OBJECTIVE: Build and validate a realistic interprofessional simulation scenario for the identification and management of sepsis by doctors and nurses. METHOD: Methodological study developed in 2021, in two stages: construction of the guide and content validation by expert judges; and development of the simulation and validation of the scenario by doctors and nurses. 15 specialists participated in the research in the first stage and eight care professionals in the second stage. 0.8 was adopted as a parameter for the Content Validation Index (CVI). RESULTS: The scenario guide addressed nosocomial sepsis with an abdominal focus in an adult patient and its validation obtained a total CVI of 0.97. All areas evaluated in the simulation scenario obtained agreement indices greater than 0.8. CONCLUSION: The construction and validation of the guide allowed the elaboration of guiding material for the development of an interprofessional simulated scenario, whose execution and validation process demonstrated its suitability in approaching the identification and management of sepsis by doctors and nurses. It is recommended to carry out future research evaluating its applicability to other situational contexts.

OBJECTIVES: To evaluate human-based Medical Subject Headings (MeSH) allocation in articles about 'patient simulation'-a technique that mimics real-life patient scenarios with controlled patient responses. METHODS: A validation set of articles indexed before the Medical Text Indexer-Auto implementation (in 2019) was created with 150 combinations potentially referring to 'patient simulation'. Articles were classified into four categories of simulation studies. Allocation of seven MeSH terms (Simulation Training, Patient Simulation, High Fidelity Simulation Training, Computer Simulation, Patient-Specific Modelling, Virtual Reality, and Virtual Reality Exposure Therapy) was investigated. Accuracy metrics (sensitivity, precision, or positive predictive value) were calculated for each category of studies. KEY FINDINGS: A set of 7213 articles was obtained from 53 different word combinations, with 2634 excluded as irrelevant. 'Simulated patient' and 'standardized/standardized patient' were the most used terms. The 4579 included articles, published in 1044 different journals, were classified into: 'Machine/Automation' (8.6%), 'Education' (75.9%) and 'Practice audit' (11.4%); 4.1% were 'Unclear'. Articles were indexed with a median of 10 MeSH (IQR 8-13); however, 45.5% were not indexed with any of the seven MeSH terms. Patient Simulation was the most prevalent MeSH (24.0%). Automation articles were more associated with Computer Simulation MeSH (sensitivity = 54.5%; precision = 25.1%), while Education articles were associated with Patient Simulation MeSH (sensitivity = 40.2%; precision = 80.9%). Practice audit articles were also polarized to Patient Simulation MeSH (sensitivity = 34.6%; precision = 10.5%). CONCLUSIONS: Inconsistent use of free-text words related to patient simulation was observed, as well as inaccuracies in human-based MeSH assignments. These limitations can compromise relevant literature retrieval to support evidence synthesis exercises.

Objetivo. Avaliar a percepção da transferência de conhecimentos das simulações clínicas para a prática assistencial em estudantes de enfermagem por meio de um debriefing eficaz. Métodos. Foi realizado um estudo observacional, descritivo e transversal com uma lista de 281 estudantes de enfermagem de uma universidade espanhola durante o curso 2020-2021, por meio de uma consulta ad hoc a partir do Debriefing Assessment for Simulation in Healthcare (DASH) em português, para valorizar as áreas competentes que um estudante de graduação deve alcançar ao finalizar seus estudos. Resultados. A consulta realizada após cada simulação mostrou que os estudantes valorizaram positivamente as sessões de debriefing realizadas por especialistas, com uma pontuação média de 6.61 sobre 7 [6.56-6.65%] com base em 675 consultas analisadas, e cada estudante realizou mais de uma simulação dentro do curso acadêmico. Foram observadas 221 respostas cumulativas que o aprendizado na simulação se traduziu na prática em 89.23% [86.39-92.06%], especificamente nas áreas de Comunicação, Segurança do paciente, Trabalho em equipe e Liderança. Conclusão. Na percepção dos estudantes participantes, o uso de um debriefing eficaz na simulação clínica permitiu a transferência de conhecimentos para a prática assistencial, mostrando ser uma ferramenta crucial que ajuda a melhorar a formação dos futuros enfermeiros.


Objetivo. Evaluar la percepción de la transferencia de conocimientos de las simulaciones clínicas a la práctica asistencial en estudiantes de enfermería mediante un debriefing eficaz.Métodos. Se realizó un estudio observacional, descriptivo y transversal con una muestra de 281 estudiantes durante el curso 2020-2021, mediante una encuesta ad hoc a partir de la Debriefing Assessment for Simulation in Healthcare (DASH) en español, para valorar las áreas competenciales que un estudiante de grado debe alcanzar al finalizar sus estudios. Resultados. La encuesta realizada después de cada simulación mostró que los estudiantes valoraban positivamente las sesiones de debriefing realizadas por expertos, con una puntuación media de 6.61 sobre 7 [6.56 % - 6.65 %] en base a 675 encuestas analizadas, ya que cada estudiante realizaba más de una simulación dentro del curso académico. Se observó en 221 respuestas cumplimentadas que lo aprendido en la simulación se transfirió a la práctica en el 89.23 % [86.39 % - 92.06 %], específicamente en áreas de Comunicación, Seguridad del paciente, Trabajo en equipo y Liderazgo. Conclusión. En la percepción de los estudiantes participantes, el uso de un debriefing eficaz en la simulación clínica permitió la transferencia de conocimientos a la práctica asistencial, mostrando ser una herramienta crucial que ayuda a mejorar la formación a de los futuros enfermeros.


Objective. This work sought to assess the perception of knowledge transfer from clinical simulations to the care practice in nursing students through effective debriefing.Methods. An observational, descriptive, and cross-sectional study was conducted with a sample of 281 students during the 2020-2021 course, through una ad hoc survey from the Debriefing Assessment for Simulation in Healthcare (DASH) in Spanish, to assess competence areas that undergraduate students must reach to complete their studies.Results. The survey conducted after each simulation showed that the students valued positively the debriefing sessions conducted by experts, with a mean score of 6.61 over 7 [6.56%-6.65%] based on 675 surveys analyzed, given that each student conducted more than one simulation within the academic course. It was observed in 221 completed answers that what was learned in the simulation was transferred to the practice in 89.23% [86.39%-92.06%], specifically in areas of Communication, Patient safety, Teamwork, and Leadership. Conclusion. In the perception by the participating students, the use of effective debriefing in clinical simulation enabled knowledge transfer to the care practice, proving to be a crucial tool that helps to improve the formation of the future nurses.

Consultations with children and their families are complex and require soft skills. However, there is a gap in the medical curriculum concerning these skills, especially as encounter training is often adult-centered. We developed, validated, and applied simulation scenarios that prioritize active participation of children to train soft skills in child-centered care for undergraduate medical students. This is a methodological study to develop three scenarios and a checklist of what is expected. The content was validated by 18 experts. A pre-test was carried out for adjustments. Then, the simulations were applied and evaluated by 18 medical undergraduate students. They included the participation of 6 pediatric simulated patients aged 9-12 years trained by a drama teacher. According to the results, the scenarios and checklist proved to be valid instruments in content terms (ICV-I > 0.8). The scripts were followed by the simulated pediatric patients, but they had difficulty mimicking a hypoactive state. Some were anxious, but everyone enjoyed participating in the feedback. The simulated parents had difficulty participating and giving space to the child's speech. Participants assessed that the simulations performed as they were proposed and, after experimenting them, felt more prepared. The simulations provided an opportunity for students to practice soft skills by interacting with children in a safe environment. Using children as simulated patients is feasible but presents some challenges. Our study has expanded the ways in which children's health content can be taught. We are investigating whether this training leads to better patient outcomes in real clinical settings.

Objective: To assess the self-confidence and satisfaction of Nursing students based on a preoperative telesimulation scenario. Method: Exploratory-descriptive quantitative study conducted from June 2021 to June 2022 at a public university in southern Brazil. The study included 28 undergraduate Nursing students who participated in a telesimulation on preoperative nursing. After the telesimulation, the Student Satisfaction and Self-confidence in Learning Scale and the Scale of Satisfaction with Simulated Clinical Experiences were applied through an electronic questionnaire. Descriptive statistics were used for data analysis. Results: Participants reported higher satisfaction (mean 4.36) than self-confidence (mean 3.83). Satisfaction with simulated clinical experiences showed that the cognitive dimension had the highest mean (9.25), followed by the realism dimension (8.83) and practical activity (8.64). Conclusion: Preoperative telesimulation is capable of promoting satisfaction and self-confidence in the Nursing students' learning. (AU)


Objetivo: Evaluar la autoconfianza y satisfacción de los estudiantes de Enfermería basándose en un escenario de telesimulación preoperatoria. Método: Estudio cuantitativo exploratorio-descriptivo realizado de junio de 2021 a junio de 2022 en una universidad pública del sur de Brasil. El estudio incluyó a 28 estudiantes de pregrado en Enfermería que participaron en una telesimulación sobre enfermería preoperatoria. Después de la telesimulación, se aplicaron la Escala de Satisfacción y Autoconfianza del Estudiante en el Aprendizaje y la Escala de Satisfacción con Experiencias Clínicas Simuladas a través de un cuestionario electrónico. Se utilizó estadística descriptiva para el análisis de datos. Resultados: Los participantes reportaron una mayor satis-facción (media de 4,36) que autoconfianza (media de 3,83). La satisfacción con las experiencias clínicas simuladas mostró que la dimensión cognitiva tuvo la media más alta (9,25), seguida de la dimensión de realismo (8,83) y actividad práctica (8,64). Conclusión: La telesimulación preoperatoria es capaz de promover la satisfacción y autoconfianza en el aprendizaje de los estudiantes de Enfermería. (AU)

OBJECTIVE: to build, evaluate and test two clinical simulation scenarios for the planning and management of infusion therapy by nurses. METHODS: methodological study, with construction of scenarios based on the NLN Jeffries Simulation Theory and the theoretical model Vessel Health Preservation; evaluation of the scenario design by judges, with calculation of the Modified Kappa Coefficient (MKC); testing scenarios with the target audience. Results: scenarios built for: 1. Patient assessment and vascular device selection; and 2. Identification and management of deep vein thrombosis. In the evaluation by judges, testing of validated scenarios in relation to educational practices and simulation design, the items evaluated presented MKC values ≥ 0.74. CONCLUSION: two evidence-based scenarios related to infusion therapy were constructed, with high levels of agreement among judges regarding their design. In testing with nurses, good results were obtained regarding the design and structuring of educational practice.

Abstract Objective: to explore and describe how perioperative nurses assess and interpret the child's behavior before entering the operating room, identifying the strategies they use to reduce anxiety and the proposals for improvements. Method: descriptive qualitative study using semi-structured interviews and participant observation of daily routines. Thematic analysis of data. This study follows the recommended criteria for publication of articles of the qualitative methodology Consolidated Criteria for Reporting Qualitative Research. Results: four topics emerged from the data: a) assessment of anxiety or close communication with the child and their family; b) evaluating what was observed; c) managing anxiety and d) improving the assessment or proposals for improvements in daily practice. Conclusion: nurses assess anxiety in their daily practice through observation using their clinical judgment. The nurse's experience is decisive for the appropriate assessment of the preoperative anxiety in child. Insufficient time between waiting and entering the operating room, lack of information from child and their parents about the surgical procedure, and parental anxiety make it difficult to assess and properly manage anxiety.


Resumo Objetivo: explorar e descrever como as enfermeiras perioperatórias avaliam e interpretam o comportamento da criança antes de entrar na sala de cirurgia, identificando as estratégias que utilizam para minimizar a ansiedade e as propostas de melhoria. Método: estudo qualitativo descritivo utilizando entrevistas semiestruturadas e observação participante das rotinas diárias. Análise temática dos dados. O estudo segue os critérios recomendados para publicação de artigos da metodologia qualitativa Consolidated Criteria for Reporting Qualitative Research. Resultados: quatro temas emergiram dos dados: a) avaliação da ansiedade ou comunicação próxima com a criança e sua família; b) analisando o que foi observado; c) controlando a ansiedade e d) melhorando a avaliação ou propostas de melhoria na prática diária. Conclusão: as enfermeiras avaliam a ansiedade em sua prática diária por meio da observação e usando julgamento clínico. A experiência da enfermeira é decisiva na avaliação adequada da ansiedade pré-operatória da criança. A falta de tempo entre a espera e o momento de entrar na sala de cirurgia, a escassez de informação que a criança e os pais têm sobre o processo cirúrgico e a ansiedade dos pais, dificultam a avaliação e o controle adequado da ansiedade.


Resumen Objetivo: explorar y describir cómo las enfermeras perioperatorias evalúan e interpretan el comportamiento del niño antes de entrar a quirófano, identificando las estrategias que utilizan para minimizar la ansiedad y las propuestas de mejora. Método: estudio cualitativo descriptivo mediante entrevistas semiestructuradas y observación participante de las rutinas diarias. Análisis temático de los datos. El estudio sigue las recomendaciones de criterios para la publicación de artículos de metodología cualitativa Consolidated Criteria for Reporting Qualitative Research. Resultados: cuatro temas surgieron de los datos: a) evaluación de la ansiedad o comunicación estrecha con el niño y su familia; b) valorando lo observado; c) manejando la ansiedad y d) mejorando la evaluación o propuestas de mejora para la práctica diaria. Conclusión: enfermeras evalúan la ansiedad en su práctica diaria de forma observacional utilizando el juicio clínico. La experiencia de la enfermera es determinante en la adecuada evaluación de la ansiedad prequirúrgica del niño. La falta de tiempo entre la espera y el momento de entrar a quirófano, la mala información que tiene el niño y los padres sobre el proceso quirúrgico y la ansiedad de los padres dificultan la evaluación y el manejo correcto de la ansiedad.

Apresenta-se a experiência da simulação clínica realística (SCR) efetuada no Laboratório de Simulação Clínica Realística e de Realidade Virtual Aumentada da Faculdade de Odontologia da Universidade de São Paulo (inaugurado em junho de 2022). Essa metodologia contribui significativamente na formação do estudante de Odontologia em todas as áreas do conhecimento odontológico. Sua aplicação deve submeter-se a protocolos corretos construção do problema (briefing), a simulação e discussão posterior (debriefing). Este cenário constitui o presente e o futuro da realidade do ensino odontológico. De fato, sua estratégia possibilita a simulação de uma multiplicidade de situações clínicas. Para tal, impõe-se a estruturação de um ambiente específico (aqui denominado "laboratório"). Este trabalho resume os fundamentos dessa simulação e a avaliação interna de sua aplicabilidade e inequívoca eficácia na nossa Faculdade. De fato, a simulação é uma estratégia facilitadora imprescindível no processo ensino-aprendizagem da Odontologia, cujo papel nesse processo deve delinear-se cada vez mais importante, na medida em que outros centros formadores forem adotando esse recurso e forem progressivamente intercambiando suas experiências e avaliações (AU).


Se presenta la experiencia de simulación clínica realista (SCR) realizada en el Laboratorio de Simulación Clínica Realista y Realidad Virtual Aumentada de la Facultad de Odontología de la Universidad de São Paulo (inaugurado en junio de 2022). Esta metodología contribuye significativamente a la formación de los estudiantes de odontología en todas las áreas del conocimiento odontológico. Su aplicación debe someterse a protocolos correctos para la construcción del problema (briefing), simulación y posterior discusión (debriefing). Este escenario constituye la realidad presente y futura de la educación odontológica. De hecho, su estrategia permite simular una multiplicidad de situaciones clínicas. Para lograrlo, es necesario estructurar un entorno específico (aquí llamado "laboratorio"). Este trabajo resume los fundamentos de esta simulación y la evaluación interna de su aplicabilidad y efectividad inequívoca en nuestra Facultad. De hecho, la simulación es una estrategia facilitadora esencial en el proceso de enseñanza-aprendizaje de la Odontología, cuyo papel en este proceso debe ser cada vez más importante, a medida que otros centros de formación adopten este recurso e intercambien progresivamente sus experiencias y valoraciones (AU).


This report on the experience of realistic clinical simulation (RCS) is linked to the Realistic Clinical Simulation and Augmented Virtual Reality Laboratory, launched in June 2022 at the Faculty of Dentistry of the University of São Paulo (São Paulo). RCS is an active methodology that can significantly contribute to the education of dental students in all areas of dental knowledge. Its application must adhere to correct protocols for problem construction (briefing), the simulation itself, and the subsequent discussion (debriefing). This scenario represents both the present and the future of dental education. Indeed, its strategy enables the simulation of a multitude of clinical situations, for which the establishment of a specific environment (herein referred to as a "laboratory") is needed. The present work summarizes the basis of this simulation and the internal evaluation of its applicability and effectiveness. It is concluded that simulation is a facilitating strategy in the dental teaching-and-learning process. In fact, its role in this process will become increasingly important as far as other educational institutions adopt this resource and progressively exchange their experiences and assessments (AU).

INTRODUCCIÓN: La simulación es una herramienta que promueve la confianza y desarrollo de habilidades en los participantes. En la evaluación de programas formativos la percepción de utilidad se vincula a la confianza y aprendizaje, y corresponde al primer nivel de evaluación según Kirkpatrick. OBJETIVO: Evaluar la percepción de internos y residentes de obstetricia y ginecología en escenarios de simulación. MATERIAL Y MÉTODOS: Investigación descriptiva cuantitativa, en una muestra a conveniencia de internos y residentes, quienes contestaron una escala de valoración global y de preferencias sobre sus simulaciones. RESULTADOS: Se obtuvieron 63 respuestas de internos (63%) y 7 de residentes (78%). La valoración promedio de las simulaciones fue de 6,42 en los internos y de 6,64 de los residentes. El 67% de los internos y el 86% de los residentes no eliminaría ninguna simulación. Los internos repetirían todas las simulaciones, en tanto que los residentes repetirían tres simulaciones de un total de 11. CONCLUSIONES: La valoración de las simulaciones fue alta en ambos grupos, difiriendo en las actividades que prefieren repetir, lo cual puede relacionarse con las competencias inherentes al rol de especialista en comparación al rol del médico general o a las diferencias en autoconfianza de internos y residentes.


INTRODUCTION: Clinical simulation is a training tool that promotes confidence and the development of procedural skills in participants, from initial training to the training of professional teams. In the evaluation of training programs, the measurement of perceived usefulness is linked to confidence and learning and corresponds to a first level of evaluation of the quality of training according to Kirkpatricks model. OBJECTIVE: To evaluate the perception of medical interns and Obstetrics and Gynecology fellows regarding simulation scenarios. MATERIAL AND METHODS: Quantitative descriptive research, in a convenience sample of interns and fellows, who answered a scale of global assessment and preferences about simulations. RESULTS: The average score was 6.42 for the interns and 6.64 for the scholarship recipients. With odd answers about the repetition of scenarios. 67% of the interns and 86% of the fellows responded that they would not eliminate any simulation. Conclusions: The evaluation of the scenarios was excellent and satisfactory by the participants.

BACKGROUND: Communication skills (CS) are competencies required by health professionals. Clinical simulation with a simulated/standardized patient (SP) is a good resource for teaching this skill. However, it requires trained teachers to guide the process. Not all teachers have this training. HC short evaluation instruments are required to develop a structured observation during the activity, especially those that can be applied by all the participants to guide and understand the process from a broader perspective. AIM: Validate a short HC assessment questionnaire to evaluate these skills from a 360° analysis, i.e., from the learner, facilitator/teacher, and PS perspective. METHOD: A brief HC questionnaire was created, subjected to construct and content validity, and thus applied to 40 Kinesiology undergraduate students, facilitator/teacher, and PS during a simulated teleconsultation scenario. We analyzed the result of the communication by descriptive statistics, Lashe, Cronbach, KMO, Bartlett, IVC, Fisher and Cohen tests, and exploratory factorial analysis. RESULTS: The questionnaire showed content validity for 5 of 6 items. Construct validity with commonalities over 50% for each item. With good reliability (Cronbach's alpha > 0.79). We found high levels of HC in the students, but the concordance between observers was weak (Cohen's Kappa < 0.4). CONCLUSION: The short questionnaire for HC is a valid assessment tool during clinical simulation. RESUMEN

OBJECTIVE: to build and validate a clinical simulation scenario on hospital nurse managerial decision-making competence for undergraduate nursing students. METHOD: a descriptive and methodological study was carried out in a higher education institution, with the participation of 10 judges and five players. To do so, the conceptual simulation model proposed by Jeffries and standards of the International Nursing Association for Clinical Simulation and Learning were used to prepare the scenario and the checklist. RESULTS: the scenario was called "Managerial decision-making of nurses in the face of adverse events in a hospital". The scenario script and checklist were built for validation. The checklist was face- and content-validated. Afterward, judges used the checklist to validate the scenario, which, in its final version, was composed of Prebriefing (seven items), Scenario in Action (18 items) and Debriefing (seven items). CONCLUSION: the scenario proved to be a teaching strategy that anticipates the reality of future nurses, bringing them the self-confidence to perform their activities and helping them to act critically and reflectively during decision-making processes.

Objetivo: relatar a implementação de simulações in situ na educação permanente dos profissionais de enfermagem de uma unidade de terapia intensiva. Métodos: relato de experiência realizada de dezembro de 2018 a fevereiro de 2019 com 37 profissionais de enfermagem. Aplicou-se dois cenários de simulação. As simulações foram gravadas em áudio e vídeo, seguidas de debriefing com Bom Julgamento. Resultados: O cenário da assistência ao paciente em parada cardiorrespiratória foi realizado sete vezes e teve a participação de 28 profissionais. O cenário assistencial voltado ao paciente em choque séptico ocorreu três vezes e envolveu nove participantes. O debriefing com Bom Julgamento associado às imagens videogravadas favoreceu a reflexão dos profissionais sobre suas ações durante as simulações. Conclusão: a simulação in situ em terapia intensiva demonstrou ser uma prática efetiva e inovadora para a educação permanente com a equipe de enfermagem. (AU)

Debriefing with Good Judgment associated with video images favored a reflection by professionals about their actions during the simulations. Objetivo: relacionar una implementación de simulaciones in situ en la educación permanente de profesionales de enfermería en una unidad de cuidados intensivos. Métodos: relación de experiencia realizada entre diciembre de 2018 y febrero de 2019 con 37 profesionales de enfermería. Se aplicaron dos escenarios de simulación. Como las simulaciones se grabaron en audio y video, siga el informe con Bom Judgamento. Resultados: el escenario de asistencia a pacientes en paro cardiorrespiratorio se realizó siete veces y contó con la participación de 28 profesionales. El escenario de atención dirigido al paciente en shock séptico ocurrió tres veces e involucró a nueve participantes. El debate con buen juicio asociado con las imágenes de video favoreció la reflexión de los profesionales sobre sus acciones durante las simulaciones. Conclusión: una simulación in situ en cuidados intensivos demostró una práctica efectiva e innovadora para la educación continua con el equipo de enfermería. (AU)

Resumo: Trata-se de um estudo metodológico para construção e validação de um cenário simulado com abordagem interprofissional, que permitirá a utilização no ensino e na educação permanente de profissionais da saúde por meio da metodologia ativa de simulação clínica. Este estudo foi realizado em uma universidade pública da região Sul do Brasil, objetivando construir e validar um cenário simulado para a pronação de pacientes críticos com Síndrome do Desconforto Respiratório Agudo (SDRA). Para isto, o estudo ocorreu em duas etapas: revisão de conteúdo, construção do cenário e de validação de conteúdo e de aparência por juízes. A revisão da literatura permitiu conhecer melhor sobre o manejo do paciente com SDRA, bem como subsidiar o desenvolvimento do caso clínico para o cenário. Um protocolo de pronação segura de um hospital universitário foi adotado. A partir desta revisão, a construção do cenário foi realizada considerando um roteiro validado. Como parte desta construção, surgiram como resultados: a descrição do cenário; a relação de materiais e equipamentos necessários para o desenvolvimento do cenário; o roteiro para o ator simulado; o guia de apoio ao facilitador; o guia de apoio ao participante; o quadro de apoio para tomada de decisão e o checklist de observação do desenvolvimento de competências e habilidades para cada profissão envolvida no cenário. Onze juízes participaram do estudo. Em relação ao perfil sociodemográfico dos juízes, a amostra foi predominantemente de enfermeiros (63,6%), seguido por fisioterapeutas (18,1%), médico (9%) e docente de enfermagem (9%). Para medir o percentual de concordância entre os juízes, adotou-se o Índice de Validade de Conteúdo (IVC) para os itens, que foram agrupados de acordo com unidades de significância. Após a leitura do cenário, os juízes responderam a um questionário do tipo Likert com 37 itens, que abordaram sobre a "Experiência Prévia do Participante/Briefing", "Conteúdo/Objetivos"; "Recursos Humanos"; "Preparo do Cenário", "Desenvolvimento do Cenário" e "Avaliação". Todos os itens obtiveram IVC superior ao desejável (0,80) e, portanto, foram considerados válidos. Além disso, os juízes realizaram sugestões de melhorias no cenário, aos quais foram acatadas ou rejeitas e discutidas com a literatura disponível. Este estudo permitiu criar e validar um cenário que reflete a prática real, ao mesmo tempo que oportuniza um ambiente seguro para os participantes e responde aos objetivos da aprendizagem.


Abstract: This is a methodological study for the construction and validation of a simulated scenario with an interprofessional approach, which will allow the use in the teaching and continuing education of health professionals through the active methodology of clinical simulation. This study was carried out in a public university in the South region of Brazil, aiming to build and validate a simulated scenario for the pronation of critically ill patients with Acute Respiratory Distress Syndrome (ARDS). To this end, the study occurred in two stages: content review, scenario construction, and content and appearance validation by judges. The literature review provided a better understanding of the management of the ARDS patient, as well as a basis for developing the clinical case for the scenario. A safe pronation protocol from a university hospital was adopted. Based on this review, the scenario was built using a validated script. As part of this process, the following results emerged: the description of the scenario; the list of materials and equipment needed for the development of the scenario; the script for the simulated actor; the facilitator support guide; the participant support guide; the decision support framework; and the checklist for observing the development of competencies and skills for each profession involved in the scenario. Eleven judges participated in the study. Regarding the sociodemographic profile of the judges, the sample was predominantly nurses (63.6%), followed by physical therapists (18.1%), physicians (9%), and nursing professors (9%). To measure the percentage of agreement between the judges, the Content Validity Index (CVI) was adopted for the items, which were grouped according to significance units. After reading the scenario, the judges answered a Likert-type questionnaire with 37 items, which addressed "Prior Participant Experience/Briefing", "Content/Objectives"; "Human Resources"; "Scenario Preparation", "Scenario Development", and "Evaluation". All items scored higher than desirable CVI (0.80) and were therefore considered valid. In addition, the judges made suggestions for improvements in the scenario, which were accepted or rejected and discussed with the available literature. This study made it possible to create and validate a scenario that reflects actual practice, while providing a safe environment for participants and meeting the learning objectives.

BACKGROUND: Clinical simulation is a strategy with broad support especially when face-to-face medical attention is difficult. In Psychiatry, the use and availability of simulation is lower than in other medical specialties, even for remote teaching. AIM: To report a pedagogical experience whose objective was to develop and implement the simulated patient technique as a teaching strategy for psychopathology and face-to-face Psychiatry for Psychiatry residents and remote for undergraduate medical students. MATERIAL AND METHODS: Six residents and 115 medical students participated in the activity. A descriptive qualitative-quantitative study was designed. Surveys were used to measure the quality and satisfaction of clinical simulation and simulation in psychopathology and psychiatry. In addition, an opinion was requested about the activity. All residents and 104 students participated in the assessment. RESULTS: The simulations were satisfactorily evaluated. The participants perceived that the pedagogical activity favored the development of generic competencies and specific skills for general Psychiatry. CONCLUSIONS: Simulation in Psychiatry does not replace face-to-face practical teaching, but it is a transitional and complementary method for clinical activities.

Objetivo: validar o conteúdo do cenário de simulação clínica sobre assistência ao paciente idoso hospitalizado em cuidados paliativos com dispneia, como ferramenta de ensino para estudantes de graduação em enfermagem. Materiais e método: estudo metodológico desenvolvido em três fases: revisão integrativa da literatura; entrevista com enfermeiros da área de gerontologia e cuidados paliativos; e validação do conteúdo do cenário de simulação. A validação contou com cinco juízes especialistas na área de simulação, gerontologia e cuidados paliativos entre abril e maio de 2022. O cenário criado foi baseado no pressuposto da Fabri: objetivos, material de estudo prévio, preparação, desenvolvimento, debriefing e avaliação. Utilizaram-se estatística descritiva e índice de validade de conteúdo. Resultados: foi identificado que os itens "duração do cenário", "características do paciente", "descrição do caso clínico", "caracterização do ator", "prebriefing" e "avaliação" obtiveram 0,80. O cenário de simulação obteve um valor geral de 0,91. Foram realizados ajustes no cenário referente à clareza da redação, às funcionalidades e à pertinência segundo as sugestões dos juízes. Conclusões: o cenário validado é uma ferramenta que auxilia no ensino da graduação em enfermagem, proporcionando uma formação nas áreas de gerontologia e de cuidados paliativos com um olhar diferenciado, o que pode levar a uma assistência humanizada e de qualidade.

El escenario de simulación obtuvo un valor general de 0,91. Se incorporaron algunos ajustes al escenario con respecto a la claridad de la escritura, las funcionalidades y su relevancia, de acuerdo con las sugerencias de los jueces. Conclusiones: el escenario validado se constituye en una herramienta que contribuirá al proceso de enseñanza del estudiante de enfermería, particularmente a la formación en las áreas de gerontología y cuidados paliativos, proporcionando una mirada diferente que redundará en cuidados de calidad y humanizados.


Objective: To validate the content of a clinical simulation scenario on the care provided to elderly patients with dyspnea hospitalized in palliative care as a teaching tool for undergraduate nursing students. Materials and method: Methodological study developed in three phases, namely: integrative literature review; interviews with nurses in the gerontology and palliative care area; and validation of the simulation scenario content. The validation phased engaged five specialist judges in the area of simulation, gerontology, and palliative care, and was conducted between April and May 2022. The scenario created was based on Fabri's assumption: objectives, previous study material, preparation, development, debriefing, and evaluation. Descriptive statistics and the content validity index were used. Results: The items "duration of the scenario", "patient characteristics", "description of the clinical case", "characterization of the actors", "prebriefing" and "evaluation" obtained a score of 0.80. The simulation scenario obtained an overall value of 0.91. Adjustments on the clarity in writing, functionalities, and the relevance of the scenario were incorporated, according to the judges' suggestions. Conclusions: The validated scenario could become a tool in the training process of undergraduate nursing courses, providing a different perspective for gerontology and palliative care training and fostering humanized and quality care.

SUMMARY STATEMENT: Psychological safety in high-fidelity simulation-based experiences ensures that learners feel safe with new experiences in the service of learning. This article is a faculty reflection on breaches in the psychological safety among nursing students in 2 independent, high-fidelity simulation-based experience studies that were conducted in 2019. Demonstrated breaches in psychological safety included fear and anxiety in the sole provider role in multiple-patient simulations and incivility directed toward nursing students in a primary nurse role in interprofessional simulations. This article adds to the growing literature on psychological safety by sharing important lessons learned regarding role clarity and interprofessional colleagues, proposing recommendations, and providing suggestions for future research on psychological safety.

BACKGROUND: Communication skills (CS) are competencies required by health professionals. Clinical simulation with a simulated/standardized patient (SP) is a good resource for teaching this skill. However, it requires trained teachers to guide the process. Not all teachers have this training. HC short evaluation instruments are required to develop a structured observation during the activity, especially those that can be applied by all the participants to guide and understand the process from a broader perspective. AIM: Validate a short HC assessment questionnaire to evaluate these skills from a 360° analysis, i.e., from the learner, facilitator/teacher, and PS perspective. METHOD: A brief HC questionnaire was created, subjected to construct and content validity, and thus applied to 40 Kinesiology undergraduate students, facilitator/teacher, and PS during a simulated teleconsultation scenario. We analyzed the result of the communication by descriptive statistics, Lashe, Cronbach, KMO, Bartlett, IVC, Fisher and Cohen tests, and exploratory factorial analysis. RESULTS: The questionnaire showed content validity for 5 of 6 items. Construct validity with commonalities over 50% for each item. With good reliability (Cronbach's alpha > 0.79). We found high levels of HC in the students, but the concordance between observers was weak (Cohen's Kappa < 0.4). CONCLUSION: The short questionnaire for HC is a valid assessment tool during clinical simulation. Incorporating the views and perceptions of all the simulation participants can improve the understanding of health communication.

AIM: To contribute to the development of disciplinary and generic competencies of medical students at Universidad Austral de Chile. through the incorporation of student- centered educational didactics such as remote simulation with a simulated patient (SP). MATERIAL AND METHOD: Quantitative, cross-sectional and descriptive research. Non-probabilistic sample included 68 third-year medical students who played two roles: interviewer and evaluator/observer. An instrument adapted from a satisfaction survey was used, applying it at the end of the experience in an anonymous and voluntary digital format (Google Forms). This tool covered 4 areas: Organization, Methodology, Self-assessment, and Evaluation of teaching performance, with closed Likert-type questions of five ordinal categories, two dichotomous questions (yes/no), and 4 open questions. The quantitative analysis was carried out using the SPSS program, and the qualitative one (open questions) with a progressive reduction scheme through three levels. Confidentiality and the fictional contract were protected with signature. RESULTS: The educational experience was very well valued by the students. Similar tendencies were showed in the perception of simulation among the groups that performed the roles of interviewer and evaluator/observer. CONCLUSIONS: It is essential to incorporate this type of educational experience into the medical curriculum, since it constitutes a valuable contribution to the development of competencies, through experiential learning, connecting the student with reality, with their emotions, knowledge, and abilities. This approach promotes empathy and communication skills, promoting reflection and self-criticism.
