p = .001). The longer lamina of the thyroid cartilage, as well as the more posterior position of the cricothyroid articulation, in men might explain the greater difficulty in exposing the arytenoid cartilage during laryngeal framework surgeries observed for this group of patients.

A aproximação cricotireóidea simula a contração do músculo cricotireóideo, aproximando as cartilagens cricóidea e tireóidea anteriormente. Distende e tensiona as pregas vocais, elevando a freqüência fundamental da voz. É indicada em pacientes que apresentam voz grave em conseqüência da paralisia do nervo laríngeo superior ou por doenças endócrinas e reumatológicas no sexo feminino. Com o objetivo de estudar os efeitos da aproximação cricotireóidea sobre a vibração das pregas vocais em ambos sexos, foram avaliadas 30 laringes excisadas de cadáveres humanos frescos pré e pós aproximação cricotireóidea. Esse trabalho experimental permitiu excluir as interferências dos mecanismos compensatórios da fonação presentes em um estudo 'in vivo'. As cartilagens aritenóideas foram aproximadas por sutura, fechando a glote. Um fluxo de ar comprimido umedecido foi injetado no interior da traquéia, e ao passar pela glote promoveu a vibração das pregas vocais. Foi então realizada a aproximação cricotireóidea através de suturas, aproximando as cartilagens tireóidea e cricóidea. O comprimento da porção membranosa das pregas vocais foi obtido através de um paquímetro. A vibração das pregas vocais foi gravada pré e pós a aproximação cricotireóidea utilizando-se uma câmera de videoquimografia. Após digitalização e análise quadro a quadro das imagens documentadas, foram realizadas medidas da freqüência fundamental, da amplitude de vibração das pregas vocais, do ciclo vibratório e suas fases (aberta, fechada, de abertura e fechamento) pré e pós aproximação cricotireóidea. Foram também calculados e analisados índices que correlacionam a duração das diferentes fases do ciclo vibratório, como o quociente de abertura, de fechamento e o quociente de velocidade. The cricothyroid approximation simulates the cricothyroid muscle contraction by approximating the cricoid and thyroid cartilage’s. By this surgery, the vocal folds are lengthened. Consequently, the fundamental frequency of the voice is increased. The main indications for this procedure are patients presenting voice hoarseness due to superior laryngeal nerve palsy or to rheumatoid and endocrinological diseases in women. Thirty fresh cadaver’s larynges were used for the purpose of studying the effects of the cricothyroid approximation on the vocal folds vibration in both genders. Ideally, this experiment would allow exclusion of the diverse phonation compensatory factors that can possible compromise the accuracy of an “in vivo” study. The surgical procedure was performed in the excised larynges. Initially, the arytenoids cartilages were approximated by suturing both together, closing the glottis. Humidified compressed air was injected inside of the tracheal tube, causing vocal folds vibration. The cricothyroid approximation was performed by sutures placed in the cricoid and thyroid cartilages. Calipers were used to measure the length of the membranous portion of the vocal folds. Pre and post cricothyroid approximation. Vocal folds vibration was recorded by a portable camera specific for videokymographic studies. After digitalizing the recorded images, each one of them was carefully evaluated. The fundamental voice frequency, the vocal folds vibratory amplitude, and the vibratory cycle with its phases (opened, closed, opening and closing phases) were measured pre and post operatively. Indexes related to the duration of the different phases of the vibratory cycle, such as open quotient, closed quotient and speed quotient were then calculated and analyzed.
