RESUMO

Brazil nuts are the second most frequent cause of nut allergy in the United Kingdom. We report the case of a 20-year-old woman with documented Brazil nut allergy who developed widespread urticaria and mild dyspnea after intercourse with her boyfriend who had earlier consumed Brazil nuts. Skin prick testing with the boyfriend's semen after Brazil nut consumption confirmed significant reactivity whereas a sample before nut consumption was negative. We believe this to be the first case of a sexually transmitted allergic reaction.

RESUMO

Motor aphasia complicating bacteriologically confirmed typhoid fever in a 20-year-old adult female is presented. Neither the cause of death nor the aetiology of this isolated neurological deficit could be determined from detailed post-mortem examination. An immune related mechanism is suggested based on the temporal characteristics of this complication and CSF abnormalities.

RESUMO

Thirty cases of infective endocarditis identified in the autopsy service of the University Hospital over a 10-year period are reviewed. The mean age of the patients was 30.6 years, and 70% were below the age of 40 years. Sixty per cent of the cases had previous heart disease of which rheumatic heart disease was the most common. Degenerative heart disease and floppy mitral valve contributed one case each. The mitral valve was most commonly affected, and there were only 2 cases of right-sided endocarditis. Evidence of systemic embolization was present in 83% of the patients. The results of this study are compared with others from both developed and under-developed countries.

RESUMO

Thirty-one cases of histologically confirmed chromomycosis seen at the University Hospital of the West Indies (UHWI) between 1967 and 1986 were reviewed. A mean age of 56 years and a 5 to 1 male to female ratio were recorded. The lower limb was infected most frequently and both the macroscopic and microscopic disease showed typical features. Fonsecaea species were isolated in all 16 cases in whom mycological information was available (14 cases of F. pedrosoi). Surgical excision and split skin grafting produced long term cure in 7 to 10 cases with limited disease. Chromomycosis is more common in Jamaica than previous records would suggest.

RESUMO

The in-vitro sensitivity to ampicillin, cotrimoxazole, nitrofuratoin, nalidixic acid and mecillinam was determined for 511 organisms isolated from 399 consecutive urine specimens. Urine specimens were divided into those of hospital in-patient origin (group B). Group B organisms were more sensitive than group A organisms. Over 75 percent of all group B organisms were sensitive to nitrofurantoin, nalidixic acid and mecillinam. Organisms resistant to multiple antibiotics were more frequently isolated from group A catheterized patients and are now less frequently isolated than in 1983. The antibiotic implications of these findings are discussed (AU)

RESUMO

Data are presented on 13 immunocompromised patients with herpes simplex virus (HSV) infection. Eleven patients had recurrent HSV infection. Eight had genital herpes, 3 had disseminated infection and two had localised extragenital infection. Nine patients had confirmed recurrent HSV-type 2 infection. The benefits of recent antiherpetic therapy in the prevention of recurrent HSV infections in immunocompromised Jamaican patients are discussed (AU)

RESUMO

Twenty-four cases of pyogenic liver abscess admitted between 1977 and 1986 are presented. A mean age of 43 years (range 5-78) with a 3:1 male:female ratio and 25% mortality were noted. Fever and abdominal pain were encountered in over 80% of cases and anorexia and malaise in over 60%. Hepatomegaly and right upper quadrant tenderness were the commonest signs. Leucocytosis, raised alkaline phosphatase and gamma-glutamyl transpeptidase, and hypoalbuminaemia were each noted in roughly 80% of cases. None of these showed any prognostic significance. Predisposing factors were noted in 11 cases. No cases of associated biliary disease were noted. Multiple, polymicrobial, aerobic and mixed aerobic/anaerobic abscesses were associated with a higher mortality. Patients aged over 50 years or more also had a higher mortality (P less than 0.05). Anaerobic abscesses were often solitary and were associated with a lower mortality (P less than 0.05). Surgical drainage and guided percutaneous drainage showed no difference in morbidity.
