Eccrine poromas are benign adnexal tumors that originate from the intraepidermal portion of the sweat gland duct, representing approximately 10 percent of all tumors of these glands. The classic variant of poroma is characterized by a skin-colored or erythematous papular-looking neoformation, with a predilection for the palms and soles. We present the case of a 53-year-old female patient with a history of type I diabetes mellitus who attended the dermatology clinic of the polyclinic in her health area due to a 2-year history of an asymptomatic lesion on the sole of her left foot. It started as a small ball, noticing that it was increasing in size, she first went to the podiatry consultation and was sent to our specialty, where she was evaluated and an eccrine poroma was clinically diagnosed. In the histopathological study it was of great value depending on the diagnostic orientation and the presence of this entity was specified. The case presented contributes to make visible the necessary review of the subject, taking into account that it represents a diagnostic challenge, due to its great morphological variety and histological studies are necessary(EU)

RATIONALE: Eccrine poroma, a benign cutaneous neoplasm originating from the intraepidermal portion of the eccrine sweat duct, is relatively common in clinical practice. Nevertheless, the 1 presenting as spindle-shaped plaque is extremely rare and easily misdiagnosed as seborrheic keratosis or other dermatoses. Thus, the current study demonstrates a case of eccrine poroma with unique clinical manifestation. PATIENTS CONCERNS: A 47-year-old man presented with a spindle-shaped plaque on his left sole for 6 years. DIAGNOSES: Based on the clinical and histopathological manifestations, diagnosis of eccrine poroma was established. INTERVENTIONS: Surgical excision under local anesthesia was performed. OUTCOMES: No recurrence or malignant transformation occurred within 6-month follow-up. LESSONS: Eccrine poroma typically presents as a dome-shaped nodule on palm or sole. But this case reminded us the lesion presenting as a spindle-shaped plaque on sole can not rule out the possibility of eccrine poroma.

Poromas are benign adnexal neoplasms originating from the intraepidermal portion of sweat gland ducts. With the possibility of malignant transformation, accurate clinical diagnosis and treatment are crucial. Numerous reports of hand poroma lesions have been reported. We present an unusual case of a distal thumb poroma originally identified as a squamous cell lesion in a shave biopsy and eventually accurately identified after excisional biopsy. This report highlights the limitations of shave biopsy associated with soft tissue hand lesions and the need to consider poroma when evaluating a soft tissue lesion of the hand.

Eccrine poroma is a rare benign adnexal neoplasm originating from a portion of the intraepidermal eccrine sweat gland duct and the acrosyringium. Typically, the lesions are asymptomatic, slow-growing nodules, which may be found in any sweat gland-bearing area. Multiple red lacunae, glomerular vessels, hairpin vessels, flower- and leaf-like vascular patterns, a polymorphic vascular pattern, globule/lacunae-like structures, a frog egg-like appearance, and comedo-like openings have been defined as characteristic dermoscopic patterns of the disease. We report a case of eccrine poroma in an unusual periungual and subungual location mimicking ingrown toenails. The dermoscopic findings of the lesions were compatible with those of eccrine poromas located in areas other than the periungual area. Recurrence was observed after the first excisional biopsy. There was no recurrence 10 months after the second surgical intervention, and near-complete regrowth of the nail plate was achieved. Eccrine poroma should be considered as a differential diagnosis in the presence of slow-growing, erythematous, painful, hemorrhagic papular lesions located in the periungual area in conjunction with a prediagnosis of ingrown toenails and malignant processes.

Hidroacanthoma simplex is a rare intraepidermal neoplasia that arises from the acrosyringial portion of the eccrine duct. Malignant transformation of hidroacanthoma simplex is reported in the literature and the treatment is performed with wide excision or Mohs micrographic surgery. We report the first case successfully treated with cryosurgery with a long-term follow up.

BACKGROUND: Eccrine poroma is a rare, benign skin appendage tumor originating from the intraepidermal portion of the eccrine sweat duct, which typically occurs on the sides and soles of the feet. Nonetheless, eccrine poroma may be found in any skin area bearing sweat glands. OBJECTIVE: Herein we report a case of an eccrine poroma in an unusual location, the surgical management of the condition, and follow-up processes.

AIM: Pigmented poromas are rarely reported and considered to be more common in non-white people and on non-acral sites. Objective of the present study was to report our cases of pigmented poromas with particular attention to the presence of clinical and/or microscopic evidence of pigmentation, their characteristics and the diagnostic pitfall with other pathologies. METHODS: All the histologically confirmed poromas observed from January 1994 to July 2012 were considered. Clinic-epidemiological data were collected. The presence of clinical pigmentation was recorded as well as the presence of melanin pigmentation or melanocytes in the histologic specimens. RESULTS: One hundred and one patients with poromas were collected. All the patients were Caucasians. All the lesions were solitary. Only three patients had a clinically visible pigmented poromas. In eight cases the presence of melanin and melanocytes did not produce a clinically visible pigmentation. All the poromas with pigmentation did not occur on palmo-plantar surfaces. CONCLUSION: Pigmented poromas may be observed even in Caucasian patients and their clinical aspect mimic basal cell carcinoma and/or melanoma. The presence of pigment visible at the histology may not be observed in the clinical expression. The absence of pigmentation on palmo-plantar location is confirmed in all the reported cases.

A 42-year-old man presented with a rapidly growing nodule on his right cheek. Except for some itching, it was asymptomatic. The tumor was excised with 3-4 mm safety margin. The clinical differential diagnostic considerations included pyogenic granuloma, amelanotic melanoma, basal cell carcinoma and eccrine poroma. An eccrine poroma is a rare tumor derived from the eccrine sweat gland. Hormonal disturbances, trauma, immunosuppression, actinic damages and radiotherapy have been suggested as possible etiologic factors; however, the exact pathophysiology remains unclear.

Eccrine poroma is a benign adnexal neoplasm composed of epithelial cells with poroid or distal ductal differentiation. We report a healthy 57-year-old woman with a 4-year history of an asymptomatic pedunculated nodule on the lateral border of the right foot that was proven to be an eccrine poroma by histopathologic examination.
