INTRODUCTION: Long-term clinical success of non-splinted, posterior, and short implants still is unclear. This prospective cohort study reports the 10-year follow-up of 6-mm implants supporting single crowns in the posterior region, and patient-reported outcomes. METHODS: Baseline sample comprised 20 patients treated with 46 screw-retained crowns supported by 6-mm implants with moderately rough implant surface. Participants were recalled for a 10-year clinical follow-up to assess survival rates, biologic and mechanic conditions, quality of life (OHIP-14), and treatment satisfaction. Data were collected with clinical-radiographic exams and analyzed using descriptive and inferential statistics. RESULTS: Fourteen patients with 35 implant-crown units were examined after 127.6 ± 11.8 months. For the entire cohort period, 7/46 implants were lost (survival estimate: 77.7% at 133 months), and mechanic complications occurred in 14/46 units (survival estimate: 66.4% at 116 months). In the Cox models, "maximum occlusal force" had a significant effect for implant loss (p = 0.038) and for prosthetic screw loosening (p = 0.038); "arch" and "bruxism" were not significant. Peri-implant bone loss was 0.4 ± 0.6 mm at 10 years. For peri-implant bone level, "crown-to-implant ratio" (p < 0.001) and "time" (p = 0.001) were significant. Bone levels differed from baseline to 12, 48, and 120 months. Satisfaction VAS was 94.0 ± 7.9 mm and OHIP-14 was 2.3 ± 2.2. CONCLUSION: Single screw-retained crowns supported by 6-mm implants have an acceptable long-term clinical performance, with stable peri-implant bone levels after 10 years of function.

OBJECTIVES: The worldwide dissemination of carbapenemase-producing Escherichia coli lineages belonging to high-risk clones poses a challenging public health menace. The aim of this work was to investigate genomic features of a colonizing multidrug-resistant strain of Klebsiella pneumoniae carbapenemase (KPC)-producing E. coli from our institution. METHODS: Whole-genome sequencing was done by Illumina MiSeq-I, and de novo assembly was achieved using SPAdes. Resistome, mobilome, plasmids, virulome, and integrons were analysed using ResFinder, AMRFinder, ISFinder, PlasmidFinder, MOB-suite, VirulenceFinder, and IntegronFinder. Sequence types (STs) were identified with pubMLST and BIGSdb databases. Conjugation assays were also performed. RESULTS: Escherichia coli HA25pEc was isolated from a rectal swab sample taken within the framework of the hospital epidemiological surveillance protocol for detection of carbapenemase-producing Enterobacterales. Escherichia coli HA25pEc corresponded to the first report of ST648 co-harbouring blaKPC-2 and blaCTX-M-15 in Latin America from a colonized patient. It had 19 antibiotic resistance genes (ARGs), including blaKPC-2, located on a Tn4401a isoform. Conjugation assays revealed that blaKPC-2 was not transferred by conjugation to E. coli J53 under our experimental conditions. CONCLUSION: Escherichia coli ST648 has been detected previously in companion and farm animals as well as in hospital- and community-acquired infections worldwide. Although scarcely reported as KPC-producers, our finding in a culture surveillance with several acquired ARGs, including blaCTX-M-15, alerts the potential of this clone for worldwide unnoticed spreading of extreme drug resistance to ß-lactams. These data reinforce the importance of carrying out molecular surveillance to identify reservoirs and warn about the dissemination of new international clones in carbapenemase-bearing patients.

Resumen Se analiza la construcción, por parte de inmigrantes internacionales, de redes sociales en el país de destino, y las características de estas redes según el país de origen. Se estudia el caso de tres colectivos residentes en Chile, utilizando metodología cualitativa basada en entrevistas semiestructuradas, que se analizan mediante análisis narrativo. Los resultados muestran que quienes tienden a incorporarse con mayor rapidez en el lugar de destino, suelen establecer redes de vínculos débiles. Por el contrario, quienes tienen más dificultades para incorporarse en el lugar de destino, o lo hacen solo parcialmente, establecen redes densas más cerradas en lo cultural.


Resumo Este artigo analisa a construção, por imigrantes internacionais, de redes sociais no país de destino e as características dessas redes de acordo com o país de origem. O caso de três grupos residentes no Chile é estudado por meio de um desenho metodológico qualitativo baseado em entrevistas semiestruturadas, que são abordadas a partir da análise sociológica do discurso. Os resultados mostram que aqueles que se incorporam com mais facilidade e rapidez no local de destino tendem a estabelecer redes de vínculos fracos. Ao contrário, aqueles que têm mais dificuldade em se incorporar ao local de destino, ou o fazem apenas parcialmente, tendem a estabelecer redes culturalmente fechadas e coesas.


Abstract This article analyzes international immigrants' construction of social networks in the country of destination, and the characteristics of these networks according to the country of origin. The case of three groups residing in Chile is studied through a qualitative methodological design based on semi-structured interviews, examined by means of narrative analysis. Results show that those who adapt more easily and quickly to the place of destination tend to establish networks with weak ties. Conversely, those who have more difficulties in adapting to the place of destination, or do so only partially, tend to establish closed, tightly-knit cultural networks.

Abstract Background In patients with rheumatic diseases, the use of biological (b) or targeted synthetic (ts) disease-modifying antirheumatic drugs (DMARDs) after discontinuation of tumor necrosis factor inhibitors (TNFi) is known to be effective. However, data on the use of TNFi after discontinuation of non-TNFi bDMARDs or tsDMARDs (non-TNFi) are scarce. This study assessed the 4-years golimumab retention in patients with rheumatic diseases when used after discontinuation of non-TNFi. Methods Adults with rheumatoid arthritis (RA; n = 72), psoriatic arthritis (PsA; n = 30) or axial spondyloarthritis (axSpA; n = 23) who initiated golimumab after discontinuation of non-TNFi from the Spanish registry of biological drugs (BIOBADASER) were analyzed retrospectively. The retention rate (drug survival or persistence) of golimumab up to 4 years was evaluated. Results The golimumab retention rate was 60.7% (51.4-68.8) at year 1, 45.9% (36.0-55.2) at year 2, 39.9% (29.8-49.7) at year 3 and 33.4% (23.0-44.2) at year 4. Retention rates did not differ significantly whether golimumab was used as second, third, or fourth/subsequent line of therapy (p log-rank = 0.462). Golimumab retention rates were higher in axSpA or PsA patients than in RA patients (p log-rank = 0.002). When golimumab was administered as third or fourth/subsequent line, the 4-years retention rate after discontinuation of non-TNFi was similar to that after discontinuation of TNFi. Conclusion In patients who discontinued non-TNFi, most of whom received golimumab as third/subsequent line of therapy, one-third of patients remained on golimumab at year 4. Retention rates were higher in patients with axSpA and PsA than in those with RA.

OBJECTIVES: Enterobacter cloacae complex (ECC) has awakened interest recently because of its increasing resistance to carbapenems codified by several genes all over the globe. Even though there are some sequence types (STs) which represent high-risk clones, there is substantial clonal diversity in the ECC. This work aimed to perform whole-genome sequencing (WGS), genomic analysis, and phylogenetic studies of a Klebsiella pneumoniae carbapenemase (KPC) -producing multidrug-resistant (MDR) ECC isolate from Argentina. METHODS: We analysed the genome of an MDR KPC-producing ECC strain isolated from a urine sample from a patient in a hospital in Argentina. The WGS was done by Illumina MiSeq-I (Illumina, San Diego, CA). The genome was assembled with SPAdes 3.9.0, and annotated with PROKKA, RAST, and Blast. Plasmids were identified with PlasmidFinder. Antibiotic resistance genes were detected using RESfinder, CARD, and Blastn. STs were identified with pubMLST. RESULTS: The strain was identified as Enterobacter hormaechei, an important emerging human pathogen. No ST could be assigned; six of seven alleles of multilocus sequence typing (MLST) were the same as for E. hormaechei ST66, which is a high-risk clone. We found multiple acquired antibiotic resistance genes, including blaKPC-2 in an IncM1 plasmid, and a secretion system VI, which can favour the prevalence of ECC strains while competing with other bacteria. CONCLUSION: Because of its MLST profile being so close to that of E. hormaechei ST66, the acquisition of multiple resistance genes, and the presence of the secretion systems, the potential of this strain for becoming a new high-risk clone cannot be discarded.

In this work, we describe how stereochemically complex polycyclic compounds can be generated by applying a synthetic sequence comprising an intramolecular Ugi reaction followed by a Pictet-Spengler cyclization on steroid-derived scaffolds. The resulting compounds, which combine a fragment derived from a natural product and a scaffold not found in nature. are both structurally distinct and globally similar to natural products at the same time, and interrogate an alternative region of the chemical space. One of the new compounds showed significant antiproliferative activity on HepG2 cells through a caspase-independent cell-death mechanism, an appealing feature when new antitumor compounds are searched.

BACKGROUND: This study assessed the relationship between insertion torque and bone quality evaluated during surgery and in preoperative computed tomographic (CT) images analyzed either visually or by rescaled mean gray values (MGVs). The study also tested the correlation between the clinical and radiographic measures of bone quality. METHODS: The consecutive sample was composed of 45 short implants (4.1 × 6 mm) placed in the posterior region of 20 patients. Intra-surgical tactile bone quality, based on the classification of bone types by Lekholm and Zarb, and insertion torque were recorded during the implant placement. Visual bone quality and normalized MGV were assessed in standardized axial, coronal, and sagittal sections of preoperative CT images. Data were analyzed by ANOVA and Spearman correlation (alpha = 0.05). RESULTS: Insertion torque was associated with all assessment methods of bone quality (tactile, CT visual, MGV). A moderate correlation was found among all methods of bone quality, except for CT visual assessment and tactile evaluation. MGVs varied as a function of arch, dental region, insertion torque, and bone types. CONCLUSIONS: The results suggest that bone quality measures affect primary stability as recorded by insertion torque, and the assessment methods are consistently related.

Over the last decades, much effort has been devoted to the design of the "ideal" library for screening, the most promising strategies being those which draw inspiration from biogenic compounds, as the aim is to add biological relevance to such libraries. On the other hand, there is a growing understanding of the role that molecular complexity plays in the discovery of new bioactive small molecules. Nevertheless, the introduction of molecular complexity must be balanced with synthetic accessibility. In this work, we show that both concepts can be efficiently merged-in a minimalist way-by using very simple guidelines during the design process along with the application of multicomponent reactions as key steps in the synthetic process. Natural phenanthrenoids, a class of plant aromatic metabolites, served as inspiration for the synthesis of a library in which complexity-enhancing features were introduced in few steps using multicomponent reactions. These resulting chemical entities were not only more complex than the parent natural products, but also interrogated an alternative region of the chemical space, which led to an outstanding hit rate in an antiproliferative assay: four out of twenty-six compounds showed in vitro activity, one of them being more potent than the clinically useful drug 5-fluorouracil.

BACKGROUND: The use of short implants is still not a consensus for challenging clinical situations, such as unfavorable crown-to-implant (C/I) ratio. PURPOSE: This prospective study evaluated the rates of prosthetic complications and implant failure, the mean marginal bone loss of 6-mm dental implants with single crowns in posterior regions and the potential risk factors. MATERIALS AND METHODS: Forty-six dental implants, 6-mm long and 4.1-mm wide, were placed in the posterior region in 20 patients. Patients were clinically and radiographically examined after the restoration with single crowns and on a yearly basis. Potential risk factors (arch, bruxism, maximum bite force, anatomical and clinical C/I ratios, and occlusal table area) were analyzed according to the following outcomes: implant survival, bone loss, and prosthetic complications. RESULTS: The mean follow-up time was 45 ± 9 (16-57) months. There was no early loss of implants. After prosthetic loading, 4 implants were lost (2 in the mandible and 2 in the maxilla; 91.3% survival), and there were 13 prosthetic complications (28.3%), yielding a 65.2% success rate. The frailty term showed a 95% greater chance of loss in the mandible than the maxilla. Mean peri-implant bone loss was of 0.2 ± 0.4 mm, 0.1 ± 0.2 mm, 0.1 ± 0.3 mm, and 0.2 ± 0.4 mm in the first, second, third, and fourth years, respectively, with a mean cumulative loss of 0.3 ± 0.5 mm at 48 months. In the multilevel model, the effects of the clinical C/I ratio and time were significant for bone loss (P < .001). It was estimated that a mean bone loss of 0.1 mm is associated with both a one-unit increase in time (12 months) and a 0.1 increase in the clinical C/I ratio. The other potential risk factors showed no significant relationship with the outcomes. CONCLUSIONS: The 6-mm implants supporting single crowns performed well, but the mandible shows a higher risk of failure. The time and clinical C/I ratio are predictors for bone loss.

Complications from anterior approaches to the cervical spine are uncommon with normal anatomy. However, variant anatomy might predispose one to an increased incidence of injury during such procedures. We hypothesized that left vertebral arteries that arise from the aortic arch instead of the subclavian artery might take a more medial path in their ascent making them more susceptible to iatrogenic injury. Fifty human adult cadavers were examined for left vertebral arteries having an aortic arch origin and these were dissected along their entire cervical course. Additionally, two radiological databases of CTA and arteriography procedures were retrospectively examined for cases of aberrant left vertebral artery origin from the aortic arch over a two-year period. Two cadaveric specimens (4%) were found to have a left vertebral artery arising from the aortic arch. The retrospective radiological database analysis identified 13 cases (0.87%) of left vertebral artery origin from the aortic arch. Of all cases, vertebral arteries that arose from the aortic arch were much more likely to not only have a more medial course (especially their preforaminal segment) over the cervical vertebral bodies but also to enter a transverse foramen that was more cranially located than the normal C6 entrance of the vertebral artery. Spine surgeons who approach the anterior cervical spine should be aware that an aortic origin of the left vertebral artery is likely to be closer to the midline and less protected above the C6 vertebral level. Investigación cuantitativa que buscó establecer la relación entre la satisfacción vital (variable principal) y las variables autoestima personal, bienestar material percibido, prejuicio grupal percibido, discriminación personal percibida e identidad nacional, identificando cuáles, entre estas últimas, podían predecir la primera. La muestra, no probabilística, estuvo conformada por 171 colombianos, hombres y mujeres, residentes en Estados Unidos. Los resultados indicaron que la autoestima y el bienestar material percibido fueron las variables más estrechamente relacionadas con la satisfacción vital, siendo, además, las únicas predictoras positivas de esta. Las expresiones de prejuicio no impactaron significativa-mente la satisfacción vital, mientras que la identidad nacional presentó un impacto nulo.


This quantitative investigation aimed at establishing the relationship between life satisfaction (main variable) and the variables personal self-esteem, perceived material well-being, perceived group prejudice, perceived personal discrimination and ethnic identity. The sample, not probabilistic, was comprised of 171 Colombians, men and women, who reside in the United States. Results indicated that self-esteem and perceived material well-being were the variables most closely related to life satisfaction, being, moreover, the only positive predictors in the model. Expressions of prejudice did not significantly impact life satisfaction while ethnic identity showed not impact at all.


Trata-se de pesquisa quantitativa que buscou estabelecer a relação entre a satisfação vital (variável principal) e as variáveis: autoestima pessoal, bem-estar material percebido, preconceito grupal percebido, discriminação pessoal percebida e identidade nacional, bem como identificar quais, entre estas últimas, podiam predizer a primeira. A amostra, não probabilística, foi conformada por 171 colombianos, homens e mulheres, residentes nos Estados Unidos. Os resultados indicaram que a autoestima e o bem-estar material percebidos foram as variáveis mais estreitamente relacionadas com a satisfação vital; além disso, foram as únicas preditoras positivas desta. As expressões de preconceito não impactaram significativamente a satisfação vital, enquanto a identidade nacional apresentou um impacto nulo.

Most sterols, such as cholesterol and ergosterol, become functional only after the removal of the two methyl groups at C-4 from their biosynthetic precursors. Nevertheless, some findings suggest that 4,4-dimethyl sterols might be involved in specific physiological processes. In this paper we present the synthesis of a collection of analogues of 4,4-dimethyl sterols with a diamide side chain and a preliminary analysis of their in vitro activity on selected biological systems. The key step for the synthesis involves an Ugi condensation, a versatile multicomponent reaction. Some of the new compounds showed antifungal and cytotoxic activity.

AIM: To assess the failures and complications of short (<10 mm) implants supporting single crowns in the posterior region and its potential risk factors (RkF). MATERIALS AND METHODS: Prospective studies were screened according to eligibility criteria, followed by contact with authors. Quality assessment was performed using a standardized protocol. Mean implant failure proportion (FP), biological and prosthetic failure proportions (BFP/PFP) and marginal bone loss (MBL) including 95% confidence intervals were estimated using random-effects models for meta-analysis. RESULTS: Sixteen studies with a medium methodological quality (mean score: 8 ± 3; 2-14) had data collected. In summary, 762 short implants were followed up for up to 120 months in 360 patients (mean follow-up: 44 ± 33.72 months; mean dropout rate: 5.1%). The means FP, BFP, PFP and MBL were 5.9% (95%CI: 3.7-9.2%), 3.8% (95%CI: 1.9-7.4%), 2.8% (95%CI: 1.4-5.7%) and 0.83 mm (95%CI: 0.54-1.12 mm) respectively. Quantitative analysis showed that placement in the mandible (p = 0.0002) and implants with length ≤8 mm (p = 0.01) increased FP, BFP and MBL, whereas qualitative assessment revealed that crown-to-implant ratio did not influence MBL. CONCLUSIONS: Single crowns supported by short implants in the posterior region are a predictable treatment option with reduced failure rates, biological/prosthetic complications and minimal bone loss.

Clásicamente se describe a la insuficiencia renal aguda pre-renal como un cuadro clínico que se caracteriza por presentarse con uremia elevada y creatininemia normal (hiperazoemia pre-renal). Asimismo se ha descripto en la literatura que la insuficiencia renal pre-renal en personas ancianas (edad ≥ 65 años) suele cursar con valores séricos elevados no sólo de urea (hiperazoemia) sino además de creatinina (hipercreatininemia). En este reporte presentamos el caso de una mujer de 86 años de edad que presenta insuficiencia renal aguda con hipercreatininemia pre-renal y se analizan además las bases fisiopatológicas de este fenómeno.(AU)

Clásicamente se describe a la insuficiencia renal aguda pre-renal como un cuadro clínico que se caracteriza por presentarse con uremia elevada y creatininemia normal (hiperazoemia pre-renal). Asimismo se ha descripto en la literatura que la insuficiencia renal pre-renal en personas ancianas (edad ≥ 65 años) suele cursar con valores séricos elevados no sólo de urea (hiperazoemia) sino además de creatinina (hipercreatininemia). En este reporte presentamos el caso de una mujer de 86 años de edad que presenta insuficiencia renal aguda con hipercreatininemia pre-renal y se analizan además las bases fisiopatológicas de este fenómeno.

El propósito de esta investigación fue estudiar la relación de la satisfacción vital con varios factores psicosociales corrientemente vinculados a los procesos de migración internacional. En concreto, este estudio exploró la relación de la satisfacción vital - componente cognitivo del bienestar subjetivo - con las variables prejuicio grupal percibido, discriminación personal percibida, autoestima global personal, identidad nacional y bienestar material percibido. La investigación, de tipo empírico y cuantitativo, se realizó através de un muestreo no probabilístico con inmigrantes colombianos en varias ciudades del Ecuador (N = 185). Confirmando las hipótesis, se halló una relación positiva entre la satisfacción vital, el bienestar material percibido y la autoestima. Se confirmó así mismo una relación negativa entre la satisfacción vital, la discriminación personal percibida y la identidad nacional. Estas mismas variables; la autoestima, el bienestar material percibido, la discriminación personal percibida y la identidad nacional, emergieron como predictoras de la satisfacción vital. No se confirmó, en cambio, la relación negativa hipotetizada entre la satisfacción vital y el prejuicio grupal percibido. Se hace un análisis de dichos resultados desde la literatura psicosocial.


The aim of this research was to study the relationship between life satisfaction and various psychosocial variables usually linked to international migration processes. Specifically, this study explored the relationship between life satisfaction-a cognitivedimension of subjective well-being- and perceived group prejudice, perceived personal discrimination, global personal self-esteem, national identity and perceived material well- being. The research, of empirical quantitative type, was conducted with a non-probabilistic sample of Colombian migrants at several cities of Ecuador. As expected, the hypothesis that life satisfaction was positively related to self -esteem and perceived material well-being was confirmed. Also, a negative correlation was confirmed between life satisfaction, perceived personal discrimination and national identity. Results show that the variables self-esteem, perceived material well-being, perceived personal discrimination and national identity are significant predictors of life satisfaction. Findings didn·t confirm the expected negative association between life satisfaction and perceived group prejudice. Results are analyzed from a psychosocial perspective.

The need to develop novel antiviral agents encouraged us to assess the antiviral activity of synthetic sterol analogues with a diamide side chains. Cytotoxicity and antiviral activity of a family of azasterol previously synthesized was evaluated against herpes simplex virus 1 (HSV-1) (KOS and B2006) and vesicular stomatitis virus (VSV). This family of compounds was extended by the synthesis of novel analogs using an Ugi multicomponent reaction and their ability to inhibit viral multiplication was also evaluated. The results show that some of the compounds tested exert an antiviral activity. Besides, the effect of the azasterols on the intracellular localization of viral glycoproteins was examined. Strikingly, alteration on the glycoprotein D (gD) of HSV-1 fluorescence pattern was observed with both the antiherpetic compounds and the inactive azasterols.

RESUMO

In this paper we report the synthesis of four ring-A difluorinated analogs of brassinosteroids. The bioactivity of these new compounds was evaluated using the rice lamina inclination test. The results show that one of these analogs elicits a bioactivity comparable to that of 28-homocastasterone, a highly active natural brassinosteroid. This finding suggests that both hydroxyls at C-2 and C-3 in active brassinosteroids are involved as hydrogen bond acceptors in their interactions with the cellular receptor.
