RESUMO

The World Health Organization developed the "iSupport for Dementia" program for family caregivers of people with dementia. Objective: To explore studies on adaptation, randomized clinical trial protocols, and preliminary results of iSupport by unpaid caregivers of people living with dementia in different countries. Methods: Systematic review. Results: Ten cultural adaptation studies, eight randomized clinical trial protocols, and two preliminary results were included. Adaptation studies showed adjustments in terminology, design, and additional resources. Clinical trial protocols included burden as the primary outcome, and baseline, three months of intervention, and follow-up after six months. Studies with preliminary results found positive effects on the mental health and well-being of caregivers after using the program. Conclusion: iSupport is an online program of the World Health Organization in response to dementia in implementation in different countries.


A Organização Mundial da Saúde desenvolveu o programa "iSupport for Dementia" para cuidadores familiares de pessoas com demência. Objetivo: Explorar estudos sobre adaptação, protocolos de ensaio clínico randomizado e resultados preliminares do "iSupport" por cuidadores não remunerados de pessoas que vivem com demência em diferentes países. Métodos: Revisão sistemática. Resultados: Foram incluídos dez estudos de adaptação cultural, oito protocolos de Ensaio Clínico Randomizado e dois resultados preliminares. Os estudos de adaptação apresentaram os ajustes nas terminologias, design e recursos adicionais. Os protocolos de ensaio clínico incluíram a sobrecarga como desfecho primário, e com linha de base, três meses de intervenção e acompanhamento após seis meses. Os estudos com resultados preliminares encontraram efeitos positivos na saúde mental e bem-estar dos cuidadores após o uso do programa. Conclusão: O iSupport é um programa online da Organização Mundial da Saúde em resposta à demência na implantação em diferentes países.

BACKGROUND: In Brazil, Leishmania (Leishmania) infantum is a widely distributed protozoan parasite. The human leishmaniasis caused by this species is often associated with visceral form. Tegumentary leishmaniasis (TL) cases due to L. (L.) infantum in the country are considered rare but may be underestimated. Although probably uncommon, these cases represent a new challenge to the prevention and control of leishmaniasis. OBJECTIVES: Here, we describe two distinct cases of TL with atypical clinical presentations caused by L. (L.) infantum. METHODS AND FINDINGS: Parasites were isolated from cutaneous lesions of the two patients and typed as L. (L.) infantum after sequencing of the ribosomal DNA internal transcribed spacer. The dermotropic L. (L.) infantum isolates were compared in terms of growth culture patterns, metacyclogenesis and in vitro infectivity in macrophages. MAIN CONCLUSIONS: This study addresses the emergence of L. (L.) infantum as a causative agent of cutaneous disease in a visceral leishmaniasis hotspot located in northeast Brazil. The data presented provides novel information about the presence of dermotropic L. (L.) infantum in the country and demonstrates the infectivity potential of theses isolates.

Apesar de predominante no ambiente de trabalho, a Síndrome de Burnout (SB) torna-se cada vez mais frequente no ambiente universitário. Com objetivo de verificar a prevalência da SB entre acadêmicos e correlacionar essa variável com aspectos sociodemográficos e de saúde, esse estudo transversal foi conduzido com uma amostra composta por 72 participantes. O protocolo de avaliação foi composto por um questionário sociodemográfico, avaliação da Síndrome de Burnout, de sintomas depressivos, saúde geral, aspectos sociais e sintomas osteomusculares. A correlação de Spearman foi utilizada para análise estatística. Houve predomínio da SB entre a amostra (65,2%). Observou-se correlações significativas entre aspectos sociais e descrença (p=0.016; r=-0.283), eficácia estudantil (p=<0.001; r=-0.399) e sintomas depressivos (p=<0.001; r=-0.547), bem como entre a saúde geral e sintomas depressivos (p=0.012; r=-0.295). Estabelecer esse tipo de medida permite planejar ações voltadas à assistência estudantil, priorizando intervenções e práticas preventivas que colaborem com a promoção da saúde no ambiente universitário.


Despite being predominant in the workplace, Burnout Syndrome (BS) is becoming more and more frequent in the university environment. With the objective of verifying the prevalence of BS among students and correlating this variable with sociodemographic and health aspects, this cross-sectional study was conducted with a sample of 72 participants. The evaluation protocol consisted of a sociodemographic questionnaire, evaluation of Burnout Syndrome, depressive symptoms, general health, social aspects, and musculoskeletal symptoms. Spearman's correlation was used for statistical analysis. There was a predominance of BS among the sample (65.2%). Significant correlations were observed between social aspects and disbelief (p=0.016; r=-0.283), student efficacy (p=<0.001; r=-0.399) and depressive symptoms (p=<0.001; r=-0.547), as well as between general health and depressive symptoms (p=0.012; r=-0.295). Establishing this type of measure allows planning actions aimed at student assistance, prioritizing interventions and preventive practices that collaborate with health promotion in the university environment.


A pesar de ser prevalente en el ámbito laboral, el Síndrome de Burnout (SB) es cada vez más frecuente en el ámbito universitario. Con el objetivo de verificar la prevalencia de SB entre los estudiantes y correlacionar esta variable con aspectos sociodemográficos y de salud, este estudio transversal se realizó con una muestra de 72 participantes. El protocolo de evaluación consistió en un cuestionario sociodemográfico, evaluación del Síndrome de Burnout, síntomas depresivos, salud general, aspectos sociales y síntomas musculoesqueléticos. Para el análisis estadístico se utilizó la correlación de Spearman. Hubo predominio de SB en la muestra (65,2%). Se observaron correlaciones significativas entre los aspectos sociales y la incredulidad (p=0.016; r=-0.283), la eficacia acadêmica (p=<0.001; r=-0.399) y síntomas depresivos (p=<0.001; r=-0.547), así como entre la salud general y los síntomas depresivos (p=0.012; r=-0.295). Establecer este tipo de medidas permite planificar acciones dirigidas a la asistencia estudiantil, priorizar intervenciones y prácticas que colaboren con la promoción de la salud en el ámbito universitario.

BACKGROUND: The Positive Aspects of Caregiving (PAC) scale is used to assess psychosocial benefits provided to caregivers by the task of caring. The PAC scale consists of nine items, assessed using a five-point Likert scale, with higher values indicating greater positive perceptions and gains from the caregiving experience. OBJECTIVE: To translate and culturally adapt the PAC scale for informal Brazilian caregivers of people with dementia. DESIGN AND SETTING: A methodological study was conducted at the Federal University of São Carlos. METHODS: The following stages were carried out: Translation; Synthesis of the translations; Back-translation; Evaluation by an experts' committee; and Pre-test. RESULTS: Two independent professionals translated the PAC scale. The consensus version was obtained by merging both translations, which were back-translated into English by a third translator. The expert committee comprised three specialists in the area and project researchers. All scale items presented a Content Validity Index of 1 (CVI = 1.0), and thus remained in the pre-final version of the instrument. The instrument was pre-tested with seven caregivers of people with dementia, the majority of whom were women (57.1%), with a degree of kinship corresponding to sons/daughters (57.1%) and an average age of 55.2 (± 4.1) years. The caregivers considered it clear and understandable and made no suggestions for changes. CONCLUSION: The PAC scale was translated and culturally adapted for use by informal caregivers of people with dementia in Brazil. However, a psychometric analysis of the instrument is necessary to provide normative data for this population group.

ABSTRACT The sense of mastery is conceptualized as a positive aspect of care targeted at people living with dementia, a coping mechanism to reduce burden, and may represent a protective factor for caregivers' mental and physical health. Objective: To translate and culturally adapt the Pearlin Mastery Scale for Brazil. Methods: A methodological study was conducted at the Federal University of São Carlos in which the initial translation stages were followed; synthesis; back-translation; review by the committee of judges by analyzing the Content Validity Index (CVI); and test of the pre-final version. Results: Two specialists translated the scale into Brazilian Portuguese and defined a consensus version with the researchers. Subsequently, another two specialists back-translated the consensus version, which was reviewed by three judges who are PhDs in the area, considering all scale items as very equivalent (CVI=1.0), and maintaining them in the pre-final version of the instrument. This was tested in a first group of caregivers for them to point out adjustments. The suggestions were accepted by modifying three items and, afterward, the scale was tested in a second group, which did not present difficulties answering the instrument. Conclusion: The Pearlin Mastery Scale was translated and culturally adapted for Brazil, showing equivalence. However, future psychometric analyses of the instrument are required to make it available for use in this population.


RESUMO O senso de domínio é conceituado como um aspecto positivo do cuidado voltado às pessoas que vivem com demência, um mecanismo de enfrentamento para reduzir a sobrecarga e que pode representar um fator protetor para a saúde mental e física do cuidador. Objetivo: Traduzir e adaptar culturalmente a Pearlin Mastery Scale para o Brasil. Métodos: Estudo metodológico conduzido na Universidade Federal de São Carlos, em que foram seguidas as etapas de tradução inicial; síntese; retrotradução; revisão pelo comitê de juízes pela análise do Índice de Validade de Conteúdo (IVC); e teste da versão pré-final. Resultados: Dois especialistas traduziram a escala para o português brasileiro e definiram uma versão consensual com os pesquisadores. Posteriormente, outros dois especialistas retrotraduziram a versão consensual, que foi revisada por três juízes doutores na área, considerando todos os itens da escala como muito equivalentes (IVC=1,0), mantendo-os na versão pré-final do instrumento. Esta foi testada em um primeiro grupo de cuidadores, a fim de se apontarem adequações. As sugestões foram acatadas mediante a modificação de três itens e, depois, a escala foi testada em um segundo grupo, o qual não apresentou dificuldades em responder ao instrumento. Conclusão: A Pearlin Mastery Scale foi traduzida e adaptada culturalmente para o Brasil, demonstrando equivalência. Entretanto, análises psicométricas futuras do instrumento são necessárias para disponibilizá-lo para uso entre essa população.

Resumo Objetivo Investigar a prevalência de polifarmácia e os seus fatores associados no espaço da Atenção Primária à Saúde em Caicó (RN, Brasil). Método Trata-se de um estudo de prevalência de abordagem quantitativa com amostra final de 295 pessoas idosas. Resultados A prevalência de polifarmácia foi de 22%. A maioria dos participantes tinha entre 60 e 79 anos (76%), cor parda (50,5%), era casada (43,1%), ensino fundamental incompleto (60,3%) e renda entre um e três salários-mínimos (74,2%). Os fármacos mais utilizados foram os bloqueadores dos receptores de angiotensina (26,1%), as estatinas (20,3%) e os diuréticos tiazídicos (19,3%). O modelo de regressão logística multivariada mostrou que a polifarmácia se associa com diabetes mellitus, hipertensão arterial sistêmica, transtorno de ansiedade generalizada, gastrite, insuficiência cardíaca, doença arterial coronariana e idade acima de 80 anos. Conclusão A polifarmácia é uma condição de alta prevalência, fortemente associada ao aumento da idade, do tipo e do número de comorbidades. A problemática aponta para a necessidade de fortalecer ações e políticas de promoção da saúde, educação profissional permanente, fortalecimento do trabalho em equipe e prestação de cuidado longitudinal e em rede, em vista do risco iatrogênico do uso de muitos medicamentos por parte dos idosos.


Abstract Objective To investigate the prevalence of polypharmacy and its associated factors in the Primary Care setting of Caicó city (Rio Grande do Norte state, Brazil). Method A quantitative prevalence study of 295 older adults was conducted. Results The prevalence of polypharmacy was 22%. Participants were predominantly aged 60-79 years (76%), brown (50.5%), married (43.1%), educated to primary (incomplete) level (60.3%) and had an income of 1-3 minimum wages (74.2%). The most used medications were angiotensin-receptor blockers (26.1%), statins (20.3%) and thiazide diuretics (19.3%). The multivariate logistic regression model showed that polypharmacy was associated with diabetes mellitus, systemic arterial hypertension, generalized anxiety disorder, gastritis, heart failure, coronary artery disease and age >80 years. Conclusion Polypharmacy prevalence was high and strongly associated with advanced age and number and type of comorbidity. These results highlight the need for greater health promotion actions and policies, continuous professional education, enhanced team work and longitudinal systemic care provision to address the iatrogenic risk of use of multiple medications by older adults.

Unpaid caregivers of people living with dementia tend to suffer mental health problems as a result of the negative effects associated with the care tasks. Thus, psychosocial interventions for this population group are necessary. iSupport is an online support program for caregivers that was created by the World Health Organization. Objective: To describe the design of a randomized clinical trial to measure the efficacy of the iSupport-Brasil version on caregivers' mental health and well-being. Methods: The participants will be randomized into Intervention Group (IG) (n=195) and Control Group (CG) (n=195). For three months, the IG will access the iSupport-Brasil platform, the CG will enter the electronic page of the Brazilian Alzheimer's Association, and both groups will be emailed the preliminary version of the "Guia de cuidados para a pessoa idosa" e-book (a guide to providing care to the elderly) from the Ministry of Health. The data will be collected at three moments: baseline, and three and six months after the beginning of the intervention. Results: It is expected that it will be possible to provide diverse validity evidence about iSupport-Brasil as an online and free intervention alternative, as a preventive means and as a way to promote mental health among caregivers of people living with dementia. Conclusion: Through the evaluation protocol of this randomized clinical trial on the effects of the iSupport-Brasil program, it may become a reference for countries that plan to adapt and improve the iSupport program using digital health solutions.


Cuidadores informais de pessoas que vivem com demência tendem a sofrer problemas de saúde mental como resultado dos efeitos negativos associados às tarefas de cuidado. Assim, intervenções psicossociais para essa população são necessárias. O iSupport é um programa de apoio online para cuidadores criado pela Organização Mundial da Saúde. Objetivo: Descrever o desenho de um ensaio clínico randomizado para medir a eficácia da versão iSupport-Brasil na saúde mental e no bem-estar de cuidadores. Métodos: Os participantes serão randomizados em Grupo Intervenção (GI) (n=195) e Grupo Controle (GC) (n=195). Durante três meses, o GI acessará a plataforma iSupport-Brasil, o CG entrará na página eletrônica da Associação Brasileira de Alzheimer e ambos os grupos receberão por e-mail a versão preliminar do e-book Guia de cuidados para a pessoa idosa, do Ministério da Saúde. Os dados serão coletados em três momentos: linha de base, três e seis meses após o início da intervenção. Resultados: Espera-se que seja possível fornecer diversas evidências sobre os efeitos do iSupport-Brasil como alternativa de intervenção online e gratuita, de forma a promover a saúde mental entre os cuidadores de pessoas que vivem com demência. Conclusão: O protocolo de avaliação deste ensaio clínico randomizado sobre os efeitos do programa iSupport-Brasil pode se tornar uma referência para os países que planejam adaptar e melhorar esta intervenção, usando soluções digitais de saúde.

Objetivo: analisar a associação entre as características de idosos e cuidadores e seus impactos na sobrecarga dos cuidadores. Método: em 2021, foram avaliados 50 pares de cuidadores informais e idosos pelo perfil sociodemográfico, de saúde, Escala Hospitalar de Ansiedade e Depressão (HADS), Entrevista de Sobrecarga de Zarit-Brief (ZBI-12) e informações sobre grau de dependência dos idosos para atividades básicas e instrumentais (ABVD e AIVD). Modelos de regressão avaliaram os escores ZBI-12 e HADS em função das características dos cuidadores e idosos assistidos. O protocolo de pesquisa foi aprovado pelo Comitê de Ética em Pesquisa. Resultados: alta morbidade e dependência para ABVD foram significativamente associadas a maiores níveis de sobrecarga do cuidador. Essas características perderam significância quando ajustado o modelo por idade do cuidador, tempo que presta o cuidado e convivência com o idoso. Conclusão: esses resultados revelam a necessidade de intervenções para garantir suporte aos cuidadores quanto a diminuição da sobrecarga relacionada ao cuidado(AU)


Objective: to analyze the association between the characteristics of elderly people and caregivers and their impact on caregiver burden. Method: in 2021, 50 pairs of informal caregivers and their elderly were evaluated through the sociodemographic and health profile, Hospital Anxiety and Depression Scale (HADS), Zarit-Brief Burden Interview (ZBI-12), and information on the degree of dependence of the elderly for basic and instrumental activities (BADL and IADL). Regression models evaluated ZBI-12 and HADS scores as a function of characteristics of caregivers and assisted elderly. The research protocol was approved by the Research Ethics Committee. Results: high morbidity and BADL dependence were significantly associated with higher levels of caregiver burden. These characteristics lost significance when we adjusted the model for the caregiver's age, time that provides care, and living with the elderly. Conclusion: these results reveal the need for interventions to guarantee support for caregivers in terms of reducing the burden related to the care(AU)


Objetivo: analizar la asociación entre las características de ancianos y sus cuidadores y el correspondiente impacto en la sobrecarga de los cuidadores. Método: en 2021, se evaluaron 50 pares de cuidadores informales y sus ancianos a través del perfil sociodemográfico, de salud, Escala de Ansiedad y Depresión Hospitalaria (HADS), Escala de Zarit Reducida para la Sobrecarga (ZBI-12) e información sobre el grado de dependencia de los ancianos respecto a actividades básicas e instrumentales (ABVD y AIVD). Modelos de regresión evaluaron las puntuaciones de ZBI-12 y HADS en función de las características de los cuidadores y ancianos asistidos. El Comité de Ética en Investigación aprobó protocolo de investigación. Resultados: la alta morbilidad y la dependencia de BADL se asociaron significativamente con niveles más altos de sobrecarga del cuidador. Estas características perdieron su importancia cuando ajustamos el modelo por la edad del cuidador, tiempo que proporciona cuidados y convivencia con los ancianos. Conclusión: estos resultados revelan la necesidad de intervenciones para garantizar apoyo a los cuidadores para reducir la sobrecarga relacionada con el cuidado(AU)

BACKGROUND: Otological complaints (OC) are highly prevalent in subjects with temporomandibular disorders (TMD) and so is the risk of neck dysfunctions. OBJECTIVE: To evaluate pain, deep neck flexor (DNF) performance, disability, and head and neck posture of individuals with TMD with and without OC. METHODS: In this cross-sectional study, 57 individuals were divided into a group with TMD and OC (n= 31) and a group with TMD without OC (n= 26). Self-reported pain intensity, masticatory and neck muscles pressure pain thresholds, DNF performance, neck disability, and head and neck posture were evaluated. Data were compared between groups using the independent t test and Mann-Whitney test with Bonferroni correction for multiple comparisons. Effect sizes were evaluated using Cohen's index. RESULTS: The TMD with OC group presented less muscle activation [26 (24-28) vs. 24 (24-26) mmHg; p< 0.05], less endurance [105 (46-140) vs. 44 (28-78) points; p< 0.05], and greater neck disability (8.15 ± 5.89 vs. 13.32 ± 6.36 points; p< 0.05). No significant difference was observed in self-reported pain, head and neck posture, or pressure pain thresholds. CONCLUSION: Individuals with TMD with OC presented decreased DNF performance and increased neck disability compared to individuals with TMD without OC.

OBJECTIVE: To evaluate current studies to establish and compare the efficacy of traditional and laser acupuncture in reducing the signs and symptoms of temporomandibular disorders (TMD). METHODS: PubMed, Cochrane, Scopus, and Web of Science databases were searched. Clinical, controlled, and randomized trials written in English and having used traditional or laser acupuncture as therapy for TMD and RDC/TMD or DC/TMD as a diagnostic criterion for TMD were included. RESULTS: Six studies that evaluated the intensity of pain and the level of mouth opening of the patients submitted to acupuncture were selected, and all showed improvement. However, similar results were also observed in the groups treated with occlusal splint and placebo acupuncture. Only one study evaluated laser acupuncture and showed a higher proportion of patients with remission of symptoms in the experimental group. CONCLUSION: The traditional acupuncture seems to relieve the signs and symptoms of TMD, as well as laser acupuncture when associated with occlusal splint. However, more rigorous and high-quality clinical trials are needed.

ABSTRACT. Unpaid caregivers of people living with dementia tend to suffer mental health problems as a result of the negative effects associated with the care tasks. Thus, psychosocial interventions for this population group are necessary. iSupport is an online support program for caregivers that was created by the World Health Organization. Objective: To describe the design of a randomized clinical trial to measure the efficacy of the iSupport-Brasil version on caregivers' mental health and well-being. Methods: The participants will be randomized into Intervention Group (IG) (n=195) and Control Group (CG) (n=195). For three months, the IG will access the iSupport-Brasil platform, the CG will enter the electronic page of the Brazilian Alzheimer's Association, and both groups will be emailed the preliminary version of the "Guia de cuidados para a pessoa idosa" e-book (a guide to providing care to the elderly) from the Ministry of Health. The data will be collected at three moments: baseline, and three and six months after the beginning of the intervention. Results: It is expected that it will be possible to provide diverse validity evidence about iSupport-Brasil as an online and free intervention alternative, as a preventive means and as a way to promote mental health among caregivers of people living with dementia. Conclusion: Through the evaluation protocol of this randomized clinical trial on the effects of the iSupport-Brasil program, it may become a reference for countries that plan to adapt and improve the iSupport program using digital health solutions.


RESUMO. Cuidadores informais de pessoas que vivem com demência tendem a sofrer problemas de saúde mental como resultado dos efeitos negativos associados às tarefas de cuidado. Assim, intervenções psicossociais para essa população são necessárias. O iSupport é um programa de apoio online para cuidadores criado pela Organização Mundial da Saúde. Objetivo: Descrever o desenho de um ensaio clínico randomizado para medir a eficácia da versão iSupport-Brasil na saúde mental e no bem-estar de cuidadores. Métodos: Os participantes serão randomizados em Grupo Intervenção (GI) (n=195) e Grupo Controle (GC) (n=195). Durante três meses, o GI acessará a plataforma iSupport-Brasil, o CG entrará na página eletrônica da Associação Brasileira de Alzheimer e ambos os grupos receberão por e-mail a versão preliminar do e-book Guia de cuidados para a pessoa idosa, do Ministério da Saúde. Os dados serão coletados em três momentos: linha de base, três e seis meses após o início da intervenção. Resultados: Espera-se que seja possível fornecer diversas evidências sobre os efeitos do iSupport-Brasil como alternativa de intervenção online e gratuita, de forma a promover a saúde mental entre os cuidadores de pessoas que vivem com demência. Conclusão: O protocolo de avaliação deste ensaio clínico randomizado sobre os efeitos do programa iSupport-Brasil pode se tornar uma referência para os países que planejam adaptar e melhorar esta intervenção, usando soluções digitais de saúde.

Solid lipid nanoparticles (SLNs) containing rutin were prepared to enhance their photochemopreventive effect on the skin. SLNs were produced by the hot melt microemulsion technique. Two 3D skin models: ex vivo skin explants and 3D tissue engineering skin were used to evaluate the photochemopreventive effect of topical formulations containing rutin SLNs, against ultraviolet B (UVB) radiation, inducing sunburn cells, caspase-3, cyclobutane pyrimidine dimers, lipid peroxidation, and metalloproteinase formation. The rutin SLNs presented average size of 74.22 ± 2.77 nm, polydispersion index of 0.16 ± 0.04, encapsulation efficiency of 98.90 ± 0.25%, and zeta potential of -53.0 ± 1.61 mV. The rutin SLNs were able to efficiently protect against UVB induced in the analysed parameters in both skin models. Furthermore, the rutin SLNs inhibited lipid peroxidation and metalloproteinase formation. These results support the use of rutin SLNs as skin photochemopreventive agents for topical application to the skin.

OBJECTIVES: to analyze the personal and work-related burden factors associated with physical and emotional symptoms of informal caregivers of the elderly. METHODS: cross-sectional study conducted with 121 informal caregivers and 121 seniors who received care, assessed individually for the risk of: physical overload, musculoskeletal symptoms, Self-Reporting Questionnaire, effort perception, and Katz index. RESULTS: a greater perception of effort raises up to 3.3 times the chances of presenting symptoms of pain in the spine region (p=0.01), and lower functional capacity of the elderly increases up to 1.3 times the chances of presenting pain symptoms in the spine region (p=0.02). The symptoms of emotional overload were associated with the caregiver's low income (p=0.02). CONCLUSIONS: the perception of effort, dependence of the elderly, caregiver's age, and symptoms of emotional overload are involved with caregivers' symptoms of physical overload, and low income, with emotional overload.

BACKGROUND: Leishmaniases are diseases caused by Leishmania protozoans that affect around 12 million people. Leishmania promastigotes are transmitted to vertebrates by female phlebotomine flies during their blood meal. Parasites attach to phagocytic cells, are phagocytosed and differentiate into amastigotes. We previously showed that PH8 and LV79 strains of Leishmania amazonensis have different virulence in mice and that their amastigotes differ in their proteomes. In this work, we compare promastigotes' infectivity in macrophages, their proteomes and morphologies. METHODS/PRINCIPAL FINDINGS: Phagocytosis assays showed that promastigotes adhesion to and phagocytosis by macrophages is higher in PH8 than LV79. To identify proteins that differ between the two strains and that may eventually contribute for these differences we used a label-free proteomic approach to compare promastigote´s membrane-enriched fractions. Proteomic analysis enabled precise discrimination of PH8 and LV79 protein profiles and the identification of several differentially abundant proteins. The proteins more abundant in LV79 promastigotes participate mainly in translation and amino acid and nucleotide metabolism, while the more abundant in PH8 are involved in carbohydrate metabolism, cytoskeleton composition and vesicle/membrane trafficking. Interestingly, although the virulence factor GP63 was more abundant in the less virulent LV79 strain, zymography suggests a higher protease activity in PH8. Enolase, which may be related to virulence, was more abundant in PH8 promastigotes. Unexpectedly, flow cytometry and morphometric analysis indicate higher abundance of metacyclics in LV79. CONCLUSIONS/SIGNIFICANCE: Proteome comparison of PH8 and LV79 promastigotes generated a list of differential proteins, some of which may be further prospected to affect the infectivity of promastigotes. Although proteomic profile of PH8 includes more proteins characteristic of metacyclics, flow cytometry and morphometric analysis indicate a higher abundance of metacyclics in LV79 cultures. These results shed light to the gaps in our knowledge of metacyclogenesis in L. amazonensis, and to proteins that should be studied in the context of infection by this species.

OBJECTIVE: To assess the level of agreement between the Subjective Frailty Assessment (SFA) and Clinical Functional Vulnerability Index (CFVI-20) for the diagnosis of frailty in older adults. METHODS: A descriptive, comparative, cross-sectional study was conducted in 2018/2019 with 492 older adults at nine Family Health Strategy units in the city of Três Lagoas, state of Mato Grosso do Sul, Brazil. Frailty was assessed using the SFA and CFVI-20 in addition to a structured interview. The Wilcoxon test and Pearson's chi-squared test were used for the comparisons. RESULTS: Agreement was 35.4%, with a Kappa coefficient of 0.11. After dichotomizing the sample into frail and non-frail individuals, agreement was 70.1%, with a Kappa coefficient of 0.41 and Cronbach's alpha coefficients of 0.61 and 0.74 for IVCF-20 and SFA, respectively. The prevalence of frailty was lower using the IVCF-20 (17.1%) compared to 59.8% using the SFA. CONCLUSION: Agreement regarding the classification of frailty between the two instruments ranged from low to moderate. This finding underscores the need for a standardized instrument for measuring frailty in community-dwelling older adults.

Leishmania spp. are parasitic protozoa that cause leishmaniasis, a disease endemic in 98 countries. Leishmania promastigotes are transmitted by the vector and differentiate into amastigotes within phagocytic cells of the vertebrate host. To survive in multiple and hostile environments, the parasite has several virulence factors. Oligopeptidase B (OPB) is a serine peptidase present in prokaryotes, some eukaryotes and some higher plants. It has been considered a virulence factor in trypanosomatids, but only a few studies, performed with Old World species, analysed its role in Leishmania virulence or infectivity.L. (L.) amazonensis is an important agent of cutaneous leishmaniasis in Brazil. The L. (L.) amazonensis OPB encoding gene has been sequenced and analysed in silico but has never been expressed. In this work, we produced recombinant L. (L.) amazonensis OPB and showed that its pH preferences, Km and inhibition patterns are similar to those reported for L. (L.) major and L. (L.) donovani OPBs. Since Leishmania is known to secrete OPB, we performed in vitro infection assays using the recombinant enzyme. Our results showed that active OPB increased in vitro infection by L. (L.) amazonensis when present before and throughout infection. Our findings suggest that OPB is relevant to L. (L.) amazonensis infection, and that potential drugs acting through OPB will probably be effective for Old and New World Leishmania species. OPB inhibitors may eventually be explored for leishmaniasis chemotherapy.

The present paper provides an overview of the state-of-the-art research, outlining the applications of the Industry 4.0 (I4.0) technologies on the aircraft manufacturing sector and their maturity state based on the technology readiness level (TRL) scale. A literature review has been conducted for the identification, selection, and evaluation of the published research. A total of 57 papers extracted from the two most relevant scientific databases for the area (Web of Science and Scopus), from 2010 to March 2021, were analysed and summarized. The research, analysis, and evaluation of these papers has provided an outlook of how the aircraft manufacturing industry is inserted into the I4.0 context, based on a classification of the I4.0 technologies maturity for this industrial branch. Then, a survey was performed with 12 specialists from 5 different aircraft manufacturing companies aiming to report the practical point-of-view in this area. Thus, this paper highlights and discusses the gaps found in the literature related to the I4.0 technologies applied to aircraft manufacturing and their main useful implications not only from the academic point-of-view but also from competitive business aspects, providing recommendations for industrial managers, engineers, and stakeholders. Finally, this paper proposes new opportunities and challenges for future research.

OBJECTIVE: To evaluate neck pain, disability, and deep neck flexor (DNF) performance of individuals with temporomandibular disorders (TMD). METHODS: Eighty individuals were divided into the following: arthrogenic TMD (n = 40), myogenic TMD (n = 12), and mixed TMD (n = 28). Neck pain intensity, neck disability, and DNF performance were evaluated. RESULTS: Individuals with arthrogenic TMD reported lower intensity of neck pain when compared to mixed TMD (p = 0.01). Individuals with arthrogenic TMD had less neck disability than individuals with myogenic TMD (p = 0.037) and mixed TMD (p < 0.001). A moderate positive correlation was found between neck pain and neck disability (p < 0.001). No differences were found for DNF performance. CONCLUSION: Neck pain and disability differs according to subtype of TMD, but performance of the deep neck flexors does not. Neck pain intensity and neck disability were correlated in patients with TMD.
