RESUMO
Desde 1981 a aids tem estado presente em nossa sociedade. Como consequência, doenças infectocontagiosas e novos patógenos oportunistas têm desenvolvido novos comportamentos e adquirido novas virulências. Mesmo com a aquisição de novas tecnologias como técnicas imunológicas, ressonância magnética (RM), tomografia computadorizada (TC), tomografia por emissão de pósitron (PET) e espectrometria , existem desafios para o diagnóstico que incluem falsos negativos e positivos. Reportamos nossa experiência com paciente imunocomprometido e suas múltiplas infecções oportunistas, e juntamos casos com diferentes patógenos que mimetizam tumores de estirpe glial com resultados falsos positivos e negativos nas provas imunológicas, seja por comprometimento imunológico do hospedeiro, seja por provas cruzadas de colagenoses.
Since 1981 AIDS has been present in our society. Following it, controlled infection diseases and new opportunistic pathogen have raised different behavior and virulence. Even with the important technical advance such as immunologic assay, magnetic resonance (MRI), computed tomography CT, positron emission tomography (PET) and spectrometry there still exists a real challenge for diagnosis, including high levels of false negatives. We report in this work our experience with imunocompromised patients and their multiple opportunistic diseases and attach it different pathogens that may mimic tumoral glial diseases with false positive and negatives results in immunological assay, developed by host imunocompromised reactivity or by cross reaction of collagenosis desease.

A retrospective study of 81 patients with low-grade astrocytoma (LGA) comparing the efficacy of aggressive versus less aggressive surgery in eloquent and non-eloquent brain areas was conducted. Extent of surgical resection was analyzed to assess overall survival (OS) and progression- free survival (PFS). Degree of tumor resection was classified as gross total resection (GTR), subtotal resection (STR) or biopsy. GTR, STR and biopsy in patients with tumors in non-eloquent areas were performed in 31, 48 and 21% subjects, whereas in patients with tumors in eloquent areas resections were 22.5, 35 and 42.5%. Overall survival was 4.7 and 1.9 years in patients with tumors in non-eloquent brain areas submitted to GTR/STR and biopsy (p=0.013), whereas overall survival among patients with tumors in eloquent area was 4.5 and 2.1 years (p=0.33). Improved outcome for adult patients with LGA is predicted by more aggressive surgery in both eloquent and non-eloquent brain areas.

Foi realizado estudo retrospectivo em 81 pacientes com astrocitoma de baixo grau (LGA) comparando a eficácia da ressecção cirúrgica com cirurgia menos agressiva em relação à área eloquente e não eloquente do cérebro. A extensão da ressecção cirúrgica foi analisada para avaliar a sobrevida geral (OS) e o tempo livre de doença (PFS). O grau da ressecção cirúrgica foi classificado como ressecção total (GTR), subtotal (STR) e biópsia. Nos pacientes com lesão em área não eloquente foram realizadas GTR, STR e biópsia em 31, 48 e 21% dos casos, enquanto, naqueles com lesão em área eloquente, em 22,5, 35 e 42,5%, respectivamente. A sobrevida geral foi de 4,7 e 1,9 anos em pacientes com lesões em área não eloquente submetidos à GTR/STR e biópsia (p=0,013). Nos pacientes com lesão em áreas eloquentes, a sobrevida geral foi de 4,5 e 2,1 anos (p=0,33), respectivamente. A extensão da ressecção é fator preditivo de sobrevida tanto nas lesões em áreas eloquentes quanto nas não eloquentes.

Os autores discutem as modificações da hemodinâmica encefálica que conferem à curva de Langfitto aspecto exponencial que se relaciona com a rápida piora da hipertensão intracraniana. Concluíram ser interessante considerar que a cascata vasodilatadora pode ser importante no desenvolvimento desse fenômeno.
The authors discuss the modification in the intracranial hemodynamic that causes the exponential aspect of the ascending part of the Langfitt curve. The hypothesis concerning the importance of the vasodilatations cascade in the genesis of the phenomenon is very interesting.

This is a review on the biochemical and hemodynamic alterations caused by the isquemia in the neural tissue. This text introduce the pathophysiology and the therapeutic effects of this ischemic alterations.

O objetivo deste texto é o de estruturar e sistematizar novas hipóteses fisiopatológicas sobre as modificações hemodinâmicas do cérebro submetido à isquemia e à reperfusão. São propostas e analisadas hipóteses bioquímicas e hemodinâmicas do fenômeno da hipoperfusão pós-hiperemia.
The aim of this publication is to elaborate and discuss a new pathophysiological hypothesis about the haemodinamic modifications into the brain that occur after the reperfusion. Biochemical and haemodinamic hypothesis are proposed and analyzed, concerning the hipoperfusion following the hiperemia phenomenon.

The authors discuss the pathophysiology of the intracranial hypertension relative to putaminal or basal ganglia spontaneous hematomas. The role of the surgery is discussed. The current pathophysiological knowledge is strong enough to be the counterpart to the previous published statistical studies, becoming obvious the necessity of the surgical drainage when the patient is going to be treated and intracranial hypertension is present.

Objetivo: Descrever a via transtentorial, por meio de craniotomia têmporo-occipital, para remoção de cavernoma pontino de localização dorsolateral, exemplificado pela apresentação de um caso. Conclusão: O acesso transtentorial demonstra-se útil e seguro para remoção de lesões localizadas nessa região da ponte.
Objective: To present the transtentorial route through a temporal-occipital craniotomy for surgical removal of a posterior-lateral pontine cavernomas, exemplified by the presentation of a case. Regional anatomy is discussed. Conclusion: The transtentorial approach may be used safely to remove lesions of highly located in the posterior-lateral region of the pons.

Objetivo: Descrever a via pré-sigmoidea ampliada para acesso a lesões localizadas na região ventrolateral da ponte, exemplificado com a apresentação de um caso de cavernoma pontino nessa localização. Conclusão: Esse acesso, amplamente revisto pela literatura, é útil para remoção cirúrgica de cavernomas pontinos ventrolaterais.
Objective: To describe the pre-sigmoid transpetrosal approach to the ventrolateral pontine region, exemplified by the presentation of a case of cavernoma in this localization. Conclusion: This approach is useful to remove pontine ventrolateral cavernomas.

BACKGROUND: Cortical seeding of a craniopharyngioma has been rarely reported. We present a case that ectopically recurred along the tract of a previous surgical route. METHODS: A 27-year-old woman presented earlier with a suprasellar craniopharyngioma. A left frontotemporal craniotomy was done with subtotal resection of the tumor because it was strongly adhered to the optic chiasm. Histopathology confirmed the diagnosis of craniopharyngioma. Six months after, the patient presented with decreased visual acuity and diplopia. She was reoperated through the previous craniotomy with a total resection. One year after the second surgery, the patient presented with seizures that were difficult to control. Magnetic resonance imaging revealed a contrast-enhancing tumor with cystic and solid components on the left temporal lobe cortex. The primary tumor bed was intact. The patient was reoperated, and the temporal lobe tumor was totally removed. Histologic studies showed an adamantinomatous craniopharyngioma. The patient was free of neurologic abnormalities, and no new lesion was found in the magnetic resonance imaging performed 1 year after the last surgery. CONCLUSIONS: Although craniopharyngiomas exhibit a benign histopathologic pattern, a total resection combined with careful inspection and irrigation of the surgical field is the optimal treatment for preventing local and ectopic recurrences. It is strongly recommended that the concerned patients have a long-term clinical and neuroimaging follow-up.

Paragangliomas são tumores benignos de origem neuroendócrina que raramente ocorrem na cauda equina. Os autores apresentam o caso de um paciente portador de dois tumores de cauda equina que foram diagnosticados como sendo uma paraganglioma e um schawannoma; eles discutem os aspectos diagnósticos e terapêuticos dos paragangliomas.

É relatada uma forma atípica de displasia fibromuscular da artéria carótida interna cuja denominação é causa de grande confusão na literatura devido a sua raridade. Há menos de 20 casos descritos. Sua aparência em forma de septo é bem singular. A lesão foi causa de isquemia cerebral embólica transitória nuam paciente de 49 anos de idade. A paciente foi submetida a arteriotomia e a membrana rígida, que se originava na parede da artéria carótida interna projetando-se para luz arterial, foi removida. O aspecto histopatológico é a presentado e as controvérsia na nomenclatura dessa lesão é discutida.

Mielopatia cervical alta causada por cisto sinovial da articulação atlantoaxial é uma entidade rara e muito pouco descrita na literatura. Os autores descreveram um caso de cisto sinovial localizado posteriormente ao processo odontóide causando compressão bulbo-medular em paciente com 88 anos de idade. O cisto foi removido através de um acesso póstero-lateral direito com hemilaminectomia de C1 e ressecção microcirúrgica transdural. A paciente apresentou uma excelente recuperação. Os aspectos etiológicos e terapêuticos são discutidos.
