RESUMEN

An ankle dislocation without an accompanying fracture is extremely rare, and an open ankle dislocation is even rarer. Due to its rarity, there is no consensus on the optimal treatment strategy. A professional basketball player (a 28-year-old male) incurred an open ankle dislocation (with no accompanying fracture) during a basketball game due to plantar flexion and inversion of his ankle during the transition from dashing to stop motion. The same day, an emergency reduction under spinal anesthesia was performed with primary closure of the wound. Considering the complications of infection and decreased ankle range of motion (ROM), primary ligament repair was not performed. He was treated conservatively with cast immobilization for four weeks, and early weight-bearing and ROM exercises were initiated. At six weeks postoperatively, stress radiography did not reveal ankle instability. After three months of conservative treatment, the patient was able to play basketball at his previous performance level. Four weeks of cast immobilization without ligament repair plus early rehabilitation with weight-bearing and ROM exercises allowed for an early return without complications. Even in high-level athletes, open ankle dislocation without an accompanying fracture can be treated adequately with conservative therapy.

Within the National Basketball Association (NBA), players and teams maintain that having healthy players sit out some games during the regular season may help them be more productive during the post-season. This decision to not play in order to rest the player, aptly noted as a DNP-Rest decision on injury reports, is in line with team and player goals, and fits with a growing body of evidence in support of the power of rest for health and performance. However, these practices conflict with some goals of the league, which has a vested interest in having the top talent play to attract broadcasters, advertisers, live spectators, and thus, enhance viewership. The current study is among the first to test the theory that strategically resting healthy players during the regular season results in better performance, as indicated by Player Efficiency Rating (PER) and Win Shares, during the post-season. Utilizing data from the 2016-17 through the 2020-21 NBA seasons, there was not sufficient evidence to suggest that resting more games during the regular season results in better performance in the post-season. Findings from a nested case-control study of 184 players (92 cases; 92 controls) also showed no differences in the change in performance from regular to post-season between cases of players who received rest during the regular season and matched controls. Although the restorative effects of rest might be considerable in the short term, the current study provides additional evidence to suggest that the impact may not carry over into the post-season.

OBJECTIVE: Recognizing the high stakes associated with winning and losing in the National Basketball Association (NBA), a deep understanding of the underlying mechanisms of NBA game-play performance would provide substantial benefit to all stakeholders involved with preparing NBA players and teams for competitive success. To the best of the authors' knowledge, this systematic review presents the first attempt to systematically amalgamate and appraise the scientific literature published in the XXI Century, following a constraints-led approach (CLA). In particular, two underpinning factors of NBA game-play performance were investigated: (1) NBA player constraints (internal variables) and (2) NBA contextual constraints (external variables). METHODS: Databases included PubMed (MEDLINE), Web of Science (WOS), ResearchGate, SPORTDiscus, SCOPUS, Google Scholar, and the World Association of Basketball Coaches' database (WABC). This review followed the Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) model and the Population, Intervention, Comparison and Outcomes (PICOS) guidelines. RESULTS: Ultimately, 43 articles met the inclusion criteria (n = 43). Promisingly, the vast majority of studies were published in recent years (>2016; n = 28; 65.1%). Topics related to 'contextual constraints' (n = 25; 58.1%) received more attention than topics related to 'player constraints' (n = 18; 41.9%). Even though the importance of longitudinal-interventional approaches to applied sports science is well-documented, descriptive-observational research emerged as the most popular method of choice (n = 27; 62.8%); interventional studies were absent; and near all researchers merely utilized secondary data sources (n = 37; 86.0%). CONCLUSIONS: Taking into account the total body of evidence (2001-2020), NBA practitioners may use this systematic review as a baseline reference to enrich their current knowledge about the nature, demands, and dynamics of the modern-day NBA ecosystem. Finally, adoption of an 'Applied Science Research Framework' is encouraged, fostering clearly outlined project incentives; standardizing taxonomies; sequencing follow-up studies; embracing holistic and cross-disciplinary viewpoints; and integrating longitudinal-interventional projects to increase the reproducibility of their findings.

BACKGROUND: Numerous studies have reported the incidence and outcomes of injuries in the men's and women's National Basketball Association (NBA and WNBA, respectively). PURPOSE: To synthesize published data regarding the incidence and outcomes of all injuries in the NBA and WNBA in a comprehensive review. STUDY DESIGN: Systematic review; Level of evidence, 4. METHODS: Following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, we searched 3 electronic databases (PubMed, MEDLINE, Embase) for studies of all levels of evidence since 1990 pertaining to injuries sustained by active players in the NBA and WNBA. Studies were excluded if the cohort of interest included ≤3 active players. RESULTS: The initial search of the 3 databases yielded 1253 unique studies, of which 49 met final inclusion criteria for this review. Only 4 studies included athletes in the WNBA. Based on the mean annual incidence, the 5 most common orthopaedic sports injuries sustained in the NBA were concussions (9.5-14.9 per year), fractures of the hand (3.5-5.5 per year), lower extremity stress fractures (4.8 per year), meniscal tears (2.3-3.3 per year), and anterior cruciate ligament tears (1.5-2.6 per year). Cartilage defects treated using microfracture, Achilles tendon ruptures, and anterior cruciate ligament injuries were 3 injuries that led to significant reductions in performance measurements after injury. CONCLUSION: With advances in sports technology and statistical analysis, there is rapidly growing interest in injuries among professional basketball athletes. High-quality prospective studies are needed to understand the prevalence and effect of injuries on player performance and career length. This information can inform preventative and treatment measures taken by health care providers to protect players and guide safe return to play at a high level.

Background and objectives: Striking a balance between maximizing performance and preventing injury remains elusive in many professional sports. The purpose of this study was to assess the relative risk of non-contact injuries in professional basketball players based on predictive cut scores on the Functional Movement Screen™ (FMS). Materials and Methods: Thirty-two professional basketball players from the National Basketball Association (NBA) and Women's National Basketball Association (WNBA) participated in this study. This observational pilot cohort study assessed and scored each participant using the FMS during training camp. Each athlete was then tracked throughout the season while recording the number, type, and time lost due to injuries. Possible exposures, actual exposures, and exposures missed due to non-contact injury (NCI) for each athlete were calculated and then used to determine the crude and specific incident rates for exposures missed due to NCI per 1000 exposures. Results: Linear regression models were used to evaluate the predictive ability of the FMS score for total missed exposures, NCI, and CI missed exposures. In all models, the FMS total score failed to attain significance as a predictor (p > 0.05). FMS scores ranged from 5 to 18. The recommended cut score of 14 showed a sensitivity of 0.474 and a specificity of 0.750. The cut score of 15 showed the best combination, exhibiting a sensitivity of 0.579 and specificity of 0.625. A total of 5784 exposures to NCI were possible for the men and women combined, and 681 possible exposures were missed. Of these, 23.5% were due to NCI, 16.5% were due to contact injuries (CI), and 60% were due to illnesses and personal reasons. Conclusions: The FMS proved to be a measure that was not associated with any injury measure in this sample of professional basketball players, suggesting the instrument lacks predictive validity in this population.

HYPOTHESIS: We hypothesized that players in the National Basketball Association (NBA) who sustained a shoulder destabilizing injury could return to play (RTP) successfully at a high rate regardless of treatment type. METHODS: We used publicly available data to identify and evaluate 50 players who sustained an in-season shoulder instability event (subluxation/dislocation) while playing in the NBA. Demographic variables, return to NBA gameplay, incidence of surgery, time to RTP, recurrent instability events, and player efficiency rating (PER) were collected. Overall RTP was determined, and players were compared by type of injury and mode of treatment. RESULTS: All players (50/50) returned to game play after sustaining a shoulder instability event. In those treated nonoperatively, athletes who sustained shoulder subluxations returned after an average of 3.6 weeks, compared with 7.6 weeks in those who sustained a shoulder dislocation (P = .037). Players who underwent operative management returned after an average of 19 weeks. Athletes treated operatively were found to have a longer time interval between a recurrent instability event (70 weeks vs. 28.5 weeks, P = .001). CONCLUSION: We found 100% rate of RTP after a shoulder instability event in an NBA athlete. Players who experience shoulder dislocations were found to miss more time before RTP and were more likely to undergo surgical intervention compared with those who experienced a subluxation. Surgical repair maintained a longer interval between recurrent instability. Future investigations should aim to evaluate outcomes based on surgical procedures and identify possible risk factors predictive of recurrent instability or failure to RTP.

BACKGROUND: Vitamin D is believed to play a role in influencing fracture risk and athletic performance. Insufficiency of vitamin D affects an estimated three-quarters of the United States population. Hypovitaminosis D has also been demonstrated to be quite common among professional basketball players in the National Basketball Association (NBA). PURPOSE: To determine whether a relationship exists between vitamin D levels and fracture risk and athletic performance (as measured by NBA draft status) among elite basketball players. STUDY DESIGN: Descriptive epidemiology study. METHODS: Data were obtained from the NBA regarding combine participants from 2009 through 2013. This information included vitamin D level, demographic information, fracture history, and NBA draft status. The data were analyzed to determine associations between vitamin D level and fracture risk and NBA draft status. RESULTS: Vitamin D levels were measured for 279 players at the NBA Combine from 2009 through 2013. Vitamin D deficiency (<20 ng/mL) was seen in 32.3% of athletes, vitamin D insufficiency (20-30 ng/mL) was seen in 41.2%, and sufficient levels of vitamin D (>30 ng/mL) were present in only 26.5%. A total of 118 players had a history of at least 1 fracture. Vitamin D level was not predictive of fracture risk. Contrary to our hypothesis, players with a history of stress fracture had a significantly greater mean vitamin D level than those without such history (30.7 vs 25.1 ng/mL; P = .04). A majority (79.6%) of participants were selected in the NBA draft. Players with deficient vitamin D levels had a significantly lower rate of being drafted into the NBA (P = .027). The NBA draft rate was found to increase with increasing levels of vitamin D (P = .007). CONCLUSION: Hypovitaminosis D is quite common among NBA Combine participants, affecting 73.5%. While no significant relationship was found between vitamin D level and fracture history, patients with a history of stress fracture had significantly greater mean vitamin D levels. Additionally, participants with greater vitamin D levels were more likely to be drafted into the NBA. This information supports the potential role of vitamin D in influencing athletic performance.
