RESUMEN

Background: Obtaining consent from potential donor families is a challenging step in the donation process and is influenced by various factors. Methods: In this cross-sectional study, we utilized a questionnaire containing 14 questions about facilitators and barriers in the family interview process. The questionnaire was distributed in March 2023 to intensive care unit (ICU) nurses who had experience with donor family interviews. We collected the opinions of these respondents on hospital performance and drew comparisons between the studied hospitals. Results: A total of 60 participating ICU nurses provided mean scores for hospital performance in family interviews of 2.60±0.84 for type I hospitals (those providing neurosurgery and trauma care) and 2.035±0.890 for type II hospitals (those without neurosurgery and trauma services; P=0.04). The mean scores for public and private hospitals were 1.86±0.86 and 2.59±0.85, respectively (P=0.008). Based on the findings, the most important facilitators were the availability of organ donation staff and access to a professional team for family discussions. Conversely, poor physician communication skills and limited communication capabilities among medical staff were identified as significant barriers. Implementation of a professional team for family interviews was found to be more critical for type II hospitals. Poor physician communication skills were a significant concern in public hospitals, while families' lack of awareness of patient prognosis emerged as a key barrier in private hospitals. Conclusions: This study highlights numerous facilitators and barriers that vary across hospitals. Addressing these issues individually and developing tailored plans to enhance hospital performance in interviewing donor families is essential.

RESUMEN

The German post-mortem organ donation rate has been low, since transplantation scandals attracted media attention in 2012. Therefore, the objective of this article is to address the willingness to document attitudes (e.g., organ donor card) towards organ donation, factors influencing the willingness to document the individual will and knowledge of organ donation among the German public. Data of surveys on attitudes towards organ and tissue donation in Germany conducted between 2010 and 2016 by the Federal Centre for Health Education (Bundeszentrale für gesundheitliche Aufklärung) were analysed. Using binary logistic regression, factors influencing the method for documenting the individual will were identified. In the 2016 survey, 35.7% of all respondents had documented their will in the form of an organ donor card or an advance health care directive. Female gender, younger age, higher level of education, discussion of organ donation with next-of-kin/friends, grasping the concept of death and knowledge of organ donation increase the likelihood of possessing an organ donor card. Being male increases the likelihood of documenting the individual will using an electronic health insurance card. Older respondents or respondents with lower level of education lack knowledge about organ donation. Persons with extensive knowledge are more likely to possess an organ donor card and to consent to donate organs of next-of-kin. Further measures to increase knowledge of organ donation among the general public should be discussed. The influence of gender on the willingness to use different methods to document attitudes needs to be evaluated.

RESUMEN

BACKGROUND: Medicines are an integral part of the health system and the accessibility hugely depends on affordability and availability of medicines. Oral health is an integral component of overall well-being but is ironically often one of the most neglected areas of healthcare. There is paucity of data on the pharmaceutical cost components of dental disease, so this study aims to address this lacuna on the pricing of medicines. OBJECTIVE: This research study can be considered a partial economic evaluation that focuses on an estimation of costs associated with medicines used in the treatment of selected oral diseases to suggest policy measures to reduce out-of-pocket expenditure on medicines. METHODS: Using cost analysis research design on the PharmaTrac dataset and cross-comparing it with public procurement rates of the Rajasthan Medical Services Corporation (RMSC) indicates that the pharmaceutical cost of treating caries and periodontitis is approximately three times higher if median retail prices are considered instead of RMSC public procurement costs. RESULTS: Medicine cost of treating a single episode of caries or periodontitis using RMSC medicine rates at all levels of care is approximately 1.9-2.5 times lower than retail prices of the same formulations. CONCLUSION: Our results strongly suggest that centralized public procurement of dental medicines with an efficient monitoring and control can reduce the out-of-pocket expenditure.

RESUMEN

Objective: To summarize the typical experiences and practices based on the quality evaluation of medical consumables in foreign countries and the practice of drug bidding in China, and to explore the establishment of a bidding grouping scheme for medical consumables in China. Methods: Using literature analysis and evaluation method of combing foreign countries typical measures of medical consumables quality assessment, this paper summa-rized the construction of index system of bidding in China Pharmaceutical Group, and used simple statistical data to reflect the present policy of China's medical supplies such as lack of relevant policies, different levels of quality etc.. Results:There are many problems in the quality evaluation of medical consumables in China. However, the typical measures of extraterritorial quality evaluation and the practice of pharmaceutical bidding in China provide a solid theo-retical and practical basis for the establishment of medical consumable supplies bidding grouping. Conclusion:Estab-lishing bidding grouping scheme is necessary in the Chinese Medical supplies context. Based on a clear difference be-tween medical supplies and medicines, and according to the"three stage"our conceptualization gradually improved. On the basis of the existing data and practical situation, the establishment of scientific selection index bidding group division standard, has been in the exploration of the formation of China's medical supplies bidding grouping scheme.

RESUMEN

The purpose of this study was to standardize ingredients used by school foodservices. This study analyzed the current notation of ingredients in used by used in school foodservices through the NEIS system employed by school foodservices of elementary schools through high schools in South Korea. Specifically, this study suggests systemized standardization of ingredient classification and quality attributes of at school foodservices by applying a case study analysis. The findings from the case analysis of the Electronic Procurement System operator are as follows. Classifications for ingredients of the NEIS system used by school food services consisted of included food group, food name, detailed food name, and description. Classification was not clearly divided between the classification scheme and the attribute system. Therefore, food group, food name, and product information of each food should be categorized as the classification scheme, whereas the detailed food name (excluding product information) and description should be standardized as the attribute system, which is composed of required attributes, recommended attributes, and other attributes. This study suggests that system standardization should be carried out in the field of school foodservices, as advancements between distributors and school food service providers could affect food ingredient quality. Thus, standardization can influence purchase and distribution in many ways.
