RESUMEN

Long noncoding RNAs (lncRNAs) are a category of noncoding RNAs characterized by their length, often exceeding 200 nucleotides. There is a growing body of data that indicate the significant involvement of lncRNAs in a wide range of disorders, including cancer. lncRNA H19 was among the initial lncRNAs to be identified and is transcribed from the H19 gene. The H19 lncRNA exhibits significant upregulation in a diverse range of human malignancies, such as breast, colorectal, pancreatic, glioma, and gastric cancer. Moreover, the overexpression of H19 is frequently associated with a worse prognosis among individuals diagnosed with cancer. H19 has been shown to have a role in facilitating several cellular processes, including cell proliferation, invasion, migration, epithelial-mesenchymal transition, metastasis, and apoptosis. This article summarizes the aberrant upregulation of H19 in human malignancies, indicating promising avenues for future investigations on cancer diagnostics and therapeutic interventions.

Resumo Durante a dentadura mista pode-se observar uma série de distúrbios do desenvolvimento que devem ser tratados a fim de devolver ao paciente uma oclusão normal ou minimizar o tratamento corretivo futuro. A perda precoce de dentes decíduos pode ocasionar alterações significativas na oclusão como a perda de perímetro do arco, o que prejudica a irrupção fisiológica dos dentes permanentes. O objetivo deste trabalho é apresentar o manejo de espaços durante a dentadura mista por meio de alinhadores ortodônticos, no qual foi diagnosticada a perda prematura de um dente decíduo no período intertransitório da dentadura mista. O tratamento, realizado com alinhadores ortodônticos, foi planejado para: verticalização do primeiro molar permanente, recuperação do espaço reduzido devido à perda do segundo molar decíduo, expansão sequencial dos arcos superior e inferior, correção da mordida profunda e alinhamento e nivelamento dos incisivos superiores e inferiores. Para obtenção dos resultados propostos, foram utilizadas 3 sequências de alinhadores. Os resultados demonstram que os alinhadores ortodônticos representam uma alternativa viável para a intervenção oportuna em pacientes com perda prematura de dentes decíduos e redução de espaço para dentes permanentes. Ainda, permitem a correção simultânea de outras alterações oclusais, o que confere eficiência ao tratamento interceptor (AU)


Abstract During the mixed dentition, a series of developmental disorders can be observed that must be treated to return the patient to a normal occlusion or to minimize future comprehensive treatment. Early loss of deciduous teeth can cause significant changes in occlusion, such as loss of arch perimeter, which impairs the physiological eruption of permanent teeth. This clinical report aims at presenting the management of spaces during the mixed dentition by means of orthodontic aligners, in which the premature loss of a deciduous tooth was diagnosed in the inter-transitional phase of the mixed dentition. The treatment, carried out with orthodontic aligners, was planned for: verticalization of the first permanent molar, recovery of the reduced space due to the loss of the second deciduous molar, sequential expansion of the upper and lower arches, correction of the deep bite and alignment and leveling of the upper and lower incisors. To obtain the proposed results, 3 sequences of aligners were used. The results demonstrated that orthodontic aligners represent an alternative for early intervention in patients with premature loss of deciduous teeth and reduced space for permanent teeth. Furthermore, they allow the simultaneous correction of other occlusal alterations, which makes the early treatment efficient. (AU)

Abstract: It is difficult how to manage acute undifferentiated leukemia in daily practice, but generally, hand mirror morphology provides ease to treat these patients. Thirty-nine years old male patient was admitted to with the complaints of echymosis, and pain at his left buttock due to an intramuscular injection for the treatment of previously diagnosed of the lower respiratory infection. Peripheral blood smear revealed >%50 blasts cells with a moderate nuclear: cytoplasmic ratio and one or more nucleoli. The blast cells showed a hand-mirror morphology and not harboring auer rods. According to the flow cytometric analysis the blastic cells do not represent to be originated from myeloid or lymphoid origin, because the cells harboring both of two cell lineages. AML-like therapy was commenced based on the positive myeloid markers including CD117 and CD135. Even though hand mirror morphology of the blasts usually demonstrates the lymphoid origin and the patients are treated as ALL like therapy, myeloid blasts rarely represents the same morphology, as was in our patient.

BACKGROUND: Oligo-astheno-teratozoospermia is frequently reported in men from infertile couples. Its etiology remains, in the majority of cases, unknown with a variety of factors to contribute to its pathogenesis. The aim of this European Academy of Andrology guideline was to provide an overview of these factors and to discuss available management options. MATERIALS AND METHODS: PubMed was searched for papers in English for articles with search terms: male infertility and oligo-astheno-teratozoospermia. For evidence-based recommendations, the GRADE system was applied. Issues related to urogenital infections/inflammations have not been included in this document as they will be covered by separate guidelines. RESULTS: For men with oligo-astheno-teratozoospermia, the European Academy of Andrology recommends: A general physical examination to assess signs of hypogonadism. A scrotal physical examination to assess (i) the testes and epididymes for volume and consistency, (ii) deferent ducts for total or partial absence, and (iii) occurrence of varicocoele. Performing two semen analyses, according to World Health Organization guidelines to define an oligo-astheno-teratozoospermia. An endocrine evaluation. A scrotal ultrasound as part of routine investigation. Karyotype analysis and assessment of Yq microdeletions in infertile men with a sperm concentration ≤5 × 106 /mL. Cystic fibrosis transmembrane conductance regulator gene evaluation in case of suspicion for incomplete congenital obstruction of the genital tract. Against quitting physical activity to improve the chance of achieving pregnancy. Against androgen replacement therapy to improve the chance of achieving pregnancy. Assisted reproduction techniques to improve the chance of achieving pregnancy, in case other treatment options are not available or not efficient. Androgen replacement therapy in patients with biochemical/clinical signs of hypogonadism, after completion of the fertility treatment. CONCLUSION: These guidelines can be applied in clinical work and indicate future research needs.

RESUMEN

The rates of breast cancer mortality remain high in Brazil. Docetaxel is a semi-synthetic taxane used to treat various tumors, particularly tumors of the breast, lung and prostate. In this study ADR that occurred in 45 docetaxel users with breast cancer were surveyed. They were identified by type, causality (Naranjo algorithm and World Health Organization categories) and, if considered probable or defined, rated for severity according to SOBRAFO proposal (2007). A total of 325 ADR were observed: 165 in the first, 137 in the second and 23 in the third cycle. Fifty seven ADR were immediate and the others, late. Fatigue and exhaustion for more than five days, classified as Grade 3 by SOBRAFO (2007), were reported as the primary late RAM. There was no significant difference in the occurrence of immediate and late ADR between cycles (p=1 and p=0.3577, respectively). The presence of a pharmacist gave the patients a better understanding of the occurrence of RAM, especially those that occur outside the hospital, between chemotherapy cycles and are often not reported to the healthcare team, creating institutional demands and reaching the goal to track, observe and correlate the RAM for each user.


As taxas de mortalidade por câncer de mama no Brasil permanecem altas. O docetaxel é um taxano semi-sintético usado para tratar vários tumores, particularmente tumores da mama, pulmão e próstata. Neste estudo, as Reações Adversas (RAM) ocorridas em 45 pacientes com câncer de mama foram monitoradas. Elas foram classificadas pelo tipo e causalidade (Algoritmo de Naranjo e categorias propostas pela Organização Mundial da Saúde) e, se consideradas prováveis ou definidas, foram classificadas também pela severidade, de acordo com a proposta da SOBRAFO (2007). Um total de 325 RAM foram observadas: 165 no primeiro, 137 no segundo e 23 no terceiro ciclo. Cinquenta e sete RAM foram imediatas e as demais tardias. Fadiga e exaustão por mais de 5 dias, classificadas como grau 3 pela SOBRAFO (2007), foram as principais RAM encontradas. Não houve diferença significativa na ocorrência de RAM imediatas ou tardias entre os ciclos (p=1 e p=0,3577, respectivamente). A presença de um farmacêutico proporcionou aos pacientes um melhor entendimento sobre a ocorrência de RAM, especialmente sobre aquelas que ocorrem fora do ambiente hospitalar, entre os ciclos da terapia, não sendo usualmente relatadas aos profissionais de saúde. Isto gerou uma demanda na instituição e permitiu alcançar a meta de acompanhar, observar e correlacionar as RAM de cada paciente.
