Introdução: Devido à crescente demanda estética, a odontologia restauradora se renova na intenção de alcançar melhores resultados na harmonia do sorriso. Entretanto, a atenção deve voltar-se também aos tecidos periodontais, visto que há uma integração entre material restaurador e os tecidos. Objetivo:Descrever e discutir as etapas clínicas para a confecção de laminados cerâmicos, atentando-se a preparos minimamente invasivos, contorno cervical e fatores que podem intervir na longevidade do procedimento.Relato de caso:Paciente do sexo masculino, 28 anos de idade, compareceu a Associação Brasileira de Odontologia, seção de Alagoas, relatando insatisfação em relação ao tamanho dos seus dentes. Os arcos superior e inferior foram moldados para a obtenção dos modelos de estudo e enceramento diagnóstico. Ao início do tratamento foi orientado ao paciente clareamento dental pela técnica combinada. O preparo dos dentes foi guiado por uma matriz de silicone, sendo estes uniformes e conservadores, seguido de moldagem, fase laboratorial e cimentação final. Finalizado o caso foi realizado o ajuste oclusal solicitando ao paciente que realizasse movimentos de lateralidade e protrusão. Conclusões:A técnica do preparo influência na longevidade dos laminados cerâmicos, por relacioanar-se à adesão e a saúde periodontal. O preparo deve ser mínino e limitado ao esmalte dental sempre que possível, determinando assim um maior e melhor prognóstico (AU).


Introduction:Due to the growing aesthetic demand, restorative dentistry is renewed to achieve better results in smile harmony. However, attention should also be turned to periodontal tissues since there is an integration between restorative material and tissues. Objective:To describe and discuss the clinical steps for the manufacture of laminate veneers, paying attention to minimally invasive preparations, cervical contour, and factors that can intervene in the procedure's longevity. Case report:A 28-year-old male patient attended the Brazilian Dental Association, the Alagoas section, reporting dissatisfaction with his teeth' size. The upper and lower arches were molded to obtain the study and diagnostic waxing models. At the beginning of treatment, the patient was instructed by the combined technique. After the case, the occlusal adjustment was performed, asking the patient to perform laterality and protrusion movements. Conclusions:The preparation technique influences the longevity of laminate veneers due to adherence and periodontal health. The preparation should be minimal and limited to dental enamel whenever possible, thus determining a higher and better prognosis (AU).


Introducción: Debido a la creciente demanda estética, la odontología restauradora se renueva para lograr mejores resultados en la armonía de la sonrisa. Sin embargo, también se debe prestar atención a los tejidos periodontales ya que existe una integraciónentre el material restaurador y los tejidos.Objetivo: Describir y discutir los pasos clínicos para la confección de carillas laminares, prestando atención a las preparaciones mínimamente invasivas, al contorno cervical y a los factores que pueden intervenir en la longevidad del procedimiento.Reporte del caso: Un paciente masculino de 28 años asistió a la Asociación Dental Brasileña, sección de Alagoas, reportando insatisfacción con el tamaño de sus dientes. Los arcos superior e inferior fueron moldeados para obtener los modelos de depilación de estudio y diagnóstico. Al comienzo del tratamiento, el paciente fue instruido por la técnica combinada. La preparación de los dientes fue guiada por una matriz de silicona, siendo estas uniformes y conservadoras, seguidas de moldeo, fase de laboratorio y cementación final. Después del caso, se realizó el ajuste oclusal, pidiendo al paciente que realizara movimientos de lateralidad y protuberancia. Conclusiones: La técnica de preparación influye en la longevidad de los laminados cerámicos, debido a la adherencia y la salud periodontal. La preparación debe ser mínima y limitada al esmalte dental siempre que sea posible, determinando así un pronóstico más alto y mejor (AU).

RESUMEN

This article presents a prototype of an intraoral device that facilitates preparation of parallel guide planes with accuracy and minimal occlusal divergence. The design of this device facilitates transfer of the desired path of insertion from the diagnostic cast to the mouth quickly and efficiently.

RESUMEN

This paper presents the advantages and limitations related to the use of a clinical microscope in restorative dentistry, and it demonstrates the aid of magnification during preparation and restoration of a direct composite veneer. Good illumination and visibility is important to adequately viewing the adjacent dental tissues so that the resin composite buildup can mimic natural teeth. The reproduction of details results in a naturally esthetic direct veneer.

UNLABELLED: During the past decades, scientific developments in cutting instruments have changed the conventional techniques used to remove caries lesions. Ultrasound emerged as an alternative for caries removal since the 1950s. However, the conventional technology for diamond powder aggregation with nickel metallic binders could not withstand ultrasonic power. Around 5 years ago, an alternative approach using chemical vapor deposition (CVD) resulted in synthetic diamond technology. CVD diamond burs are obtained with high adherence of the diamond as a unique stone on the metallic surface with excellent abrading performance. This technology allows for diamond deposition with coalescent granulation in different formats of substrates. When connected to an ultrasonic handpiece, CVD diamond burs become an option for cavity preparation, maximizing preservation of tooth structure. Potential advantages such as reduced noise, minimal damage to the gingival tissue, extended bur durability, improved proximal cavity access, reduced risk of hitting the adjacent tooth resulting from the high inclination angles, and minimal patient's risk of metal contamination. These innovative instruments also potentially eliminate some problems regarding decreased cutting efficiency of conventional diamond burs. CLINICAL SIGNIFICANCE: This clinical report presents the benefits of using CVD diamond burs coupled with an ultrasonic handpiece in the treatment of incipient caries. CVD diamond burs coupled with an ultrasonic device offer a promising alternative for removal of carious lesions when ultraconservative cavity preparations are required. Additionally, this system provides a less-painful technique for caries removal, with minimal noise.

STATEMENT OF PROBLEM: Highly polished enamel surfaces are recommended for axial tooth surfaces that will serve as guiding planes and be contacted by component parts of a removable partial denture. There is little evidence to support the assumption that this tooth modification will provide accurate adaptation of the framework and prevent build-up of plaque. PURPOSE: The aim of this investigation was to evaluate the surface roughness of the tooth enamel, prepared to serve as guiding planes, with different polishing systems. MATERIAL AND METHODS: Four different methods (designated A, B, C, and D) for finishing and polishing the prepared enamel surfaces of 20 freshly extracted third molar teeth were studied. Each method involved 3, 4, or 5 different steps. The roughness of each specimen was measured at the start of each method before recontouring, after recontouring, and after each step of the 4 finishing and polishing procedures. The 4 experimental finishing methods were applied after recontouring the axial surfaces (buccal, lingual, and proximal) of each tooth. Thus the 20 teeth (60 surfaces) were finished and polished by use of 1 of the experimental methods. Surface roughness was measured with a profilometer (microm); the readings of the unpolished enamel surfaces were recorded as control measurements. Results were statistically analyzed with one-way analysis of variance followed by Tukey's test at the 95% level of confidence. RESULTS: The highest roughness mean values (14.41 microm to 16.44 microm) were found when the diamond bur was used at a high speed for tooth preparation. A significant decrease in roughness values was observed with the diamond bur at a low speed (P<.05). Analysis of the roughness values revealed that all polishing methods produced surface roughness similar to that of the corresponding control teeth. CONCLUSION: Within the limitations of this study, all finishing procedures tested effectively promoted an enamel surface similar to the original unpolished enamel.

PURPOSE: A data baseline on dental cutting methodologies was established by means of a survey of North American dental school teaching. MATERIALS AND METHODS: Sixty-four North American dental schools were surveyed regarding their recommendations on handpiece usage and coolant flow rates in fixed prosthodontics and operative dentistry. RESULTS: High-speed handpieces were the instruments of choice for tooth preparation in fixed prosthodontics. In operative procedures, recommendations for sole use of the high-speed, the low-speed, or both handpiece types were more uniform. CONCLUSIONS: North American dental schools advocate greater use of high-speed than low-speed handpieces. Although the use of high-speed handpieces predominate in schools in Canada and Puerto Rico, there is a proportionately higher use of low-speed handpieces than in US dental schools. Few (approximately 1 in 5) schools made recommendations on coolant flow rates during cutting procedures.

RESUMEN

Standardization of measurements for marginal fit of castings is critical. This study describes the fabrication of a device that allowed fixation of specimens on a Toolmakers microscope with identical conditions according to tri-dimensional positioning of specimens, measuring location, and seating force. The device also allows mapping of the marginal discrepancies on the entire marginal perimeter of the tooth preparation.
