RESUMEN

Background: Cystic endometrial hyperplasia is a hormone-dependent disease induced by systemic increase in progesteronethat can occur in several domestic species, such as the rabbit. This disease may be associated with sex steroid hormones,especially progesterone, and may be asymptomatic, and it is diagnosed using complementary imaging tests such as totalabdominal ultrasound. However, surgical excisional biopsy with histopathological tissue analysis is the gold standard. Thisstudy reports a case of asymptomatic cystic endometrial hyperplasia in a female Miniature Lion Lop rabbit (Oryctolaguscuniculus domesticus) treated with therapeutic ovariohysterectomy.Case: A domestic, adult, female Miniature Lion Lop rabbit (Oryctolagus cuniculus domesticus), aged approximately 5 yearsand weighing 3.2 kg, was referred to specialized care to undergo ovariohysterectomy, an elective procedure. The tutor onlyreported the occurrence of a single episode of vulvar secretion more than 2 years ago, treated with antibiotics, with remissionof clinical signs. In the intraoperative period after celiotomy, the uterine horn and uterine body showed a significant increasein volume, with abnormal color changes and tissue consistency; however, both changes were clinically asymptomatic. Subsequently, biopsy was performed during the ovariohysterectomy procedure. The excised uterus and ovaries were placed in 10%formalin and histopathologically analyzed. The macroscopic histopathological examination of the sectioned tissue revealed aslight amount of brownish fluid inside the uterine horns, in addition to multiple cystic areas in the uterine mucosa. Microscopicexamination revealed marked hyperplasia of well-differentiated endometrial epithelial cells, occasionally forming cystic structuresof different sizes. Background: Hydronephrosis is the dilation of the pelvis and renal calyxes due to post-renal obstruction. The obstruction is often associated with extraluminal masses, blood clots and ureter ligation in castration procedures. Ureter ligation is reported as a malpractice. The renal function is reestablished if ligation is rapidly undone, but not for obstructions longer than four weeks. Often, clinical signs are results from months to years after the castration, when nephrectomy is the best therapeutic option. This paper aims to report a case of asymptomatic unilateral hydronephrosis in a 10-year-old dog caused by chronic ureter occlusion with Nylon 3.0 suture during an elective procedure. Case: A 10-year-old female pinscher dog, spayed 3 years ago was admitted at the Surgery Department of the Veterinary Hospital of the Federal University of Jataí (HV-UFJ). The animal was taken for periodontal treatment. In the physical and laboratory examination (complete blood count, hepatic and renal biochemical tests) no significant and noteworthy alterations were found. Ultrasonographic examination showed no changes in the topography and echotexture of the left kidney, however the right kidney was not visualized, with an anechoic structure suggestive of advanced and severe hydronephrosis. Therefore, exploratory laparotomy was proposed to identify the observed structure, with the periodontal treatment considered for a later time. So, a retroumbilical incision was made, followed by linea alba and the removal of simple isolated suture remaining from previous surgical procedure. In the cavity, the viscera were isolated and the left kidney was identified, observing preserved anatomy. On the other hand, the right kidney had altered topography and morphology, being exposed after release of adhesions in adjacent structures. The right renal artery and vein were dissected and a double ligature was made. Then, the right ureter was dissected, observing marked dilatation in the proximal portion and the presence of local ligation with Nylon 3.0. Right ureterectomy and right nephrectomy were performed. After nephrectomy, the capsule was ruptured, observing dark fluid in it and absence of tissue compatible with renal parenchyma. The material was preserved in 10% formaldehyde and sent for histopathological examination. Histopathology revealed risk of rupture of the renal capsule due to the advance of renal degeneration and complete absence of parenchyma. However, contrary to the severity of the histopathological, surgical and ultrasonographic findings, the patient did not present clinical signs at the time of diagnosis. Discussion: In the intraoperative evaluation, the cause of the hydronephrosis was verified to be in fact the ligation of the ureter, which may have been accidental or due to the malpractice of the veterinarian surgeon. Other possible causes such as adhesions and granulomas were ruled out because the Nylon 3.0 suture was found in the proximal portion of the right ureter. It is believed that the patient may have presented clinical signs of hydronephrosis that may have been confused by the tutors as postoperative complications, changes that if identified and performed in time, could have avoided the occurrence or worsening of hydronephrosis and subsequent nephrectomy. Clinically, the bitch did not show clinical signs presented in the literature as consistent with hydronephrosis such as polyuria, polydipsia, abdominalgia, external fistula and anorexia. Therefore, it is believed that this case is one of the first reports of the occurrence of severe hydronephrosis without typical clinical manifestations of hydronephrosis. This fact raises a warning about the thorough monitoring in the postoperative period by owners and veterinarians, in addition to highlighting concerns regarding the occurrence of medical malpractice versus surgical accidents.

This case report presents a celioscopy procedure followed by partial salpingectomy (PS) in a pregnant bitch referred to elective video assisted ovariohysterectomy. Through celioscopy it was visualized the embryonic vesicles and PS was performed after conversion to minioceliotomy. The puppies were born by eutocic delivery without clinical alterations. The case suggested that short and regular pneumoperitoneum is not deleterious to dog fetus and video surgery could be used in pregnant bitches. PS allows the fetus development and promotes bitch neutering.(AU)


Este trabalho relata um caso de celioscopia seguido de salpingectomia parcial (SP) em fêmea canina gestante encaminhada à ovariohisterectomia eletiva videoassistida. Por celioscopia foram identificadas vesículas gestacionais e a SP foi aplicada após conversão para miniceliotomia. Não foram observadas alterações clínicas nos filhotes, os quais nasceram por parto eutócico. Sugere-se que o pneumoperitônio regular por tempo reduzido não seja deletério para fetos caninos e a videocirurgia pode ser aplicada em caninos gestantes. A SP permite a continuidade do desenvolvimento fetal e a esterilização de fêmeas caninas.(AU)

Background: The nylon clamp, marketed as an accessory of electrical devices, has been used as an alternative method for definitive surgical hemostasis by ligature of arteriovenous vessels in ovariosalpingohisterectomy (OSH) in bitches and female cats. However, since May 2015, the Brazilian Veterinary Statutory Body (CFMV - Conselho Federal de Medicina Veterinária) banned its application, once it is a material still not regulated to be used in animals, in addition to the numerous reports of postoperative complications. This paper reports the migration of the clamp used in the OSH to the urinary bladder of the cat and the implications of the presence of the foreign body, in addition to discussing the use of this device in surgeries. Case: A 1-year-old Persian female cat, which, three months after an elective OSH in a private veterinary clinic, presented polyuria and hematuria. The patient presented pain at abdominal palpation; in the WBC were observed leucopenia and eosinophilia, indicating a chronic inflammatory process installed; the urinalysis showed the presence of crystals, increased urinary density, milky aspect indicating presence of pus, proteinuria and hematuria; on ultrasound were observed the presence of two amorphous mobile structures inside the bladder, producing acoustic shadow, measuring approximately 0.4 and 0.6 cm in diameter, suggesting the presence of bladder urolithiasis. Considering the results, the patient was sent for a cystotomy. The association of acepromazine (0.02 mg/kg), midazolam (0.2 mg/kg), morphine (0.2 mg/kg) and ketamine (10 mg/kg) was used intramuscularly as preanesthetic medication. For anesthetic induction, 5 mg/kg of propofol was applied intravenously, in addition to epidural anesthesia with 1 mL/4 kg of lidocaine along 0.1 mg/kg of morphine and, for maintenance, isoflurane in oxygen at 100%...(AU)

Background: The ovariosalpingohysterectomy (OSH) is one of the most performed surgeries in bitches often used in population control. Surgical techniques that decrease the chances of operative complications and cause less nociceptive stimulation and acute pain to the patient are highly sought after by veterinarians. We evaluated the inverted OSH technique (IT), which, after celiotomy, begins with the ligature execution, transection of the uterine body and subsequent ligation and ovarian disinsertion. The purpose of this investigation was to analyze aspects of IT execution in bitches, such as visceral manipulation, access to the uterus and ovaries, and surgical time compared to the traditional technique. Materials, Methods & Results: Inverted OSH was compared to the so-called traditional OSH technique, characterized by the initial removal of the ovaries followed by uterine section. The OSH techniques were performed in 24 healthy animals, with a body condition score of up to six (on a nine-point scale) in dogs aged between seven months and five years. All dogs were from the Birth Control Program of Dogs and Cats of the UFRRJ and were distributed into two groups: traditional technique (TT) and inverted technique (IT). Surgeries and anesthesia were carried out by the same veterinarian crew (surgeon, surgical assistant, and anesthesiologist). OSH techniques were evaluated by the surgeon and surgical assistant using semiquantitative analysis (multiple-choice questionnaire with closed questions that pointed scores related to the ease of uterine and ovarian manipulation, as well as to the level of manipulation of the intestinal and urinary vesicle) and qualitative descriptive methods (report of techniques with the opinion of surgeons and an external evaluator-veterinarian surgeon who attended surgeries). O objetivo deste estudo foi avaliar os efeitos analgésicos transoperatórios da infusão contínua de morfina e cetamina, associada ou não à lidocaína, em gatas submetidas à OSH eletiva. Foram utilizadas 16 fêmeas adultas, hígidas, pré-medicadas com acepromazina (0,1mg/kg) e morfina (0,5mg/kg), ambas pela via intramuscular, induzidas com cetamina (1mg/kg) e propofol (4mg/kg), pela via intravenosa, e mantidas sob anestesia geral inalatória com isoflurano a 1,4 V%. Os animais foram alocados aleatoriamente em dois grupos: grupo morfina, lidocaína e cetamina (MLK, n=8), que recebeu bolus de lidocaína (1mg/kg), pela via IV, seguido de infusão de morfina, lidocaína e cetamina (0,26mg/kg/h, 3mg/kg/h e 0,6mg/kg/h, respectivamente); e grupo morfina e cetamina (MK, n=8), que recebeu bolus de solução salina, seguido de infusão de morfina e cetamina, nas mesmas doses do MLK. Os momentos avaliados foram: M0, basal, cinco minutos após a indução; M1, imediatamente após a aplicação do bolus de lidocaína ou solução salina; M2, M3, M4 e M5, a cada cinco minutos, até completar 20 minutos do início da infusão; M6, após a incisão da musculatura; M7, após pinçamento do primeiro pedículo ovariano; M8, após pinçamento do segundo pedículo ovariano; M9, após pinçamento da cérvix; M10, após sutura da musculatura; M11, ao final da cirurgia; e M12, M13 e M14, intervalos de cinco minutos, até completar uma hora de infusão. A FP no M0 foi maior no MLK quando comparado ao MK. Em ambos os grupos, a PAS foi maior no M7 e no M8 em relação ao M0, porém no MK, além da PAS, a FP foi maior do M7 ao M13, assim como a f. Os animais do MK necessitaram de um número maior de resgates transoperatorios, total de 23, do que o MLK, total de sete. Conclui-se que a adição de lidocaína incrementou a analgesia oferecida, reduzindo o número de resgates analgésicos transoperatórios, a dose total de fentanil, bem como a probabilidade de os animais necessitarem dese tipo de resgate.(AU)


The aim of this study was to evaluate the trans-operative analgesics, continuous infusion of morphine and ketamine, with or without lidocaine in cats undergoing elective OSH. Sixteen adult cats were used, otherwise healthy, pre-medicated with acepromazine (0.1mg/kg) and morphine (0.5mg/kg), both intramuscularly, induced with ketamine (1mg/kg) and propofol (4mg/kg), intravenous, maintained under general inhalation anesthesia with isoflurane 1.4 V%. The animals were randomly allocated into two groups: morphine, lidocaine and ketamine (MLK, n= 8), which received intravenous bolus of lidocaine (1mg/kg) followed by infusion of morphine, lidocaine and ketamine (0.26mg / kg/h, 3mg / kg/h and 0.6mg / kg/h, respectively); Morphine and ketamine (MK, n= 8), who received bolus of saline followed by infusion of morphine and ketamine at the same doses of MLK. The evaluated moments were: M0, basal, 5 minutes after induction; M1 immediately after the application of lidocaine bolus injection or saline; M2, M3, M4 and M5, every 5 minutes to complete 20 minutes after the start of infusion; M6, after the incision of the musculature; M7, after clamping of the first ovarian pedicle; M8, after clamping of the second ovarian pedicle; M9, after clamping of the cervix; M10, after suturing of the musculature; M11, at the end of surgery; And M12, M13 and M14, 5 minute intervals until completing one hour of infusion. The time to extubating and full recovery of animals, and the need for rescue analgesic fentanyl intraoperatively were also evaluated. HR in M0 was higher in MLK when compared to MK. In both groups the SBP was higher in M7 and M8 compared to M0, but the MK, addition of SAP, HR was greater M7 to M13, as well as f. MK animals required a greater number of trans-operative rescues than the MLK. It was concluded that the addition of lidocaine to the protocol using morphine and ketamine increased its analgesia.(AU)

Sixteen adult cats were used, otherwise healthy, pre-medicated with acepromazine (0.1mg/kg) and morphine (0.5mg/kg), both intramuscularly, induced with ketamine (1mg/kg) and propofol (4mg/kg), intravenous, maintained under general inhalation anesthesia with isoflurane 1.4 V%. The animals were randomly allocated into two groups: morphine, lidocaine and ketamine (MLK, n= 8), which received intravenous bolus of lidocaine (1mg/kg) followed by infusion of morphine, lidocaine and ketamine (0.26mg / kg/h, 3mg / kg/h and 0.6mg / kg/h, respectively); Morphine and ketamine (MK, n= 8), who received bolus of saline followed by infusion of morphine and ketamine at the same doses of MLK. The evaluated moments were: M0, basal, 5 minutes after induction; M1 immediately after the application of lidocaine bolus injection or saline; M2, M3, M4 and M5, every 5 minutes to complete 20 minutes after the start of infusion; M6, after the incision of the musculature; M7, after clamping of the first ovarian pedicle; M8, after clamping of the second ovarian pedicle; M9, after clamping of the cervix; M10, after suturing of the musculature; M11, at the end of surgery; And M12, M13 and M14, 5 minute intervals until completing one hour of infusion. The time to extubating and full recovery of animals, and the need for rescue analgesic fentanyl intraoperatively were also evaluated. HR in M0 was higher in MLK when compared to MK. In both groups the SBP was higher in M7 and M8 compared to M0, but the MK, addition of SAP, HR was greater M7 to M13, as well as f. MK animals required a greater number of trans-operative rescues than the MLK. It was concluded that the addition of lidocaine to the protocol using morphine and ketamine increased its analgesia.(AU)

Objetivou-se comparar os efeitos fisiológicos, analgésicos e sobre a taxa de infusão de propofol, decorrentes da anestesia epidural com lidocaína, associada ao tramadol ou à dexmedetomidina, em felinas submetidas à ovariosalpingohisterectomia (OSH). Para tal, 16 felinas hígidas foram pré-tratadas com acepromazina 0,08mg/kg/IM, utilizando-se propofol para a indução (dose-efeito) e manutenção anestésicas. Após indução, as gatas foram aleatoriamente distribuídas em dois grupos (n=8), designados: grupo lidocaína-tramadol (GLT), tratado com lidocaína (3,0mg/kg) associada ao tramadol (2,0mg/kg); e grupo lidocaína-dexmedetomidina (GLD), tratado com lidocaína (3,0mg/kg) associada à dexmedetomidina (2µg/kg), pela via epidural. Durante a OSH, a infusão de propofol foi aumentada ou reduzida, objetivando-se manutenção de plano anestésico cirúrgico. Foram avaliados os parâmetros: f, FC, SPO2, EtCO2, PAS, PAD, PAM, T°C, nos períodos pré (M1) e transoperatórios (M2 a M7); a taxa mínima de propofol necessária; o tempo de recuperação anestésica e a qualidade da analgesia pós-cirúrgica durante seis horas. Ambos os tratamentos garantiram baixas taxas mínimas de infusão de propofol, todavia o uso da dexmedetomidina resultou em bradicardia inicial, elevação da pressão arterial, maior tempo de recuperação e menor qualidade analgésica, quando comparada ao tramadol.(AU)

Anomalias reprodutivas em fêmeas domésticas são de rara ocorrência e caracterizam-se na grande maioria dos casos pela ausência de sintomatologia clínica que possa ser percebida pelos proprietários, dessa maneira o diagnóstico ocorre muitas vezes de forma incidental durante OSH, necropsia ou celiotomia exploratória. Este artigo descreve o achado incidental de agenesia ovariana unilateral direita em gata SRD submetida a OSH em programa de castração solidária. Na ocasião, não foram realizados exames complementares pré-cirúrgicos, estando a avaliação do animal restrita apenas a exame físico geral e palpação abdominal. A abordagem foi realizada por meio de acesso mediano ventral. Após a exposição do corno uterino esquerdo foi constatada inexistência do ovário esquerdo, que foi confirmada mediante palpação cranial do complexo arteriovenoso ovariano (CAVO) até a superfície renal. Não houve dificuldades na realização das manobras de hemostasia, no entanto, a ausência de exames de imagem pode aumentar o risco cirúrgico, sobretudo para profissionais ou estudantes inexperientes. Apesar de ser uma condição de simples resolução cirúrgica, deve-se evitar demasiada e brusca tração do CAVO devido ao risco de complicações decorrentes de sua ruptura.(AU)


Reproductive anomalies in domestic cats are uncommonand in most cases, they are characterized by the absence of clinical symptoms that can be perceived by the owners, this way the diagnosis often occurs incidentally during OSH, necropsy or exploratory celiotomy. This article describes the incidental finding of right unilateral ovarian agenesis in a female cat submitted to OSH in a solidarity castration program. At the time, no pre-surgical complementary exams were performed, the evaluation of the animal is restricted to general physical examination and abdominal palpation. The approach was performed through the ventral median approach. After exposure of the left uterine horn there was no left ovary, confirmed by cranial palpation of the ovarian arteriovenous complex (OAVC) to the surface kidney. No difficulties were found in performing hemostasis maneuvers, however, the absence of imaging may increase the surgical risk, especially for inexperienced professionals or students. Despite being a condition of simple surgical resolution, excessive and sudden traction should be avoided OAVC due to the risk of complications arising from its rupture.(AU)

Este artículo describe el trabajo incidental de agenesia ovárica unilateral en una gata SRD sometido a OSH en programa de castración solidaria. En la ocasión, se mantuvieron las pruebas pre quirúrgicas, con la evaluación del animal restringido sólo un examen físico general y palpación abdominal. Background: Elective ovariossalpingohysterectomy is one of the most accomplished surgeries in veterinary practice, presenting moderate degree of pain. Aiming at balanced anesthesia, α2-agonists have been increasingly used, with dexmedetomidine being characterized by its sympatholytic, sedative, analgesic properties and synergism with opioids, benzodiazepines and other drugs. The objective of this study was to evaluate the effects of dexmedetomidine alone and associated with morphine under the cardiovascular, respiratory and body temperature variables in the pre and trans-operative periods in bitches submitted to elective ovariossalpingohisterectomy under general inhalation anesthesia.Materials, Methods & Results: Sixteen bitches were used in this study, which were submitted to elective ovariossalpingohisterectomy, all of which were clinically healthy based on clinical and hematological results. The animals were randomized into two groups, in which intramuscular dexmedetomidine alone and 5 µg/kg (DG) and 0.3 mg/kg morphine (GDM). In both groups, anesthesia was induced with propofol administered and maintained with isoflurane diluted in 100% oxygen administered through a calibrated vaporizer and appropriate anesthetic system based on the weight of the animal and kept under spontaneous ventilation. After stabilization of the patient, the surgical procedure started, which lasted 45 min. The parameters evaluated were heart rate (HR), respiratory rate (ƒ), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), body temperature (TºC), expired isoflurane concentration , partial carbon dioxide gas pressure (ETCO2 ) and oxygen saturation in hemoglobin (SatO2 ), which were measured before and after the application of preanesthetic medication and at specific surgical times:[...]

The aim of this study was to evaluate cardiorespiratory parameters and anesthetic recovery in bitches submitted to continuous infusion of fentanyl, lidocaine and ketamine associated with total intravenous anesthesia with propofol and submitted to elective ovariohisterectomy. Twelve Balb-c mice were randomly divided into three groups of four animals each: G1 - non-transplanted animals, G2 - animals which received the ovarian fragment in the perimetrium, G3 - animals which had the abdominal subcutaneous tissue as a receptor site. For transplantation, mice were subjected to bilateral salpingectomia. The left ovary was submitted for histological processing. The right ovarian cortex was fragmented and reimplanted in the specified places according to the groups. Transplants were recovered in 3/4 of the mice in each treatment group. In vaginal washes, the return of the ovarian activity in half of the females of G3 and 3/4 females of G2 was observed. It was detected the presence of corpora lutea in 1/4 of G3 transplants and half in G2. In follicular population all stages of development were observed. Comparing the receptor sites, it was observed a higher percentage of return to cyclicity, as well as of follicles, in animals which had the perimetrium as the site of transplantation. In the subcutaneous, there was a lower follicular growth and lower quantity of corpus luteum. Thus, ovarian autologous transplantation allocated in the subcutaneous tissue and perimetrium allowed both tissue maintenance and follicular development; however, the site of choice will depend on the final goal one aims with the technique


Objetivou-se comparar dois sítios receptores no transplante autólogo ovariano em camundongas. Doze camundongas BALB/c foram divididas aleatoriamente em três grupos experimentais com quatro animais em cada. O G1 - animais não submetidos ao transplante, G2 - animais que receberam o fragmento ovariano no perimétrio, G3 - animais que tiveram o tecido subcutâneo abdominal como sítio receptor. Para o transplante, as fêmeas foram submetidas à ovariossalpingectomia bilateral. O ovário esquerdo foi encaminhado para processamento histológico. O córtex do ovário direito foi fragmentado e reimplantado nos locais especificados de acordo com os grupos. Os transplantes foram recuperados em 3/4 das camundongas em cada grupo tratado. No lavado vaginal, observou-se em metade das fêmeas do G3 e em 3/4 das fêmeas do G2 retorno da ciclicidade. Observou-se presença de corpos lúteos em 1/4 dos transplantes do G3 e metade nos do G2. Na população folicular, foram observados todos os estádios de desenvolvimento. Comparando os sítios receptores observou-se maior porcentagem de retorno à ciclicidade, bem como de folículos, nos animais que tiveram o perimétrio como local do transplante. Já no subcutâneo houve menor crescimento folicular e menor quantidade de corpos lúteos. Dessa forma, conclui-se que o transplante autólogo ovariano alocado no tecido subcutâneo e no perimétrio permitiu tanto a manutenção tecidual quanto o desenvolvimento folicular, porém, o sítio de escolha vai depender do objetivo final que se almeja com a técnica
