RESUMEN
OBJECTIVE: To map and summarize the current scientific evidence concerning the active ingredients, effectiveness, and adverse effects of over-the-counter (OTC) bleaching products. DATA AND SOURCE: This study was conducted according to the PRISMA-ScR guidelines for scoping reviews and registered on the Open Science Framework platform. STUDY SELECTION: Database searches were conducted in PubMed/MEDLINE, Embase, and Scopus up to January 2024. All in vitro, in situ, and clinical studies evaluating the effectiveness and adverse effects of OTC bleaching products were included. A descriptive analysis of the included studies was performed. RESULTS: A total of 88 studies were included. Most of them were in vitro studies (n = 49), followed by randomized clinical trials (n = 28). The main OTC bleaching products identified were whitening or stain-removing toothpastes (n = 42), followed by whitening strips (n = 39). Most clinical studies indicate that whitening strips are effective in improving tooth color and providing whitening benefits. In contrast, the bleaching effectiveness of toothpastes, mouth rinses and whitening trays was mainly supported by in vitro studies. The main adverse effects associated with OTC bleaching agents were tooth sensitivity and gingival irritation. CONCLUSION: A wide variety of OTC bleaching products is available for consumer self-administered use. Clinical studies have mainly confirmed the bleaching effectiveness of whitening strips, while the validation for toothpastes, mouth rinses and whitening trays has mainly relied on in vitro studies. Nevertheless, the use of OTC bleaching products may result in adverse effects, including tooth sensitivity, gingival irritation, and enamel surface changes. CLINICAL SIGNIFICANCE: Some over-the-counter bleaching products may have whitening properties supported by clinical studies, particularly those containing hydrogen or carbamide peroxide. Nonetheless, clinicians must be aware of the potential risks associated with excessive self-administration of these products, which may result in adverse effects.

A indústria farmacêutica tem encontrado novas técnicas para promover os medicamentos isentos de prescrição médica junto do público geral. Neste contexto, a publicidade testemunhal permite incrementar a credibilidade da mensagem, ficando o seu enquadramento dependente da categoria do emissor nos ordenamentos jurídicos português e brasileiro. Adotando uma perspectiva de direito comparado, o presente trabalho visou analisar a publicidade testemunhal, com especial enfoque nos depoimentos veiculados por influenciadores digitais que recomendam o consumo de medicamentos. Trata-se de um estudo qualitativo, assente no levantamento bibliográfico e legislativo, bem como na análise detalhada de decisões dos organismos de autorregulação competentes, tanto em Portugal como no Brasil. Concluiu-se que alguns influenciadores digitais podem ser considerados pessoas famosas, e não meros consumidores, o que determina a ilicitude da publicidade difundida, atenta a necessidade de acautelar a objetividade e o uso racional dos medicamentos. No entanto, resultou das decisões analisadas uma abordagem tradicional destas novas técnicas de promoção de medicamentos, centrada ainda nas menções obrigatórias e nos princípios gerais vigentes em matéria de publicidade.
The pharmaceutical industry has found new techniques to promote over-the-counter drugs to the public. In this context, testimonial advertising makes it possible to increase the credibility of the message, its framework being dependent on the category of the sender, in the Portuguese and Brazilian legal systems. Adopting a comparative law perspective, the present work aimed at analysing testimonial advertising, with a special focus on testimonies given by digital influencers who recommend the consumption of drugs. It is a qualitative study, based on a bibliographic and legislative survey, as well as on a detailed analysis of decisions by competent self-regulatory bodies, both in Portugal and Brazil. It is concluded that some digital influencers can be considered famous people and not just consumers, which determines the unlawfulness of widespread advertising, considering the need to safeguard objectivity and the rational use of drugs. However, a traditional approach to these new techniques of drugs promotion, still focused on mandatory mentions and the general principles of advertising, resulted from the decisions analyzed.

Although restricting over-the-counter (OTC) antimicrobial drug sales is recommended globally, no data track its effect on antimicrobial resistance (AMR) in bacteria. We evaluated the effect of a national policy restricting OTC antimicrobial sales, put in place in November 2010, on AMR in a metropolitan region of São Paulo, Brazil. We reviewed associations between antimicrobial sales from private pharmacies and AMR in 404,558 Escherichia coli and 5,797 Streptococcus pneumoniae isolates using a dynamic regression model based on a Bayesian approach. After policy implementation, a substantial drop in AMR in both bacterial species followed decreased amoxicillin and trimethoprim/sulfamethoxazole sales. Conversely, increased ciprofloxacin sales were associated with increased ciprofloxacin resistance, and extended spectrum ß-lactamases-positive E. coli isolates and azithromycin sales increases after 2013 were associated with increased erythromycin resistance in S. pneumoniae isolates. These findings suggest that restricting OTC antimicrobial sales may influence patterns of AMR, but multifaceted approaches are needed to avoid unintended consequences.

OBJECTIVE: To determine the effect and safety of IV lipid emulsion in rabbits with acute ivermectin toxicosis. DESIGN: Randomized controlled trial. SETTING: University research facility. ANIMALS: Twenty-four healthy male adult New Zealand rabbits. INTERVENTIONS: Three groups of rabbits (IV, IV_RL, and IV_LE) received 80 mg/kg of ivermectin (8 mL/kg) through a nasogastric tube, and 1 group (LE) received an equivalent volume (8 mL/kg) of 0.9% sodium chloride. Group IV_RL was treated with Ringer's lactate (2 mL/kg bolus, followed by 0.25 mL/kg/min for 60 minutes), whereas groups IV_LE and LE received 20% lipid emulsion. The rabbits were submitted to clinical and neurological evaluation, and blood samples were collected for biochemical analysis. All animals were euthanized, and tissue samples were collected and processed for histopathological evaluation and ivermectin quantification. MEASUREMENTS AND MAIN RESULTS: All animals exposed to ivermectin manifested clinical changes consistent with toxicosis, but the ones that received IV lipid emulsion infusion showed no significant clinical improvement. Intense increase in serum glucose and triglyceride concentrations was seen after ivermectin exposure, along with increased urea and creatinine concentrations, but the last 2 remained within the reference range. Lipid emulsion caused an intense increase in triglycerides and cholesterol concentrations. No pathological abnormalities were seen in the organs sampled. Toxicological analysis showed greater ivermectin concentration in adipose tissue and liver, followed by kidney and, finally, brain. The treatments did not change ivermectin tissue concentration. CONCLUSIONS: When given to rabbits intoxicated with ivermectin, IV lipid emulsion was biochemically and histologically safe but was not effective in treating, delaying, or reversing clinical signs and progression, nor did it alter ivermectin tissue concentration.

BACKGROUND: Acetaminophen (Paracetamol) is one of the most used and prescribed anti-inflammatory and analgesic drugs worldwide. It has become one of the main drugs related to accidental and intentional overdoses in many countries, including Chile. The objective of this work was to characterize acetaminophen poisonings occurred in Chile between the years 2001 and 2020. METHODS: A retrospective study of acetaminophen poisonings among patients who were hospitalized in Chilean public and private hospitals was carried out between the years 2001 and 2020. Data was obtained from the Medical Outcome Statistical Report database. Inclusion criteria were cases of patients who were admitted into either public or private healthcare settings with diagnosis of acetaminophen poisoning according to the WHO ICD-10. Statistical analyses were run to establish associations between variables selected in the study. RESULTS: A total of 2,929 cases were included in the study. 77 % of the cases corresponded to female patients (p<0.05). Patients' age range went from 0 to 81 years old. Adolescents and young adults resulted more involved in reported cases during the 2001-2020 period (p<0.001). During the first period of the study, accidental poisonings were more commonly reported, however in the recent years intentional cases increased their occurrence, especially among female patients (p<0.05). A multivariate logistic regression model considered as statistically significant (p<0.05) the interaction between the variables age, gender and year of the event. CONCLUSION: The present study identified a large number of acetaminophen poisonings reported in Chile during the 2001-2020 period. Cases were characterized including patients' gender, age, and poisoning intent. Health authorities should consider these findings as an opportunity to improve public health associated with the use and misuse of over-the-counter drugs, including acetaminophen.
RESUMEN
This article presents the prevalence of stimulant doping among Brazilian athletes, the analytical approaches used, as well as a general evolution of the detectability of the stimulants being used. Results from the Brazilian accredited doping control laboratory are compared with the global statistics disclosed by the World Anti-Doping Agency. The high prevalence of stimulant doping in Brazil can be attributed to several reasons, including "self-administration," a "body-shaping" culture, and the use of nutritional supplements.

Al suponer que ha existido confusión con la elección de un medicamento de venta libre, este estudio analiza de qué manera la emoción de frustración que expresa un consumidor da lugar a intenciones conductuales particulares. La actitud de un consumidor ante la confusión es la dinámica entre sus emociones y las conductas que este anticipa (i.e., cambio del producto, búsqueda de información e intención de consumo). Esta dinámica es analizada en una situación experimental simulando una decisión errónea. Los resultados demuestran que a) la emoción ante la confusión explica la intención de cambio y que b) la intención de cambio a su vez está asociada con un menor interés en consumir un producto equivocado y con una mayor intención de obtener información.
Because of consumers' confusion with an over the counter drug, feelings of frustration lead an individual to particular behavioral intentions. Consumers' attitude towards confusion is the relationship between their emotions and behavioral intentions (i.e., changing the product, intention to seek for information, and intention to consume). This relationship is analyzed in an experimental setting simulating a wrong decision. Results demonstrate that a) emotions due to confusion explain higher intentions to exchange a product and that b) this is then associated with lower intentions of taking a medicine and higher intentions of gathering information.

Tooth discoloration is commonly found in the dental clinic and tooth bleaching has been considered the preferred esthetic alternative, being more conservative, safe and with predictable results. Supervised home-use of 10 percent Carbamide Peroxide (CP) bleaching with custom-trays is the most common bleaching procedure dispensed by dentists to their patients. The good results obtained with this technique stimulated the flourishing of new products and techniques. Over-the-counter (OTC) bleaching products appeared as a low-cost alternative to bleach discolored teeth without dentist supervision. Different OTC products are available in supermarkets, drug stores or on the Internet, including rinses, paint-on brushes, toothpastes, chewing guns, dental floss, and whitening strips. There is lack of clinical evidence regarding the safety and effectiveness of these products, being most of the studies supported by the manufacturers'. Basically, toothpastes, chewing gums, and dental floss are removal agents of superficial stains. Rinses and paint-on brushes with low levels of hydrogen peroxide have some whitening effect, but without clinical relevance. Strips present similar esthetic results and side-effects, compared to bleaching with 10 percent CP using trays; however, the studies have financial support from the manufacturers and were based on short term evaluations. Legislation varies widely in different countries regarding OTC dental bleaching. Concerns have appeared due to the potential abusive use of these self-medication agents, especially in young patients, with potential harmful results. Dentists should be acquainted with this kind of products to be able to inform their patients. In conclusion, there is a need for independent clinical trials to provide sufficient evidence regarding the use of OTC bleaching products.
