RESUMEN

PURPOSE: The aim of this study is to describe the variable phenotype of congenital corneal opacities occurring in patients with biallelic CYP1B1 pathogenic variants. METHODS: A retrospective chart review was conducted to identify patients with congenital corneal opacities and CYP1B1 pathogenic variants seen at UPMC Children's Hospital of Pittsburgh. Ophthalmic examination, high-frequency ultrasound, anterior segment optical coherence tomography, histopathologic images, and details of genetic testing were reviewed. RESULTS: Three children were identified. All presented with raised intraocular pressure. Two patients showed bilateral limbus-to-limbus avascular corneal opacification that did not resolve with intraocular pressure control; 1 showed unilateral avascular corneal opacity with a crescent of clear cornea, iridocorneal adhesions, iridolenticular adhesions, and classical features of congenital glaucoma in the fellow eye (enlarged corneal diameter, Haab striae, and clearing of the corneal clouding with appropriate intraocular pressure control). The first 2 patients were visually rehabilitated with penetrating keratoplasty. Histopathology revealed distinct features: a variably keratinized epithelium; a thick but discontinuous Bowman-like layer with areas of disruption and abnormal cellularity; Descemet membrane, when observed, showed reduced endothelial cells; and no pathological changes of Haab striae were identified. Two patients had compound heterozygous pathogenic variants in CYP1B1 causing premature stop codons, whereas 1 was homozygous for a pathogenic missense variant. CONCLUSIONS: Congenital corneal opacities seen in biallelic CYP1B1 pathogenic variants have a variable phenotype. One is that commonly termed as Peters anomaly type 1 (with iridocorneal adhesions, with or without iridolenticular adhesions) and the other is a limbus-to-limbus opacity, termed CYP1B1 cytopathy. Clinicians should be aware of this phenotypic variability.

RESUMO A ceratopigmentação teve seu primeiro registro pelo filósofo Galeno há muitos séculos como uma estratégia utilizada para o tratamento estético de pacientes com leucomas. As córneas com leucoma são patológicas e, muitas vezes, intolerantes a lentes de contato cosméticas ou próteses oculares, sendo comum a queixa de desconforto excessivo, proporcionado pela superfície corneana irregular. Assim, a ceratopigmentação é uma alternativa para a melhora estética de pacientes com opacidades corneanas. Descrevemos o caso de um paciente do sexo masculino, 39 anos, que apresentou despigmentação precoce em caso de ceratopigmentação associado a quadro de ceratite herpética necrotizante. O paciente foi submetido ao tratamento com aciclovir 2g ao dia e doxiciclina 200mg ao dia, evoluindo com melhora do quadro clínico, apesar da má adesão medicamentosa.


ABSTRACT Keratopigmentation was first recorded many centuries ago by the philosopher Galeno, as a strategy used for the aesthetic treatment of patients with leukomas. Corneas with leucoma are pathological and often intolerant of cosmetic contact lenses or ocular prostheses, with complaints of excessive discomfort provided by the irregular corneal surface being common. Therefore, keratopigmentation is an alternative for the aesthetic improvement of patients with corneal opacities. We describe the case of a 39-year old male patient, who presented early depigmentation in a case of keratopigmentation associated with necrotizing herpetic keratitis. The patient was treated with Acyclovir 2g/day and Doxycycline 200mg/day, evolving with clinical improvement, despite poor medication adherence.

RESUMO Objetivo: Relatar, por meio de uma série de casos, a percepção de pacientes com opacidade corneana sobre a eficácia da tatuagem na melhoria estética de seus olhos, utilizando a combinação de duas técnicas. Métodos: Oito pacientes responderam a um inquérito sobre sua satisfação estética com o procedimento, o desconforto pós-operatório e o impacto social observado após a cirurgia. Resultados: Todos os pacientes consideraram-se muito satisfeitos com os resultados. Em relação ao grau de desconforto no pós-operatório, 75% disseram ter tido pouco desconforto, e 25% relataram desconforto moderado. Todos os pacientes relataram melhora significativa no bem-estar social e pessoal. Da mesma forma, todos os pacientes disseram que repetiriam o procedimento. Conclusão: A tatuagem corneana surge como um método alternativo às lentes de contato e às próteses oculares em pacientes cegos com leucomas, trazendo resultados estéticos satisfatórios, duradouros e que podem promover impactos sociais na vida desses pacientes.


ABSTRACT Objective: To report, through a case series, the perception of patients about the effectiveness of corneal tattooing in the cosmetic improvement of their eyes with leukomas, using a combination of two techniques. Methods: Eight patients answered a survey regarding their cosmetic appearance satisfaction regarding the procedure, postoperative discomfort, and social impact observed after surgery. Results: All patients considered themselves very satisfied with the cosmetic results. Regarding the degree of postoperative discomfort, 75% said they had little discomfort, while 25% reported moderate discomfort. All patients reported significant improvement in social and personal well-being. Likewise, all patients said they would repeat the procedure. Conclusion: Corneal tattooing appears as an alternative method to contact lenses and ocular prostheses in impaired eyes with leukomas, bringing satisfactory and long-lasting cosmetic improvement that can promote social impacts for these patients.

Background: Bullous Keratopathy (BK) is characterized by decompensation of the corneal stroma resulting in visual impairment by corneal opacity, discomfort, excessive tearing, blepharospasm and pain. The aim of this study was to report a case of BK in a dog and the complete recovery of the ocular structure and visual function, with a third eyelid flap associated with the use of autologous blood serum topically. Case: A 2-year-old Shih Tzu male dog, weighing 4.3 kg, with recurrent bilateral eye discomfort was brought to Ophthalmologist Veterinarian Assistance. The patient had already been seen by other veterinarians, but the signs returned in a short period of time after the prescription was suspended. During physical examination it was observed moderate blepharospasm in the right eye, epiphora in the left eye, and in both eyes was noted exophthalmos, medial corneal entropion, caruncle trichiasis and ectopic cilia in the upper and lower eyelids. Fluorescein test was positive in both eyes, and a large bullous, opaque and gelatinous lesion with irregular appearance was observed occupying a large part of the cornea of the right eye. Superficial corneal ulcer was diagnosed in left eye and BK in right eye, both probably in consequence of ectopic cilia presence. Surgical intervention was made, with the cauterization and excision of the ectopic cilia and third eyelid flap in the right eye. The cauterization of caruncle trichiasis was not authorized by the owner. Moxifloxacin eye drops in the dose of one drop four times a day in both eyes, lubricant based on hyaluronic acid in the dose of one drop four times a day in both eyes, and autogenous blood serum, obtained from the centrifugation of a blood sample of the dog itself, in the dose of one drop every hour in the right eye for 24 h, and meloxicam in the dose of...(AU)

ABSTRACT Purpose: To evaluate causes and management of congenital corneal opacities (CCO) diagnosed in a tertiary care eye center and to compare the data with a previous study at the same institution. Methods: Computerized medical records in all patients with congenital corneal opacities diagnosed in the Cornea Service at Wills Eye Hospital (Philadelphia, PA) between January 1, 2007, and December 31, 2015, were retrospectively reviewed. Children aged 12 years and younger at the first visit were included in the study. Patients' demographics, ocular diagnosis, laterality, associated ocular abnormalities, other ocular surgery performed prior or subsequent to the first visit, and their treatment were extracted from the medical records. Results: A total of 77 eyes in 56 patients were examined. The mean age at presentation was 32.8 ± 44.2 months, with the mean follow-up period of 26.7 ± 30.1 months. The most frequent diagnosis was Peters anomaly (53.2%), followed by limbal dermoid (13.0%), aniridia with glaucoma and microphthalmos (6.5%), sclerocornea and congenital glaucoma (5.2%), idiopathic (3.9%), Axenfeld-Rieger anomaly and Hurler syndrome (2.6%), and microcornea (1.3%). Primary keratoplasty was performed in 26 eyes, with the outcome rate in the clear cornea of 76.0% during the follow-up. Conclusion: Peters anomaly is the most common cause of congenital corneal opacities encountered at our institution. Penetrating keratoplasty is the most frequent choice of corneal surgery to treat congenital corneal opacities. Additional interventions during penetrating keratoplasty were moderately positively correlated with graft failure. This study also shows the rates of some etiologies of that changed over the recent decades in our tertiary care Cornea Service. Although Peters anomaly remains the most common presenting reason for congenital corneal opacities, its rate appears to be increasing over the recent decade. Congenital corneal opacities due to birth trauma, which is one of the preventable causes, were observed in a previous study in our clinic; however, no new cases were noted in this study.


RESUMO Objetivo: Avaliar as causas e o controle das opa cidades corneanas congênitas diagnosticadas em um centro oftal mológico de atendimento terciário e comparar os dados com um estudo anterior realizado na mesma instituição. Métodos: Prontuários médicos informatizados de todos os pacientes com opacidade corneana congênita diagnosticada no Serviço de Córnea no Wills Eye Hospital (Filadélfia, PA) entre 1º de ja neiro de 2007 e 31 de dezembro de 2015 foram revisados retrospectivamente. Crianças com 12 anos ou menos na primeira consulta foram incluídas no estudo. A demografia dos pacientes, o diagnóstico ocular, a lateralidade, as anormalidades oculares associadas, outras cirurgias oculares realizadas antes ou após a primeira consulta e o tratamento foram extraídos dos prontuários médicos. Resultados: Um total de 77 olhos de 56 pacientes foi examinado. A idade média de apresentação foi de 32,8 ± 44,2 meses, com um tempo médio de acompanhamento de 26,7 ± 30,1 meses. O diagnóstico mais frequente foi anomalia de Peters (53,2%), seguido por dermóide límbico (13,0%), aniridia com glaucoma e microftalmia (6,5%), esclerocórnea e glaucoma congênito (5,2%), idiopático (3,9%), síndrome de Axenfeld-Rieger e síndrome de Hurler (2,6%) e microcórnea (1,3%). Ceratoplastia primária foi realizada em 26 olhos, com desfecho de córnea clara de 76,0% durante o acompanhamento. Conclusão: A anomalia de Peters é a causa mais comum de opacidade corneana congênita encontrada em nossa instituição. A ceratoplastia penetrante é a escolha mais frequente de cirurgia corneana para o tratamento de opacidades corneanas congênitas. Intervenções adicionais durante a ceratoplastia penetrante foram moderadamente correlacionadas positivamente com a falha do enxerto. Este estudo também mostra as taxas de algumas etiologias do que mudou ao longo faz últimas décadas em nosso serviço de córnea de atendimento terciário. Embora a anomalia de Peters continue a ser a causa mais comum das opacidades congênitas da córnea, sua taxa parece estar aumentando na última década. Opacidades congênitas da córnea devido a trauma no nascimento, que é uma das causas evitáveis, foram observadas em um estudo anterior em nossa clínica; no entanto, nenhum caso novo foi observado neste estudo.

ABSTRACT The following report describes a case of apical leucoma syndrome after hyperopic photorefractive keratectomy following hyperopic laser in situ keratomileusis and the subsequent treatment of this complication with focal phototherapeutic keratectomy. The patient underwent focal transepithelial phototherapeutic keratectomy of 70 mm ablation and 1.5 mm diameter after pupil and ablation offset correction. After 1 year, significant improvement in vision was observed and only slight residual opacity remained on biomicroscopy and optical coherence tomography. Focal phototherapeutic keratectomy was effective and safe for the treatment of this complication.


RESUMO O relato a seguir descreve um caso da síndrome do Leucoma Apical após cirurgia ceratorrefrativa hipermetrópica depois de Laser hiperópico in situ keratomileusis, e o subsequente tratamento desta complicação com ceratectomia fototerapêutica focal. O paciente foi submetido à ceratectomia fototerapêutica focal transepitelial com ablação de 70 mm e diâmetro de 1,5 mm, após correção de offset pupilar. Depois de um ano, foi observada uma melhora significativa da acuidade visual permanecendo apenas leve opacidade residual na biomicroscopia e tomografia de coerência óptica. A ceratectomia fototerapêutica focal foi efetiva e segura para o tratamento desta complicação.

We report the case of a patient who received conjunctival tattooing for cosmetic purposes with associated complications. A 28-year-old male experienced severe ocular pain after undergoing conjunctival tattooing. Slit-lamp examination revealed blue deposits over the conjunctiva in association with intense conjunctival edema and a 4+ anterior chamber cellular reaction. The patient underwent clinical treatments to control ocular inflammation. Two months after the injury, the patient was still in follow-up to ensure that any further complications were managed and documented. This unusual case of conjunctival tattooing resulted in conjunctival edema and anterior uveitis. Because of the increasing popularity of eyeball tattooing, the potentially severe complications of this procedure may become more common.

RESUMO Relatamos um caso de um paciente que realizou tatuagem conjuntival para fins cosméticos com complicações associadas. Um homem de 28 anos de idade apresentou dor ocular importante após tatuagem conjuntival. O exame da lâmpada de fenda revelou depósitos azuis sobre a conjuntiva, associados a um intenso edema conjuntival e uma reação de câmara anterior de 4+ de células. O paciente foi submetido a tratamentos clínicos para controlar a inflamação ocular. Dois meses após o ocorrido, o paciente ainda estava em acompanhamento para garantir que quaisquer outras complicações fossem gerenciadas e documentadas. Um caso incomum de tatuagem conjuntival resultou em edema conjuntival e uveíte anterior. Devido à crescente popularidade da tatuagem no globo ocular, complicações potencialmente graves deste procedimento podem se tornar mais comuns.

RESUMEN

PURPOSE: To describe the clinical signs of Descemet membrane (DM) detachment due to forceps-related birth injury and its subsequent management using optical coherence tomography. METHODS: Case report. RESULTS: A 3-day-old term infant presented with left eye corneal clouding and a definitive history of traumatic forceps-assisted delivery. Despite topical therapy, corneal clouding persisted, necessitating an examination under anesthesia using ultrasound and handheld optical coherence tomography. This revealed not only a tear in DM but also a large detachment. Injection of air alone failed to achieve apposition of DM to the posterior stroma. Apposition was achieved only after penetration of the overlying cornea with the needle of a 10-0 nylon suture and release of clear viscous fluid. The cornea cleared within the first week and continued in the months to follow. CONCLUSIONS: Prolonged corneal edema should alert the physician to probable DM detachment after forceps-related birth injury. Injecting air alone may not be sufficient to reattach the detached DM.

Objetivo: correlacionar los valores queratométricos obtenidos por el programa Holladay Report del Pentacam en ojos operados de queratotomía radial con diagnóstico de catarata, y los obtenidos a través del método de Maloney.Métodos: se realizó un estudio observacional prospectivo y descriptivo a 18 ojos miopes de 14 pacientes portadores de opacidades cristalinianas previamente sometidos a queratotomía radial que acudieron al Servicio de Catarata del Instituto Cubano de Oftalmología Ramón Pando Ferrer en el período comprendido de febrero a noviembre del año 2013. Se utilizó el Pentacam para obtener de forma directa el poder refractivo corneal utilizando programas diseñados con este fin, y posteriormente compararlo con el obtenido por el método de Maloney.Resultados: se obtuvo una correlación positiva al comparar las lecturas queratométricas equivalentes a 1 mm (r= 0,962); 2 mm (r= 0,845); 3 mm (r= 0,985); 4 mm (r= 0,988); 4,5 mm (r= 0,988) y central (r= 0,976) obtenidas en el programa Holladay Report del Pentacam, y los valores queratométricos aportados por el método de Maloney, lo cual fue estadísticamente significativo (p= 0,001). Conclusiones: el programa Holladay Report del Pentacam aporta poderes corneales que no difieren estadísticamente de los obtenidos por el método de Maloney en pacientes con cirugía refractiva corneal previa y catarata con criterio quirúrgico(AU)

They had gone to the cataract service of Ramon Pando Ferrer Cuban Institute of Ophthalmology from February to November 2013. Pentacam was used to directly estimate the corneal refractive power by using tailor-made software for this purpose, and then compare it with the values obtained by Maloney´s method. Results: there was positive correlation when comparing the keratometric readings of 1 mm (r= 0.962); 2 mm (r= 0.845); 3 mm (r= 0.985); 4 mm (r=0.988); 4.5 mm (r= 0.988) and central (r= 0.976) in the Pentacam´s Holladay Report software and the keratometric values of the Maloney's method, which was statistically significant (p= 0,001). Conclusions: pentacam´s Holladay Report software reached corneal power values that do not differ statistically from those of Maloney´s method in patients who had previously undergone corneal refractive surgery and cataract with surgical criteria(AU)

RESUMO O surgimento de uma membrana fibrótica opacificada na córnea transplantada é pouco descrito nas literaturas nacional e mundial. O objetivo é relatar o caso de um paciente com leucoma total de olho esquerdo que foi submetido à ceratoplastia penetrante levando a formação de dupla câmara anterior devido ao surgimento de uma membrana fibrótica cicatricial. Paciente do sexo masculino, 54 anos, com leucoma total secundário a ceratite herpética, diabético há 20 anos, em uso de insulina, com retinopatia diabética não proliferativa. Realizou-se cirurgia de membranectomia com complicações pós-operatória.


ABSTRACT The emergence of opaque fibrotic membrane in transplanted cornea is little described in national and world literature. The goal is to report the case of a patient with leucoma total of left eye that was submitted to the penetrating keratoplasty leading to formation of double anterior chamber due to the emergence of a fibrotic scar membrane. Male patient, 54 years, with total herpetic keratitis secondary leucoma, diabetic for 20 years, using insulin, with non-proliferative diabetic retinopathy. Held membranectomia surgery with postoperative complications.

OBJETIVO: Determinar os diagnósticos clínicos corneanos na indicação da Ceratoplastia Penetrante (CP) de pacientes inscritos no Banco de Olhos do Hospital de Base de São José do Rio Preto, correlacionando-os com outras variáveis, como a faixa etária, sexo e situação sócioeconômica. MÉTODOS: Realizou-se análise retrospectiva de 1085 fichas de inscrição de pacientes submetidos à Ceratoplastia Penetrante, no Hospital de Base de São José do Rio Preto, no período de 2000 a 2009. Os diagnósticos para indicação das cirurgias foram posteriormente correlacionados com a faixa etária, o sexo e o nível socioeconômico dos pacientes, por meio do teste do Qui-quadrado. RESULTADOS: Do total de Ceratoplastias Penetrantes, 57,4% foram realizadas em pacientes do sexo masculino, e a média de idade encontrada foi de 52,3 anos (DP=21,42). Verificou-se que em 90,8% dos casos a cirurgia ocorreu por meio do Sistema Único de Saúde (SUS), enquanto nos demais (9,2%) os pacientes possuíam convênio ou custearam todo o procedimento. A indicação mais frequentemente encontrada foi o leucoma (23%), o qual ocorreu em 14% das CPs realizadas em homens e em 9% das ocorridas em mulheres. Em relação às CPs com este diagnóstico, 40% ocorreram em pacientes com idade superior a 60 anos, com porcentagem decrescendo com as faixas etárias mais jovens, e 94,4% efetuaram-se através do SUS, enquanto que apenas 5,6% dos casos se encaixaram no quesito convênio/particular. CONCLUSÃO: A principal indicação à Ceratoplastia Penetrante encontrada foi o leucoma, o qual predominou em homens, nas idades acima de 60 anos, em pacientes do SUS.


OBJECTIVE: To determinate the corneal diagnoses of patients underwent Penetrating Keratoplasty (PK) in the university hospital of São José do Rio Preto, São Paulo, Brazil, and their relation to age group, gender and socioeconomic factors. METHODS: Retrospective analysis of 1085 registration forms of patients underwent Penetrating Keratoplasty in the university hospital of São José do Rio Preto, São Paulo, Brazil, from 2000 to 2009. The surgical indications were then correlated to age group, gender and socioeconomic factors, by chi-square test. RESULTS: Fifty-seven percent of all Penetrating Keratoplasties were performed in male patients, and the mean age was 52.3 years. Most surgeries (90.8%) occurred by means of public health care (SUS). The main diagnosis was leukoma (23%), representing 14% of the indications for PK in males and 9% in females. Leukoma was also the diagnosis in 40% of PK performed in patients older than 60 years and in 94.4% of these surgeries in public health care users. CONCLUSION: The main indication for Penetrating Keratoplasty was leukoma, predominating in males, ages above 60 years and in public health care users.

RESUMEN

A Síndrome de Urrets-Zavalia apresenta achados oculares bem descritos, porém sua fisiopatologia ainda é incerta. A isquemia iriana é o mecanismo proposto mais comum. Descrevemos dois casos submetidos à ceratoplastia lamelar profunda (CLP) realizadas pelo mesmo cirugião que desenvolveram a síndrome. No primeiro caso, a indicação cirúrgica foi para o tratamento de opacidade corneana e, no segundo, para o de ceratocone. No pós-operatório, ambos os pacientes evoluíram com pupila dilatada fixa que não regrediu totalmente apesar do tratamento administrado.


The Urrets-Zavalia Syndrome presents well described ocular findings, even though its physiopathology is still unsure. Iris ischemia is the most common proposing mechanism. We describe two cases that underwent deep lamellar keratoplasty (DLK) performed by the same surgeon and developed the syndrome. In the first case, the surgical indication was for corneal opacity treatment and, in the second case, for keratoconus treatment. During the post-operatory, both patients developed fixed dilated pupil, which didn't regress completely inspite of the onset treatment.

PURPOSE: To assess visual results and compare methods of measuring central corneal thickness (CCT) and corneal opacity thickness (COT) in patients with corneal opacities induced by epidemic keratoconjunctivitis (EKC) and treated with phototherapeutic keratectomy (PTK) using low-dose mitomycin C (MMC). DESIGN: Prospective consecutive case series. METHODS: Patients with chronic adenoviral corneal opacity underwent transepithelial PTK with MMC 0.002% for 1 minute. The presence of photophobia, the best spectacle-corrected visual acuity (BSCVA), and the contrast sensitivity were evaluated. CCT measurements were obtained with ultrasound pachymeter (US), ultrasound biomicroscopy (UBM), Scheimpflug tomography (Pentacam Oculus), and optical coherence tomography (OCT Visante). COT measurements were obtained with UBM, Pentacam, and OCT. RESULTS: Thirty-one eyes of 23 patients, comprising 15 women (65.2%) and 8 men (34.8%), mean age 41.8 years, were enrolled in the study. Duration of visual disturbance was 19.1 ± 14 months. The number of patients with photophobia was reduced from 100% to 29% after surgery. BSCVA improved 2 or more lines in 78% of the patients at 12 months. A hyperopic shift of 1.52 ± 0.91 diopters was achieved. Contrast sensitivity improved in both photopic and mesopic conditions. For each of the instruments, the CCT postoperative mean was significantly smaller than the preoperative measurement (P < .0001) and COT values were significantly reduced in comparison to the preoperative values (P < .001). CONCLUSION: Improvements in photophobia, BSCVA, and contrast sensitivity were observed in patients treated using excimer laser PTK with low-dose MMC for subepithelial infiltrates.
