RESUMEN

BACKGROUND/AIM: Tooth loss affects oral health-related life quality. More than a third of edentulous patients are not fully satisfied with their complete dentures and mainly complain of insufficient stability, retention, and pain during mastication. Solving the problem may include relining by materials that are based on silicone or acrylic. The aim of this study was to determine the level of patients' satisfaction before and after relining upper dentures with soft and rigid liners. METHODS: The patients (n=24) were divided into two study groups. Maxillary denture relining of the first group of patients was performed with hard acrylic based resins while in the second group of patients complete denture was relined with a silicone-based soft liner. They were asked the questions from the specifically adapted the Oral Health Impact Profile Questionnaire for edentulous patients before and three months after relining dentures. RESULTS: After relining the patients showed a higher degree of satisfaction with their dentures in all the tested domains (masticatory function, psychological discomfort, social disability and retention and hygiene). The padents with soft denture relines were more satisfied. CONCLUSION: Refining of maxillary complete dentures significantly positively impacts the quality of life of patients in all the tested domains (masticatory function, psychological discomfort, social disability, pain and oral hygiene). Better results were achieved using a silicone-based soft liner, which recommends it as the material of choice for relining dentures.

PURPOSE: The purpose was to assess and compare self-reported oral health and oral and general health related quality of life (OHRQoL and HRQoL) in two groups of edentulous adults who reported dissatisfaction with their mandibular dentures, and who were treated with a conventional relining of this denture or by having it converted into an implant-retained one. MATERIALS AND METHODS: Sixty subjects were randomly allocated into two equal groups, a relined conventional denture (RCD) group and an implant-retained overdenture (IOD) group. Data on demographics, oral health, OHRQoL, and HRQoL were recorded by means of a self-administered questionnaire at baseline, 3 months, and 2 years. RESULTS: Fifty-four subjects completed the protocol, 28 in the IOD-group and 26 in the RCD-group. The IOD group reported significant improvement in oral health and Oral Health Impact Profile (OHIP-20) sum score and all its domains after 3 months. The improvements remained stable at the 2-year control. The RCD group reported almost no significant improvements. Neither group reported improved HRQoL. CONCLUSION: The results of this study support the findings from other RCT studies that to implant-retain the mandibular denture significantly improves self-reported oral health and OHRQoL. This treatment modality should be a minimum standard of care in complete denture wearers dissatisfied with their mandibular denture.

BACKGROUND: Knowledge of benefits caused by a treatment on quality of life is very relevant. Despite the wide use and acceptance of soft denture liners, it is necessary to evaluate the patient's response about the use of these materials with regard to improvement in oral health related quality of life (OHRQoL). OBJECTIVES: The aim of this study was to evaluate the influence of denture relining in the OHRQoL of edentulous patients. MATERIALS AND METHODS: Thirty-two complete denture wearers had their lower dentures relined with a silicone-based material (Mucopren soft, Kettenbach, Germany) according to chairside procedures. OHRQoL was assessed before and after 3 months of relining by means of OHIP-EDENT, and the median scores were compared by Wilcoxon test (p ≤ 0.05). RESULTS: After 3 months of relining, participants reported significant improvement of their OHRQoL (p ≤ 0.01). CONCLUSION: Denture relining with a soft liner may have a positive impact on the perceived oral health of edentulous patients.

Clinicians reline or replace dentures to improve occlusion, stability, retention and facial support. They believe that well-fitting dentures will maintain supporting tissues in good health and satisfy patients by improving oral function and self-esteem. In this study, satisfaction of 21 patients with poorly fitting dentures was assessed before and after the dentures were modified to improve occlusion, vertical dimension of occlusion, and retention and stability or the dentures were replaced with new ones. More than 55% of the patients were moderately to fully satisfied with their poorly fitting dentures. Most patients perceived improvements in chewing comfort, chewing ability, eating enjoyment, food choices, security, and speech after each denture modification and with new dentures. The relative importance of the four sequential modifications could not be determined because they were made sequentially and their effects became cumulative. Chewing ability, eating enjoyment, particles under dentures, and food choices were highly correlated with chewing comfort and overall patient satisfaction. The results support the belief of many clinicians that patients benefit from relining of poorly fitting dentures or replacement with new dentures. Improvements in chewing function were perceived by most patients despite the lack of improvement in masticatory performance or masseter muscle activity with modified or new dentures. It is likely that denture wearers perceive chewing ability in terms of chewing comfort rather than ability to comminute food, an objective measure of chewing performance. The latter is not altered markedly by the clinical excellence of dentures.

In 1992 a follow-up on the Dutch National Dental Survey of 1986 (DNDS) was performed. The main objective was to detect and analyze changes in oral self care, dental attendance, and oral health status. This paper will focus on the DNDS adults wearing complete dentures. Changes over a 6-yr period will be presented of: dental attendance, denture satisfaction and wearing of the complete dentures, and denture treatments. A postal questionnaire was used: 232 persons (64%) participated in the study. Analysis of Variance (ANOVA) was used to study the effect of dental and social variables on observed changes since 1986. Risk ratios (RR) were computed to clarify the actual effects of these variables. With respect to dental attendance, a shift towards visits for denture check-ups was found, thought to reflect the respondents' concern for their complete dentures (RR = 1.6). A small shift towards more visits because of denture problems was found. This was attributed to denture treatments that had taken place between 1986 and 1992 (RR = 2.8). With respect to changes in denture satisfaction, no influence of denture treatments could be noted, but a relation was found with a variable indicating the imperfection of the complete dentures as felt by the respondents (RR = 2.8). In 31% of the respondents some denture treatment had been performed since 1986. No objective dental criteria of complete dentures and denture bearing areas had influenced these denture treatment decisions, but only respondents' subjective criteria (RR = 1.9-2.9).
