RESUMEN
The work analyses the results of the multimodality treatment of 80 patients among whom 20 suffered from endarteritis obliterans and 60 from atherosclerosis obliterans of the lower limb vessels. In the process of complex treatment intravascular laser irradiation of blood (ILIB) was carried out by introducing the light guide into a peripheral vein. In 31 (73.8%) patients with decompensated circulatory disorders in the limbs, a course of ILIB produced a good clinical effect. A positive effect was encountered in 35 (92.1%) of patients with subcompensated circulatory disorders in the lower limbs. The clinical efficacy of ILIB is due to normalization of blood coagulative properties, increase of the fibrinolytic potential, improvement of hemorheology, and normalization of the lipoprotein ratio.

Fifty-seven patients with obliterating diseases of the lower limb arteries were examined. The concentration of lactate in the venous blood and the venous-arterial difference in the blood lactate level in the affected limb were found to be increased. A relationship between the marked character of these shifts and the severity of the ischemic syndrome was noted. The authors discuss the possibility of using the values of the lactate content in the regional blood as criteria for evaluating the efficacy of treatment and the prognosis of obliterating diseases of the lower limb arteries.

The examination of 25 patients after intraarterial infusions of hydrogen peroxide has shown that temporary oxygen supersaturation of blood can improve the oxygenation of tissues.

The ultrastructure of blood capillaries and muscle fibers was examined in the I, II, III, and IV stages of obliterating endarteritis (OE) of the lower extremities. A number of morphological parameters were analysed quantitatively. The pathogenetic importance of changes in the fine structure of capillaries and muscle fibers for each stage of the disease was demonstrated. Each stage of OE is characterized by definite morphological features of the structure of blood capillaries and muscle fibers which permit to use these data clinically for biopsy diagnosis of different OE stages.

Based on observations of 317 patients with obliterating endarteritis, the most persistant effect following the complex therapy was noted in patients showing lymphocytosis after the treatment. The overwhelming majority of patients with unfavourable issues of the therapy showed lymphopenia. So, of 67 patients showing lymphocytosis after 2--3 courses of the therapy 48 patients proved to be practically recovered. But there was no recovery among 30 lymphopenic patients, and in 13 cases the extremity had to be amputated. Among 220 patients showing the normal amount of lymphocytes the repeat course of the therapy resulted in a complete recovery in 19, in 2 cases the extremity was amputated.
