BACKGROUND AND IMPORTANCE: Complete posterior atlantoaxial dislocation (PAAD) with an unfractured odontoid process is a rare condition where a dislocated but intact odontoid process is positioned ventrally to the anterior arch of C1. This lesion is related to transverse and alar ligament rupture secondary to hyperextension and rotatory traumatic injury and is often associated with neurological deficit. The treatment strategy remains controversial, and in many cases, odontoidectomy is required. Traditional approaches for odontoidectomy (transnasal and transoral) are technically demanding and are related to several complications. This article describes a 360° reduction and stabilization technique through a navigated anterior full-endoscopic transcervical approach (nAFETA) as a novel technique for odontoidectomy and C1-C2 anterior transarticular fixation supplemented with posterior fusion. CLINICAL PRESENTATION: A 21-year-old man presented to the emergency room by ambulance after a motorcycle accident. On evaluation, incomplete ASIA B spinal cord injury was documented. Imaging revealed a complete PAAD. We performed a two-staged procedure, a nAFETA odontoidectomy plus C1-C2 anterior transarticular fixation followed by posterior C1-C2 wired fusion. At a 2-year follow-up, the patient had a 10-point Oswestry Disability Index score and neurological improvement to ASIA E. CONCLUSION: PAAD can be successfully treated through minimally invasive nAFETA. Noteworthy, the risks of the transoral and endonasal routes were avoided through this approach. In addition, nAFETA allows anterior transarticular fixation during the same procedure providing spinal stability. Further studies are required to expand the use of nAFETA in this field.

The Os odontoideum is a variable oval or round ossicle with a smooth cortical border, which partially corresponds to the odontoid process, without having continuity with the rest of C2 bone. The multifactorial etiology causes instability and clinically translates into pain and compression data into neural structures. The treatment of choice is surgical and techniques have been developed that focus on preserving the stability of the segment. We present the case of a 23-year-old female patient, who begins to suffer at 8 years of age, refers to moderate to severe cervicalgia, which develops with paresthesias in the left hemisphere and later paresis of the left thoracic limb. Physical examination showed hypoaesthesia of the left hemisphere, as well as paresis of the left thoracic limb. Extension studies demonstrate chronic axonal lesion from C1 to C3, predominantly left, imaging studies showing axial instability and magnetic resonance bulbar compression. The patient receives surgical treatment consisting of posterior fixation C1-C2, evolving satisfactorily.
Os odontoideum es un osículo oval o redondo de tamaño variable con un borde cortical liso, que se corresponde parcialmente con la apófisis odontoides, sin tener continuidad ósea con el resto de C2. La etiología es multifactorial, causa inestabilidad y clínicamente se traduce en dolor y datos de compresión a las estructuras neurales. El tratamiento de elección es quirúrgico y se han desarrollado técnicas que se enfocan en conservar la estabilidad del segmento. Presentamos el caso de una mujer de 23 años, inicia padecimiento a los ocho años, refiere cervicalgia moderada a intensa, que evoluciona con parestesias en hemicuerpo izquierdo y posteriormente paresia de miembro torácico izquierdo. A la exploración física se evidencia hipoestesia de hemicuerpo izquierdo, así como paresia de miembro torácico izquierdo. Los estudios de extensión demuestran lesión axonal crónica de C1 a C3, de predominio izquierdo, los estudios de imagen evidencian inestabilidad axial y la resonancia magnética compresión bulbar. La paciente recibe tratamiento quirúrgico consistente en fijación posterior C1-C2, evolucionando satisfactoriamente.

PURPOSE: Morphology measures of the odontoid process in children under 12 years old were carried out to demonstrate the viability of anterior internal fixation in this population once their active profile may not be compatible with successful conservative treatment. METHODS: During a 6-month period, 36 tomographic examinations of the cervical spine region that provided visualization of the odontoid process were selected. Group 1 included children between 6 and 9 years of age, and group 2 contained children from 9 to 12 years of age. There were 23 (63.8%) male patients and 13 (36.2%) female patients. Patients diagnosed with a tumor, an infection, fracture non-union, or congenital malformation were excluded. Exams were ordered as part of a protocol applied to non-specific neck pain and pediatric trauma entries. The following parameters were analyzed: (1) screw attack angle, (2) height of the odontoid process, and (3) minimal transverse diameter of the odontoid process. RESULTS: In Groups 1 and 2, the average values of the screw attack angle were 55.9° ± 2.3° and 54.8° ± 4.5°, respectively; the average heights of the odontoid process were 26.58 ± 3.28 and 29.48 ± 3 mm, respectively, and the average minimal transverse diameter of the odontoid process were 6.57 ± 1.08 and 6.23 ± 0.88 mm, respectively. The minimal transverse diameter of the odontoid process was statistically higher in males than that in females, regardless of age (p = 0.007). CONCLUSION: In both groups, the minimal transverse diameter of the odontoid process allowed for the use of one 3.5-4.5 mm screw for anterior internal fixation. INTRODUCTION: The Chiari I malformation (CIM) is commonly encountered by neurosurgeons and can have different etiologies and clinical presentations. CASE REPORT: We report a CIM patient who presented with symptoms of ventral brain stem compression and was found to have a large peri-odontoid pannus. Posterior fossa decompression was performed with a planned second-stage odontoidectomy. However, at the 6-month follow-up, postoperative images demonstrated a mostly resolved pannus and improvement of the brain stem compression symptoms, and the patient progressed uneventfully without the need for odontoidectomy. CONCLUSIONS: This case illustrates the resolution of a significant and symptomatic peri-odontoid pannus in a patient with CIM without craniocervical fusion or odontoidectomy. Such a case indicates that not all peri-odontoid pannus formations in CIM patients are due to hypermobility at the craniocervical junction.

OBJECT: Most of the craniometric relationships of the normal craniocervical junction (CCJ), especially those related to angular craniometry, are still poorly studied and based on measurements taken from simple plain radiographs. In this study, the authors performed a craniometric evaluation of the CCJ in a population without known CCJ anomalies. The purpose of the study was to evaluate the normal CCJ craniometry based on measurements obtained from CT scans. METHOD: The authors analyzed 100 consecutive CCJ CT scans obtained in adult patients who were admitted at their tertiary hospital for treatment of non-CCJ conditions between 2010 and 2012. A total of 17 craniometrical measurements were performed, including the relation of the odontoid with the cranial base, the atlantodental interval (ADI), the clivus length, the clivus-canal angle (CCA)-the angle formed by the clivus and the upper cervical spine, and the basal angle. RESULTS: The mean age of the 100 patients was 50.6 years, and the group included 52 men (52%) and 48 women (48%). In 5 patients (5%), the tip of the odontoid process was more than 2 mm above the Chamberlain line, and in one of these 5 patients (1% of the study group). it was more than 5 mm above it. One patient had a Grabb-Oakes measurement above 9 mm (suggesting ventral cervicomedullary encroachment). The mean ADI value was 1.1 mm. The thickness of the external occipital protuberance ranged from 7.42 to 22.36 mm. The mean clivus length was 44.74 mm, the mean CCA was 153.68° (range 132.32°-173.95°), and the mean basal angle was 113.73° (ranging from 97.06°-133.26°). CONCLUSIONS: The data obtained in this study can be useful for evaluating anomalies of the CCJ in comparison with normal parameters, potentially improving the diagnostic criteria of these anomalies. When evaluating CCJ malformations, one should take into account the normal ranges based on CT scans, with more precise bone landmarks, instead of those obtained from simple plain radiographs.

OBJECT: The goal of this study was to compare the indications, benefits, and complications between the endoscopic endonasal approach (EEA) and the microscopic transoral approach to perform an odontoidectomy. Transoral approaches have been standard for odontoidectomy procedures; however, the potential benefits of the EEA might be demonstrated to be a more innocuous technique. The authors present their experience with 12 consecutive cases that required odontoidectomy and posterior instrumentation. METHODS: Twelve consecutive cases of craniovertebral junction instability with or without basilar invagination were diagnosed at the National Institute of Neurology and Neurosurgery in Mexico City, Mexico, between January 2009 and January 2013. The EEA was used for 5 cases in which the odontoid process was above the nasopalatine line, and was compared with 7 cases in which the odontoid process was beneath the nasopalatine line; these were treated using the transoral microscopic approach (TMA). Odontoidectomy was performed after occipital-cervical or cervical posterior augmentation with lateral mass and translaminar screws. One case was previously fused (Oc-C4 fusion). The senior author performed all surgeries. American Spinal Injury Association scores were documented before surgical treatment and after at least 6 months of follow-up. RESULTS: Neurological improvement after odontoidectomy was similar for both groups. From the transoral group, 2 patients had postoperative dysphonia, 1 patient presented with dysphagia, and 1 patient had intraoperative CSF leakage. The endoscopic procedure required longer surgical time, less time to extubation and oral feeding, a shorter hospital stay, and no complications in this series. CONCLUSIONS: Endoscopic endonasal odontoidectomy is a feasible, safe, and well-tolerated procedure. In this small series there was no difference in the outcome between the EEA and the TMA; however, fewer complications were documented with the endonasal technique.

STUDY DESIGN: Retrospective matched cohort study. OBJECTIVE: To compare mortality in elderly patients with odontoid fractures after operative and nonoperative treatment. In addition, to evaluate potential factors that may increase the risk of mortality in the geriatric population after odontoid fracture. SUMMARY OF BACKGROUND DATA: Odontoid fractures represent the most common cervical spine fracture in patients over 70. In this population controversy exists as to the optimal treatment of odontoid fractures, especially type II fractures. METHODS: A retrospective review of all odontoid fractures in patients 75 years of age or older at our institution from 1996 to 2010 was performed. Comorbidities were stratified using the Charlson comorbidity index. Mortality was determined at 3 months, 1, and 5 years. RESULTS: A total of 96 patients were identified of which 75 met inclusion criteria. The average age of patients included was 82.3 years. The average Charlson comorbidity score for those operated on (2.37) was not significantly different from the nonoperative group (2.46), (P=0.45). At 3 months the operative group had a significantly lower mortality rate than the nonoperative group (P=0.024). There was no significant difference in 1-year (P=0.42) or 5-year (P=0.21) mortality between the operative and nonoperative cohorts. Of the patients treated nonoperatively, those who died within 3 months of treatment had a significantly higher Charlson score (3.98) than those who survived (2.14). Patients who survived at least 1 year after surgery had a significantly lower Charlson score (1.53) than those who died (3.22) within that time frame after surgery (P=0.05). CONCLUSIONS: There was no significant difference between 1- and 5-year mortality in patients treated operatively or nonoperatively. Regardless of treatment Charlson score had a significant impact on mortality. Age significantly affected mortality in patients treated surgically.

OBJECTIVE: To report the difficulties in managing a case of os odontoideum. METHODS: Female patient, 12 years old who developed a quadiparesis after minor cervical trauma in October 2005. In the emergency department a congenital cervical anomaly was identified. The patient was placed in a Stryker® frame and, few days later, in a halo bracing. After 3 months, an infection around the pins emerged and the halo vest had to be removed. A severe C1-2 instability persisted and a C1-C2 Gallie procedure was attempted. In the following weeks the bone disappeared and another procedure was attempted in June 2006 - C1 laminectomy and occiput-C3 fusion. In the following months the neurological status of the patient improved and a complete mass of occiput-C3 fusion was observed. RESULTS: We choose a posterior cervical arthrodesis of C1-C2 using the Gallie technique. Since the condition was not resolved we performed a second surgery, C1 laminectomy (determined by SAC of 8, 3 mm in MRI) followed by posterior occiput-C3 fusion. In our case, until now, there is no evidence of axial decompensation, but a more prolonged follow-up is needed. CONCLUSIONS: The treatment of os odontoideum has many considerations but the essential that in the presence of instability and neurological deficit a solid fusion is achieved. In case of failure of posterior atlantoaxial wiring, the occiput-C2 or C3 fusion with rods seems to be an excellent option with a high rate of success, avoiding the need for additional support.
OBJETIVO: Relatar as dificuldades no tratamento de um caso de os odontoideum. MÉTODOS: Paciente do sexo feminino com 12 anos de idade que desenvolveu tetraparesia após trauma cervical discreto em outubro de 2005. No departamento de emergência, constatou-se uma anomalia cervical congênita. A paciente foi colocada em cama Stryker® e alguns dias depois, em colete Halo Vest. Depois de 3 meses, ocorreu infecção em torno dos pinos e foi preciso remover o colete. Houve persistência de instabilidade C1-C2 grave, tentando-se a resolução com a técnica de Gallie C1-C2. Nas semanas seguintes, o enxerto desapareceu e outro procedimento foi tentado em junho de 2006 - laminectomia de C1 e fusão occipital-C3. Nos meses posteriores, o estado neurológico da paciente melhorou e verificou-se massa de fusão completa occipital-C3. RESULTADOS: Escolhemos a artrodese cervical posterior C1-C2 usando a técnica de Gallie. Uma vez que o problema não se resolveu, realizamos uma segunda cirurgia, laminectomia de C1 (determinada por SAC de 8, 3 mm na RM) seguida de fusão occipital-C3. Em nosso caso, até agora, não há evidência de descompensação axial, mas é necessário um período maior de acompanhamento. CONCLUSÕES: o tratamento de os odontoideum tem muitas considerações, mas é essencial, na presença de instabilidade e déficit neurológico, obter fusão sólida. Em caso de falha de amarrilho atlanto-axial posterior, a fusão occipital-C2 ou C3 com hastes parece ser uma opção excelente, com alta taxa de sucesso, evitando a necessidade de suporte complementar.
OBJETIVO: Informe sobre las dificultades para tratar un caso de os odontoideum. MÉTODOS: Paciente, una adolescente, de 12 años de edad, que desarrolló una cuadriparesia después de un traumatismo cervical leve en octubre de 2005. En el Departamento de Primeros Auxilios, se identificó una anomalía cervical congénita. La paciente fue colocada en un marco de Stryker®, unos días después, en un braguero de halo. Cuando transcurrieron 3 meses, surgió una infección alrededor de los pernos y se tuvo que retirar el chaleco de halo. Persistió una grave inestabilidad C1-2 y se intentó un procedimiento de Gallie C1-C2. En las semanas siguientes, el alambre desapareció y, en junio de 2006, se intentó otro procedimiento - laminectomía C1 y fusión de occipucio-C3. En los meses siguientes, el estado neurológico de la paciente mejoró y se observó una masa completa de fusión occipucio-C3. RESULTADOS: Elegimos una artrodesis cervical posterior de C1-C2 usando la técnica de Gallie. Como la condición no se solucionó, realizamos una segunda cirugía, laminectomía C1 (determinada por SAC de 8, 3 mm en Imágenes de Resonancia Magnética), seguida por una fusión posterior occipucio-C3. En nuestro caso, hasta la fecha, no hay evidencia de descompensación axial, pero se necesita de un seguimiento más prolongado. CONCLUSIONES: El tratamiento de os odontoideum tiene muchas consideraciones, pero lo esencial es que, en la presencia de inestabilidad y de déficit neurológico, se obtiene una fusión sólida. En el caso de falla del sistema de alambres posteriores atloidoaxoideos, la fusión occipucio-C2 o C3, con bastoncillos, parece ser una opción excelente que tiene una tasa alta de éxito, evitando la necesidad de apoyo adicional.

INTRODUCTION: Odontoid diameter in some individuals may not be large enough to accommodate two 3.5-mm cortical screws for anterior odontoid fracture fixation. The study was performed to evaluate, in a Brazilian population, the diameter of the odontoid process and the feasibility of using two 3.5-mm cortical screws for anterior odontoid fracture fixation. MATERIALS AND METHODS: Computed tomographic (CT) scans of 88 adult patients (aged 18-78 years) were analyzed; 40 patients (45%) were male (mean age: 43.08 years) and 48 (55%) were female (mean age: 43.39 years). The minimum external and internal anteroposterior and transverse diameters of the odontoid process on sagittal and coronal planes were measured on CT multiplanar reconstructions of the cervical spine. RESULTS: The mean value of the minimum external anteroposterior diameter was 10.83 ± 1.08 and 7.53 ± 1.10 mm for the minimum internal anteroposterior diameter. The mean value of the minimum external transverse diameter was 9.19 ± 0.91 and 6.07 ± 1.08 mm for the minimum internal transverse diameter. The mean AP diameter was significantly larger than the mean transverse diameter; 57 (65%) individuals had the minimum external transverse diameter >9.0 mm that would allow the insertion of two 3.5-mm cortical screws with tapping, and five (6%) individuals had the minimum internal transverse diameter >8.0 mm that would allow the insertion of two 3.5-mm cortical screws without tapping. CONCLUSIONS: The insertion of two 3.5-mm cortical screws was possible for anterior fixation of odontoid fracture in 57 (65%) individuals of our study, and there was no statistical difference between males and females.

OBJETIVO: Este artigo faz a avaliação retrospectiva clínica e radiológica das fraturas do odontóide de 20 pacientes atendidos no IOT-HCFMUSP, durante o período de 2004-2010. MÉTODOS: Foi realizado a estratificação destas fraturas segundo sua classificação (AO/Anderson e D'Alonzo), perfil epidemiológico, tipo de tratamento, tempo de consolidação e complicações. RESULTADOS: Observou-se maior número de casos da fratura do odontóide em pacientes do sexo masculino (4:1), com idade entre 3ª e 4ª década de vida (60 por cento), vítimas de quedas de altura (60 por cento) e acidentes automobilísticos (25 por cento) como principais mecanismos de trauma, e 15 por cento dos casos apresentaram-se com déficit neurológico. A fraturas do odontóide de maior prevalência foram as do tipo II (55 por cento), seguidas pelas fraturas do tipo III (40 por cento). Os tratamentos mais empregados nas fraturas do tipo II e III foram, respectivamente, o cirúrgico (73 por cento) e conservador (87,5 por cento). A consolidação ocorreu em até 16 semanas para 87,5 por cento dos casos de tratamento cirúrgico e para 54,5 por cento dos tratados conservadoramente. Não houve casos de pseudoartrose. CONCLUSÃO: O tratamento cirúrgico das fraturas do odontóide do tipo II apresentou resultados satisfatórios em relação ao tempo consolidação e baixa incidência de complicações, bem como o tratamento conservador destinado às fraturas do tipo III.Nivel de Evidência IV, série de casos.
OBJECTIVE: This article describes a clinical and radiologic retrospective analysis of odontoid fractures in20 patients accompanied byThe IOT-HCFMUSP,from 2004 to 2010. METHODS: These fractures were stratified according to their classification (AO/Anderson andD'Alonzo), epidemiologic profile, type of treatment, time to consolidation of the fracture, and complications. RESULTS: It was observed that there was a higher number of odontoid fractures in males (4:1), between the third and fourth decades of life (60 percent), and that the main causes of the trauma were falling from heights (60 percent) and car accidents (25 percent). Also, 15 percent of the cases presented neurological deficits. The most prevalent type of odontoid fracture was Type II (55 percent) followed by Type III (40 percent). The most prevalent type of treatment used for Type II and III fractures was surgical (73 percent) and non-surgical (87.5 percent), respectively. Consolidation of the fracture took place within 16 weeks in 87.5 percent of surgically treated cases, and in 54.5 percent of those treated non-surgically. No cases of pseudoarthrosis were found. CONCLUSION: The surgical treatment of Type II odontoid fractures showed satisfactory results in relation totime to consolidation of the fracture and low incidence of complications, as did the non-surgical treatment used for the Type III fractures. O objetivo deste trabalho foi realizar um estudo retrospectivo dos casos de subluxação atlantoaxial em cães, por meio de consulta dos registros neurológicos do Hospital Veterinário Universitário (HVU), entre os anos de 2003 e 2008. Foram identificados a raça, o sexo, a idade, a etiologia, os sinais neurológicos, a duração dos sinais clínicos, o tratamento empregado, a resposta ao tratamento, o tempo de recuperação, a recidiva e a relação entre a duração dos sinais clínicos e a recuperação pós-operatória. Foram feitos o diagnóstico de subluxação atlantoaxial em 14 cães, sendo as raças Poodle (35,7 por cento), Pinscher (21,4 por cento) e Yorkshire Terrier (21,4 por cento) as mais acometidas e a maioria (92,8 por cento) com idade inferior a 24 meses. A principal causa da instabilidade foi a agenesia do processo odontoide do áxis (71,4 por cento) e os sinais clínicos variaram desde hiperestesia cervical até tetraparesia não ambulatória. O tratamento predominante foi o cirúrgico, que demonstrou ser eficaz com recuperação satisfatória em 90 por cento dos casos e menor possibilidade de recidiva, quando comparado ao trata,mento clínico. O tempo de recuperação predominante foi de 30-60 dias após a cirurgia, não existindo relação deste com a duração dos sinais clínicos.
A retrospective study on atlantoaxial subluxation in dogs was done by reviewing the cases filed from 2003 to 2008 in the neurological records of the Veterinary Hospital of the Universidade Federal de Santa Maria, at Santa Maria, Rio Grande do Sul, Brazil. The following data were identified: Breed, sex, age, etiology, clinical signs, duration of clinical course, assessment of the therapy employed and its efficacy, response to treatment and relapse. Fourteen dogs were diagnosed as affected by atlantoaxial subluxation and the condition was more frequent in dogs under twenty-four month old years and of toy breeds, such as Poodle (35.7 percent), Pinscher (21.4 percent) and Yorkshire terrier (21.4 percent). The main cause found for the instability was agenesis of the odontoid process. Clinical signs ranged from cranial cervical pain to non-ambulatory tetraparesis. The predominant treatment employed was surgical which demonstrated to be efficacious in 90 percent of the cases with minor risks of relapse when compared with clinical treatment. The predominant time of recovery was 30-60 days after surgery. No correlation was found between the duration of clinical signs before surgery and the time of recovery.

O objetivo deste trabalho foi realizar um estudo retrospectivo dos casos de subluxação atlantoaxial em cães, por meio de consulta dos registros neurológicos do Hospital Veterinário Universitário (HVU), entre os anos de 2003 e 2008. Foram identificados a raça, o sexo, a idade, a etiologia, os sinais neurológicos, a duração dos sinais clínicos, o tratamento empregado, a resposta ao tratamento, o tempo de recuperação, a recidiva e a relação entre a duração dos sinais clínicos e a recuperação pós-operatória. Foram feitos o diagnóstico de subluxação atlantoaxial em 14 cães, sendo as raças Poodle (35,7 por cento), Pinscher (21,4 por cento) e Yorkshire Terrier (21,4 por cento) as mais acometidas e a maioria (92,8 por cento) com idade inferior a 24 meses. A principal causa da instabilidade foi a agenesia do processo odontoide do áxis (71,4 por cento) e os sinais clínicos variaram desde hiperestesia cervical até tetraparesia não ambulatória. O tratamento predominante foi o cirúrgico, que demonstrou ser eficaz com recuperação satisfatória em 90 por cento dos casos e menor possibilidade de recidiva, quando comparado ao trata,mento clínico. O tempo de recuperação predominante foi de 30-60 dias após a cirurgia, não existindo relação deste com a duração dos sinais clínicos.(AU)

Objetivo: Propor uma alternativa cirúrgica ao acesso do terço médio da face incluindo a rinofaringe, seio esfenoidal, fossa pterigopalatina, processo odontóide e clivus. Expor como a técnica apresentada oferece exposição ampla da região e mínimas complicações que são avaliadas neste trabalho. Método: Os registros médicos de doze pacientes submetidos a translocação médio-facial no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto - USP, de 1995 a 2008, foram analisados quanto a doença tratada, localização das lesões, técnica cirúrgica e complicações. Resultados: Seis pacientes eram portadores de cordoma do clivus, neste grupo três pacientes não apresentaram complicações, um desenvolveu ectrópio, um apresentou hipoestesia do nervo infra-orbitário e um evoluiu com insuficiência velo-palatina leve. Um paciente com diagnóstico de estesioneuroblastoma apresentou granulama nasal. Os outros cinco pacientes tinham compressão vértebro-basilar e, no pós-operatório, três deles não apresentaram complicações. Dentre os que apresentaram complicações, um evoluiu com deiscência parcial do palato mole e outro apresentou pequenos granulomas em uma cavidade nasal, paresia do ramo oftálmico e leve insuficiência velo-palatina. Conclusão: A técnica de translocação médio-facial fornece uma ampla abordagem à base do crânio com poucas seqüelas e de baixa morbidade para o paciente no pós-operatório.

The transoral approach provides a safe exposure to lesions in the midline and the ventral side of the craniovertebral junction. The advantages of the transoral approach are 1) the impinging bony pathology and granulation tissue are accessible only via the ventral route; 2) the head is placed in the extended position, thus decreasing the angulation of the brainstem during the surgery; and 3) surgery is done through the avascular median pharyngeal raphe and clivus. We analyzed the clinical effects of odontoidectomy after treating 38 patients with basilar invagination. The anterior transoral operation to treat irreducible ventral compression in patients with basilar invagination was performed in 38 patients. The patientsÆ ages ranged from 34 to 67 years. Fourteen patients had associated Chiari malformation and eight had previously undergone posterior decompressive surgery. The main indication for surgery was significant neurological deterioration. Symptoms and signs included neck pain, myelopathy, lower cranial nerve dysfunction, nystagmus and gait disturbance. Extended exposure was performed in 24 patients. The surgery was beneficial to the majority of patients. There was one death within 10 days of surgery, due to pulmonary embolism. Postoperative complications included two cases of pneumonia, three cases of oronasal fistula with regurgitation and one cerebrospinal fluid leak. In patients with marked ventral compression, the transoral approach provides direct access to the anterior face of the craniovertebral junction and effective means for odontoidectomy.
O acesso transoral é uma via direta e segura às lesões situadas na linha média e na face anterior da junção craniocervical. As vantagens do acesso transoral são as seguintes:1) a compressão óssea e o tecido de granulação localizam-se anteriormente e são accessíveis pela via anterior; 2) a cabeça do paciente é colocada em extensão, diminuindo a angulação do tronco cerebral durante a cirurgia; e 3) a cirurgia é feita através de um plano avascular na linha média faríngea e clivo. Analisamos os resultados obtidos após odontoidectomia por via transoral em 38 pacientes portadores de invaginação basilar. Trinta e oito pacientes com compressão ventral da junção craniocervical foram submetidos a odontoidectomia por via transoral. A idade dos pacientes variou de 34 a 67 anos. Quatorze pacientes apresentavam associação com malformação de Chiari tipo I e 8 já haviam sido submetidos à cirurgia descompressiva por via posterior. A maioria dos pacientes apresentou nucalgia, mielopatia, déficits dos nervos cranianos baixos, nistagmo, e distúrbio da marcha. Em 24 pacientes foi necessário ampliar o acesso transoral através de miotomia do palato mole, ou osteotomia do palato duro ou maxilotomia. A cirurgia proporcionou melhora dos sintomas na maioria dos pacientes. Um paciente faleceu no pós-operatório imediato por causa de embolia pulmonar. Dois pacientes tiveram pneumonia, três apresentaram fístula oronasal com regurgitação, e um teve fístula liquórica. Em pacientes com compressão ventral irredutível da junção craniocervical, a via transoral proporcionou uma abordagem direta e ampla ao processo odontoide.

OBJECTIVE: A surgical variation of the technique of facial translocation procedure is proposed, which has been called midfacial translocation for approach to the entire medial and lateral region of the middle third of the face, including the rhinopharynx, sphenoid sinus, pterygomaxillary fossa, odontoid process, and clivus. PATIENTS AND METHODS: The medical records of five treated patients accordingly were reviewed for an analysis of the surgical technique, the disease, the topography of the lesion, and the complications. RESULTS: The approach permitted ventral decompression of the bulbomedullary junction with resection of the C1 arch and the odontoid process in four patients and resection of a chordoma of the clivus located along the midline and extending intradurally in the fifth patient. Only one patient presented with dehiscence of the posterior half of the soft palate, this being the only complication observed following surgery in these patients. Three months postoperatively, no patient presented any aesthetic alteration of the face. Functionally, there was only infraorbital hypoaesthesia on the side of flap rotation. CONCLUSION: The technique of midfacial translocation provides both good surgical approach and access to the rhinopharynx, pterygomaxillary fossa, high odontoid process and clivus, with few adverse sequelae for the patient.
