INTRODUCTION: Erectile dysfunction (ED) is a common sexual problem among men with chronic kidney disease (CKD). The severity of sexual dysfunction tends to worsen with kidney damage. This study aims to evaluate the erectile function and sexual quality of life of adult male CKD patients undergoing hemodialysis (HD) in a hospital located in the Brazilian Amazon. METHODS: A cross-sectional quantitative study was performed within the HD Sector of the Nephrology Unit including men with CKD aged ≥ 18 years, undergoing ≥ 3 weekly HD sessions for ≥ 3 months who had been sexually active for ≥ 6 months. We used the Male Sexual Quotient (MSQ) to measure sexual satisfaction and the International Index of Erectile Function (IIEF5) to establish erectile function. Statistical analysis was performed with SPSS 21.0 using appropriate tests, such as Mann-Whitney and Kruskal-Wallis (P < 0.05). RESULTS: Ninety-eight patients (51.68 ± 15.28 years) were evaluated. They were primarily married/or living with a partner (60.20%), with HD time between 1 to 5 years (55.10%), and an average KTV of 1.17. ED prevalence was 66.30%, and it was associated with a higher age group (p = 0.01), lower family income (p = 0.02), diabetes (p = 0.01), lower mean corpuscular hemoglobin (p = 0.04), higher total calcium (p = 0.04), and lower albumin (p = 0.03). Around 75% classified their sex life as regular to excellent. CONCLUSION: Despite the high ED prevalence, most men with CKD in HD reported experiencing regular to excellent sex life. The study underscores the importance of establishing effective screening and conducting routine evaluations regarding sexual issues in these men.

OBJECTIVE: To assess and compare the functional and quality of life results in patients treated with curative intent for localized prostate cancer during 2015 in our hospital. METHOD: 77 patients treated by radical prostatectomy or external radiotherapy with androgen deprivation were prospective enrolled. Expanded Prostate Cancer Index Composite for Clinical Practice (EPIC-CP) questionnaire at 3-year follow-up and Spanish Questionnaire on Quality of Life in Patients with Prostate Cancer (CAVIPRES-30) at diagnosis and at 3-year follow-up were registered. RESULTS: 68 patients were included, 39 patients treated by radical prostatectomy and 29 received external radiotherapy with androgen deprivation. Among the operated patients, 61.5% were dry and 17.9% use three or more daily pads, compared to 72.4% and 6.8%, respectively, in the radiotherapy group. 48.7% of prostatectomized patients reported very poor or no capacity to have a sufficiently rigid erection, compared to 69% of the radiated group. After surgery, 43.6% considered bad or very bad quality-of-life, compared to 68.9% in the radiotherapy group. In the comparison of the data of the pre- and post-treatment questionnaire can be seen that the patients had a superior perception before the procedure. CONCLUSIONS: Patients treated by surgery have a better perception of quality-of-life compared to those treated by radiotherapy.


OBJETIVO: Determinar y comparar los resultados funcionales y de calidad de vida de pacientes con cáncer de próstata tratados con intención curativa durante el año 2015 en nuestro centro. MÉTODO: Se incluyeron 77 pacientes sometidos a prostatectomía radical (PR) o radioterapia externa con terapia de deprivación androgénica (TDA). Se realizaron el Expanded Prostate Cancer Index Composite for Clinical Practice (EPIC-CP) tras 3 años de seguimiento y el Cuestionario Español de Calidad de Vida en Pacientes con Cáncer de Próstata (CAVIPRES-30) al diagnóstico y a los 3 años. RESULTADOS: Se incluyeron 68 pacientes, 39 con PR y 29 con radioterapia más TDA. De los pacientes intervenidos, el 61.5% están secos y el 17.9% usan tres o más compresas, diarias frente al 72.4% y el 6.8%, respectivamente, en el grupo de radioterapia. El 48.7% de los prostatectomizados refieren erecciones muy malas o ninguna, frente al 69% de los radiados. Tras la cirugía, el 43.6% refieren mala o muy mala calidad de vida, frente al 68.9% de los radiados. En la comparación de los datos del cuestionario pre- y postratamiento, los pacientes tenían una percepción superior antes del procedimiento. CONCLUSIONES: Los pacientes tratados mediante cirugía tienen una mejor percepción de su calidad de vida relacionada con la salud que los radiados.

Erectile dysfunction is observed in about 50% of men. It has been found that diabetes mellitus increases its prevalence to 19-86.3%, necessitating attention to a therapeutic strategy. Among the available treatment methods, intracavernosal injections of mesenchymal stem cells have proven to be particularly effective. OBJECTIVE: The purpose of study is to assess and analyse the effectiveness of their use in the treatment of erectile dysfunction in patients with diabetes mellitus. MATERIALS AND METHODS: The literature search was conducted using systematic methods and analysis in databases such as Web of Science, Scopus, PubMed, Elsevier, and Springer, with 41 sources included for further review. RESULTS: The study highlights microangiopathic and neuropathic links as key factors in erectile dysfunction development in diabetic patients, stemming from endothelial dysfunction and conductivity disturbances. Mesenchymal stem cell therapy from bone marrow, adipose tissue, and umbilical cord mitigates pathogenic impact through regenerative and anti-apoptotic effects. Due to this, most studies indicate high efficacy of the treatment and rapid therapeutic action through intracavernosal administration. Some studies suggest an increase in the body's receptor sensitivity to other drugs, such as sildenafil. CONCLUSION: From the perspective of further research on this issue, standardising the preparation of stem cells and the treatment method using a large sample size is essential to introduce such a method as an extremely promising therapy for this delicate issue in men into practical medicine. The practical value of the study lies in the systematisation of information on different sources of mesenchymal stem cells for treating erectile dysfunction.

BACKGROUND: It is known that aortic stiffness (AS) increases in patients with erectile dysfunction (ED). Phosphodiesterase type-5 (PDE-5) enzyme inhibitors are used in the treatment of ED, and patients' responses to this treatment may vary. OBJECTIVES: We aimed to investigate the role of AS in predicting the response of patients planned to take PDE-5 enzyme inhibitors due to ED. METHODS: A total of 96 male patients with ED were included in the study. The International Index of Erectile Function (IIEF) questionnaire was used to evaluate the presence and severity of ED and the response to treatment. Transthoracic echocardiography was used to evaluate AS. RESULTS: There was a statistically significant difference between the aortic strain and aortic distensibility values of the study groups (p<0.001). The delta IIEF score had a high level of positive correlation with aortic strain (p<0.01, r=0.758) and a moderate level of positive correlation with aortic distensibility (p<0.01, r=0.574). CONCLUSION: We determined that in patients with ED, aortic strain and aortic distensibility measured non-invasively using transthoracic echocardiography are important parameters in predicting patients' response to PDE-5 inhibitor therapy.


FUNDAMENTO: Sabe-se que a rigidez aórtica (RA) aumenta em pacientes com disfunção erétil (DE). Os inibidores da enzima fosfodiesterase tipo 5 (PDE-5) são usados no tratamento da DE, e as respostas dos pacientes a esse tratamento podem variar. OBJETIVOS: Nosso objetivo foi investigar o papel da RA na previsão da resposta de pacientes planejados para tomar inibidores da enzima PDE-5 devido à DE. MÉTODOS: Um total de 96 pacientes do sexo masculino com DE foram incluídos no estudo. O questionário do Índice Internacional de Função Erétil (IIEF) foi utilizado para avaliar a presença e gravidade da DE e a resposta ao tratamento. A ecocardiografia transtorácica foi utilizada para avaliar RA. RESULTADOS: Houve diferença estatisticamente significativa entre os valores de deformação aórtica e distensibilidade aórtica dos grupos de estudo (p<0,001). O escore delta IIEF apresentou alto nível de correlação positiva com a deformação aórtica (p<0,01, r=0,758) e um nível moderado de correlação positiva com a distensibilidade aórtica (p<0,01, r=0,574). CONCLUSÃO: Determinamos que em pacientes com DE, a deformação aórtica e a distensibilidade aórtica medidas de forma não invasiva por meio de ecocardiografia transtorácica são parâmetros importantes na previsão da resposta dos pacientes à terapia com inibidores da PDE-5.

A hiperplasia prostática benigna (HPB) representa o crescimento não maligno do tecido da próstata. Proliferação de células estromais e epiteliais na zona de transição da próstata causa compressão uretral e obstrução do fluxo vesical. Isso pode levar a manifestações de LUTS (lower urinary tract symptoms): urgência urinária, noctúria, dificuldades de micção, esvaziamento incompleto da bexiga, menor força e/ ou interrupção do jato e inflamações. Mecanismos do LUTS/HPB compartilhados afetam a função sexual masculina. Atividade aumentada de subtipos de receptores α1-adrenérgicos na próstata está associada à HPB. Tais receptores, também presentes no pênis, podem inibir a ereção, devido à HPB. Quanto à correlação entre LUTS e disfunção sexual, aventa-se que ambas resultem da contração anômala da musculatura lisa, por ativação dos receptores α1-adrenérgicos. LUTS/HPB causam desconforto nos homens, devido à obstrução urinária, ejaculação dolorosa, disfunção erétil (DE), distúrbios ejaculatórios e baixa libido, que prejudicam a qualidade de vida, deles e de suas parcerias. A noctúria interfere negativamente na qualidade do sono e na disposição para a atividade sexual. Tratamentos para LUTS/HPB podem induzir disfunções sexuais. Entre eles, ressecção transuretral (RTU), α1-bloqueadores, inibidores da 5α-redutase e terapia combinada (α1-bloqueador e 5α-redutase). Os efeitos prejudiciais do LUTS/HPB e de seu tratamento sobre a função sexual ainda são subdiagnosticados e insuficientemente tratados. A atividade sexual deve ser investigada antes e durante o tratamento, também orientando o paciente sobre os possíveis efeitos de cada opção terapêutica sobre a função sexual, evitando-se assim o abandono do tratamento.

INTRODUCTION: The superiority of the functional results of robot-assisted radical prostatectomyis still controversial. Despite this, it is known that minimally invasive surgery obtains better results when analyzing blood loss, blood transfusion and length of stay, for example. Several studies have analyzed the impact of the resident physician's involvement on the results of urological surgeries. The simple learning curve for robot-assisted radical prostate surgery is estimated to be around 10 to 12 cases. Learning curve data for robotic surgeons is heterogeneous, making it difficult to analyze. Rare studies compare the results of a radical prostatectomy of an inexperienced surgeon starting his training in open surgery, with the results of the same surgeon, a few years later, starting training in robotic surgery. OBJECTIVE: to analyze the results of open radical prostatectomy surgeries (ORP) performed by urology residents, comparing them to the results of robot-assisted radical prostatectomy (RARP), performed by these same surgeons, after completing their training in urology. MATERIALS AND METHODS: a retrospective analysis of the cases of only 3 surgeons was performed. 50 patients underwent ORP (group A). The surgeons who operated on the ORP patients were in the 3rd and final year of the urology residency program and beginners in ORP surgery, but with at least 4 years of experience in open surgery. The same surgeons, already trained urologists, began their training in robotic surgery and performed 56 RARP surgeries (group B). For the comparative analysis, data were collected on age, number of lymph nodes removed, surgery time, hospitalization time, drain volume, drain permanence time, indwelling bladdercateter (IBC) permanence time, positive surgical margin, biochemical recurrence, risk classification (ISUP), intra and postoperative complications, urinary incontinence (UI) and erectile dysfunction (ED). The console used was the Da Vinci Si, from Intuitive®. For statistical analysis, the Shapiro-Wilk test verified that the data did not follow normality, the Levene test guaranteed homogeneity, and the Mann-Whitney test performed the comparative analysis of the quantitative data. For the analysis of qualitative data, the Chi-square test was used for nominal variables and the Mann-Whitney U test for ordinal variables. Additionally, the Friedman test analyzed whether there was an improvement in the perception of UI or ED over the months, for each group individually (without comparing them), and the post-hoc Durbin-Conover test, for the results with statistically significant difference. We used a p-value < 0.05, and the Jamovi® program (Version 2.0). RESULTS: there was no statistically significant difference between the groups for age, number of lymph nodes removed, positive surgical margin, biochemical recurrence, risk classification and urinary incontinence. Additionally, we observed that the surgical time was longer in group B. On the other hand, the length of stay, drain volume, drain time, IBC time, complication rate and levels of erectile dysfunction in the third and sixth months were higher in group A, when compared to group B. We also observed that there was no evolutionary improvement in ED over the months in both groups, and that there was a perception of improvement in UI from the 1st to the 3rd month in group A, and from the 1st to the 6th month, and from the 3rd to the 12th month, in group B. CONCLUSION: the learning curve of RARP is equivalent to the curve of ORP. In general, the results for the robotic group were better, however, the functional results were similar between the groups, with a slight tendency of advantage for the robotic arm.

This is an opinion-based article that aims to reflect on the antagonism established between the National Policy of Integral Attention to Men's Health (PNAISH) with specific focus on ED (Erectile Dysfunction), directed to access to medication. It is well-known that PNAISH presents objectives that go beyond sexual and reproductive health, especially, even after 13 years of its publication, since there was no incorporation of medications to treat ED within the SUS. This article was developed based on the scenario observed in the daily care of patients who undergo treatment in the Men's Health Outpatient Service of a High-Complexity Hospital. According to this perspective, it is important to emphasize, that the prescription of medications within the SUS should faithfully follow the Rename; as recommended by Decree 7,508, observing the Resolution SS-83 of 2015, in the state of São Paulo. This establishes that the cost of dispensing medications not included in the SUS pharmaceutical assistance protocols, through judicialization, prescribed by a physician in the state health network, may be funded by the institution to which this physician is affiliated.


Trata-se de um artigo embasado em opinião que procura refletir sobre o antagonismo estabelecido entre a Política Nacional de Atenção Integral à Saúde do Homem (PNAISH) com o recorte específico para DE (Disfunção Erétil) direcionado ao acesso de medicamentos. É notório que a PNAISH apresenta objetivos que vão além da saúde sexual e reprodutiva, sobretudo, mesmo após 13 anos de sua publicação, não houve incorporação de medicamentos para tratar DE no âmbito SUS. O artigo foi desenvolvido com base no cenário observado no dia a dia de atendimento aos pacientes que recebem cuidados no Serviço Ambulatorial da Saúde Masculina de um Hospital de Alta Complexidade. Isto por constatar a dificuldade dos médicos em prescreverem medicamentos para DE, por ausência de padronização no contexto SUS. Importante ressaltar, nesta perspectiva, que a prescrição de medicamentos no âmbito SUS deve seguir fidedignamente a Rename; conforme preconiza o Decreto 7.508 e, se atentar à Resolução SS-83 de 2015, vigente no estado de São Paulo. Ela estabelece que o custo da dispensação de medicamentos não contemplados na Rename, por meio da judicialização, prescritos por médico da rede estadual de saúde, poderá ser custeado pela instituição ao qual este esteja vinculado.

OBJECTIVE: To present the patient-reported quality of life (QoL) outcomes from a prospective, randomized controlled trial comparing the use of pelvic floor muscle training (PFMT) and duloxetine after robot-assisted radical prostatectomy (RARP). METHODS: We identified 213 men with organ-confined disease having post-RARP urinary incontinence who were randomly assigned to received PFMT, duloxetine, combined PFMT-duloxetine and pelvic floor muscle home exercises. Urinary symptoms burden was measured by marked clinical important difference improvement (MCID) defined by using the International Prostate Symptom Score (IPSS) difference of - 8 points (ΔIPSS ≤-8). QoL was assessed according to Visual Analog Scale (VAS), King's Health Questionnaire (KQH), and International Index of Erectile Function (IIEF-5). Multivariable regression analyses aimed to predict MCID, burden of urinary symptoms (IPSS ≥8), and patients reporting to be satisfied (IPSS QoL ≤2) or comfortable (VAS ≤1) post-RARP. RESULTS: Moderate to severe urinary symptoms decreased from 48% preoperatively to 40%, 34%, and 23% at 3, 6, and 12months post-RARP. After surgery, MCID improvement was observed in 19% of patients, and deterioration in 3.3%. Large prostate was the only factor associated to MCID (OR 1.03 [95%CI 1.01-1.05], P = .005). At 6months, patients reached the same degree of preoperative satisfaction. Neurovascular bundle preservation was the only predictor of being comfortable regarding urinary symptoms postoperatively (OR 12.8 [CI95% 1.47-111.7], P = .02 at 3months) and was also associated to higher median postoperative IIEF-5. CONCLUSION: Despite urinary incontinence following RARP, patients with larger prostates experience a reduction of lower urinary tract symptoms within a year, which subsequently elevates QoL. Furthermore, nerve-sparing surgery augments erectile function and urinary outcomes, shaping postoperative QoL.

PURPOSE: A step-based anastomotic urethroplasty is a standard technique for repairing the posterior urethra in patients with pelvic fracture urethral injury (PFUI). We aim to identify pre-operative factors, including results of conventional radiological imaging, for prediction of elaborated perineal or a combined abdominoperineal procedure. METHODS: Retrospective observational study on 114 consecutive patients undergoing urethroplasty for PFUI between January 2020 and December 2022 was conducted. Surgical procedures were categorized according to the Webster classification into two groups: steps 1-2 (group 1) and steps 3-4 or a combined abdominoperineal repair (group 2). Pre-operative pattern results of RGU/VCUG were categorized regarding the relation between the proximal urethral stump with the pubic symphysis: posterior urethral stump below (pattern 1) or above (pattern 2) the lower margin of the pubic symphysis. Patient demographics were assessed. Univariate and multivariate logistic regression analyses were utilized. RESULTS: Overall, 102 patients were enrolled in the study for data analysis. On the multivariate logistic regression analysis, the presence of erectile dysfunction (OR 4.5; p = 0.014), prior combined treatment (endoscopic and urethroplasty) (OR 6.4; p = 0.018) and RGU/VCUG pattern 2 (OR 66; p < 0.001) significantly increased the likelihood of the need of step 3 or higher. CONCLUSIONS: The need of step 3 or higher during urethroplasty for PFUI can be predicted pre-operatively with conventional imaging (RGU/VCUG). Patients with proximal urethral stump above the lower margin of pubic symphysis were about 66 times more likely to need step 3 or higher during urethroplasty.

Objetivo: La prostatectomía radical se asocia con complicaciones como incontinencia urinaria y disfunción eréctil. Múltiples estudios reportan discrepancia entre el tiempo de recuperación de la incontinencia urinaria y disfunción eréctil percibida por el paciente comparado con lo informado en la historia clínica. El objetivo de nuestro estudio es comparar la concordancia que existe entre la recuperación de la continencia urinaria y función eréctil reportados por el paciente y la historia clínica. Métodos: Se recolectó una muestra de hombres con cáncer de próstata llevados a prostatectomía radical. Se interrogó el tiempo de recuperación de continencia urinaria y función eréctil posterior a cirugía. El seguimiento mínimo fue un año. Resultados: Entre 2000-2018 se escogieron 158 pacientes. La recuperación de continencia urinaria reportada por la paciente a los 6 meses fue del 82,3 vs. 86,6% reportada en historia clínica, a los 12 meses de la cirugía el reporte de recuperación de la continencia por la paciente fue del 92,5 vs. 95,4 % en la historia clínica, con una diferencia del 2,9%. La recuperación de función eréctil reportada por el paciente a los 6 meses fue de del 69 vs. 77,8% por historia clínica, a los 12 meses la tasa de recuperación reportada por el paciente fue del 77,8 vs. 65,9% por historia clínica. Conclusiones: Se demostró una baja tasa de discrepancia de continencia urinaria y recuperación de la función eréctil entre lo informado por el paciente y la historia clínica. El tiempo entre la cirugía y la solicitud de recuperación no afecta los resultados.


Objective: Radical prostatectomy is associated with complications like urinary incontinence and erectile dysfunction. Multiple studies report a discrepancy between recovery time of urinary continence and erectile function perceived by the patient compared to report on clinical history. The objective of our study is to compare the concordancy of recovery of urinary continence and erectile function between the patient's report and clinical history. Methods: A sample of men with prostate cancer who underwent radical prostatectomy was collected. Recovery time of urinary continence and erectile function after surgery by the patient and clinical history was reported. The time of follow-up was at least one year. Results: Between 2000-2018, 158 patients were collected. The recovery time of urinary continence reported by the patient at 6 months was 82.3 vs. 86.6% reported on clinical history. At 12 months after surgery, the recovery of urinary continence reported by the patient was 92.5 vs. 95.4% on clinical history, with a difference of 2.9%. The recovery of erectile function reported by the patient at 6 months was 69 vs. 77.8% on clinical history, and 12 months after surgery the recovery rate reported by the patient was 77.8 vs. 65.9% on clinical history. Conclusion: A low discrepancy of recovery of urinary continence and erectile function between the report by the patient and clinical history was observed. The time between surgery and the recovery request does not change the results

Describir la utilidad de la cavernosografía por tomografía computarizada (TC) de la vasculatura venosa peneana para el diagnóstico de disfunción eréctil de origen venoso. Método: Entre el año 2017 y 2021 en el Servicio de Radiología de la Clínica de la mujer de Bogotá, Colombia, se realizaron 32 cavernosografías como método radiológico para el diagnóstico vascular de pacientes con disfunción eréctil y sospecha de escape venoso. El procedimiento se hizo con un método estandarizado y la lectura de la imagen se hizo utilizando la clasificación del drenaje venoso anómalo mediante TC para la clasificación de escapes venosos: tipo A, sin escape; tipo B, escape sistema venoso profundo; tipo C, escape sistema venoso superficial, y tipo D, escape en sistema venoso superficial y profundo. Resultados: Treinta y dos pacientes fueron diagnosticados con insuficiencia venosaL: tipo A dos pacientes, escape tipo B tres pacientes, escape tipo C cuatro pacientes y escape tipo D 20 pacientes. En dos pacientes poscirugía de ligadura venosa se encontró neovascularización que escapaba y en un paciente con Peyronie se encontró escape venoso superficial y profundo con falta de llenado de los cuerpos cavernosos distales. Conclusión: El diagnóstico etiológico en la disfunción eréctil debe ser un objetivo clínico para poder ofrecer tratamientos exitosos más específicos. Uno de los métodos potenciales de diagnóstico es la realización de cavernosografía por TC en pacientes seleccionados, lo que permite una imagen más detallada anatómicamente profunda del drenaje venoso bajo una visualización tridimensional similar a la que se observaría en una disección quirúrgica.


To describe the usefulness of computed tomography (CT) cavernosography of the penile venous vasculature for the diagnosis of erectile dysfunction of venous origin. Method: Between 2017 and 2021 in the Radiology Service of the Women's Clinic of Bogota, Colombia, 32 cavernosographies were performed as a radiological method for the vascular diagnosis of patients with erectile dysfunction and suspected venous leakage. The procedure was performed with a standardized method and the reading of the image was done using the classification of anomalous venous drainage by CT for the classification of venous leaks: type A, no leak; type B, deep venous system leak; type C, superficial venous system leak, and type D, leak in superficial and deep venous system. Results: Thirty-two patients were diagnosed with venous insufficiency: type A two patients, type B leak three patients, type C leak four patients and type D leak 20 patients. In two patients after venous ligation surgery, neovascularization was found escaping and in one patient with Peyronie's disease, superficial and deep venous leakage was found with lack of filling of the distal cavernous bodies. Conclusion: The etiological diagnosis of erectile dysfunction should be a clinical objective to offer more specific and successful treatments. One of the potential diagnostic methods is the performance of CT cavernosography in selected patients, which allows a more detailed anatomically deep image of the venous drainage under a three-dimensional visualization like what would be seen in a surgical dissection

Las prótesis peanas marcaron un cambio importante en la historia del manejo de la disfunción eréctil, principalmente en hombres refractarios a otros métodos existentes. En la actualidad se cuenta con tres opciones de implantes de prótesis peneanas: prótesis de dos piezas, de tres piezas y las semirrígidas; cada una de ellas con una indicación específica. Existen también diferentes tipos de abordajes quirúrgicos, de los cuales los más usados son el abordaje penoescrotal y suprapúbico. En cuanto a las complicaciones quirúrgicas, la infección es una de las más frecuentes, por lo que se han desarrollado implantes impregnados con antibióticos e implantes irrigables con solución antibiótica y/o antifúngicas a elección con el fin de mitigar la infección, puesto que se ha reportado que los dispositivos recubiertos reducen la incidencia de infección del dispositivo en aproximadamente un 50%, al igual que la implementación de otras técnicas como la de no tocar para reducir la infección. Penile prostheses marked an important change in the history of erectile dysfunction management, mainly in men refractory to other existing methods. Currently, there are three options for penile prosthesis implants, which are: two-piece, three-piece and semi-rigid prostheses; each with a specific indication. There are also different types of surgical approaches, of which the most used are the penoscrotal and suprapubic approaches. Describir y sintetizar la evidencia existente sobre el impacto en la calidad de vida y el bienestar emocional de los pacientes con cáncer de próstata según la intervención recibida. Realizamos una revisión de la literatura en cuatro bases de datos (Medline, Embase, Bolsa de Valores en Salud [BVS] y PsycNet), incluyendo términos relacionados con cáncer de próstata, calidad de vida y bienestar emocional, y su tratamiento. Incluimos 36 artículos, los cuales señalan un impacto por parte de todas las terapias evaluadas, que, a la vez, puede estar mediado en parte por el curso natural de la enfermedad u otros factores asociados. Se encontró a los síntomas relacionados con la función urinaria y sexual de los pacientes a largo plazo como más prevalentes en el impacto negativo de la calidad de vida. No obstante, algunos estudios mostraron una mejoría en la calidad de vida de los pacientes años después de iniciar el tratamiento, lo cual puede estar relacionado con un proceso adaptativo, y con el fenómeno de response shift. Los diferentes tratamientos para el cáncer de próstata tienen un efecto en la calidad de vida impactando en las actividades de la vida cotidiana, lo cual hace pertinente la valoración y el seguimiento en salud mental periódico y oportuno, con enfoques cognitivos y educativos para tratar los síntomas de salud mental y disminuir su carga de la enfermedad.


To describe and synthesize the existing evidence on the impact on the quality of life and emotional well-being of patients with prostate cancer according to the intervention received. We reviewed the literature in four databases (Medline, Embase, Bolsa de Valores en Salud [BVS], and PsycNet), including terms related to the treatment of prostate cancer, quality of life, and emotional well-being, and prostate cancer. We include 36 articles, which indicate an impact on the part of all the therapies evaluated, which, at the same time, may be mediated in part by the natural course of the disease or other associated factors. Symptoms related to urinary and sexual function of long-term patients were found to be more prevalent in negatively impacting the quality of life. However, some studies showed improved patient quality of life years after starting treatment, which may be related to an adaptive process and the response shift phenomenon. The different treatments for prostate cancer affect the quality of life due to the impact on activities of daily living. It makes periodic and timely assessment and follow-up in mental health pertinent, with cognitive and educational approaches for treating mental health symptoms and decreasing their symptoms burden of disease.

Objective: Determine the association and utility of measuring estradiol-to-testosterone ratio on erectile dysfunction (ED) and libido. Materials and methods: This is a case­control study involving 32 adult male patients with chronic pathologies who consulted for sexual dysfunction. Cases defined as subjects with hypoactive sexual desire. Results: Diabetes mellitus showed a significant LR of 6.58. Testosterone levels differed between cases and controls, yet no differences emerged for estradiol or estradiol-to-testosterone ratio. Subgroups defined by testosterone and LH levels were established. Low testosterone and inappropriately normal LH showed an OR of 13 and significant LR. Analysis of normal and inappropriately normal LH categories revealed significant differences in E/T ratio and adjusted testosterone. ROC curves assessed E/T ratio effectiveness for diagnosing hypogonadotropic hypogonadism and SD. A cutoff point of 5.98 for E/T ratio yielded 0.85 sensitivity and 0.79 specificity. Conclusions: No significant differences were observed between cases and controls. Differences were observed when analyzed in subgroups based on LH response, indicating estradiol-to-testosterone ratio may be useful in studying inappropriate LH response. A larger sample size is warranted to be able to extrapolate our results.


Objetivo: Determinar asociación y utilidad de medir la relación estradiol-testosterona en disfunción eréctil y libido. Método: Casos y controles, involucrando 32 pacientes con patologías crónicas que consultaron por disfunción sexual, definiendo casos como adultos con libido hipoactiva. Resultados: En los sujetos con diabetes mellitus se evidenció likelihood ratio (LR) significativo (6,58). Hubo diferencias significativas en testosterona entre casos y controles, sin diferencias en estradiol o la relación estradiol-testosterona. Se establecieron subgrupos según testosterona y hormona luteinizante (LH). Testosterona baja y LH inapropiadamente normal, mostraron una odds ratio (OR) de 13 y LR significativo. Análisis de LH normal e inapropiadamente normal, reveló diferencias significativas en la relación estradiol-testosterona y testosterona corregida. Curvas ROC evaluaron la eficacia de relación estradiol-testosterona para diagnosticar hipogonadismo hipogonadotrópico y disfunción eréctil (DE). Un punto de corte de 5,98 para la relación estradiol-testosterona arrojó una sensibilidad del 0,85 y una especificidad del 0,79. Conclusiones: Se observaron diferencias al analizar en subgrupos basados en la respuesta de LH, sugiriendo que medir la relación estradiol-testosterona puede ser útil para estudiar la respuesta inapropiada de LH. INTRODUCTION: Erectile dysfunction (ED) is a common condition that negatively affects men's quality of life. It can have various causes, including psychological, vascular, and neurologic factors. Existing treatments for ED mainly focus on symptom relief rather than addressing the underlying cause. Stem cells (SCs) have shown potential as a therapeutic approach for ED due to their anti-inflammatory properties. OBJECTIVES: This systematic review aims to assess the current status of trials and determine the potential impact of SCs on male sexual health. METHODS: A comprehensive search strategy was employed to gather relevant articles from 6 electronic databases. The search included articles published until March 2023. The reference lists of articles were manually reviewed to identify additional studies of relevance. The eligibility criteria for inclusion in the analysis focused on clinical trials involving humans that evaluated the safety and efficacy of SC therapy for ED. Exclusion criteria encompassed case reports, case series, abstracts, reviews, and editorials, as well as studies involving animals or SC derivatives. Data extraction was performed via a standardized form with a focus on erectile outcomes. RESULTS: A total of 2847 articles were initially identified; 18 were included in the final analysis. These studies involved 373 patients with ED and various underlying medical conditions. Multiple types of SC were utilized in the treatment of ED: mesenchymal SCs, placental matrix-derived mesenchymal SCs, mesenchymal SC-derived exosomes, adipose-derived SCs, bone marrow-derived mononuclear SCs, and umbilical cord blood SCs. CONCLUSION: SC therapy shows promise as an innovative and safe treatment for organic ED. However, the lack of standardized techniques and controlled groups in many studies hampers the ability to evaluate and compare trials.

OBJECTIVE: The aim of this study was to reveal the learning curve of early apical release en bloc laser prostatectomy using a high-power thulium (200 W) laser device. METHODS: We obtained data on the initial 60 patients who had thulium laser enucleation of the prostate by a single surgeon between October 2021 and August 2022 to treat the signs and symptoms of benign prostatic hyperplasia at our clinic. The cases were split into three groups, each consisting of 20 patients. Prostate volumes, prostate-specific antigen and hemoglobin levels, the International Prostate Symptom Score, Quality of Life scores, the International Index of Erectile Function-5 scores, and uroflowmetry parameters were documented preoperatively. The enucleation weight, the enucleation and morcellation times, as well as the efficiency, hospitalization, and catheterization durations were calculated. The patients were re-evaluated at 6 months postoperatively, examined for functional results, and compared to baseline conditions. RESULTS: Enucleation times, morcellation times, enucleation weight, and enucleation efficiency were significantly different among the groups. However, there was no statistically significant difference in total operative time and morcellation efficiency. In terms of postoperative statistics, the reduction in hemoglobin was significantly greater in Group 1 compared to Group 2. Six months after surgery, all groups had comparable validated ratings (International Prostate Symptom Score, Quality of Life, and the International Index of Erectile Function-5) on postoperative examinations. There were no long-term complications in either group throughout the perioperative period. CONCLUSION: Completing 40 first cases would be sufficient for managing the learning curve for early apical release en bloc thulium laser enucleation of the prostate.

OBJECTIVE: To assess the difference in outcomes between single dilation (SingD) and sequential dilation (SeqD) in primary penile implantation, hypothesizing that patients who undergo SeqD had higher rates of noninfectious complications. METHODS: We performed a multicenter, retrospective study of men undergoing primary inflatable penile prosthesis placement. Intraoperative complications and postoperative noninfectious outcomes were assessed between the two groups. Multivariable analysis was performed to identify predictors of complications. RESULTS: A total of 3293 patients met inclusion criteria. After matching, there were 379 patients who underwent SingD and 379 patients who underwent SeqD. There was no significant difference in intraoperative complications between patients who underwent SingD vs SeqD, nor was there any difference in cylinder length (20 cm with interquartile range [IQR] 18-21 cm vs 20 cm with IQR 18-20 cm respectively, P = .4). On multivariable analysis, SeqD (OR 5.23 with IQR 2.74-10, P < .001) and older age (OR 1.04 with IQR 1.01-1.06, P = .007) were predictive of postoperative noninfectious complications. There was no significant difference in intraoperative complications between patients who underwent SingD vs SeqD, nor was there any difference in cylinder length. SeqD and older age were predictive of postoperative noninfectious complications. CONCLUSION: During inflatable penile prosthesis placement in the uncomplicated patient without fibrosis, SingD is a safe technique to utilize during implantation that will minimize postoperative adverse events, and promote device longevity without loss of cylinder length.
