RESUMEN

Objetivo: Demostrar los beneficios del uso de cuero cabelludo como área donadora de injertos cutáneos de espesor parcial. Método: Estudio retrospectivo descriptivo realizado de 2013 a 2015 en la Unidad de quemaduras pediátricas del hospital Roosevelt de 111 pacientes en que se utilizaron injertos del cuero cabelludo. Resultados: La mayoría de los pacientes sufrieron quemaduras por inmersión en líquidos calientes con disponibilidad limitada de zonas donadoras de injertos. El otro uso que se le dio a los injertos de cuero cabelludo fue en quemaduras y lesiones faciales debido a la similitud de la pigmentación de estas áreas. Este estudió demostró el corto tempo de epitelización de esta zona donadora (3.5 días), lo que permitió que se utilizara en varias ocasiones. La principal complicación fue la alopecia (1.8%) resultado de una toma de un injerto muy grueso. Conclusión: El estudio sugiere que el cuero cabelludo es una zona donadora útil cuando las principales áreas donadoras están limitadas. Más investigación tendrá que realizarse para establecer otros beneficios observados del uso de esta región como zona donadora de injertos cutáneos.


Objectve: Demonstrate the benefts of using scalp as a donor area of partal split thickness skin grafs. Methods: Retrospectve descriptve study from 2013 to 2015 shows 111 patents that required a skin graf from the scalp in a pediatric burn unit. Results: Most of the patents were burn victms, but few other cases are described as well. The most common use of this donor area was for patents that had sufered pot immersion scalds where the back, gluteus and lower limbs were afected and no longer available as skin donor areas. The other main use was in facial burns and traumas, because the color similitude between the face and scalp renders a beter cosmetc result of facial skin grafs. This study demonstrates the short period of healing (3.5 days) of the donor area making possible its reuse in a shorter period of tme. The main complicaton was alopecia (1.8%) where the graf take was too deep. Conclusions: The study suggests the use of scalp donor area when other main donor areas are not available and for facial burns. Further studies should be performed in order to establish other observed benefts of the scalp as a donor area for split thickness skin grafs.

RESUMEN

The temporoparietal fascia flap has been extensively used in craniofacial reconstructions. However, its use for eyebrow reconstruction has been sporadically reported. We describe a successfully repaired hair-bearing temporoparietal fascia flap after traumatic avulsion of eyebrow. Temporoparietal fascia flap is a versatile tool and should be considered as a therapeutic option by all plastic surgeons.

O retalho de fáscia temporoparietal tem sido usado em inúmeras reconstruções craniofaciais. No entanto, sua utilização para a reconstrução de sobrancelha tem sido relatada esporadicamente. Descreve-se aqui o reparo com retalho de fáscia temporoparietal associado à ilha para a reconstrução de sobrancelha, após avulsão traumática, realizado com sucesso. O retalho de fáscia temporoparietal é uma ferramenta versátil e deveria ser considerada no arsenal terapêutico de todos os cirurgiões plásticos.

PURPOSE: To investigate the reproducibility of the experimental model of face allotransplantation in rats in Brazil. METHODS: Eighteen rats were operated, nine-nine donors recipients. Animals underwent transplantation of the left hemiface, with periorbital and scalp. Transplants were made from donor Wistar rats to recipients Lewis rats. Flaps were based on the common carotid artery and the external jugular vein of the donor animal and the anastomosis in the recipient area was performed in common carotid artery (end-to-side) and in external jugular vein (end-to-end). RESULTS: Of the nine recipient animals operated, six survived and three progressed to death in the first days after surgery (survival rate = 67%). The mean time of the procedure was 252 minutes and the mean time of flap ischemia was 95 minutes. The five surviving animals were sacrificed at 14 days, in good general condition and without signs of tissue rejection. CONCLUSIONS: The experimental model of face allotransplantation in rats is reproducible in our midst. Duration of surgery, time of flap ischemia, animal survival rate and complications observed were similar to those described in the literature.

AIM: This study aims to describe the incidence of complications on scalp from which a thin split-skin graft was harvested (0.005-0.007 in.) of the donor site in children and adult burn victims. METHODS: We reviewed the medical records of 295 burn patients admitted in the Burn Unit of the Clinical Hospital of the Faculty of Medicine of Ribeirão Preto, from January 1998 to December 2007, whose scalps were used as donor site for grafts. Skin-graft thickness varied from 0.005 in. to 0.007 in. The occurrence of pathological healing was evaluated clinically and the time of epithelisation by the main surgeon and a plastic surgeon or a staff nurse. RESULTS: Of the 295 patients whose scalps were used as donor site, 274 were followed from 6 months to 10 years after the procedure (median 18.2 months). Twenty-one patients were lost to follow-up in the first 6 months. No hypertrophic scarring or keloids on the donor site was observed. Five patients (1.82%) presented with folliculitis and two of them were evaluated with small areas of alopecia (0.7%), treated with resection of these areas and primary suture. The average time of epithelisation of the donor site was 7 days. CONCLUSION: The harvest of thinner split graft from the scalp is a safe procedure.

RESUMEN

The absence of an eyebrow, either partial or total, has been observed in patients with craniofacial clefts, such as the Tessier 9 to 13 cleft. Several techniques have been used to improve the appearance of the region, such as island scalp flaps and scalp strip grafting, with limited or marginally satisfactory aesthetic results. The authors report 2 patients with craniofacial clefts in whom a novel technique combining 2 separate surgical approaches, micrografting and tattooing, was used. The use of micrografting with single or double hair units, properly angulated, produces natural-looking and satisfactory results with a minimum of morbidity. Excellent volume and appearance of the eyebrow may be achieved in a single session using this technique. Tattooing performed subsequently over the microimplanted hairs provides the illusion of greater density to the eyebrow, resulting in an appearance closer to normal.

RESUMEN

Se presentan los resultados obtenidos en el tratamiento de 230 pacientes alopécicos, mediante trasplante autólogo de cuero cabelludo. Se analizan sexo, raza, edad, causa de la alopecia y número de operaciones por paciente. Se comunican los factores que han influido en la mejoría de los resultados alcanzados por este grupo multidisciplinario, en cuatro años de trabajo continuo.

RESUMEN

The technique and results of a new scalp flap that solves frontal baldness are described. As a bipediculate flap it offers a great amount of hair when advanced and has a low rate of complications. This kind of flap has been used by us for four years in 36 patients in the age group between 20 and 60 years. The results obtained with the flap have been very satisfactory.
