Background: Free flap reconstruction in acute burns is high risk but often required for limb salvage and coverage of vital structures. Prior studies have shown a flap loss rate up to 44%. This study aimed to compare the complications associated with muscle and fasciocutaneous free flaps in acute burn reconstruction. Methods: A systematic review and meta-analysis was conducted according to PRISMA guidelines and registered on the PROSPERO database (CDR42023471088). The databases accessed were Embase, PubMed, Web of Science, and Cochrane Library. The primary outcome was free flap failure rate based on flap type. Secondary outcomes included venous congestion, arterial thrombosis, amputation, and need for reintervention. Results: Twelve studies with 181 free flaps were included: 87 muscle flaps and 94 fasciocutaneous flaps. Muscle flaps had a higher risk ratio (RR) for total flap loss [RR: 2.32, 95% confidence interval (CI): 1.01-5.32, P = 0.04], arterial thrombosis (RR: 3.13, 95% CI: 1.17-8.42, P = 0.02), and amputations (RR: 8.89, 95% CI: 1.27-70.13, P = 0.03) compared with fasciocutaneous flaps. No significant differences were found in venous thrombosis (RR: 1.33, 95% CI: 0.37-4.78, P = 0.65) or need for reinterventions (RR: 1.34, 95% CI: 0.77-2.32, P = 0.29). Conclusions: Muscle flaps in burn injuries are associated with higher risks of flap failure, arterial thrombosis, and amputations. Fasciocutaneous free flaps in acute burns seem to be safer with better outcomes, though further research is needed to confirm these findings.

The entero-mammary pathway is a specialized route that selectively translocates bacteria to the newborn's gut, playing a crucial role in neonatal development. Previous studies report shared bacterial and archaeal taxa between human milk and neonatal intestine. However, the functional implications for neonatal development are not fully understood due to limited evidence. This study aimed to identify and characterize the microbiota and metabolome of human milk, mother, and infant stool samples using high-throughput DNA sequencing and FT-ICR MS methodology at delivery and 4 months post-partum. Twenty-one mothers and twenty-five infants were included in this study. Our results on bacterial composition suggest vertical transmission of bacteria through breastfeeding, with major changes occurring during the first 4 months of life. Metabolite chemical characterization sheds light on the growing complexity of the metabolites. Further data integration and network analysis disclosed the interactions between different bacteria and metabolites in the biological system as well as possible unknown pathways. Our findings suggest a shared bacteriome in breastfed mother-neonate pairs, influenced by maternal lifestyle and delivery conditions, serving as probiotic agents in infants for their healthy development. Also, the presence of food biomarkers in infants suggests their origin from breast milk, implying selective vertical transmission of these features.

El empiema pleural es una de las complicaciones de las infecciones del tracto respiratorio inferior y se caracteriza por la presencia de pus en la toracocentesis. Raramente Trichomonas hominis está asociada al empiema como agente causal. En este artículo presentamos el caso de una mujer de 39 años que desarrolló un empiema causado por T. hominis, además de una revisión de la literatura disponible de esta rara infección. Hasta donde sabemos, este es el primer caso de empiema pleural causado por Trichomona hominis reportado en México (AU)


Pleural empyema is one of the complications of lower respiratory tract infections and is characterized by the presence of pus on thoracentesis. Trichomonas hominis is rarely associated with empyema as the causative agent. In this article we present the case of a 39-year-old woman who developed an empyema caused by T. hominis, as well as a review of the available literature on this rare infection. To the best of our knowledge, this is the first case of pleural empyema caused by Trichomona hominis reported in Mexico (AU)

INTRODUCTION: Diabetic macular edema (DME) is one of the leading causes of vision impairment. The relationship between DME and estimated glomerular filtration rate (eGFR) has not been clearly evaluated in Hispanic or Latino populations. The objective of this study was to evaluate the eGFR in a Latino population with DME. METHODS: A cross-sectional, observational, and descriptive study was carried out on the basis of a multicenter phase III clinical trial. RESULTS: A total of 82 subjects diagnosed with DME (36 women and 46 men) were included in the study. The mean age was 61.93 ± 6.71 years. Mean values of the blood chemistry parameters glycated hemoglobin and eGFR were 7.20 ± 0.95% and 74.42 ± 26.82 mL/min/1.73 m2, respectively. The time elapsed since diagnosis of diabetes mellitus was 15.30 ± 7.35 years, while the duration of DME was 1.41 ± 1.75 years. Mean values for central macular thickness (CMT) and total macular volume (TMV) were 440.99 ± 132.22 µm and 11.97 ± 2.11 mm3, respectively. DME duration had a negative correlation with TMV (Rho - 0.26, p < 0.05) and a positive correlation with mean arterial pressure (Rho 0.26, p < 0.05). CMT was correlated with TMV (Rho 0.43, p < 0.0001) and visual acuity (Rho 0.26, p < 0.05). No significant correlations were observed between eGFR and CMT, TMV, or any demographic variable (p > 0.05). Chronic kidney disease (CKD) was associated with hypertension (OR 9.32, p = 0.035), elevated intraocular pressure (IOP) (OR 0.03, p = 0.011), and advanced age (OR 0.45, p = 0.011). CMT was significantly associated with TMV (ß = 27.69, p < 0.0001). CONCLUSIONS: We did not find a correlation between eGFR and DME. Our findings suggest that the presence of hypertension is associated with a decrease in the GFR < 60 mL/min/1.73 m2, and CKD may be associated with advanced age and elevated IOP which may increase the risk for the development of glaucoma. BACKGROUND: The COVID-19 pandemic brought with it changes in the way of working and the well-being of workers. OBJECTIVE: To identify research trends on work life quality and its impact on productivity in the context of the COVID-19 pandemic. METHOD: The Proknow-C constructivist methodology was used, with which a bibliographic portfolio of 49 articles obtained from the Web of Science was prepared, resulting from four stages of search between 2012 and 2022; a bibliometric analysis was performed on that database and bibliometric networks were created with the software VOSviewer; the systemic analysis of the articles was carried out, thus showing theories, definitions and indicators; and areas of opportunity of research were identified. RESULTS: The most important high-impact specialized journals and authors, articles considered central, most used keywords such as job satisfaction, quality of work life and COVID-19, as well as the most representative countries such as European and Asian, are presented. CONCLUSION: It was found that the health sector is one of the most studied, allowing researchers from other sectors the opportunity to delve into the issue of the affectations of the quality of work life reflected in productivity and common variables were synthesized such as job satisfaction, well-being, motivation and security, among others.

Natural zeolite is a widely used material with excellent environmental cleaning performance, especially in water and wastewater treatment. Natural zeolite (Zini) calcined by CO2-laser radiation (ZL) was tested as a catalyst for the photodegradation and the adsorption of industrial azo dye Lanasol Yellow 4G (LY4G) in water. Morphology, chemical structure, and surface composition of Zini and ZL were analyzed by XRD, SEM, EDS, and XPS. UV/Visible spectrophotometry was used to evaluate the photocatalytic activity of Zini and ZL. The photocatalytic activity of the studied zeolites was associated with the presence of Fe oxides in their composition. Laser-treated natural zeolite showed higher efficiency as a photocatalyst compared to untreated natural zeolite.

The aim of this work was to obtain insights of the participation of the autonomic nervous system in different stages of calcific aortic valve disease (CAVD) by heart rate variability (HRV) analysis. Studying subjects with no valve impairments and CAVD patients, we also sought to quantify the independent contribution or explanatory capacity of the aortic valve echocardiographic parameters involved in the HRV changes caused by active standing using hierarchical partitioning models to consider other variables or potential confounders. We detected smaller adjustments of the cardiac autonomic response at active standing caused specifically by the aortic valve deterioration. The highest association (i.e., the highest percentage of independent exploratory capacity) was found between the aortic valve area and the active standing changes in the short-term HRV scaling exponent α1 (4.591%). The valve's maximum pressure gradient echocardiographic parameter was present in most models assessed (in six out of eight models of HRV indices that included a valve parameter as an independent variable). Overall, our study provides insights with a wider perspective to explore and consider CAVD as a neurocardiovascular pathology. This pathology involves autonomic-driven compensatory mechanisms that seem generated by the aortic valve deterioration.

Heart rate variability (HRV) is a method used to evaluate the presence of cardiac autonomic neuropathy (CAN) because it is usually attributed to oscillations in cardiac autonomic nerve activity. Recent studies in other pathologies suggest that HRV indices are strongly related to mean heart rate, and this does not depend on autonomic activity only. This study aimed to evaluate the correlation between the mean heart rate and the HRV indices in women patients with well-controlled T2DM and a control group. HRV was evaluated in 19 T2DM women and 44 healthy women during basal supine position and two maneuvers: active standing and rhythmic breathing. Time-domain (SDNN, RMSSD, pNN20) and frequency-domain (LF, HF, LF/HF) indices were obtained. Our results show that meanNN, age, and the maneuvers are the main predictors of most HRV indices, while the diabetic condition was a predictor only for pNN20. Given the known reduced HRV in patients with T2DM, it is clinically important that much of the HRV indices are dependent on heart rate irrespective of the presence of T2DM. Moreover, the multiple regression analyses evidenced the multifactorial etiology of HRV.

Aortic stenosis is a progressive heart valve disorder characterized by calcification of the leaflets. Heart rate variability (HRV) analysis has been proposed for assessing the heart response to autonomic activity, which is documented to be altered in different cardiac diseases. The objective of the study was to evaluate changes of HRV in patients with aortic stenosis by an active standing challenge. Twenty-two volunteers without alterations in the aortic valve (NAV) and twenty-five patients diagnosed with moderate and severe calcific aortic valve stenosis (AVS) participated in this cross-sectional study. Ten minute electrocardiograms were performed in a supine position and in active standing positions afterwards, to obtain temporal, spectral, and scaling HRV indices: mean value of all NN intervals (meanNN), low-frequency (LF) and high-frequency (HF) bands spectral power, and the short-term scaling indices (α1 and αsign1). The AVS group showed higher values of LF, LF/HF and αsign1 compared with the NAV group at supine position. These patients also expressed smaller changes in meanNN, LF, HF, LF/HF, α1, and αsign1 between positions. In conclusion, we confirmed from short-term recordings that patients with moderate and severe calcific AVS have a decreased cardiac parasympathetic supine response and that the dynamic of heart rate fluctuations is modified compared to NAV subjects, but we also evidenced that they manifest reduced autonomic adjustments caused by the active standing challenge.

BACKGROUND: Arsenic exposure is associated with cardiovascular risk in adults; however, few epidemiologic studies have evaluated biomarkers of cardiovascular risk in children who are environmentally exposed to arsenic. OBJECTIVE: The aim of this study was to assess the associations between urinary arsenic, plasma natriuretic peptides and echocardiographic parameters in Mexican children exposed to arsenic through the drinking water. METHODS: We conducted a cross-sectional study with 192 children (3-8 years old) from Zimapan, Hidalgo, Mexico. B-type natriuretic peptide (BNP), NT-proBNP and atrial natriuretic peptide (ANP) were measured by ELISA, urinary arsenic concentration (UAs) were measured via by hydride generation-cryotrapping-atomic absorption spectrometry, and cardiac parameters were measured by echocardiography. RESULTS: The median plasma concentrations of ANP, BNP and NT-proBNP were 36.9 ng/mL, 49.7 pg/mL, and 226.1 pg/mL, respectively. Using multivariable models, a dose-response relationship was observed between BNP concentrations and UAs tertiles (<47 ng/mL: reference, 47-72 ng/mL: 48.7 pg/mL, >72 ng/mL: 52.2 pg/mL, P-trend = 0.020). BNP concentrations also increased with increasing U-tAs as continuous variables (0.43 pg/mL increase per 1 ng/mL increase of U-tAs; P-Value = 0.008). Additionally, BNP was positively associated with arsenic methylated metabolites (U-MAs and U-DMAs). On the other hand, BNP was inversely related to relative wall thickness (RWT). No associations were found for other cardiac parameters. Finally, neither ANP nor NT-proBNP were significantly related to arsenic exposure or echocardiographic parameters. CONCLUSIONS: In this study, we showed associations between plasma BNP and arsenic exposure. Our results support the importance of reducing childhood arsenic exposure, which may have cardiovascular effects early in life.

BACKGROUND AND AIMS: CAPN10 gene is associated with type 2 diabetes (T2D). Specific members of the calpain system (CAPN1, CAPN2 and CAPN10) are implicated in glucose metabolism. The aim of this study was to evaluate the calpain activity in leukocytes of control subjects and patients with T2D and its association with the calpain family members involved in glucose metabolism and with biochemical parameters that are altered in T2D. METHODS: Calpain activity under extracellular glucose concentrations (70-280 mg/dL) was evaluated in leukocytes from subjects with and without T2D. Protein and mRNA levels of CAPN1, CAPN2 and CAPN10 were evaluated. Calpain inhibitors assays were performed in leukocytes from subjects without T2D to evaluate glucose uptake. Calpain activity at 100 mg/dL glucose was correlated with biochemical parameters by multivariate regression. RESULTS: Calpain activity in control subjects increased with extracellular glucose concentration in a dose-dependent manner, showing a negative association with HbA1c levels and total amount of CAPN10 protein. In contrast, calpain activity is decreased in patients with T2D and do not respond to changes in glucose concentration. A reduction of CAPN1 autolytic fragments were observed in the subjects with diabetes. Calpain inhibitors decreased calpain activity but did not altered glucose uptake in leukocytes. CONCLUSIONS: Calpain activity induced by glucose in leukocytes was associated with biochemical markers of glucose metabolism and with CAPN10 protein abundance. Calpain activity is low in subjects with T2D. Thus, calpain activity induced by extracellular glucose in leukocytes could be a potential marker for T2D early risk detection.

Objective: The objective of the study was to describe the feasibility of single catheter intervention using the transradial approach for percutaneous coronary intervention (PCI). Background: The transradial approach for PCIs has fewer vascular events and complications and lower mortality rate. However, complications can result from forearm artery tortuosity, a longer learning curve and artery spasm that can complicate, delay and impede coronary artery interventions. The latter is usually exacerbated by the changing and manipulation of catheters. Methods: We performed a study using a single catheter on patients undergoing coronary assessment and treatment. Procedural outcomes including success, procedural time, bleeding, access site complications, and contrast used were all analyzed. Results: We included 327 patients, of whom 70% were male. The mean age was 63.3 ± 11.1 years, mean height was 165.9 ± 7.7 cm, mean weight was 73.3 ± 11.3 kg, and mean body index was 26.5 ± 3.5 kg/m2. Contrast use averaged 158.5 ± 60.5 ml. Three vessels were treated in 3% of all cases, two vessels in 32%, and one vessel in 65%. Procedural success was achieved in 94.5% of the cases. A second catheter was required in 9 cases (2.7%), and crossover to the femoral approach was performed in 9 cases (2.7%) due to a lack of support, artery spasm, difficult anatomy, or the need for a larger catheter. Three complications were related to access, including a Class 2 hematoma that was treated conservatively with no further complications. Conclusions: Our study showed that using a single catheter to perform both diagnostic and therapeutic procedures has a higher success rate, lower spasm incidence, and fewer complications than reported in literature.


Objetivo: Describir la factibilidad del uso de un solo catéter en el intervencionismo coronario percutáneo por vía transradial. Antecedentes: El abordaje transradial en las intervenciones coronarias ha mostrado menores eventos cardiovasculares y complicaciones, y menor mortalidad. Sin embargo, algunos eventos adversos pueden resultar por trotuosidad de las arterias del brazo, curva de aprendizaje más larga o espasmo arterial que puede complicar, retardar o impedir la intervención coronaria. Ésta última es usualmente exacerbada por el intercambio de catéterres o la manipulación de los mismos. Métodos: Realizamos un studio utilizando un solo catéter en pacientes sometidos a coronariografía e intervención coronaria. Los desenlaces del procedimiento incluyendo éxito, tiempo de procedimiento, sangrado, complicaciones en el sitio de acceso y uso de medio de contraste fueron analizados. Resultados: Incluimos 327 pacientes, 70% de los cuales fueron varones. La edad promedio fue de 63.3 ± 11.1 años, la estatura promedio fue de 165.9 ± 7.7 cm, peso promedio de 73.3 ± 11.3 kg y el índice de masa corporal promedio de 26.5 ± 3.5 kg/m2. El contraste utilizado promedio fue 158.5 ± 60.5 ml. El total de vasos tratados fue de tres en 3% de los casos, dos en 32% de los casos y uno en 65%. El éxito del procedimiento fue logrado en 94.5% de los pacientes No obstante, un Segundo catéter fue requerido en 9 intervenciones (2.7%), y cambio en la vía de acceso fue realizado en 9 casos (2.7%) por falta de apoyo, espasmo arterial, anatomía dificil o necesidad de un catéter de mayor lumen. Tres complicaciones asociadas al sitio de acceso incluyendo un hematoma clase 2 fueron registradas el cual se trató conservadoramente. Conclusiones: Nuestro estudio mostró que el uso de un catéter único para realizar tanto procedimientos diagnósticos como terapéuticos tiene una tasa de éxito mayor, con menor incidencia de espasmo y complicaciones reportadas en la literatura.

Abstract Objective: The objective of the study was to describe the feasibility of single catheter intervention using the transradial approach for percutaneous coronary intervention (PCI). Background: The transradial approach for PCIs has fewer vascular events and complications and lower mortality rate. However, complications can result from forearm artery tortuosity, a longer learning curve and artery spasm that can complicate, delay and impede coronary artery interventions. The latter is usually exacerbated by the changing and manipulation of catheters. Methods: We performed a study using a single catheter on patients undergoing coronary assessment and treatment. Procedural outcomes including success, procedural time, bleeding, access site complications, and contrast used were all analyzed. Results: We included 327 patients, of whom 70% were male. The mean age was 63.3 ± 11.1 years, mean height was 165.9 ± 7.7 cm, mean weight was 73.3 ± 11.3 kg, and mean body index was 26.5 ± 3.5 kg/m2. Contrast use averaged 158.5 ± 60.5 ml. Three vessels were treated in 3% of all cases, two vessels in 32%, and one vessel in 65%. Procedural success was achieved in 94.5% of the cases. A second catheter was required in 9 cases (2.7%), and crossover to the femoral approach was performed in 9 cases (2.7%) due to a lack of support, artery spasm, difficult anatomy, or the need for a larger catheter. Three complications were related to access, including a Class 2 hematoma that was treated conservatively with no further complications. Conclusions: Our study showed that using a single catheter to perform both diagnostic and therapeutic procedures has a higher success rate, lower spasm incidence, and fewer complications than reported in literature.


Resumen Objetivo: Describir la factibilidad del uso de un solo catéter en el intervencionismo coronario percutáneo por vía transradial. Antecedentes: El abordaje transradial en las intervenciones coronarias ha mostrado menores eventos cardiovasculares y complicaciones, y menor mortalidad. Sin embargo, algunos eventos adversos pueden resultar por trotuosidad de las arterias del brazo, curva de aprendizaje más larga o espasmo arterial que puede complicar, retardar o impedir la intervención coronaria. Ésta última es usualmente exacerbada por el intercambio de catéteres o la manipulación de los mismos. Métodos: Realizamos un studio utilizando un solo catéter en pacientes sometidos a coronariografía e intervención coronaria. Los desenlaces del procedimiento incluyendo éxito, tiempo de procedimiento, sangrado, complicaciones en el sitio de acceso y uso de medio de contraste fueron analizados. Resultados: Incluimos 327 pacientes, 70% de los cuales fueron varones. La edad promedio fue de 63.3 ± 11.1 años, la estatura promedio fue de 165.9 ± 7.7 cm, peso promedio de 73.3 ± 11.3 kg y el índice de masa corporal promedio de 26.5 ± 3.5 kg/m2. El contraste utilizado promedio fue 158.5 ± 60.5 ml. El total de vasos tratados fue de tres en 3% de los casos, dos en 32% de los casos y uno en 65%. El éxito del procedimiento fue logrado en 94.5% de los pacientes No obstante, un Segundo catéter fue requerido en 9 intervenciones (2.7%), y cambio en la vía de acceso fue realizado en 9 casos (2.7%) por falta de apoyo, espasmo arterial, anatomía dificil o necesidad de un catéter de mayor lumen. Tres complicaciones asociadas al sitio de acceso incluyendo un hematoma clase 2 fueron registradas el cual se trató conservadoramente. Conclusiones: Nuestro estudio mostró que el uso de un catéter único para realizar tanto procedimientos diagnósticos como terapéuticos tiene una tasa de éxito mayor, con menor incidencia de espasmo y complicaciones reportadas en la literatura.

RESUMEN La Comarca Lagunera, con una participación del 25 % de la producción nacional, es la región melonera más importante del país. No obstante su importancia, se carece de estudios recientes que reflejen su situación en materia socioeconómica. El objetivo del presente trabajo fue determinar la problemática a la que se enfrentan los productores de melón de la región de la Comarca Lagunera en los temas de financiamiento, comercialización y rentabilidad del cultivo, con un enfoque de siembras por etapas. Para obtener la información necesaria, se elaboró y aplicó un cuestionario a 60 productores de melón de la zona de estudio. El cuestionario estuvo integrado por 30 preguntas, divididas en cinco bloques, en los temas de aspectos técnicos del cultivo, financiamiento, costos de producción, ingresos y comercialización. En la comercialización se identificaron tres "etapas" de siembra y cosecha durante el año, siendo las fechas "tempranas" y "tardías" las que se vendieron a mejores precios, pero con mayores riesgos climatológicos y biológicos; mientras que, las "intermedias" fueron las de mayor producción y menor riesgo, pero de menor rentabilidad, al venderse a menores precios. La situación en materia de financiamiento sigue siendo una limitante para la rentabilidad de este cultivo, con créditos escasos y tasas altas. En la comercialización se encontró que se sigue dependiendo de los intermediarios. Se concluye que es necesario reforzar la organización para poder hacer una mejor planeación de las etapas de siembra y cosecha, obtener financiamientos más accesibles y con mayor cobertura, así como fortalecer la capacidad de negociación con los intermediarios.


ABSTRACT The Comarca Lagunera, with a participation of 25 % of the national production, is the most important melon region in the country. Despite its importance, recent studies reflecting its socio-economic situation are lacking. In this work, the objective was to generate information on the issues of financing, commercialization and profitability of melon cultivation with a "step by step" approach, which will serve as the basis to promote the development of this crop in the Coahuila Lagunera Region. To obtain the necessary information, a questionnaire was prepared and administered to 60 melon producers in the study area. The questionnaire was composed of 30 items, divided into five blocks, on the topics of: technical aspects of cultivation, financing, production costs, revenue and marketing. In marketing, three "stages" of planting and harvesting were identified during the year, with "early" and "late" dates being sold at better prices, but with greater climatic and biological risks; while, the "intermediaries" were those with higher production and lower risk, but with lower profitability, when sold at lower prices. The situation regarding financing continues to be a limitation for the profitability of this crop, with expensive and scarce credits. In commercialization, it was found that it continues to depend on intermediaries. It is concluded that it is necessary to reinforce the organization to be able to do a better planning of the sowing and harvesting stages, to obtain more accessible financing and with greater coverage, as well as to strengthen the negotiation capacity with the intermediaries.

RESUMO

We evaluated the risk for the Spanish Olympic Team acquiring Zika virus in Rio de Janeiro, Brazil, during 2016. We recruited 117 team members, and all tested negative for Zika virus. Lack of cases in this cohort supports the minimum risk estimates made before the Games.

Background: the learning styles are cognitive, emotional, and psychological characteristics, which function as relatively stable indicators of how teachers and students perceive, interact, and respond to their learning environments. Knowing students' styles allows teachers to have tools to improve medical education. Our objective was to identify learning styles in pediatric residents and professors from a pediatric hospital. Methods: a learning styles questionnaire was applied to residents and theirs professors; data was analyzed in SPSS 12 software. Results: the dominant learning style in pediatric residents was reflexive and for professors was theoretical. There wasn't any difference between sexes or between medical or surgical specialities. There was more correlation between professors and residents when there was an increase in training time. Conclusions: the learning styles between professors and residents are different, especially at the beginning of the medical specialty courses; that's why it is necessary to realize a confrontation between the students' learning styles and teaching methods used by professors to improve significant learning. To know learning styles gives residents an important alternative to find a better study strategy.


Introducción: los estilos de aprendizaje son indicadores de cómo los docentes y los alumnos perciben, interaccionan y responden a sus ambientes de aprendizaje. El objetivo de esta investigación fue identificar los estilos de aprendizaje de residentes de pediatría y de especialidades de rama pediátricas y sus profesores. Métodos: estudio transversal, comparativo y analítico realizado en un hospital de pediatría. Se aplicó el cuestionario de Honey-Alonso. El resultado se obtuvo con la suma del número de reactivos positivos para cada estilo de aprendizaje y el resultado se verificó de acuerdo con los baremos de interpretación para estudiantes de la carrera de medicina. En cada encuesta se realizó el diagrama complementario para identificar el estilo de aprendizaje preferente. Resultados: el estilo de aprendizaje predominante en los residentes fue el reflexivo y en sus profesores, el teórico, sin diferencias significativas según sexo, especialidad y si esta era médica o quirúrgica. Hubo mayor correlación entre los estilos de profesores y residentes conforme se incrementaba el tiempo de entrenamiento. Conclusiones: se requiere confrontar los estilos de aprendizaje de los estudiantes y los métodos de enseñanza empleados por sus profesores, con la finalidad de favorecer un aprendizaje significativo.

Magnetic resonance (MR) imaging is the method of choice to evaluate the cranial nerves (CN). These nerves constitute a group of structures that have acquired during their phylogenetic development a high degree of specialization. There are 12 pairs of CN to which we use their specific name or number. The olfactory (I) and optic (II) pairs are not real nerves but tracts from the encephalon. The spinal nerve (XI) derives from superior cervical segment of the spine. The other 9 pairs of CN are related with the brain stem. Although the skull base foramina can be seen on computed tomography, the nerves themselves can only be visualized in detail on MR. That means, in order to see the different segments of nerves I to XII, the right sequences must be used. It is important to provide detailed clinical information to the radiologist so that a tailored MR study can be performed. In this review, the basic imaging anatomy of the 12 CN is discussed and illustrated briefly with an emphasis on more advanced extra-axial anatomy, illustrated with high-resolution MR images. Clinicians looking for complete anatomic descriptions and/or MR illustrations are advised to consult specialized textbooks considering it is not possible to describe all of the anatomy in one article. This manuscript is intended to be a practical review for clinicians.

Las vías de abordaje para la realización de la colecistectomía han cambiado dramáticamente en los últimos 20 años, desde la cirugía abierta hasta la cirugía de invasividad mínima y ahora desde 2007 se comienza a realizar cirugía a través de orificios naturales. Con la ausencia de instrumentos ideales, puede ser posible crear accesos seguros a través del abordaje trans-vaginal con el uso de instrumentos laparoscópicos convencionales en pacientes bien seleccionados. Describir una nueva técnica quirúrgica para la realización de colecistectomía transvaginal video asistida con instrumentos laparoscópicos convencionales. Descripción de la técnica: con la paciente bajo anestesia general, colocada en posición ginecológica, accedemos a la cavidad abdominal a través de la cicatriz umbilical. Insertando un trocar de 10mm bajo visión directa, a través de este se vigilará la entrada a través de la colpotomía, a través de la vagina se introducirá una pinza de grasper de 5mm y la óptica de 30°. Con el grasper transvaginal se sujetara la vesícula y a través del portal umbilical el cirujano realizará la disección de los elementos de triangulo de Calot, extrayendo la pieza por vía vaginal. El acceso trans-vaginal es factible y seguro con material laparoscópico convencional, para la realización de colecistectomía. Es indispensable el entrenamiento en laparoscopia convencional. El cirujano debe adaptarse a los ángulos de visualización ofrecidos.


The access to perform a cholecystectomy has changed dramatically in the last 20 years, from open surgery to minimal invasive surgery such as laparoscopy and now since 2007, starting toperform through natural orifices. In the absence of ideals instruments, it could be possible to create safes access through trans-vaginal way with regular laparoscopic instruments in well selected patients. Describe a new surgical technique to perform a trans-vaginal cholecystectomy video assisted, with common laparoscoic instruments. Technical description: with the patient under general anesthesia, placed her in gynecological position, we get access to the abdominal cavity through umbilicus, inserting a 10mm trocar, them under direct vision we perform the trans-vaginal access inserting one grasping forceps and 30º scope. The vaginal grasping forceps will hold the gallbladder between the fundus and the infundibulus, exposing the Calot triangle. Through the umbilicus trocar the surgeon dissect de cystic duct and artery, them it will be separated from its bed. The gallbladder is taking out side from the abdomen through vaginal way. The trans-vaginal access is a feasibly way to perform cholecystectomy with common laparoscopic instruments. It is indispensable to dominate basic laparoscopic techniques, and before start practicing this procedure, it has to perform under animal’s models.
