RESUMEN

Purpose: To investigate: 1) the impact of reduced contrast sensitivity (CS) in specific areas of vision on subjective assessment of vision-related quality of life (VRQoL) and objective performance-based measures in patients with primary open-angle, primary angle-closure, and pseudoexfoliation glaucoma; and 2) correlations between measurements of contrast sensitivity (CS) in specific areas of vision and visual field (VF) tests. Methods: The Spaeth/Richman Contrast Sensitivity Test (SPARCS) measured CS in the inferior, superior, and central areas, while the Pelli-Robson test measured central CS. Visual fields were tested in participants using standard-automated perimetry. The National Eye Institute Visual Function Questionnaire (NEI-VFQ-25) assessed VRQoL, and the Compressed Assessment of Ability Related to Vision (CAARV) assessed vision-related performance. Results: Three hundred and twenty-two eyes of 161 participants were included in our analysis. Significant correlations were found between CS and VF scores in the inferior, central, and superior areas (P < .0001 for all). Significant correlations were found between SPARCS scores in the inferior areas in both eyes and most CAARV scores (P < .05). Significant correlations were also found between SPARCS scores in the inferior and superior areas in the worse eye and most NEI-VFQ -25 scores (P < .05). Conclusion: Contrast sensitivity and VF scores significantly correlated in all tested areas. Reduced CS in the inferior areas of both eyes affected most performance-based measures. Measurement of CS areas using SPARCS is a meaningful way to assess VRQoL and ability to perform daily activities in participants with glaucoma.

PURPOSE: The aim of this study was to evaluate if there is any relation between the Alzheimer's peptides (amyloid ß-40 and ß-42) concentration in plasma and aqueous humor as well as cognitive functions with pseudoexfoliation syndrome (PEX). METHODS: One-hundred forty-two patients with PEX have been included for this study; median age: 75 years (Q1 = 71,5; Q3 = 80,0). Control group comprised 93 subjects aged 74 years (Q1 = 68,0; Q3 = 80,0). Amyloid ß-40 and ß-42 (Aß-40, Aß-42) concentrations were assessed in plasma in 73 PEX patients and 49 controls. Aß-40 concentration in aqueous humor was measured in 31 patients from each group. Mini Mental State Examination (MMSE) and Clock Drawing Tests were performed in 83 PEX patients and 36 controls. RESULTS: The differences between amyloid concentrations both in plasma and in aqueous humor in PEX and control groups were not statistically significant. There were no differences in MMSE and Clock Drawing Tests between groups. CONCLUSION: Our results do not indicate any relation between PEX and Alzheimer's amyloids or cognitive functions in cataract patients.

PURPOSE: To determine the association between vision-related quality of life (VRQOL) and levels of visual function loss in the Early Manifest Glaucoma Trial (EMGT). METHODS: Two hundred and fifty-five patients were included in the EMGT between 1993 and 1997 and followed regularly by ophthalmic examinations. A Swedish translation of the National Eye Institute Visual Function Questionnaire 25 (NEI VFQ-25) was self-administered at several follow-up visits until 2014. We analysed the association between Rasch-calibrated NEI VFQ-25 scores and visual function in the best eye at the final follow-up visit. RESULTS: Ninety-one per cent (233/255) of all participants completed the NEI VFQ-25 at least once. In univariate logistic regression analysis, NEI VFQ-25 scores were modestly associated with visual acuity (VA) (r(2)  = 0.330, p < 0.001), visual field index (VFI) (r(2)  = 0.200, p < 0.001) and perimetric mean deviation (MD) (r(2)  = 0.193, p < 0.001). In multivariate analysis, VA and VFI together accounted for approximately 40% (r(2)  = 0.380) of the NEI VFQ-25 scores. NEI VFQ-25 scores were significantly higher for patients with no visual impairment (mean 73 ± 22) than for visually impaired patients (mean 31 ± 15, p < 0.001). VFI worse than 50% or MD worse than -18 dB was significantly associated with low VRQOL scores (p < 0.001). CONCLUSIONS: Our results support the widespread, albeit arbitrary, use of a better-eye visual field of <50% as an important threshold for a significant reduction in VRQOL.

PURPOSE: To validate the Italian version of the Glaucoma Symptom Scale (GSS) Questionnaire and its symptoms and function subscales. METHODS: This transversal validation study enrolled nonhospitalized patients with glaucoma, and a reference sample of patients without eye diseases. Eligible participants had to be cognitively able to respond to a health status interview. The Italian self-administered versions of the 25-item National Eye Institute-Visual Function Questionnaire and the GSS Questionnaire were administered to all participants. Reliability and validity of the Italian translation of the GSS Questionnaire were tested using standard statistical methods for questionnaire validation. RESULTS: Ninety-seven patients were enrolled. Cronbach α coefficient ranged from 0.72 to 0.92 across subscales and eyes. Test-retest stability was >85% for each subscale and eye. The control group of participants had better scale scores across all dimensions of vision-targeted health-related quality of life captured by the GSS Questionnaire (P<0.05) and there were good correlations between responses GSS Questionnaire subscales and analogous domains of the 25-item National Eye Institute-Visual Function Questionnaire. CONCLUSIONS: The Italian version of the GSS Questionnaire has good validity, discriminatory power, internal consistence and reliability, showing psychometric properties comparable with those of the English version, and can therefore be used in clinical research as a specific measure of vision-related quality of life in Italian-speaking patients with ocular hypertension or glaucoma.
