RESUMO

Globally, millions of lives are impacted every year by infectious diseases outbreaks. Comprehensive and innovative surveillance strategies aiming at early alert and timely containment of emerging and reemerging pathogens are a pressing priority. Shortcomings and delays in current pathogen surveillance practices further disturbed informing responses, interventions, and mitigation of recent pandemics, including H1N1 influenza and SARS-CoV-2. We present the design principles of the architecture for an early-alert surveillance system that leverages the vast available data landscape, including syndromic data from primary health care, drug sales, and rumors from the lay media and social media to identify areas with an increased number of cases of respiratory disease. In these potentially affected areas, an intensive and fast sample collection and advanced high-throughput genome sequencing analyses would inform on circulating known or novel pathogens by metagenomics-enabled pathogen characterization. Concurrently, the integration of bioclimatic and socioeconomic data, as well as transportation and mobility network data, into a data analytics platform, coupled with advanced mathematical modeling using artificial intelligence or machine learning, will enable more accurate estimation of outbreak spread risk. Such an approach aims to readily identify and characterize regions in the early stages of an outbreak development, as well as model risk and patterns of spread, informing targeted mitigation and control measures. A fully operational system must integrate diverse and robust data streams to translate data into actionable intelligence and actions, ultimately paving the way toward constructing next-generation surveillance systems.

RESUMO

Despite the considerable morbidity and mortality of yellow fever virus (YFV) infections in Brazil, our understanding of disease outbreaks is hampered by limited viral genomic data. Here, through a combination of phylogenetic and epidemiological models, we reconstructed the recent transmission history of YFV within different epidemic seasons in Brazil. A suitability index based on the highly domesticated Aedes aegypti was able to capture the seasonality of reported human infections. Spatial modeling revealed spatial hotspots with both past reporting and low vaccination coverage, which coincided with many of the largest urban centers in the Southeast. Phylodynamic analysis unraveled the circulation of three distinct lineages and provided proof of the directionality of a known spatial corridor that connects the endemic North with the extra-Amazonian basin. This study illustrates that genomics linked with eco-epidemiology can provide new insights into the landscape of YFV transmission, augmenting traditional approaches to infectious disease surveillance and control.

RESUMO

BACKGROUND: Insufficient sleep is common nowadays and it can be associated with chronic pain. OBJECTIVE: To describe the main polysomnographic findings in patients with chronic musculoskeletal pain and to estimate the association between sleep quality, polysomnography variables and chronic musculoskeletal pain. METHODS: This cross-sectional research analyzed a database from polysomnography type 1 exams results and then collected data via an electronic form from these patients. The form collected sociodemographic data and presented clinical questionnaires for measuring sleep quality, sleepiness, pain intensity and central sensitization signs. Pearson's correlation coefficient and odds ratio were used to estimate the associations. RESULTS: The mean age of the respondents was 55.1 (SD 13.4) years. The mean score of the Central Sensitization Inventory showed signs of central sensitization (50.1; SD 13.4) in the participants. Most patients (86%) had 1 or more nocturnal awakenings, 90% had one or more episodes of sleep apnea, 47% had Rapid Eye Movement sleep phase latency greater than 70-120 min and the mean sleep efficiency among all participants was 81.6%. The Pittsburgh Sleep Quality Index score was correlated with the CSI score (r = 0.55; 95% CI: 0.45, 0.61). People with central sensitization signs have 2.6 times more chance to present sleep episodes of blood oxygen saturation below 90% (OR = 2.62; 95% CI:1.23, 6.47). CONCLUSION: Most people with central sensitization signs had poor sleep quality, night waking episodes and specific disturbances in sleep phases. The findings showed association between central sensitization, sleep quality, nocturnal awakening, and changes in blood oxygen saturation during sleep.

RESUMO

Mexico has experienced one of the highest COVID-19 mortality rates in the world. A delayed implementation of social distancing interventions in late March 2020 and a phased reopening of the country in June 2020 has facilitated sustained disease transmission in the region. In this study we systematically generate and compare 30-day ahead forecasts using previously validated growth models based on mortality trends from the Institute for Health Metrics and Evaluation for Mexico and Mexico City in near real-time. Moreover, we estimate reproduction numbers for SARS-CoV-2 based on the methods that rely on genomic data as well as case incidence data. Subsequently, functional data analysis techniques are utilized to analyze the shapes of COVID-19 growth rate curves at the state level to characterize the spatiotemporal transmission patterns of SARS-CoV-2. The early estimates of the reproduction number for Mexico were estimated between Rt ~1.1-1.3 from the genomic and case incidence data. Moreover, the mean estimate of Rt has fluctuated around ~1.0 from late July till end of September 2020. The spatial analysis characterizes the state-level dynamics of COVID-19 into four groups with distinct epidemic trajectories based on epidemic growth rates. Our results show that the sequential mortality forecasts from the GLM and Richards model predict a downward trend in the number of deaths for all thirteen forecast periods for Mexico and Mexico City. However, the sub-epidemic and IHME models perform better predicting a more realistic stable trajectory of COVID-19 mortality trends for the last three forecast periods (09/21-10/21, 09/28-10/27, 09/28-10/27) for Mexico and Mexico City. Our findings indicate that phenomenological models are useful tools for short-term epidemic forecasting albeit forecasts need to be interpreted with caution given the dynamic implementation and lifting of social distancing measures.

RESUMO

BACKGROUND: Low testing rates and delays in reporting hinder the estimation of the mortality burden associated with the COVID-19 pandemic. During a public health emergency, estimating all cause excess deaths above an expected level of death can provide a more reliable picture of the mortality burden. Here, we aim to estimate the absolute and relative mortality impact of COVID-19 pandemic in Mexico. METHODS: We obtained weekly mortality time series due to all causes for Mexico, and by gender, and geographic region from 2015 to 2020. We also compiled surveillance data on COVID-19 cases and deaths to assess the timing and intensity of the pandemic and assembled weekly series of the proportion of tweets about 'death' from Mexico to assess the correlation between people's media interaction about 'death' and the rise in pandemic deaths. We estimated all-cause excess mortality rates and mortality rate ratio increase over baseline by fitting Serfling regression models and forecasted the total excess deaths for Mexico for the first 4 weeks of 2021 using the generalized logistic growth model. RESULTS: We estimated the all-cause excess mortality rate associated with the COVID-19 pandemic in Mexico in 2020 at 26.10 per 10,000 population, which corresponds to 333,538 excess deaths. Males had about 2-fold higher excess mortality rate (33.99) compared to females (18.53). Mexico City reported the highest excess death rate (63.54) and RR (2.09) compared to rest of the country (excess rate = 23.25, RR = 1.62). While COVID-19 deaths accounted for only 38.64% of total excess deaths in Mexico, our forecast estimate that Mexico has accumulated a total of ~ 61,610 [95% PI: 60,003, 63,216] excess deaths in the first 4 weeks of 2021. Proportion of tweets was significantly correlated with the excess mortality (ρ = 0.508 [95% CI: 0.245, 0.701], p-value = 0.0004). CONCLUSION: The COVID-19 pandemic has heavily affected Mexico. The lab-confirmed COVID-19 deaths accounted for only 38.64% of total all cause excess deaths (333,538) in Mexico in 2020. This reflects either the effect of low testing rates in Mexico, or the surge in number of deaths due to other causes during the pandemic. A model-based forecast indicates that an average of 61,610 excess deaths have occurred in January 2021.

RESUMO

Abstract Aims: to analyze the choices of handball coaches for structuring the defensive return throughout the teaching-learning process (U-12 to U-18 teams). Methods: Nineteen coaches from teams U-12 to U-18 from two leagues in the state of São Paulo were interviewed. The speeches from the semi-structured interviews were analyzed through thematic analysis, which produced two themes and their respective subthemes. Results: Most coaches structure their defensive return and emphasize content to make it difficult for an opponent to counterattack. A higher percentage of directed play was observed in teams U-12 and U-14, which suggests a possible early specialization process. In the U-16 and U-18 teams, higher percentages of free play were observed when compared to the others. In older teams, a higher percentage of directed play would be expected, mainly because these are stages with an increase in player specialization. Conclusion: The findings suggest that the simultaneous training of different teams may be leading to sports specialization procedures in the younger teams and, simultaneously, underestimating the possibilities in the older teams.

RESUMO

The GHEP-ISFG organized a collaborative study to estimate mutation rates for the markers included in the Investigator Argus X-12 QS kit Qiagen. A total of 16 laboratories gathered data from 1,612 father/mother/daughter trios, which were used to estimate both maternal and paternal mutation rates, when pooled together with other already published data. Data on fathers and mothers' age at the time of birth of the daughter were also available for ∼93 % of the cases. Population analyses were computed considering the genetic information of a subset of 1,327 unrelated daughters, corresponding to 2,654 haplotypes from residents in several regions of five countries: Argentina, Brazil, Ecuador, Portugal and Spain. Genetic differentiation analyses between the population samples from the same country did not reveal signs of significant stratification, although results from Hardy-Weinberg and linkage disequilibrium tests indicated the need of larger studies for Ecuador and Brazilian populations. The high genetic diversity of the markers resulted in a large number of haplotype combinations, showing the need of huge databases for reliable estimates of their frequencies. It should also be noted the high number of new alleles found, many of them not included in the allelic ladders provided with the kit, as very diverse populations were analyzed. The overall estimates for locus specific average mutation rates varied between 7.5E-04 (for DXS7423) and 1.1E-02 (for DXS10135), the latter being a troublesome figure for kinship analyses. Most of the found mutations (∼92 %) are compatible with the gain or loss of a single repeat. Paternal mutation rates showed to be 5.2 times higher than maternal ones. We also found that older fathers were more prone to transmit mutated alleles, having this trend not been observed in the case of the mothers.

RESUMO

ABSTRACT Objective: To propose an inexpensive blenderized tube feeding formula consisting of foods with standard nutritional composition that meets the nutritional requirements of individuals aged more than 51 years. Methods: The enteral diets were formulated mainly with fresh foods and tested for their physical (homogeneity, stability, osmolality, pH, and flow rate) and chemical (moisture, ash, protein, lipids, energy, crude fiber, vitamin C, calcium, iron, magnesium, and zinc) characteristics. The cost was determined by surveying item prices in supermarkets and stores that specialize in nutritional support. Results: The blenderized tube feeding formula was stable and homogeneous, and had slightly acidic pH, hypertonic osmolality (603mOsm/kg), and flow rate comparable with gravity drip (21 minutes). Proximate composition analysis indicated appropriate levels of proteins, lipids, vitamin C, and zinc. The mean cost of 2000kcal of the standard blenderized tube feeding formula was R$ 12.3±1.4, which is 70% cheaper than the mean cost of similar commercial enteral formulas. Conclusion: The planned diet can be an excellent choice for patients using blenderized tube feeding formulas as it consisted of habitual food items, had physical and nutritional quality, and was inexpensive.

Introduction Sports nutrition and body composition are important factors for athletic performance. Street running has become popular worldwide, especially in the last decade. Street runners require a nutritionally balanced diet and nutrition knowledge to maintain an adequate nutritional profile. Objective To assess dietary adequacy, the anthropometric profile and nutritional knowledge of street runners. Methods Adult street runners of both genders (n=51) were interviewed about nutrition knowledge, evaluated by a standardized modified questionnaire. Assessment of food intake was obtained from two 24-hour recalls on non-consecutive days and the analysis of macronutrients, micronutrients and dietary fiber adequacy. Anthropometric evaluation was determined by body mass index and body fat percentage. Results The majority of male runners had adequate body fat percentage and underweight was observed in about 30% of men and women. Dietary inadequacies of macronutrients were characterized by insufficient intake of carbohydrate (59.6% of individuals) and high protein intake (32.7%). Fiber, calcium, thiamin, riboflavin and niacin were consumed below recommended levels, especially among men. Nutritional knowledge was associated with adequate micronutrient intake. Women had better level of basic knowledge and higher prevalence of adequate dietary intake. Conclusion Despite of high nutrition knowledge and adequate anthropometric profile demonstrated by street runners, the majority showed eating habits and nutrition inappropriate for sports performance.

INTRODUCTION: Baseline Energy Expenditure (BEE) is the amount of energy necessary to the human body to keep the normal physiological processes and homeostasis. Currently, the BEE is calculated from equations based on the North American and/or European population. OBJECTIVE: To compare the equations of Harris-Benedict, Food and Agriculture Organization and Institute of Medicine with the indirect calorimeter in men living in Goiânia. METHODS: Forty- four male volunteers, average age of 29.80, Standart Deviation - SD = 6.21 years, stature 1.79, SD = 0.06m, weight 77.79, SD = 8.49 kg, BMI 24.30, SD = 1.96 kg/m2, body fat 15.84, SD = 4.49%, waist circumference 80.48, SD = 5.89 cm, were evaluated. BEE was determined by direct gas analyses using the gas analyzer VO2000. RESULTS: There was significant difference (p < 0.05) for the BEE estimated by the Harris-Benedict (1824.66 ± 138.25 kcal/day) and Food and Agriculture Organization (1821.77, SD = 115.39 kcal/day) equations when compared to the BEE acquired by the indirect calorimeter (1709.81, SD = 329.75 kcal/day). The BEE estimated by the equation IOM/2005 (1791.81, SD = 329.74) did not present significant difference when compared to the measured BEE. CONCLUSION: The Food and Agriculture Organization and Harris-Benedict equations can overestimate the BEE of healthy young men with similar characteristics to those reported in this work.

RESUMO

OBJECTIVE: This study aimed to review the congenital malformation known as agenesis of the dorsal pancreas (ADP) and other pancreatic birth defects, based on a rare and exemplary clinical case of pancreatic malformations. The intent was to review the latest information published in the national and international literature on pancreatic birth defects, and to investigate the diversity of clinical presentations of ADP and other congenital pancreas abnormalities. The purpose was to identify which situations have therapeutic indication, the most appropriate time to institute treatment, and the currently available medical or surgical treatment of pancreatic congenital malformations. RESULTS: ADP is a very rare malformation that occurs during organogenesis. In the last decades, a large volume of embryological and genetic information has been obtained, helping to understand the causes of pancreatic malformations, which must be studied and understood as a whole. CONCLUSION: Pancreatic malformations are infrequently studied causes of acute and chronic pancreatic in adults. The possibility of pancreatic malformations should always be considered in patients with acute or chronic pancreatitis with no evident cause.

BACKGROUND: the narrative abilities provide valuable information about the linguistic, cognitive and social development of school-age children with typical language development (TLD). AIM: to examine the temporal ordering of figures and the narrative abilities of school-age children with TLD in relation to the type of discourse presented. METHOD: participants of this study included 60 children with TLD aged between seven and ten years of age. Fifteen stories were used in the study. Each story was illustrated by four figures. The sequences of figures were created and classified as mechanical, behavioral and intentional according to the relationships presented among the characters. Data were transcribed and analyzed regarding the type of discourse (descriptive, causal and intentional) and the type of organization of the figures. RESULTS: no differences between age groups were observed for temporal ordering. For all age groups, the most frequently presented discourse type was the causal one. Statistically significant differences were observed among age groups for the causal and intentional discourse type. Inasmuch as the age increased, school-age children with TLD reduced the use of the descriptive discourse and increased the use of the intentional one. CONCLUSION: the ability of temporal ordering is already developed in children with TLD at seven-years of age. The type of discourse was influenced by age and by the type of story presented.

RESUMO

BACKGROUND: children with Specific Language Impairment (SLI) have difficulties with speech processing. These difficulties affect the development of phonologic representations. AIM: to evaluate the abilities of children with normal language development (NLD) and those with SLI in distinguishing words from non-words in a lexical decision task. METHOD: two groups were involved in this study: the Control Group (GC), with no language disorders, composed by 36 subjects, and the Research Group (RG), with 18 subjects, all diagnosed with SLI, aging form 4 to 8:9 years. Children from both groups were arranged in three subgroups, according to the receptive vocabulary. Forty eight three syllable words were selected, being 24 real words and 24 that were manipulated in order to obtain non-words. Three variables were considered: (a) modification extension, (b) modification positioning and (c) modification type. Children had to decide whether a phonological sequence consisted of a word or a non-word. RESULTS: even though children were matched by lexical age, there were differences between GC and RG. The RG presented more difficulty in lexical decision, not only for words but also for non-words. Both groups, with lexical age of 4 years, struggled more in this task when compared with groups with lexical age of 5 and 6 years. CONCLUSION: children with SLI presented deficit in phonological representation when compared with children with NLD. This difference in performance can be explained by differences in the formation and retention of working memory representations, auditory discrimination and motor planning and execution.

BACKGROUND: children with Specific Language Impairment (SLI) show lexical deficits as the first noticeable sign of such disorder, characterized as difficulties in lexical access during naming and speech tests. Studies that compare picture naming and drawings seem perfect to clarify lexical deficits. AIM: to compare the performance of children with normal language development (NLD) to that of children with SLI in naming, drawing and definition tasks, aiming to explore the the quality of semantic representation of the lexicon. METHOD: Two groups were involved in this study: the Control Group (CG), with no language disorders, composed by 40 subjects, and the Research Group (RG), with 20 subjects, all diagnosed with SLI, aging from five to seven years. Tasks of naming, picture drawing and definition were performed, using 20 different pictures. In the naming task, the types of errors were analyzed and sorted as follows: semantic, phonological, none specified and others. The analysis of the drawing and definition tasks was based only on the correct answers, semantic and none specified errors. RESULTS: children of the RG presented a greater number of semantic errors in the picture naming task when compared to the CG. Besides that, definitions presented by the RG seemed more simple and incomplete even when the child was capable of naming the picture correctly. Drawings of correctly named objects were better than those that were named incorrectly. CONCLUSIONS: it was possible to discriminate within SLI children those that present greater lexical deficits. It was also possible to explore the possible reasons for failures in naming tasks.
