RESUMEN

The aim of this manuscript is to report the case of a 22-year-old adolescent who presented with brownish patches on the skin of her lower legs persistent since the age of eleven years. She was treated by a dermatologist since the age of twelve years with a clinical diagnosis of ochre dermatitis confirmed by a biopsy. The patient was treated for two years without a success and was sent to a vascular surgeon at fourteen years of age. The diagnosis was confirmed, and the venous duplex scan discarded the possibility of a macrocirculation abnormality. The patient was treated with aminaphtone with the normalization of the skin for two years, after which the patches returned and were controlled again with the same medication. As ochre dermatitis may be associated with capillary fragility, the use of aminaphtone is a therapeutic option.

Objective: To develop green banana peel (Musa sapientum) powder for the treatment of peristomal dermatitis, and to evaluate its effectiveness and healing time. Methods: A clinical, analytical and longitudinal study. In total, 44 volunteers of both genders, aged>18 years, with intestinal ostomy, who presented peristomal dermatitis, participated in the research. The sample was divided into two groups: the study group used a powder containing 10% of green banana peel, and the control group used ostomy powder. The research was approved by the Ethics in Research Committees (Comitês de Ética em Pesquisa, CEP, in Portuguese) under opinion n° 2.381.904. Results: The healing time of the study group was shorter than that of the control group (p=0.022). To analyze the results, we used the two-sample t-test (mean days until healing for both study groups). Conclusion: The powder containing 10% of green banana peel was developed, and effective healing of the peristomal dermatitis was observed. Compared to the ostomy powder, it presented a shorter healing time. (AU)


Objetivo: Desenvolver o pó composto da casca da banana verde (Musa sapientum) para o tratamento de dermatite periestomal, e avaliar a eficácia e o tempo de cicatrização. Métodos: Estudo clínico, analítico e longitudinal. Participaram da pesquisa 44 voluntários, de ambos os gêneros, maiores de 18 anos, com estomias intestinais, e que apresentavam dermatite periestomal. A amostra foi dividida em dois grupos: o grupo de estudo usou umpó que continha 10% de casca de banana verde, e o grupo de controle usou pó para estomia. A pesquisa foi aprovada pelos Comitês de Ética em Pesquisa (CEP), sob o parecer n° 2.381.904. Resultados: O tempo de cicatrização do grupo de estudo foimenor do que o do grupo de controle (p=0.022). Para a análise dos resultados, empregou-se o teste t para duas amostras (média de dias de cicatrização de ambos os grupos). Conclusão: Desenvolveu-se o pó contendo 10% de casca de banana verde, e observouse cicatrização eficaz da dermatite periestomal. Em comparação com o pó para estomia, apresentou tempo menor até a cicatrização. (AU)

RESUMEN

Galectin 3 is a unique ~31 kDa protein that recognizes the N-acetyl-lactosamine structure of several glycoconjugates. It mainly occurs in epithelial and myeloid cells, but is also found in a variety of human cell types. In view of the crucial role played by galectin 3 in the regulation of cellular processes of essential importance and in the pathogenetic mechanisms of diverse disorders, it is not surprising that, particularly in the last three decades, the attention of the scientific community has been increasingly drawn to this extraordinary and multifunctional galectin. In this paper the authors summarize current knowledge on the expression of galectin 3 in normal and diseased human skin, its implications in the pathogenesis, diagnosis and prognosis of cutaneous disorders, and the perspectives of a novel approach to the treatment of the latter using galectin 3 or its inhibitors/antagonists.

As dermatopatias estão entre os problemas mais observados na rotina clínica de pequenos animais. A fim de minimizar o uso indiscriminado de antibióticos e de promover a cicatrização de lesões cutâneas, a ozonioterapia é uma técnica que utiliza o ozônio como agente terapêutico. Objetiva-se com este trabalho analisar clinicamente a evolução da reparação tecidual de animais submetidos à ozonioterapia. Foi atendido dois cães, que apresentavam lesões dermatológicas macroscópicas espontâneas de etiologia bacteriana, submetidos ao tratamento utilizando o gás ozônio na forma tópica. Ao termino do tratamento os mesmos apresentaram resposta satisfatória, observando reparação tecidual completa.

In order to minimize the indiscriminate use of antibiotics and promote the healing of cutaneous lesions, ozone therapy is a technique that uses ozone as a therapeutic agent. The objective of this study was to analyze clinically the evolution of tissue repair of animals submitted to ozonotherapy. Two dogs, with spontaneous macroscopic lesions of bacterial etiology, were submitted to treatment using ozone gas in the topical form. At the end of the treatment they presented satisfactory response, observing complete tissue repair.

BACKGROUND: Limited information is available describing the features of canine atopic-like dermatitis (ALD) compared with atopic dermatitis (AD). OBJECTIVES: To compare demographic data, disease severity and response to therapy between ALD and AD dogs. ANIMALS: Two hundred and fifty-three atopic dogs with intradermal and serum allergen-specific IgE test results were selected retrospectively. METHODS AND MATERIALS: Dogs were enrolled into the ALD group if both IgE tests were negative and into the AD group if at least one test was positive. Demographic data, pruritus level and number of body sites affected before and during therapy, in addition to maintenance therapy protocols, were compared between groups. RESULTS: There were 216 (85.38%) dogs in the AD group and 37 (14.62%) in the ALD group. The soft-coated wheaten terrier, American Staffordshire terrier, English bulldog and Labrador retriever were over-represented in the AD group. No significant differences between the groups were noted regarding the other demographic variables evaluated. There were no differences in the mean pruritus scores and number of affected body sites at the first visit or during treatment. Furthermore, no significant differences between the groups were noted for the maintenance treatment scores and reduction of pruritus level and number of body sites affected during treatment. CONCLUSIONS AND CLINICAL SIGNIFICANCE: The soft-coated wheaten terrier, American Staffordshire terrier, English bulldog and Labrador retriever were over-represented in the AD group. No significant differences in the other demographic data and clinical features were noted between dogs with ALD and AD in the present study.

O presente relato revela um caso clínico de dermatite alérgica por picada de ectoparasitas (DAPE), com lesões atípicas em asa nasal de canino. O paciente não recebia controle parasitário periódico pois o proprietário não observava parasitas. As suspeitas iniciais eram de lúpus ou leishmaniose ou farmacodermia, o exame histopatológico revelou tratar-se de caso de alergopatia com piodermite secundária. O quadro foi totalmente controlado pela aplicação de coleira parasiticida com imidacloprid e flumetrina, sem nenhuma outra medida associada. A coleira parasiticida revelou-se efetiva no controle da DAPE.


The actual report reveals a clinical case of Flea and Tick allergic dermatitis, with atypical injuries on the nasal ala in a dog. The patient did not receive periodic parasitic control because the owner did not observed parasites. The initial suspicions were about lupus or leishmaniosis or drug reaction, the histopathological exame revealed been a case of allergic disease with secondary pyoderma. The case was fully controlled by application of ectoparasiticide collar with imidacloprid and flumetrina, without any other measures. The collar proved to be effective in the control of Flea and Tick allergic dermatitis.


Este informe revela un caso de dermatitis alérgica de picadura de ectoparásitos (DAPE) em perro com lesiones atípicas en ala nasal. El paciente no recibió el control de parásitos regular porque el propietario no observaba parásitos. Las sospechas iniciales fueron el lupus o la leishmaniasis o farmacodermia, el examen histopatológico reveló que se trataba de un caso de alergia y pioderma secundaria. El caso fue completamente controlado por la aplicación de um collar parasiticida con imidacloprid y flumetrina sin ninguna otra acción asociada. El collar antiparasitario demostrado su eficacia en el control de la DAPE.

Due to the high prevalence of incontinence among skilled nursing facility (SNF) residents, incontinence-associated derma- titis (IAD) is a common occurrence. In addition, facility staff may mistakenly identify IAD as a pressure injury. A prospective, descriptive, multicenter study was conducted in 3 Connecticut facilities to evaluate the effect of substituting a disposable, high- uid capacity underpad for nonpermeable disposable and reusable containment products on the rate of IADs. Residents with and without IAD but with high IAD risk scores who were bed- or chairbound or ambulatory and used disposable nonpermeable briefs and underpads or reusable, laundered containment products when in bed longer than 2 hours were randomly enrolled and observed for a 4-week period. Facility staff were trained on the importance of differentiating between IAD and pressure injury; they substituted the study product (a disposable, high- uid capacity underpad) for all previously used containment products. Patient risk for IAD and skin condition were assessed using the Perineal Assessment Tool (PAT) and the Skin Condition Assessment Tool (SAT), respectively, at 5 time points: baseline, week 1, week 2, week 3, and week 4. The PAT is a 4-item instrument based conceptually on the 4 determinants in perineal skin breakdown; subscales are rated from 1 (least risk) to 3 (most risk), with a total score range of 4 to 12. The SAT is used to evaluate IAD speci cally, generating a cumulative severity score ranging from 0 to 3 on area of skin affected, degree of redness, and depth of ero- sion. Final data analysis was conducted on 40 residents: 25 had IAD present at enrollment and 15 were deemed high risk for developing IAD. Mean SAT scores in the 25 participants with IAD decreased with signi cance at week 1 (P = .0016), week 2 (P = .0023), week 3 (P = .0005), and week 4 (P <.0001). Baseline IAD severity scores averaged 3.3 ± 1.7. Overall IAD average severity scores in this group decreased from baseline mean of 3.3 ± 1.7 to 0.7 ± 1.4 at week 4 (P <.001). The 15 participants with intact, nondamaged skin at enrollment did not develop IAD from baseline to week 4, and PAT score risk levels decreased from high (7 or greater) to low (6 or less) as a result of a speci c reduction in the duration of irritant exposure category for 11 (73%) of this group of participants by week 4. PAT risk level scores for both IAD and non-IAD participants at baseline averaged 8.1 ± 1.4; after 4 weeks, they averaged 7.0 ± 1.5). Although change was not significant, results suggest the use of a disposable, high- uid capacity underpad improved SAT scores over time. IAD rates increased in each facility, but pressure injury incidence rates decreased for the study duration. Replacing a nonpermeable, reusable containment product with a disposable, high- uid capacity underpad when SNF residents are in bed longer than 2 hours may impact the severity of IAD and reduce its incidence. The inverse impact reported on IAD and pressure injury incidence rates 1 month after training suggest study educational efforts had a short-lasting effect. Future research is indicated to determine the most effective method to improve nurses' ability to identify and distinguish IAD from pressure injury in the SNF setting.

Este reporte describe el primer aislamiento en Argentina de una espécie de Chrysosporium en un ejemplar juvenil de iguana verde (Iguana iguana). El animal presentaba una lesión en el tercio distal de la cola, refractario a diversos tratamentos y se decidió la amputación de la zona afectada. Se tomaron muestras estériles sembradas en agar Sabouraud glucosado con inhibidores, agar sangre ovino 8% y agar McConkey (AMe) incubadas a 37 °C, obteniéndose colonias blanco opacas y en su reverso un color marrón claro. Microsc6picamente se identificó el género Chrysosporium por las hifas hialinas tabicadas, rectas, ramificadas irregularmente con presencia de conidios globosos y de superfICie lisa. Histol6gicamente se observo pérdida de solución de continuidad del epitelio con infiltrado de células granulocíticas y presencia del microorganismo saprofito. En la dermis, granulomas con escasa reacción fibroblástica y angioblástica.


This article reports for the first time the isolation of a Chrysosporium species from a juvenile green iguana (Iguana iguana) in Argentina. Since the animal presented a lesion in the distal third of the tail and had been refractory to different treatments, caudal amputation was the elected approach. Sterile tissue samples were seeded in Sabouraud agar with glucose and inhibitors, 8% ovine blood agar and McConkey agar (AMc), and incubated at 37"C. Under these conditions, white opaque fungal colonies with Iight brown pigmentation on the underside developed. Microscopically, the genus Chrysosporium was identified by the observation of septate, straight, irregularly branched hyphae with smooth surface and globose conidia. Histological findings included loss of epithelial continuity with infiltration of granulocytes and the presence of the saprophytic organism. In the dermis, granulomatous Iesions with scarce fibroblastic and angioblastic reactions were observed.(AU


O presente trabalho descreve o primeiro isolamento na Argentina de urna espécie de Chrysosporium em urna iguana-verde (Iguana iguana) jovem. O animal apresentava urna lesão no terço distal da cauda e não havia respondido a vários tratamentos, razão pela qual se decidiu amputar a zona afetada. Obtiveram-se amostras estéreis do tecido, semeadas em ágar Sabouraud glicosado com inibidores ágar sangue ovino 8% e ágar McConkey (AMe) incubadas a 37 °C; as colônias obtidas eram brancas e opacas, e no reverso, marrom-claras. Na microscopia foram identificadas hifas hialinas tabicadas típicas do género Chrysosporium, que se apresentavam retas, ramificadas de forma irregular e com presença de conídios globosos e de superfície lisa. Na histologia se observou perda de solução de continuidade do epitélio, com infiltrado de células granulosas e presença do microrganismo saprófito. Na derme foram observados granulomas com escassa reação fibroblástica e angioblástica.

BACKGROUND: Allergen immunotherapy has proved effective in the treatment of various allergies, but whether the clinical benefits outweigh the economic costs has been little evaluated. OBJECTIVE: To explore the economic consequences of immunotherapy in asthma, rhinitis and dermatitis. METHODS: Descriptive study. Patients with at least 18 months' follow-up were selected, whose medical history allowed evaluation of drugs received, manner of use, check-ups, treatment changes, reasons for these, and so on. Patients were classified according to whether they received drug treatment + immunotherapy (active group) or drug therapy alone (control group). RESULTS: 1848 patients were included: 648 in the active group and 1200 in the control. Immunotherapy increased the cost of treatment during the first few months, but after 9 to 12 months drug treatment decreased significantly compared to the control group. In patients with asthma or various diseases, immunotherapy reduced costs of treatment (p < 0.05). The active group saw fewer relapses at 18 months (p < 0.05). CONCLUSION: Allergen immunotherapy allows the sustained reduction in drug treatment, with medium-term financial savings for the patient and the health system.


Antecedentes: La inmunoterapia con alérgenos ha demostrado ser efectiva en el tratamiento de diversas alergias, pero poco se ha evaluado si los beneficios clínicos compensan los costos económicos. Objetivo: Explorar las consecuencias económicas de la inmunoterapia en asma, rinitis y dermatitis. Métodos: Estudio retrospectivo analítico. Se seleccionaron pacientes con seguimiento por al menos 18 meses, cuya historia clínica permitiera evaluar los medicamentos recibidos, forma de uso, citas de control, cambios en el tratamiento, motivos de estos, etcétera. Los pacientes fueron clasificados de acuerdo a si recibieron tratamiento farmacológico + inmunoterapia (grupo activo) o solo tratamiento farmacológico (grupo control). Resultados: 1848 pacientes fueron incluidos: 648 en el grupo activo y 1200 en el control. La inmunoterapia aumentó el costo del tratamiento durante los primeros meses, pero después de 9 a 12 meses disminuyó significativamente el tratamiento farmacológico en comparación con el grupo control. En los pacientes con asma o varias enfermedades, la inmunoterapia redujo los costos del tratamiento (p < 0.05). En el grupo activo se observó menos recaídas a los 18 meses (p < 0.05). Conclusión: La inmunoterapia con alérgenos permite la reducción sostenida del tratamiento farmacológico, con ahorro económico a mediano plazo para el paciente y el sistema de salud.

La dermatitis o eccema numular es una patología poco frecuente en pediatría. El cuadro se caracteriza por un inicio con diminutas pápulas y vesículas eritematovioláceas que confluyen en placas exudativas de forma circular y evolucionan a placas eccematosas o liquenificadas de forma discoide o anular. Aparecen, predominantemente, en superficies extensoras de las extremidades, aunque pueden encontrarse en el tronco, las manos o los pies. El diagnóstico es clínico; solo en casos con mala evolución pueden ser necesarias pruebas complementarias. La base del tratamiento son los corticoides tópicos y, si se encuentra un desencadenante infeccioso, el tratamiento de la causa. Su evolución suele ser crónica o recidivante. Se presentan dos casos clínicos en población pediátrica con la finalidad de difundir entre los pediatras su conocimiento y manejo.

It is presented as red-purplish small papules and vesicles that join to form exudative circular patches and then to eczematous or lichenified patches with discoid shape. The lesions appear predominantly on the extensor surface of extremities, although they can appear in trunk, hands or feet. This pathology has a clinical diagnosis; only few cases require complementary test. The topical corticosteroids are the mainstay of the treatment, and the causal treatment whether an infectious trigger is found. The patients have chronic or recurrent evolution. We report two cases in children with the aim of spreading knowledge among pediatricians.

Cutaneous food allergy is an adverse immunological response, triggered by antigenic dietary components that may escape the digestion process and are absorbed intact through the gastrointestinal mucosa. In Brazil, there are only a few reports on cutaneous food allergy and antigenicity of food components used in pet food production. Thus, the aim of this report was to assess and describe data from medical records of canine patients diagnosed with cutaneous food allergy at a Veterinary Teaching Hospital from Jaboticabal, SP, Brazil, in order to evaluate epidemiological and clinical aspects related to this skin problem. From 2007 to 2012, 29 dogs received the diagnosis of cutaneous food allergy after an eight-week hypoallergenic elimination trial. Among them, purebred dogs were more frequent, as well as females, and their mean age was 50.4 months old. Pruritus was reported in all cases, especially in interdigital areas. In order to establish the diagnosis, most patients received home-cooked elimination diet, based on potato and lamb in approximately 80% of the cases. Besides, it was highlighted the importance of the hypoallergenic elimination diet being rigorously followed by dogs' owners, aiming the adequate control of the clinical signs related to cutaneous food allergy.(AU)


A dermatite trofoalérgica é uma resposta imunológica adversa, desencadeada por componentes dietéticos antigênicos, que escapam do processo de digestão e são absorvidos intactos pela mucosa gastrointestinal. No Brasil, há poucos relatos acerca da dermatite trofoalérgica e da antigenicidade dos componentes utilizados na produção de alimentos para animais. Assim, o objetivo deste trabalho foi avaliar e relatar dados de prontuários de pacientes caninos diagnosticados com dermatite trofoalérgica em um Hospital Veterinário de Jaboticabal, SP, Brasil, a fim de analisar os aspectos epidemiológicos e clínicos relacionados a este problema de pele. De 2007 a 2012, 29 cães receberam o diagnóstico de dermatite trofoalérgica após oito semanas de dieta de eliminação hipoalergênica. Dentre eles, destacam-se os cães de raça definida e as fêmeas como mais frequentes, com idade média de 50,4 meses. Prurido foi a queixa relatada em todos os casos, especialmente na região interdigital. Para fins diagnósticos, a maioria dos animais recebeu dieta caseira de eliminação, a base de batata e carneiro em aproximadamente 80% dos casos. Ademais, destaca-se a importância de que a dieta de eliminação hipoalergênica seja criteriosamente seguida pelo proprietário, a fim de controlar adequadamente os sinais clínicos da dermatite trofoalérgica.(AU)

PURPOSE: To compare the biomodulatory effects of LED and ultrasound combined with semipermeable dressing in the repair of cutaneous lesions. METHODS: Eighty-four Wistar rats were submitted to surgical injury (2.5 cm) and divided into four groups (n=21): Group I (control), Group II (LED therapy, LED), Group III (LED therapy + dressing, LED+D) and Group IV (ultrasound + dressing, US+D). At seven, 14 and 21 days, the animals were euthanized, and the specimens of interest removed for histological analysis. RESULTS: Histological and histomorphometric analysis revealed a greater percent wound regression in animals receiving the dressing (group III: 55.97; group IV: 53.06), as well as a greater reduction in the inflammatory infiltrate (group III: 29.14; group IV: 31.71) since day 7. A later effect, with progression of the tissue repair process only after 14 and 21 days, was observed in the LED group intense fibroblast proliferation and greater collagen fiber production and organization were seen in the LED+D and US+D groups compared to the other groups. CONCLUSION: LED combined with a dressing was more effective at accelerating in the repair of cutaneous lesions.

Pododermatitis is currently one of most frequent and important clinical complications in seabirds kept in captivity or in rehabilitation centers. In this study, five Magellanic penguins with previous pododermatitis lesions on their footpad were treated with photodynamic therapy (PDT). All PDT treated lesions successfully regressed and no recurrence was observed during the 6-month follow-up period. PDT seems to be an inexpensive and effective alternative treatment for pododermatitis in Magellanic penguins encouraging further research on this topic.

BACKGROUND: Dermatology is primarily an outpatient clinical and surgical specialty, but substantial numbers of patients are admitted to hospital for inpatient treatment in dermatology wards. METHODS: We performed a retrospective study of patients admitted to dermatology beds between September 1, 2002, and September 30, 2010. Patient data were analyzed for age, gender, ethnicity, length of stay (LoS), dermatologic disease, comorbidities, hospital-acquired infection (HAI), transfer to the intensive care unit (ICU), and mortality. RESULTS: A total of 3308 patients admitted during this 8-year period were identified for analysis. The most frequent admissions were for eczema/dermatitis (17.5%) and cutaneous infections (15.9%). The mean LoS was 13.0 days. The mean ± standard deviation (SD) number of comorbidities per patient was 1.0 ± 1.2, among the most frequent of which were hypertension and diabetes mellitus. The rate of HAI was 6.2%; bloodstream infection was regarded as the most commonly acquired type and Staphylococcus aureus as the infectious agent most commonly found in culture. Of the patients admitted, 3.7% were transferred to the ICU and 2.5% died. In these latter two groups, the most common dermatologic diagnoses were immunobullous diseases, and the mean hospital LoS and rate of HAI were higher than in the total admissions cohort. CONCLUSIONS: Higher value should be placed on dermatology inpatient services in order to expand the availability of dermatology beds, mainly in tertiary hospitals, in view of the potentially high severity of the dermatologic diseases found in many patients referred to this type of service.
