RESUMEN

Randomized controlled trials have reported psychoanalytic psychotherapy to improve longer-term post-treatment outcomes in patients with treatment-resistant depression. In this case study, we examine the therapy process of a female trial participant diagnosed with treatment-resistant depression. Structured clinical assessments indicated that the patient's level of depression remained unchanged during and after treatment. Over the course of the therapy, she repeatedly broke away from important others and finally also from the therapy itself, which we linked to the impact of earlier experiences of abandonment on her internal world. In the discussion, we present a variety of reflections that were put forward by the authors during a series of case discussion meetings. Some of these reflections relate to how the inner world of this patient might have triggered a negative therapeutic reaction and a destructive pattern of repetition. The interpretative stance, in which the therapist interpreted this reaction as indicative of a psychic conflict and linked this conflict to the therapeutic relationship, seemed to be experienced by the patient as unhelpful and persecutory. Other elements that were brought up include basic distrust, lack of symbolization and trauma in the patient, as well as the constraints of the research context.

RESUMEN

Freud's publication of The Ego and the Id sparked a diverging set of psychoanalytic models - ego psychology, structural conflict theory, Kleinianism, object relations theories, Lacanianism, etc. - each of which attempted to deal with the clinical limitations of his first topography in regard to unconscious guilt, negative therapeutic reactions and primitive character organizations. This paper attempts to look back on these developments from the perspective of contemporary, post-Freudian psychoanalytic theories.

RESUMEN

This paper examines two major issues related to the concept of identity. The first of these concerns the place of this concept in psychoanalytic theory and practice, particularly taking note of its limited presence in the psychoanalytic literature of the British School of psychoanalysis. My argument is that the concept and phenomena of identification has been preferred to that of identity in the discourse of British Object Relations and considers why that might be the case. The second issue concerns the salience of the concept of identity in contemporary political and cultural debate, as this has come to denote differences of a socially-constructed kind such as those of race, gender, ethnicity, and religion. In this context, the idea of identity has become an important point of reference in much recent psychoanalytic thinking. The significance of this development will be considered in its relevance for psychoanalytic and wider social practices.

The present psychobiography took up the three main psychoanalytic conceptions of love to illuminate the psychological development of famed experimental writer William Seward Burroughs (1914-1997). The study found evidence of all three concepts of love in the subject's life strategies: (1) Love as cathexis was present in Burroughs' fascination with centipedes and other vermin that appeared in his dreams and which symbolised, in part, his terror over early childhood traumas as well as his concomitant struggle to integrate sex with intimacy. (2) Love as eroto-philiac fusion was observed in Burroughs' unstable and even exploitative relationships with others. This tendency was most salient in Burroughs' abortive attempts to seduce straight men, as well as in his failed efforts to be a traditional husband and father. (3) Reparative love became the subject's primary mode of interaction late in life. The study showed that, in his declining years, Burroughs was able to overcome partially the maladaptive strategies of his early life through his numerous pet cats, upon whom he projected aspects of his past romantic partners and friends.

RESUMEN

Contemporary models of the diagnosis and classification of personality disorders have diverged from the categorical diagnostic framework of the DSM system. One response to this movement can be found in the Alternative DSM-5 Model for Personality Disorders (AMPD), which uses dimensions of personality functioning, coupled with identification of dominant pathological traits, to define and classify personality disorders. By applying psychodynamic object relations theory to the AMPD, therapists can enrich the understanding and assessment of personality functioning and pathology as described in the AMPD.

Currently, psychiatry lacks a field that can be called "theoretical psychiatry", which uses theoretical concepts and explanatory models: The main stream of research is to collect data of all kinds in the hope that the computational Big Data approach will shed a bright light on the black box of mental disorders. Accordingly, the biology-based Research Domain Criteria of the National Institute of Mental Health have been established. However, as philosophical analyses of concepts and methods have shown, several epistemological gaps stand in the way of a consistent multilevel understanding of mental disorders. Also, the implicit ontological problems in the biological reduction of the psychosocial level and in the integration of so-called hard and soft disciplines are mostly left out. As a consequence, a non-reductive psychological theory of mental disorders is sought that also integrates correlating biological and sociological issues. In this context, one example of promising nonreductive psychiatric research is the option of systems/network psychopathology. The possibilities for integrating different psychological perspectives are highlighted for the field of addiction research and treatment, where pragmatic behaviorist approaches dominate over the theory-based practice of psychoanalysis. In comparing the theoretical constructs of these two approaches, the relevance of the concept of "(social) environment" as the wealth of influential sociocultural factors is discussed at levels superior to the interpersonal micro-level, namely the organizational meso- and societal macro level, which is not sufficiently considered in current biopsychiatry. On this basis of argumentation, the usefulness of grounding and framing psychiatry through the field of ecological sciences, especially human ecology, is demonstrated. Finally, to this end, an outline of an ecological model of mental health and illness is presented.

In contrast to the fruitful relationship between psychoanalysis/psychoanalysts and the humanities, institutionalized psychoanalysis has been largely resistant to the integration of psychoanalysis with other empirical branches of knowledge (infant observation, psychotherapy research, psychological and neurobiological sciences), as well as clinical ones [primarily cognitive-behavioral therapy (CBT)]. Drawing from two decades of theoretical and empirical work on psychopathology, psychotherapy, and psychoanalysis, the author aims to show how a reformulation of object relations theory (RORT) using (neuro-)psychological science may enhance a clinical-psychoanalytic understanding and treatment of suicidal depression, which constitutes one of the most formidable health challenges of our time. Specifically, he rewrote the notion of Melanie Klein positions-primarily the depressive position-using extant knowledge of structure of emotions, the centrality of mental representations of the future ("prospection") and the toxic nature of criticism-based emotions. This reformulation enables a dialog between clinical psychoanalysis and other therapeutic schools of thought and sheds light on the understanding and treatment of suicidal depression.

The current classification of personality disorder in ICD-11 includes a description of personality functioning, derived from a number of theoretical paradigms, but most notably consistent with the psychodynamic approach. Concurrently, an object-relations model of personality functioning in a dimensional assessment of severity is provided in the Structured Interview of Personality Organization-Revised (STIPO-R). To date, there are no published measures of International Classification of Diseases-11 (ICD-11) personality severity, though the construct is very comparable to the concepts assessed in the Diagnostic and Statistical Manual of Mental Disorders-5 (DSM-5) levels of personality functioning concept, which is measured by the Level of Personality Functioning Scale-Self-Report (LPFS-SR). This study examined the validity of ICD-11 personality functioning, as measured by the LPFS-SR, by evaluating its associations with the STIPO-R in Kurdistan region. The samples included 231 University students and 419 inpatient participants across four hospitals (267 with a diagnosed personality disorder). All the components of LPFS-SR and STIPO-R were positively and significantly intercorrelated. The components of each measure discriminated PD and non-PD patients from a University, non-clinical group adequately. Despite slightly better performance of the STIPO-R in this discrimination, the measures had a high congruence in predicting personality dysfunction. Overall, the findings of the present study support the validity of ICD-11 construct for evaluating personality functioning.

The use of a verbal or written contract has been employed in many different modalities of treatment as a means of establishing the expectations and parameters of treatment. Traditionally, the contract is seen as serving a somewhat utilitarian purpose: setting up the practical conditions of treatment and providing a containing frame for the patient. Contracts, however, can extend to establishing agreed-upon goals of treatment, anticipating obstacles that may arise in the treatment and clarifying how they will be dealt with should they arise, and clearly defining the roles of the patient and therapist in a way that aids the exploratory process once the treatment has begun. Importantly, the mutually agreed-upon contract serves as a useful roadmap to keep the treatment focused and on track. In this article we will emphasize how the treatment contract can facilitate in-depth understanding of the patient's internal world, particularly when challenges to the contract are enacted by the patient. We will begin by briefly summarizing the functions of the contract and then focus on the key role of the treatment contract in furthering the exploratory process of the patient's dynamics as expressed in the transference/countertransference matrix.

Recent advances in the understanding of personality pathology have contributed to an emphasis on the core of personality pathology as deficits in self-functioning and interpersonal functioning at different levels of severity that must be assessed for clinical intervention. In concert with these conceptual and empirical advances, transference-focused psychotherapy (TFP), an empirically supported psychodynamic treatment for borderline personality disorder, has been in constant development with extensive clinical use. This article describes an object relations model for conceptualizing and assessing levels of personality organization, a transdiagnostic approach to personality pathology, and related treatment modifications, thus expanding the utilization of TFP beyond borderline personality disorder to the full range of personality dysfunction. The core of this treatment approach is a sequential interpretive process between patient and therapist. This process takes place within the context of a structured treatment frame tailored to the unique individual with problems in self-functioning and interpersonal functioning in his/her particular environment.

RESUMEN

The new ICD-11 introduces a fully dimensional classification of personality disorders representing a fundamental change in personality disorder diagnosis with major implications for clinical practice and research. The new system centers on the evaluation of the severity of impairment in the areas of self and interpersonal functioning. This focus on personality functioning converges with long-standing psychoanalytic/psychodynamic conceptualizations of personality pathology. In a detailed conceptual analysis and review of existing empirical data, points of convergence and notable differences between major exponents of the psychodynamic tradition-object relations theory as developed by Kernberg et al. and the Operationalized Psychodynamic Diagnosis-and the ICD-11 system are critically discussed. Personality functioning can be considered to be the current "common ground" for the assessment of personality disorders and constitutes a considerable step forward in making personality disorder diagnosis both clinically meaningful and suitable for research purposes.

Narcissistic personality disorder (NPD) remains a controversial diagnosis, with lack of consensus on essential features of the disorder and its boundaries. Within the framework of object relations theory (ORT), core organizing, structural features define NPD and provide a coherent conceptual framework for understanding clinical features of the disorder. In the ORT model, both grandiose and vulnerable presentations of NPD are characterized by a specific form of self-pathology, reflecting the impact of a grandiose self-structure in the setting of borderline personality organization. The grandiose self-structure provides some stability of self-functioning but does not confer the self-regulatory capacities provided by normal identity formation and is reliant on maintaining a sense of the self as exceptional. We compare the ORT model of NPD to diagnostic criteria in the Alternative Model for Personality Disorders (AMPD) of the DSM-5, highlighting significant correspondence between the two models as well as conceptual differences.

O artigo irá aprofundar o debate acerca dos primórdios da constituição psíquica e sua relação com a etiologia dos casos-limite. Assim, apesar de não se tratar de um artigo sobre a infância, eixo principal desta revista, ele aporta a uma discussão teórica sobre a constituição psíquica e sobre a importância das interações iniciais da vida na etiologia dos casos-limite. Em seu primeiro eixo, discutiremos os primórdios do psiquismo como fonte de subsídios para uma clínica dos casos-limite, já que estes casos são compreendidos como marcados por traumas no âmbito das primeiras relações de objeto. Observa-se, na relação transferencial com estes pacientes, mecanismos de defesa verticais, como cisões, mais frequentemente do que mecanismos de defesa horizontais, como o recalque. O recalque foi o conceito central para a constituição da técnica psicanalítica clássica. Este fato faz com que nos defrontemos com a necessidade de refinamento da técnica analítica para abordar de forma mais eficaz estes casos que desafiam a técnica clássica. Neste sentido, estudaremos, no segundo eixo deste trabalho, a contratransferência sob uma perspectiva da intersubjetividade e da valorização da qualidade da relação analítica.


El artículo busca profundizar en el debate acerca de los principios de la constitución psíquica y su relación con la etiología de los casos dudosos. Aunque su tema no es sobre la infancia, línea principal de esta revista, plantea una discusión teórica sobre la constitución psíquica y la importancia de los primeros años de vida en la etiología de los casos límite. En su primer eje central, se exponen los principios del psiquismo como una fuente de subvenciones para una clínica de casos límite, puesto que estos son marcados por traumas en el contexto de las primeras relaciones de objeto. Se observa en la relación de transferencia con estos pacientes los mecanismos de defensa vertical como divisiones más frecuentes que mecanismos de defensa horizontal, como la represión. La represión fue el concepto central para el establecimiento de la técnica psicoanalítica clásica. Este hecho nos hace abordar la necesidad de refinamiento de la técnica analítica para exponer más eficazmente estos casos que desafían a la técnica clásica. En este sentido, en el segundo eje de este trabajo, vamos a estudiar la contratransferencia bajo una perspectiva de la intersubjetividad y la apreciación de la calidad de la relación analítica.


The article will deepen the debate about the beginnings of psychic constitution and its relation to the etiology of borderline cases. So, although this is not an article about childhood, main axe of this journal, it brings a theoretical discussion about the psychic constitution and the importance of the early interactions of life in the etiology of borderline cases. In the first axe, early objects relationships and the reorganization of the initials attachments as a source of subsidies for a clinic of borderline cases will be discussed. These cases are understood as marked by early trauma in the context of the first object relationships. It is noted, in relation to transference with these patients, vertical defense mechanisms, such as splitting, emerge more frequently than horizontal defense mechanisms, such as repression. The repression was the central concept for the establishment of classical psychoanalytic technique. This fact makes us tackle the need for refinement of analytical technique for approaching more effectively these cases that challenge the classical technique. In this sense, we will study, in the second axis of this paper, the countertransference under a intersubjectivity perspective and the appreciation of the quality of the analytical relationship.

This article discusses the concept of psychotic personality organization. I relate psychotic personality organization to observations and speculations about psychotic transferences and to behavioral developments during the treatment of chronically psychotic patients (particularly those with schizophrenia). The overwhelming theoretical perspectives used are twofold: (1) object relations theory and (2) a model integrating neurobiological advances in the study of psychosis with contemporary psychodynamic perspectives.

Despite considerable progress in depression research and treatment, the disorder continues to pose daunting challenges to scientists and practitioners alike. This article presents a novel conceptualization of the psychological dynamics of depression which draws from Melanie Klein's notion of the positions, reformulated using social-cognitive terms. Specifically, Klein's notion of position, consisting of anxieties (persecutory vs. "depressive"), defense mechanisms ("primitive"/split based vs. neurotic/repression based), and object relations (part vs. whole) is reformulated to include (1) affect, broadly defined, (2) affect regulatory strategies (defense mechanisms, coping strategies, and motivation regulation), and (3) mental representations of self-with-others, all pertaining to the past, present, and future. I reformulate the depressive position to include-beyond sadness, anxiety, and anhedonia-also anger/agitation, shame, disgust, and contempt, all of which are down-regulated via diverse mechanisms. In the depressive position, the self is experienced as wronged and others as punitive, albeit seductive. Attempts to appease internal others (objects) are projected into the future, only to be thwarted by awkward and inept interpersonal behavior. This might propel the use of counter-phobic, counter-dependent, and "manic" affect regulatory mechanisms, potentially leading to suicidal depression.

W. R. D. Fairbairn believed that the psychoanalyst's motivations and theories must ultimately be rooted in a need to resolve personal conflicts. His self-analytic and other records, now publicly available, indicate how his struggles with unacceptable sexual feelings and their symptomatic manifestations affected not only his theorizing, especially about sexuality, but also his clinical practice, as well as his personal and family life. Fairbairn's case affords a unique opportunity to document the effects of homophobia in a major psychoanalyst.

In this commentary on Paul Denis's paper 'The drive revisited: mastery and satisfaction', the author defends the idea of a plurality of metapsychologies that must be contrasted with and distinguished from each other while avoiding incompatible translations between models. In this connection he presents various theoretical approaches to aggression and the death drive, and demonstrates the differences between the drive model and the model underlying the theory of internalized object relations. The author holds that the concept of the internal object differs from Freud's notion of the representation (Vorstellung). He also considers that the imago as defined by Paul Denis in fact corresponds to the concept of the internal object. Lastly, he addresses the complex issue of listening to archaic forms of psychic functioning and their non-discursive presentation within the analytic process, which affects the transference-countertransference link.

The authors historically situate the London Kleinian development in terms of the small-group collaborations and adversaries that arose during the course of Melanie Klein's career. Some collaborations later became personally adversarial (e.g., those Klein had with Glover and Schmideberg); other adversarial relationships forever remained that way (with A. Freud); while still other long-term collaborations became theoretically contentious (such as with Winnicott and Heimann). After the Controversial Discussions in 1944, Klein marginalized one group of supporters (Heimann, Winnicott, and Riviere) in favor of another group (Rosenfeld, Segal, and Bion). After Klein's death in 1960, Bion maintained loyalty to Klein's ideas while quietly distancing his work from the London Klein group, immigrating to the United States in 1968.
