BACKGROUND: Bowel obstruction due to accidental ingestion of foreign objects occurs rarely in children because 80 to 90% of the objects can pass freely through the gastrointestinal tract. CASE REPORT: We report a case of a 14-month-old infant who presented bowel obstruction caused by the ingestion of hydrogel beads (sodium polyacrylate). Hydrogel beads are used as sensory and didactic toys that can increase their initial size 200 to 400 times by liquid absorption. An abdominal X-ray was perfomed in anteroposterior supine projection, where a round filling defect at the loop of the right flank was detected; this came to our attention because hydrogel beads are usually radiolucent. The diagnosis was established by abdominal ultrasound where free intraperitoneal fluid was reported with data of small bowel pseudo-obstruction by foreign objects. Conservative treatment was prescribed, finding persistence of increased abdominal perimeter, so an enterotomy was performed for their removal; finding impacted hydrogel beads 30 centimeters from the ileocecal valve. CONCLUSIONS: Hydrogel beads are dangerous for the pediatric population. The evolution of the patient was favorable thanks to the knowledge of the foreign objects ingested. The expectant behavior that had to be executed, stands out because we had no knowledge as to the maximum size of the hydrogel in the gastrointestinal tract.
INTRODUCCIÓN: La obstrucción intestinal por ingesta accidental de cuerpos extraños se presenta muy rara vez en la edad pediátrica debido a que del 80 al 90% de los objetos pueden pasar libremente por el tracto gastrointestinal. CASO CLÍNICO: Se aborda el caso de una paciente de sexo femenino de 1 año 2 meses quien presentó obstrucción intestinal debido a la ingesta de esferas de hidrogel (poliacrilato de sodio). Dichas esferas, que son utilizadas como juguetes didácticos o sensoriales, aumentan de 200 a 400 veces su tamaño inicial mediante la absorción de agua. Se realizó radiografía abdominal en proyección decúbito supino, donde llamó la atención el hallazgo de defecto de llenado redondeado en asa de flanco derecho, ya que las esferas de hidrogel son radiolúcidas. El diagnóstico se estableció mediante ultrasonido abdominal, donde se reportó líquido libre peritoneal con datos de suboclusión por cuerpos extraños a nivel intestinal. Se indicó tratamiento conservador, encontrando persistencia de aumento de perímetro abdominal. Se realizó enterotomía y se encontraron las esferas impactadas a 30 centímetros de la válvula ileocecal. CONCLUSIONES: Las esferas de hidrogel son peligrosas para la población pediátrica. La evolución de la paciente fue favorable debido al conocimiento del objeto extraño ingerido. Sobresale la conducta expectante que se tuvo que desempeñar debido a que se desconocía el crecimiento de las esferas y en qué momento no podrían continuar su paso por el tracto gastrointestinal.

BACKGROUND: Foreign bodies adherent to the hard palate often mimic oral lesions. Case report: A 10-month-old female infant presented with the sudden development of a hard palate lesion. With photography and visualization, the "lesion" was a false nail, which belonged to the child's caregiver. Discussion/conclusion: The differential diagnosis of palatal lesions in infants should include foreign bodies, and if identified as such, evaluation under anesthesia may be avoided.

The presence of esophageal foreign body (EFB) is a common emergency in gastroenterology. The protocol for management and endoscopic intervention can be variable among institutions. OBJECTIVE: to define the clinical characteristics of EFB in adults, its radiological and endoscopic diagnosis, and complications based on a sample of patients in a gastroenterology center. MATERIALS AND METHODS: case series of patients admitted from the emergency department and referred to the gastroenterology department with a presumptive diagnosis of EFB. Clinical variables were collected, as well as characteristics, comorbidities, time of evolution and diagnostic opportunity, confirmatory studies, and complications. RESULTS: 84 subjects, 70% men, mean age 45 (range: 17-87; SD 12.5) years. Urgent upper endoscopy was performed in 98.8% of the patients, with an average in-hospital stay of 2.5 days. 93% had no associated underlying pathology, in 6/84 (7.14%) patients structural or functional esophageal pathology was documented. 59/84 (70.2%) patients consulted in the first 24 hours, in 57.6% the presence of foreign body was confirmed endoscopically. In 67/84 (79.76%) patients radiography was performed prior to endoscopy, of which 62/67 (92.5%) had an abnormal result. Seventy percent of confirmed EFB were fish bones. The most frequent site of localization was in the cricopharyngeal region in 90% of the cases. In 66/84 (78.6%) subjects there was absence of complications, followed by deep laceration in 10/84 (11.9%) cases. In 3/84 (3.6%) cases complications requiring surgery were identified. CONCLUSIONS: Endoscopic intervention in the first 24 hours is an opportune moment to identify complications and provide the indicated treatment.

OBJECTIVE.: To determine the association between pre-bronchoscopy time of illness and post-bronchoscopy discharge time in pediatric patients with foreign body aspiration. MATERIALS AND METHODS.: Retrospective cohort study. Medical records were studied and reviewed at the Pediatric Emergency Hospital of Lima, Peru. We reviewed 324 medical records, and selected 183 because they had the diagnosis of foreign body aspiration and complete data. Fisher's exact test and Mann Whitney U test were used for the bivariate analysis, while Poisson regression was used to calculate the Relative Risk (RR) and its 95% confidence interval (CI). RESULTS.: We included 183 patients, of whom 65.6% were men with a mean age of 2.4 years. The most frequent location of the foreign body was the right bronchial tree and most of them were made of organic material. The majority (72.7%) of patients were discharged before 24 hours. We found an association between the time of illness prior to bronchoscopy and post-bronchoscopy discharge time (RR: 2.94, 95%CI: 1.72 - 5.01). CONCLUSIONS.: The time of illness prior to bronchoscopy and the length of hospital stay after removal of the foreign body were significantly associated when adjusted for age, sex, type of foreign body and mouth sweep maneuver as a rescue measure. Our findings are relevant because they highlight the importance of early care, timely diagnosis and early management of pediatric patients.
OBJETIVOS.: Determinar la asociación entre el tiempo de enfermedad previo a broncoscopia y el tiempo de alta posbroncoscopia en pacientes pediátricos con aspiración de cuerpo extraño. MATERIALES Y MÉTODOS.: Estudio de cohorte retrospectivo. El estudio y la revisión de historias clínicas se realizó en el Hospital de Emergencias Pediátricas de Lima, Perú. Se revisaron 324 historias clínicas, de las cuales, 183 historias fueron seleccionadas por contar con diagnóstico de cuerpo extraño en vías aéreas y estar adecuadamente llenadas. Para el análisis bivariado se utilizó la prueba exacta de Fisher y U de Mann Whitney, mientras que para el cálculo del riesgo relativo (RR) y su respectivo intervalo de confianza (IC) al 95% se utilizó regresión de Poisson. RESULTADOS.: Se incluyeron 183 pacientes, de los cuales, el 65,6% fueron hombres con una media de 2,4 años. La localización más frecuente fue el árbol bronquial derecho y cuerpo extraño de material orgánico. La mayoría (72,7%) de pacientes tuvieron un alta temprana antes de las 24 horas. Se encontró una asociación entre el tiempo de enfermedad previo a la broncoscopia y el tiempo alta posbroncoscopia (RR: 2,94, IC 95%: 1,72-5,01). CONCLUSIONES.: Existe una asociación estadísticamente significativa entre el tiempo de enfermedad previo a la broncoscopia y el tiempo de estancia hospitalaria posextracción del cuerpo extraño al ajustar por edad, sexo tipo de cuerpo extraño y maniobra de boca como medida de rescate. Este hallazgo es relevante debido a que pone en evidencia la importancia de una atención temprana, diagnóstico oportuno y manejo precoz en el paciente pediátrico.

Meckel's diverticulum is the most common congenital defect of the gastrointestinal tract. It is most often silent or asymptomatic. However, acute abdominal signs of obstruction, inflammation, hemorrhage and perforation are possible in other cases. Different foreign bodies can cause perforation of Meckel's diverticulum. We present an 18-year-old male who presented to the emergency department with abdominal pain within 48 previous hours. After evaluation, acute appendicitis was diagnosed and he underwent laparoscopic appendectomy. Meckel's diverticulum perforated by a fish bone was intraoperatively discovered. Incidental appendectomy and segmental resection of the ileum were performed.

Introducción: la faringe es el sitio más común de alojamiento de cuerpos extraños dentro del área otorrinolaringológica. Usualmente su diagnóstico y extracción resultan fáciles. Sin embargo, y afortunadamente, unos pocos se encarnan profundamente y causan infecciones en el cuello, o más raro aún, logran migrar y se sitúan inmediatamente por debajo de la piel. Presentación del caso: presentamos el caso de un paciente masculino de 27 años que fue atendido durante nuestra misión en Namibia en el 2018, quien se tragó una espina de pescado, la cual se alojó inicialmente en el espacio retrofaríngeo para luego migrar hasta el tejido celular subcutáneo del cuello. Discusión: el mecanismo por el cual un cuerpo extraño migra puede estar dado por la combinación del peristaltismo del esófago y los movimientos del cuello u otros factores como la configuración lineal y afilada del cuerpo extraño, la deglución forzada de alimentos de consistencia dura inmediatamente a la impactación del cuerpo extraño y la asistencia tardía del paciente a los servicios de salud. Conclusiones: la migración de un cuerpo extraño ingerido es una rara forma de evolución. Su tratamiento depende fundamentalmente de potenciales complicaciones que podrían poner en riesgo la vida del paciente(AU)
Introduction: the pharynx is the most common site of lodging of foreign bodies within the otorhinolaryngological area. Diagnosis and removal are usually easy. Fortunately, however, a few become deeply ingrown and cause neck infections, or more rarely, manage to migrate and lie just under the skin. Case presentation: we present the case of a 27-year-old male patient who was treated during our mission in Namibia in 2018, who swallowed a fish bone, which initially lodged in the retropharyngeal space and then migrated to the cellular tissue subcutaneous neck. Discussion: the mechanism by which a foreign body migrates may be due to the combination of peristalsis of the esophagus and movements of the neck or other factors such as the linear and sharp configuration of the foreign body, the forced swallowing of food with a hard consistency immediately after impaction of the foreign body and late attendance of the patient to health services. Conclusions: the migration of an ingested foreign body is a rare form of evolution. Its treatment fundamentally depends on potential complications that could put thepatient's life at risk(EU)

OBJECTIVE: To examine, using the National Poison Data System (the data warehouse for poison control centers in the US), magnet foreign body injuries in pediatric patients. We sought to report demographic data, outcome data, and case trends between 2008 and 2019. STUDY DESIGN: We conducted a retrospective analysis of the National Poison Data System for patients younger than 19 years of age with a magnet "exposure," which poison centers define as an ingestion, inhalation, injection, or dermal exposure to a poison. RESULTS: A total of 5738 magnet exposures were identified. Most were male (3169; 55%), <6 years old (3572; 62%), with an unintentional injury (4828; 84%). There were 222 patients (3.9%) with a confirmed medical "effect," defined as signs, symptoms, and clinical findings not including therapeutic interventions (eg, endoscopy). There was a 33% decrease in cases from 418 (2008-2011) to 281 per year (2012-2017) after high-powered magnet sets were removed from the market. Calls subsequently increased 444% to 1249 per year (2018-2019) after high-powered magnet sets re-entered the market. Cases from 2018 and 2019 increased across all age groups and account for 39% of magnet cases since 2008. CONCLUSIONS: Significant increases in magnet injuries correspond to time periods in which high-powered magnet sets were sold, including a 444% increase since 2018. These results reflect the increased need for preventative or legislative efforts.

ABSTRACT This report aims to discuss the occurrence and differential diagnosis in hard palate foreign bodies in infants. We present the case of a 16-month-old girl who was found to have plastic part of a toy embedded in her hard palate as a foreign body. The foreign body was extracted with care to avoid aspiration. The possibilities of misdiagnosis and airway aspiration are discussed. Great care was focused on the prevention of airway aspiration.

Penetrative cardiac injury can often result in life-threatening sequelae such as myocardial contusion or rupture, coronary vessel and valvular damage, pericardial effusion with tamponade, and arrhythmias of which gunshot injury is a chief culprit. We report a case of a suspected acute coronary syndrome after a cardiac gunshot injury that was conservatively managed.

The ingestion of foreign bodies, mainly metallic (MFB- metallic foreign body), by cattle is frequent and can lead to death. For diagnostic purposes, reticular pain tests are routinely used, and ferroscopy is still unusual, even as a diagnostic aid. The objective of this study was to validate the application of the GMS 120 Professional BOSCH® metal detector (MD) in the detection of MFB in the bovine species and describe its use in field cases. Fifty MFBs of varying shape, size, and weight were evaluated with an MD, initially on a flat and inert surface, and then in a permanent rumen fistulated cow. Retrospectively, a database from 2000 to 2017 with 122015 cattle was accessed, of which 31865 were examined after the introduction of MD usage in 2013. Fifteen cows positive for reticular pain tests were prospectively evaluated and eight cows were diagnosed with MFB through MD usage. The mean distance for MFB best detection on the inert surface was 4.6 cm. In the fistulated cow, it was not possible to detect any MFBs, regardless of the size or weight of the material. The MFB detection capacity increased with MD use, which can be considered a diagnostic aid in specific situations. Depending on the body condition of the bovine, false negatives may occur with the use of GMS 120 Professional Bosch® metal detector.(AU)
A ingestão de corpos estranhos, principalmente metálicos (CEM), pelos bovinos é frequente e pode levar à sua morte. Para fins diagnósticos, as provas de dor reticular são usados de forma rotineira, sendo ainda pouco usual a ferroscopia, mesmo como auxílio diagnóstico. Neste trabalho procurou-se validar a aplicação do detector de metais (DM), marca GMS 120 Professional BOSCH® no reconhecimento de CEM na espécie bovina, além de descrever seu uso em casos a campo. CEM (n=50) de forma, tamanho e peso variados foram selecionados e avaliados com o DM, inicialmente sobre uma superfície plana e inerte e depois in vivo em uma vaca com fístula ruminal. Retrospectivamente, acessou-se um banco de dados dos anos 2000 a 2017 com 122015 bovinos, com 31865 bovinos atendidos após a introdução do DM no ano de 2013. Prospectivamente, avaliaram-se 15 vacas positivas para o teste de dor reticular, oito vacas foram diagnosticadas com CEM através do DM. A distância média para a melhor detecção dos CEM na superfície inerte foi de 4,6 cm. Em vaca fistulada não foi possível detectar nenhum CEM, independentemente do tamanho e peso do material. A campo, após a introdução do uso do DM aumentou a capacidade de detecção dos CEM. O DM pode ser considerado um auxílio diagnóstico em situações específicas. Dependendo da condição corporal do bovino, falsos negativos podem ocorrer com o uso do aparelho GMS 120 Professional Bosch®.(AU)

Na rotina da clínica médica de pequenos animais, o corpo estranho é uma ocorrência relativamente comum, sobretudo quando se trata de animais jovens, de pequeno a médio porte ou apetite exagerado e grande curiosidade. Casos de corpos estranhos lineares são particularmente mais complexos que outros tipos de corpos estranhos devido à área que acabam por acometer ser de maior extensão e assim resultam em casos mais graves com maior processo inflamatório com acometimento das populações bacterianas da microbiota local. Quando acometem a região entérica, o corpo estranho linear pode desenvolver quadros de intussuscepção. A sintomatologia pode ser ampla, incluindo desde sinais característicos do trato gastrointestinal, como vômito e falta de apetite, até sintomas mais sistêmicos como febre, apatia e desordens inflamatórias. O diagnóstico deve incluir anamnese, exames físico e clínico, observação do estado geral do animal, histórico e informações relevantes e fatores predisponentes como raça e idade podem ser importantes para o caso. Exames de imagens e laboratoriais auxiliam no diagnóstico e tem papel fundamental para estadiar o tempo de acometimento e precisar o local em que o corpo estranho está situado no trato gastroentérico. Este trabalho tem como objetivo relatar um caso de corpo estranho linear excepcional devido à sua extensão de aproximadamente 30 metros e à complexidade da técnica cirúrgica de sua remoção que associou uma gastrotomia e quatro enterotomias. Apesar da complexidade da intervenção cirúrgica, o resultado e prognóstico observado no animal foi positivo, com rápida recuperação pós-cirúrgica.
In the routine of the small animal medical clinic, the foreign body is a relatively common occurrence, especially when it comes to young animals, from small to medium size or exaggerated appetite and great curiosity. Cases of linear foreign bodies are particularly more complex than other types of foreign bodies due to the area that they end up being of greater extent and thus result in more severe cases with a greater inflammatory process affecting the bacterial populations of the local microbiota. When they affect the enteric region, the linear foreign body may develop intussusception. The symptoms can be wide ranging, ranging from signs characteristic of the gastrointestinal tract, such as vomiting and poor appetite, to more systemic symptoms such as fever, apathy and inflammatory disorders. The diagnosis must include anamnesis, physical and clinical examinations, observation of the animal's general condition, history and relevant information and predisposing factors such as breed and age may be important for the case. Imaging and laboratory tests assist in diagnosis and have a fundamental role in staging the time of onset and specifying the location where the foreign body is located in the gastroenteric tract. This work aims to report a case of an exceptional linear foreign body due to its extension of approximately 30 meters and the complexity of the surgical technique of its removal that associated a gastrotomy and four enterotomies. Despite the complexity of the surgical intervention, the result and prognosis observed in the animal was positive, with rapid post-surgical recovery.

INTRODUCTION: The term «gossypiboma¼ comes from the Latin gossypium, which refers to a genus of cotton plants, and from the Swahili word boma, which translates as «place of concealment¼. It may be mistaken for tumorous lesions or abscesses due to the way it is encapsulated, as evidenced in imaging examinations, and its variable and non-specific clinical features, which give rise to difficulty in its diagnosis and significant morbidity. AIM: To synthesise the available evidence on the presence of gossypibomas during neurosurgical procedures. DEVELOPMENT: A review was performed that included a search for articles in English and Spanish published in the last 15 years in PubMed, Ebsco Host, Embase, Mediclatina, Cochrane, Lilacs and Scopus, between January and June 2019, using the keywords «gossypiboma¼, «textiloma¼, «neurosurgery¼ and «neurosurgical procedures¼. In all, a total of 630 articles were found in the search, although, after selecting them by title and abstract, 22 case report articles were included for this review process. Altogether 36 individuals were identified, of whom 21 (58.3%) were women, and whose mean age was 56.1 years. Surgical sponges were observed as gossypibomas in 20 cases (55.6%). CONCLUSIONS: Gossypiboma is a complication secondary to surgical procedures that presents fairly unspecific signs and symptoms. The time that elapses before it appears usually ranges from a few days to several years after surgery and is correlated with multiple medical and legal implications.
TITLE: Gossypibomas en neurocirugía.Introducción. El término «gossypiboma¼ proviene del latín gossypium, que hace referencia a un género de plantas de algodón, y de la palabra kiswahili boma, que se traduce como «lugar de escondite¼. Puede confundirse con lesiones tumorales o abscesos debido a la forma de encapsulación evidenciada en los exámenes imaginológicos y su clínica variable e inespecífica, situación que genera dificultad para el diagnóstico y una morbilidad importante. Objetivo. Sintetizar la evidencia disponible sobre la presencia de gossypibomas durante la realización de procedimientos quirúrgicos en neurocirugía. Desarrollo. Se realizó una revisión en la cual se incluyó una búsqueda de artículos en inglés y castellano publicados en los últimos 15 años en PubMed, Ebsco Host, Embase, Mediclatina, Cochrane, Lilacs y Scopus, entre enero y junio de 2019, utilizando las palabras clave «gossypiboma¼, «textiloma¼, «neurosurgery¼ y «neurosurgical procedures¼. El total de artículos encontrados en la búsqueda fue de 630; sin embargo, tras la selección por título y resumen fueron 22 los artículos de informe de caso que se incluyeron. Se identificó a un total de 36 individuos, de los cuales 21 (58,3%) eran mujeres, y cuya edad media era de 56,1 años. En 20 casos (55,6%) se observaron esponjas quirúrgicas como gossypibomas. Conclusiones. El gossypiboma es una complicación secundaria a procedimientos quirúrgicos, con signos y síntomas bastante inespecíficos. Su tiempo de aparición suele oscilar entre unos cuantos días hasta varios años después de realizada la cirugía y se correlaciona con múltiples implicaciones médicas y legales.

Se presenta el caso clínico de un niño de un año de edad, quien fue atendido en el Servicio de Pediatría del Hospital de Serrekunda en Gambia por presentar, desde hacía 2 días, dificultad respiratoria, tos y falta de aire, todo ello de inicio rápido. Ante la posible presencia de un cuerpo extraño en las vías respiratorias altas se realizó un examen físico minucioso que incluyó el laringoscopio, pero debido a su ubicación profunda no fue posible una correcta observación de este, por lo cual se indicaron otros estudios (resonancia magnética y endoscopia), que permitieron una correcta visualización de dicho cuerpo. Debido al tratamiento multidisciplinario y oportuno, el paciente tuvo una evolución favorable y egresó sin complicaciones(AU)
The case report of a year-old child is presented who was assisted in the Pediatrics Service of the Serrekunda Hospital in Gambia for presenting breathing difficulty for 2 days, cough and lack of air, all of them of fast beginning. Taking into account the possible presence of a strange body in the high breathing ways a meticulous physical exam was carried out that included the laryngoscope, but due to its deep location it was not possible a correct observation of it, reason why other studies were indicated (magnetic resonance and endoscopy) that allowed a correct visualization of this body. Due to the multidisciplinary and opportune treatment, the patient had a favorable clinical course and was discharged without complications(AU)

A female dog was treated at the Veterinary Hospital Governador Laudo Natel for symptomscaused by an intra-abdominal gossypiboma (gossypium: cotton; boma: place of hiding). Showing high morbidity and mortality, the gossypiboma is a granulomatous reaction that is formed in response to exposure to a textile matrix. The objective of this report is to describe the clinical and pathological findings of an intra-abdominal gossypiboma in a female dog. This patient arrived at urgent care showing increased abdominal volume, pain, and cachexia. Ultrasonography showed a regular contour formation that could not be completely delimited. The patient underwent exploratory laparotomy and died in the postoperative period. The gossypiboma had a smooth, firm, and purple-gray exterior surface. Microscopy revealed a severe reaction of desmoplasia around the necrotic region, residual textile material, and adipose tissue. Negligence by veterinarians leading to this condition can result in patients' death as well as causing psychological damage to owners.(AU)
Uma cadela foi atendida no Hospital Veterinário "Governador Laudo Natel", apresentando sinais clínicos oriundos de gossipiboma intra-abdominal (gossypium: algodão e boma: local de ocultação), tais como aumento de volume abdominal, regurgitação e caquexia. A ultrassonografia evidenciou formação de contorno regular que não podia ser totalmente delimitada. A paciente foi submetida à laparotomia exploratória e veio a óbito no período pós-operatório. O gossipiboma mostrou-se com superfície externa lisa, firme e roxo-acinzentada apresentando-se à microscopia como reação severa de desmoplasia ao redor de material necrótico, resíduos de material têxtil e do tecido adiposo. A negligência por parte dos médicos veterinários pode levar pacientes ao óbito, bem como trazer prejuízos psicológicos aos proprietários. Objetiva-se, com este relato, descrever os achados clínicos e patológicos de uma cadela que apresentou um gossipiboma intra-abdominal.(AU)
