RESUMEN

The aim of this study was to investigate the potential use of salivary IL1ß in early-stage diagnostics of peri-implant inflammation in partially and totally edentulous patients rehabilitated with dental implants. Patients were classified according to peri-implant probing depth and bleeding upon probing in groups of healthy individuals or in groups of individuals with peri-implant inflammation. Data on plaque index, clinical attachment loss, suppuration, and mobility were also assessed. Saliva was collected without stimulation, and the levels of IL-1ß were determined by ELISA. Healthy groups demonstrated significantly lower levels of IL-1ß compared with the inflammation groups. No difference in IL-1ß levels was observed between partially edentulous or totally edentulous patients. Salivary IL-1ß may be useful for the diagnosis and monitoring of early peri-implant inflammation, particularly in edentulous patients.

OBJECTIVE: We analyzed the association of coronary heart disease (CHD) and serology of periodontitis in a random sample (n=1163) of men (aged 45 to 74 years) by determining serum IgG-antibodies to Actinobacillus actinomycetemcomitans and Porphyromonas gingivalis. METHODS AND RESULTS: CHD (n=159) was more prevalent among edentulous than dentate subjects (19.8% and 12.1%, P=0.003). In the dentate population, CHD was more common among subjects seropositive for P. gingivalis compared with those seronegative (14.0% and 9.7%, P=0.029). Accordingly, CHD was more prevalent in subjects with a high combined antibody response than those with a low response (17.4% and 11.1%, P=0.026). When adjusted for age and several CHD risk factors, the subjects with a high combined antibody response had an odds ratio of 1.5 (95% CI, 0.95 to 2.50, P=0.077) for prevalent CHD. In a linear regression model, the combined antibody response was directly associated with prevalent CHD (P=0.046) and inversely with serum HDL cholesterol concentration (P=0.050). CONCLUSIONS: In conclusion, edentulousness and serum antibodies to major periodontal pathogens were associated with CHD. This suggests that periodontal infection or response of the host against the infection may play a role in the pathogenesis of CHD.

The 19 patients included in this study had all been successfully treated for total or partial edentulism with single crystal sapphire implants as retention for overdentures or fixed bridges. As there is a need for more reliable non-invasive parameters for detecting changes surrounding endosseous implants, the aim of this study was to assess the content and the activity of neutrophils in crevicular fluid samples from 3 categories of sites: (1) crevices around implants from edentulous patients (2) crevices around implants in partially edentulous patients, and (3) crevices surrounding teeth. Fluid samples were taken with paper strips from 9 partially edentulous and 10 edentulous patients and the volume measured with a Periotron 6000. Elastase activity was measured as a marker of neutrophil activity and lactoferrin concentration as a marker of the number of neutrophils. Elastase activity per microliter and lactoferrin concentration was, despite similar clinical and radiographic signs, significantly higher in samples from crevices surrounding teeth and implants in the partially edentulous patients compared to samples from crevices around implants in the totally edentulous patients. There were no differences between teeth and implants in partially edentulous patients. The increased elastase activity and lactoferrin concentration indicates a higher neutrophil activity in patients with remaining teeth.

RESUMEN

Analyzes the clinical and immunologic data evidencing that implantation, as a rule, induces immunologic shifts in patient's body. Traces the direct relationship between the frequency and severity of complications and the type of the patient's immune response to implantation, the intensity of this response being directly related to the implantation material and volume. Basing on the relationship between the clinical and immunologic data, recommends examination of the patients' immune status for the prediction of the results of the surgical stage of dental implantation and for the assessment of new implantation material intended for introduction into practical dentistry.

An antibody-capture radioimmunoassay was used to measure levels of IgG class antibodies to rubella and hepatitis A viruses in serum and saliva of 30 edentulous, 30 partially dentate and 31 dentate individuals. The prevalence of seropositivity for rubella was 98.9 per cent and for hepatitis A 73.6 per cent. The serum reactivities were generally greater than those for saliva. There were 8 false-negative results for saliva out of the 182 tests performed, of which 4 were in the edentulous group, 3 in the partially dentate and 1 in the dentate group. For both rubella and hepatitis A virus antibodies the (geometric) mean ratios between the saliva and serum reactivities were similar across the three dental groups. The values for sensitivity, specificity and positive predictive value suggest that assay of saliva for antiviral IgG antibody is a satisfactory technique regardless of dental status.

The concentrations of IgA, IgG and IgM in whole saliva of predentate babies, dentate adults, and edentulous old people, and the salivary IgG concentration of dentate old people, were determined using a solid phase radioimmunoassay. A highly significant difference in IgG concentrations was found between toothless and dentate people. The average IgG concentration in the saliva of edentulous old people was 5.2 mg/1, which was about 20% of that of dentate old people. The salivary IgG content of babies was only 1.7 mg/1, which was in concordance with the low postnatal serum IgG values. The salivary IgA level in this age group was also low. The edentulous old people tended to have decreased IgM values, and the predentate babies tended to have increased IgM values compared with the dentate adults.
