RESUMEN

Expansins are proteins without catalytic activity, but able to break hydrogen bonds between cell wall polysaccharides hemicellulose and cellulose. This proteins were reported for the first time in 1992, describing cell wall extension in cucumber hypocotyls caused particularly by alpha-expansins. Although these proteins have GH45 and CBM63 domains, characteristic of enzymes related with the cleavage of cell wall polysaccharides, demonstrating in vitro that they extend plant cell wall. Its participation has been associated to molecular processes such as development and growing, fruit ripening and softening, tolerance and resistance to biotic and abiotic stress and seed germination. Structural insights, facilitated by bioinformatics approaches, are highlighted, shedding light on the intricate interactions between alpha-expansins and cell wall polysaccharides. After more than thirty years of its discovery, we want to celebrate the knowledge of alpha-expansins and emphasize their importance to understand the phenomena of disassembly and loosening of the cell wall, specifically in the fruit ripening phenomena, with this state-of-the-art dedicated to them.

This dataset provides a comprehensive collection of vibrational data for the purpose of structural health monitoring, particularly focusing on the detection of bolt loosening in offshore wind turbine jacket foundations. The data set comprises 780 comma-separated values (CSV) files, each corresponding to specific experimental conditions, including various structural states of the wind turbine's support structure. These states are systematically varied considering three main aspects: the amplitude of a white noise (WN) signal, the type of bolt damage, and the level at which damage has occurred. The data were meticulously collected using eight triaxial accelerometers (PCB R Piezotronic model 356A17), strategically placed at different locations on a scaled-down replica of an offshore jacket-type wind turbine. This setup facilitated the acquisition of detailed vibrational data through a National Instruments' data acquisition (DAQ) system, comprising six input modules (NI 9234 model) housed in a chassis (cDAQ model). The white noise signal, simulating wind disturbance at the nacelle, was produced by a modal shaker and varied in three amplitudes (0.5, 1, and 2), directly proportional to the induced vibration in the wind turbine. The dataset uniquely captures the vibrational behaviour under different scenarios of bolt loosening in the turbine's foundation. The conditions include a healthy state (bolts tightened to 12 Nm) and various degrees of loosening (bolts loosened to 9 Nm, 6 Nm, and completely absent), examined at four distinct levels of the turbine's base structure. This granular approach offers a nuanced view of how varying degrees of bolt loosening impact the vibrational characteristics of the structure. The value of this dataset lies in its potential for wide-ranging applications in the field of structural health monitoring. Researchers and engineers can leverage this data for developing and testing new methodologies for early damage detection and progressive damage assessment in offshore wind turbines. The dataset's comprehensive coverage of damage scenarios makes it a valuable resource for the validation and enhancement of existing damage detection algorithms. Furthermore, the dataset can serve as a benchmark for comparing the efficacy of different vibrational analysis techniques in the context of wind turbine maintenance and safety. Its application is not only limited to wind turbines but can extend to other structures where bolt integrity is critical for operational safety. This dataset represents a significant contribution to the field of structural health monitoring, providing a detailed and practical resource for enhancing the reliability and safety of offshore wind turbines and similar structures.

Classically, particle-induced periprosthetic osteolysis at the implant-bone interface has explained the aseptic loosening of joint replacement. This response is preceded by triggering both the innate and acquired immune response with subsequent activation of osteoclasts, the bone-resorbing cells. Although particle-induced periprosthetic osteolysis has been considered a foreign body chronic inflammation mediated by myelomonocytic-derived cells, current reports describe wide heterogeneous inflammatory cells infiltrating the periprosthetic tissues. This review aims to discuss the role of those non-myelomonocytic cells in periprosthetic tissues exposed to wear particles by showing original data. Specifically, we discuss the role of T cells (CD3+, CD4+, and CD8+) and B cells (CD20+) coexisting with CD68+/TRAP- multinucleated giant cells associated with both polyethylene and metallic particles infiltrating retrieved periprosthetic membranes. This review contributes valuable insight to support the complex cell and molecular mechanisms behind the aseptic loosening theories of orthopedic implants.

Soybean is one of the most widely grown crops in the world and technologies are increasingly needed to increase productivity without impacting environmental degradation. In this context, the aim was to evaluate the action of forage plants of the genus Brachiaria sp. in crop-livestock integration on physical soil, agronomic and environmental aspects of soybean cultivation. The experiment was conducted in a subdivided plot design with seven integrated systems corresponding to the previous cultivation of Paiaguas palisadegrass, Xaraes palisadegrass and Ruziziensis grass in monocropping and intercropped with maize, as well as maize in monocropping. In the subplots, two grass management systems were evaluated: free growth and a grazing simulation cut. The bulk density and least limiting water range were assessed using soil samples and, after the pastures were desiccated when the soybean crop was planted, straw decomposition and plantability. A soil physics diagnosis by the bulk density and least limiting water range showed that the Paiaguas palisadegrass and Xaraes palisadegrass improved the soil environment due to biological soil loosening. The remaining mulch biomass did not affect soybean sowing and the adoption of Brachiaria sp. grass in the off-season, in addition to contributing to the provision of environmental services, and did not compromise grain productivity in succession.

Introduction: Aseptic and septic loosening by mechanical failure is one of the main causes of proximal femur endoprosthesis failure with different clinical consequences such as pain, inflammation, progressive loosening of muscular volume, and strength with functional limitation. We present a case series of four patients with aseptic and septic loosening of femur endoprosthesis that was treated with a novel technique that combines different methods to achieve primary stability. Case Report: Four patients with an average age of 49.5 years (35-70 years), two women and two men, were referred to the Orthopedic Surgery Department of the San Ignacio University Hospital of Bogota. Two of them with a history of oncological disease, ruling out a tumor relapse through local images and extension examinations; another patient with a history of firearm injury to the left thigh with multiple secondary complications, which required various reconstructive procedures; and the oldest patient with a history of total hip replacement. The main complaint of the four patients was thigh pain not caused by trauma and initial assessment radiographs of all of them showed signs of loosening of the prosthetic material, with septic etiology in two of the patients and aseptic etiology in the other two. Therefore, they were scheduled for revision surgery of prosthetic components. These patients recover functional status and resolved pain in the early post-operative period and during long-term follow-up. Only one of the patients, time after the surgical procedure, required removal of the osteosynthesis material due to persistent bone infectious process. Conclusion: Septic and aseptic loosening is one of the most common complications of proximal femur endoprosthesis, resulting in significant pain and functional decline in patients. We present a novel surgical technique that allows primary stabilization of the construct that allows early rehabilitation, improvement of functionality, and no signs of new loosening.

Os implantes osseointegrados demonstram, com o passar dos anos, que são uma opção viável na reabilitação de pacientes desdentados parciais ou totais, tendo resultados satisfatórios e com considerável longevidade clínica quando utilizados em reabilitação oral. Porém, o afrouxamento e fratura dos parafusos de retenção do pilar são as principais complicações em próteses sobre implantes, o que compromete a longevidade da reabilitação, sendo necessário buscar alternativas viáveis para reduzir ao máximo tal intercorrência. O objetivo do trabalho foi avaliar a manutenção do torque remanescente e ocorrência de falhas em parafusos de retenção de pilares protéticos, para implantes de conexão hexagonal externa. Os parafusos foram revestidos por filmes de carbono tipo diamante (DLC) dopados ou não com nanopartículas de prata (Ag). Foram utilizados 30 implantes de conexão hexagonal externa com plataforma regular e seus respectivos pilares protéticos, distribuídos em 3 grupos de acordo com o tipo de tratamento superficial do parafuso protético (n=10): Grupo A (controle) - sem tratamento na superfície do parafuso; Grupo B - parafuso revestido com filme de DLC; Grupo C - parafuso revestido com filme de DLC/Ag. Os filmes de DLC e DLC/Ag foram depositados pelo processo PECVD (Plasma Enhanced Chemical Vapour Deposition). A manutenção do torque após envelhecimento mecânico foi avaliada em todos os grupos. Será realizada microscopia eletrônica de varredura (MEV) antes e após o teste de fadiga, para avaliar possíveis alterações no filme de DLC. Os resultados serão analisados primeiramente de forma descritiva para determinação das análises estatísticas inferenciais a serem realizadas. (AU)


Over the years, osseointegrated implants have shown that they are a viable option in the rehabilitation of partially or totally edentulous patients, with satisfactory results and considerable clinical longevity when used in oral rehabilitation. However, loosening and fracture of the abutment retention screws are the main complications in implant-supported prostheses, which compromises the longevity of rehabilitation, making it necessary to seek viable alternatives to minimize this complication. This study aimed to evaluate the maintenance of the remaining torque and the occurrence of failures in retention screws of prosthetic abutments, for implants with external hexagonal connection. The screws were coated with diamond-like carbon (DLC) films, doped or not with silver nanoparticles (Ag). Thirty external hexagonal connection implants with a regular platform and their respective prosthetic abutments were used, divided into 3 groups according to the type of surface treatment of the prosthetic screw (n=10): Group A (control) - no treatment on the screw surface ; Group B - screw coated with DLC film; Group C - screw coated with DLC/Ag film. The DLC and DLC/Ag films were deposited by the PECVD (Plasma Enhanced Chemical Vapour Deposition) process. Torque maintenance after mechanical aging was evaluated in all groups. Scanning electron microscopy (SEM) will be performed before and after the fatigue test to evaluate possible changes in the DLC film. The results will be analyzed first in a descriptive way to determine the inferential statistical analyzes to be performed. (AU)

Objectives: With regard to the prevalence of abutment screw loosening (SL) and bone height reduction, particularly in the posterior regions of the jaws, as well as the contradictory issue of applying short implants instead of surgeries, along with all preparations associated with longer implants, the present study aimed to compare the amount of torque loss in short implants with increased vertical cantilever abutments and standard ones. Material and Methods: In this experimental study, a total number of 20 implants (MegaGen Implant Co., Ltd, South Korea) with 4.5 mm diameter including 10 short implants (7 mm) and 10 standard ones (10 mm) were utilized. Using a surveyor, fixtures were perpendicularly mounted in 13×34 mm resin for short implants and 19×34 mm resin for standard ones. The abutments of the same height but different cuff heights (2.5 mm for the standard implants and 5.5 mm for the short ones) were then tightened with 30 N.cm, via a digital torque meter. To compensate the settling effect, the abutment screw was re-tightened with 30 N.cm after 10 min. Upon applying 500,000 cycles at 75 N.cm and 1 Hz along the longitudinal axis on each sample, blind reverse torque value (RTV) was measured with a digital torque meter. The data were finally analyzed using Student's t-test. Results:Both groups experienced torque loss, but there was no statistically significant difference between the case and control groups in terms of abutment SL (p = 0451). Conclusion: Short implants seem to be a good mechanical alternative in emergencies with respect to torque loss and abutment SL. (AU)


Objetivos: Considerando a prevalência de afrouxamento de parafuso dos pilares, redução da altura óssea especialmente nas regiões posteriores, a questão contraditória da aplicação de implantes curtos em vez de cirurgias e todos os preparos associados a implantes mais longos, este estudo buscou comparar implantes curtos com pilares cantilever verticais aumentados e implantes padrão na quantidade de perda de torque. Material e métodos: Neste estudo experimental, foram utilizados 20 implantes (Megagen, Coreia do Sul) com diâmetro de 4,5 mm, incluindo 10 implantes curtos (7 mm) e 10 implantes padrão (10 mm). A fixação foi realizada perpendicularmente em uma resina 13 × 34 mm para implantes curtos e uma resina 19 × 34 mm para implantes padrão, usando um topógrafo. Os pilares da mesma altura, mas com diferentes comprimentos de manguito (2,5 mm para os implantes padrão e 5,5 mm para os implantes curtos) foram apertados com 30 N, utilizando um torquímetro digital. Para compensar o efeito de sedimentação, o parafuso do pilar foi reapertado com 30 N após 10 min. Depois de aplicar 500.000 ciclos a 75 N e 1 Hz ao longo do eixo longitudinal em cada amostra, o valor de torque reverso cego foi medido com um medidor de torque digital. Os dados foram analisados pelo teste t de Student. Resultados: Todos os grupos tiveram perda de torque, mas não houve diferença estatisticamente significativa entre os grupos caso e controle em termos de afrouxamento do parafuso do pilar (p = 0451). Conclusão: Os implantes curtos parecem ser uma boa alternativa mecânica em emergências em termos de perda de torque e afrouxamento do parafuso do pilar. (AU)

BACKGROUND: Aspirin has been shown to be a safe and cost-effective thromboprophylaxis agent with equivalent preventive efficacy to warfarin and fewer side-effects. However, animal studies have suggested delayed bone healing with aspirin and other inhibitors of prostaglandin synthesis. The impact of aspirin on aseptic loosening following cementless total hip arthroplasty (THA) has yet to be explored. Our aim was to determine if patients receiving aspirin for thromboprophylaxis had higher rates of aseptic loosening vs patients receiving warfarin after THA. METHODS: We identified 11,262 consecutive primary uncemented THA performed between 2006 and 2017. Postoperatively, either warfarin (target international normalized ratio 1.5-2.0) or aspirin chemoprophylaxis were prescribed for 4 weeks. We recorded demographics, length of stay, body mass index, preoperative nonsteroidal antiinflammatory drug use, and Elixhauser comorbidity index. All revisions because of aseptic loosening within 1 year of the index procedure were identified radiographically, confirmed intraoperatively, and did not fulfill Musculoskeletal Infection Society criteria for periprosthetic infection. Multivariate logistic regression analysis was performed. RESULTS: There was no difference (P = .14) in the rates of revision for aseptic loosening between patients in the aspirin cohort (14/4530; 0.31%; P = .14) and the warfarin cohort (36/6682; 0.54%). After accounting for confounding variables, no significant difference was noted in aseptic loosening rates between patients treated with aspirin vs those treated with warfarin (adjusted odds ratio 0.51; P = .11). Perioperative nonsteroidal antiinflammatory drug was not significantly associated with aseptic loosening (adjusted odds ratio 1.20; P = .67). CONCLUSION: While multiple agents are available for venous thromboprophylaxis, there is increasing evidence in favor of the use of aspirin. This study allays the notion that aspirin increases the rates of aseptic loosening following uncemented hip arthroplasty.

Abstract The aim of this study was to evaluate the failure probability of two types of abutment screws after compressive load and to analyze the stress distribution with finite element method. Sixty (60) single-tooth implant restorations were assembled on titanium implants (e-fix, A.S. Technology - Titanium Fix). The groups were divided into Conventional screw (Screw neck 1.5 ø mm) and Experimental screw (Screw neck constricted with 1.2 ø mm). Specimens were subjected to single load to failure with compressive test according ISO 14801. The fractured specimens were subjected to stereomicroscopy for measurement of remaining screws inside the implant and characterization of fracture origin. Representative specimens were analyzed by scanning electronic microscopy. For finite element method (FEM), an identical 3D model of the two in vitro test groups were used with similar conditions (30º, 100 N load). The stress in the abutment screw was analyzed by von-Mises criteria. The results of strength means were 4132.5 ± 76 MPa and 4528.2 ± 127.2 for conventional and experimental groups, respectively. During microscopy, the mean (mm) of the remaining screw piece inside the implants were 0.97 ± 0.23 and 1.32 ± 0.12 for conventional and experimental groups, respectively. In FEM, the conventional group showed stress concentered in an unfavorable region (peak of 39.23 MPa), while the experimental group showed more stress areas but less concentration than the conventional group (36.6 MPa). In using the tested experimental geometry, the abutment screw can have its strength improved, and the origin of failure can be more favorable to clinical resolution.


Resumo O objetivo deste estudo foi avaliar a probabilidade de falha de dois tipos de parafusos para pilar protético após a compressão e analisar a distribuição da tensão com o método dos elementos finitos. Sessenta (60) restaurações unitárias foram montadas em implantes de titânio (e-fix, A.S. Technology - Titanium Fix). Os grupos foram divididos em parafusos convencionais (parafuso de pescoço 1,5 ø mm) e parafuso experimental (parafuso de pescoço estreitado com 1,2 ø mm). As amostras foram sujeitas ao teste de compressão de acordo com ISO 14801. Os espécimes fraturados foram submetidos a estereomicroscopia para a mensuração dos parafusos restantes dentro do implante e caracterização da origem da fratura. Os espécimes representativos foram analisados ​​por microscopia eletrônica de varredura. Para o método de elementos finitos (FEM), utilizou-se um modelo 3D idêntico dos dois grupos de teste in vitro com condições semelhantes (30º, 100 N). A tensão no parafuso do pilar foi analisada pelo critério de von-Mises. Os resultados de resistência a compressão foram 4132,5 ± 76 MPa e 4528,2 ± 127,2 para grupos convencionais e experimentais, respectivamente. Durante a microscopia, a média do remanescente do parafuso restante dentro dos implantes foi de 0,97 ± 0,23 e 1,32 ± 0,12 mm para os grupos convencionais e experimentais, respectivamente. Em FEM, o grupo convencional mostrou tensão concentrada em uma região desfavorável (pico de 39,23 MPa), enquanto o grupo experimental apresentou mais áreas de tensão, porém menor concentração do que o grupo convencional (36,6 MPa). Ao usar a geometria experimental testada, o parafuso do pilar pode ter sua resistência melhorada e a origem da falha pode ser mais favorável à resolução clínica.

The degradation of pastures in Brazil is a problem of agronomic and environmental importance because it impairs the productive capacity of the soil and the hydrological cycle. In this context, the integration of agriculture and livestock emerges as a promising practice in recovering degraded soils through aggregation, increasing organic carbon in the soil and breaking compacted layers via the growth of grass roots. This study aimed to evaluate suitable indicators of physical quality in regard to mitigating structural changes of a Latosol cultivated with different species of the genus Brachiaria. We evaluated the grasses Brachiaria brizantha (Xaraés, Piatã, Marandu), Brachiaria decumbens and Brachiaria ruziziensis and adopted fallow and conventional tillage as controls. Soil samples were used to evaluate the parameters total organic carbon, soil aggregation via wet sieving and the least limiting water range (LLWR). Total organic carbon and aggregate stability were not suitable indicators for quantifying soil structural changes, remaining high in the evaluated conservation systems, with a difference observed only in the control with conventional tillage. The LLWR varied among all systems and layers evaluated. Xaraés palisadegrass, followed by the Piatã and Marandu cultivars, stood out in terms of biological soil loosening ability, being similar to the recovery caused by soil tillage. Furthermore, B. decumbens showed intermediate performance, while B. ruziziensis showed activity similar to that in the fallow treatment, indicating limited potential for soil physical recovery.


A degradação das pastagens no Brasil é um problema de ordem agronômica e ambiental, pois compromete a capacidade produtiva do solo e o ciclo hidrológico. Neste contexto, a integração agricultura-pecuária surge como uma prática promissora na recuperação de solos degradados, através da agregação, aumento dos estoques de carbono no solo e do rompimento de camadas compactadas pelo crescimento radicular das gramíneas. Este trabalho objetivou avaliar o indicador adequado de qualidade física na mitigação da compactação de um Latossolo cultivado com espécies do gênero Brachiaria. Foram avaliadas as gramíneas: Brachiaria brizantha (Xaraés, Piatã, Marandu); Brachiaria decumbens e Brachiaria ruziziensis. Adotou-se como controle: pousio e cultivo convencional. Amostras de solo foram utilizadas para avaliar o carbono orgânico total, a agregação do solo via peneiramento úmido e o intervalo hídrico ótimo (IHO). O carbono orgânico e a estabilidade dos agregados não foram indicadores adequados para quantificação das alterações estruturais do solo, permanecendo alta nos sistemas de conservação avaliados e diferindo apenas do controle com preparo convencional. O IHO variou entre todos os sistemas e camadas avaliadas. O capim Xaraés, seguido por Piatã e Marandu, destacou-se na escarificação biológica do solo, similarmente à recuperação via preparo do solo, e B. decumbens apresentou desempenho intermediário, enquanto B. ruziziensis apresentou atividade semelhante ao pousio, indicando potencial limitado de recuperação física do solo.

Furthermore, B. decumbens showed intermediate performance, while B. ruziziensis showed activity similar to that in the fallow treatment, indicating limited potential for soil physical recovery.(AU)


A degradação das pastagens no Brasil é um problema de ordem agronômica e ambiental, pois compromete a capacidade produtiva do solo e o ciclo hidrológico. Neste contexto, a integração agricultura-pecuária surge como uma prática promissora na recuperação de solos degradados, através da agregação, aumento dos estoques de carbono no solo e do rompimento de camadas compactadas pelo crescimento radicular das gramíneas. Este trabalho objetivou avaliar o indicador adequado de qualidade física na mitigação da compactação de um Latossolo cultivado com espécies do gênero Brachiaria. Foram avaliadas as gramíneas: Brachiaria brizantha (Xaraés, Piatã, Marandu); Brachiaria decumbens e Brachiaria ruziziensis. Adotou-se como controle: pousio e cultivo convencional. Amostras de solo foram utilizadas para avaliar o carbono orgânico total, a agregação do solo via peneiramento úmido e o intervalo hídrico ótimo (IHO). O carbono orgânico e a estabilidade dos agregados não foram indicadores adequados para quantificação das alterações estruturais do solo, permanecendo alta nos sistemas de conservação avaliados e diferindo apenas do controle com preparo convencional. O IHO variou entre todos os sistemas e camadas avaliadas. O capim Xaraés, seguido por Piatã e Marandu, destacou-se na escarificação biológica do solo, similarmente à recuperação via preparo do solo, e B. decumbens apresentou desempenho intermediário, enquanto B. ruziziensis apresentou atividade semelhante ao pousio, indicando potencial limitado de recuperação física do solo.(AU)

BACKGROUND: In an era of innovation in surgical approaches for total hip arthroplasty (THA), there is concern for increasing trends of early failure. The purpose of this study is to evaluate the incidence of early failure of primary THA stratified by surgical approach. METHODS: A retrospective review was performed on consecutive primary THAs completed from 2007 to 2014 at a high-volume center. THAs were stratified by surgical approach. Only the direct anterior (DAA) and posterior approaches (PA) were included. The primary outcome measure was early revision (<5 years). Descriptive statistics were performed using SAS software. RESULTS: In total, 6894 primary THAs performed between 2007 and 2014 were included. Across 2431 DAA THAs and 4463 PA THAs, there were 103 revisions overall. There was no difference in the overall revision rate for DAA THAs (1.69%) compared to PA THAs (1.39%) (P = .33). The DAA had a higher rate of early revisions for femoral component loosening compared to the PA (P = .0003). About 35.7% of DAA THAs were revised for femoral loosening compared to 8% for the PA (P = .0003). Early failure by femoral loosening occurred more often via the DAA in Dorr A bone (P = .03). The PA had a higher incidence of revision for instability (P = .04). There was no difference in modes of failure with regards to time to failure, acetabular loosening, early periprosthetic fracture, or infection. CONCLUSION: The DAA had a higher incidence of femoral loosening while PA had a higher mode of failure due to instability. Overall revision rates were not statistically different between approaches.

The imbalance between bone formation and osteolysis plays a key role in the pathogenesis of aseptic loosening. Strontium ranelate (SR) can promote bone formation and inhibit osteolysis. The aim of this study was to explore the role and mechanism of SR in aseptic loosening induced by wear particles. Twenty wild-type (WT) female C57BL/6j mice and 20 sclerostin-/- female C57BL/6j mice were used in this study. Mice were randomly divided into four groups: WT control group, WT SR group, knockout (KO) control group, and KO SR group. We found that SR enhanced the secretion of osteocalcin (0.72±0.007 in WT control group, 0.98±0.010 in WT SR group, P=0.000), Runx2 (0.34±0.005 in WT control group, 0.47±0.010 in WT SR group, P=0.000), β-catenin (1.04±0.05 in WT control group, 1.22±0.02 in WT SR group, P=0.000), and osteoprotegerin (OPG) (0.59±0.03 in WT control group, 0.90±0.02 in WT SR group, P=0.000). SR significantly decreased the level of receptor activator for nuclear factor-κB ligand (RANKL) (1.78±0.08 in WT control group, 1.37±0.06 in WT SR group, P=0.000) and improved the protein ratio of OPG/RANKL, but these effects were not observed in sclerostin-/- mice. Our findings demonstrated that SR enhanced bone formation and inhibited bone resorption in a wear particle-mediated osteolysis model in wild-type mice, and this effect relied mainly on the down-regulation of sclerostin levels to ameliorate the inhibition of the canonical Wnt pathway.

AIMS: The Corail stem has good long-term results. After four years of using this stem, we have detected a small group of patients who have presented with symptomatic metaphyseal debonding. The aim of this study was to quantify the incidence of this complication, to delineate the characteristics of patients presenting with this complication and to compare these patients with asymptomatic controls to determine any important predisposing factors. PATIENTS AND METHODS: Of 855 Corail collarless cementless stems implanted for osteoarthritis, 18 presented with symptomatic metaphyseal debonding. A control group of 74 randomly selected patients was assembled. Clinical and radiological parameters were measured and a logistic regression model was created to evaluate factors associated with metaphyseal debonding. RESULTS: The prevalence of this complication was 2.1% in our series. In the multivariable model, the presence of a Dorr B-type proximal femur was associated with metaphyseal debonding (odds ratio (OR) 10.73, 95% confidence interval (CI) 2.31 to 49.97, p = 0.002), as was a body mass index > 25 kg/m2 (OR 6.85, 95% CI 1.06 to 44.28, p = 0.04). Smaller stems and the use of a polyethylene acetabular liner appeared to be protective when compared with metal and ceramic setting hard-on-hard bearings. CONCLUSION: We have described an uncommon but important mode of failure of the Corail stem. Surgeons should be aware of this phenomenon; overweight patients with Dorr B-type femurs and in whom hard bearings are used appear to be particularly at risk. PURPOSE: It is still unclear whether four, six, or more implants should be used when restoring fully edentulous maxillae. This research evaluated the in vitro screw torque loss of zirconia frameworks supported by six implants and cantilevered zirconia frameworks supported by four implants. MATERIALS AND METHODS: Computer aided design/computer-assisted machining was used to fabricate 10 one-piece frameworks. Standardized pressable porcelain crowns were fabricated and luted to the frameworks. Specimens were divided into two groups (n = 5): AO4, cantilevered 12-unit full-arch fixed dental prosthesis supported by four implants; AO6, 14-unit supported by six implants. An opposing mandibular dental arch was fabricated with bis-acrylic composite resin. Specimens were submitted to 200 N underwater cyclic load at 2-Hz frequency for 1 × 106 cycles in a controlled 37°C temperature. A digital torque gauge assessed the initial and postload screw removal torque. Linear mixed-effects model was used for statistical analysis (α = 0.05). RESULTS: Significant screw torque loss was found for AO6 after cyclic loading (before: 36.20%/after: 52.82%; p < 0.05). Group AO6 (36.20%) presented lower preload loss before the cyclic loadings compared with AO4 (60.10%) (p < 0.05). CONCLUSIONS: Cyclic loading and lower implant-to-replaced-units ratio do not seem to compromise screw stability compared with higher implant-to-replaced-units ratio; however, a steep drop in preload was found before cyclic loading for both groups.

PURPOSE: The aim of the study was to evaluate the effect of a prosthetic cylinder and casting on the misfit and loosening torque of screw-retained multiple-unit implant-supported dental prostheses under masticatory simulation. MATERIALS AND METHODS: Screw-retained, three-unit fixed dental prostheses (FDP) and screw-retained full-arch FDP frameworks were waxed using calcinable (plastic cylinders) or overcasted (premachined cast-on cylinders) on the dental implant abutments. The cylinders were cast in Co-Cr alloy to obtain four groups according to cylinder type and prosthesis type (n = 10). The screws were tightened with 20 N/cm (abutment) and 10 N/cm (prosthetic) torque according to the manufacturer's recommendation. After 24 hours, the initial loosening torque was analyzed. The initial misfit measurements were performed according to the Schiffleger test. The screws were retightened, and the specimens were submitted to 10(6) mechanical cycles (2 Hz/280 N). Loosening torque and misfit were reevaluated (final measurements), and data were submitted to ANOVA, Tukey's HSD, and Pearson's correlation tests (α = 0.05). RESULTS: The calcinable three-unit FDP demonstrated greater misfit (initial: 107.53 ± 40.36 µm; final: 99.00 ± 40.85 µm) than did the overcasted three-unit FDP frameworks (initial: 51.50 ± 22.98 µm; final: 44.33 ± 14.14 µm) (initial: p = 0.0005; final: p = 0.0007). No difference was noted between the calcinable and overcasted full-arch FDP frameworks (p > 0.05). Masticatory simulation did not affect the misfit (p > 0.05). The overcasted full-arch FDP presented a lower abutment screw loosening torque (12.05 ± 1.80 N/cm) than did the calcinable ones (14.75 ± 1.72 N/cm) in the final measurement (p = 0.0024). The calcinable groups presented a lower prosthetic screw loosening torque than did the overcasted groups in the final evaluation (p < 0.05). After masticatory simulation, the prosthetic screw loosening torque of the calcinable three-unit FDP decreased (initial: 5.49 ± 1.07 N/cm; final: 3.73 ± 1.15 N/cm; p = 0.0044). Correlation between misfit and loosening was observed only for the prosthetic screws (p < 0.05). CONCLUSIONS: The overcasted components provided a better fit in three-unit FDPs but did not influence the fit of full-arch FDPs. Prosthetic screws of overcasted frameworks presented higher stability, whereas masticatory simulation did not influence misfit but did reduce the prosthetic screw loosening torque of calcinable three-unit FDP frameworks.

PURPOSE: The purpose of this study was to evaluate the influence of tightening technique and the screw coating on the loosening torque of screws used for Universal Abutment fixation after cyclic loading. MATERIALS AND METHODS: Forty implants (Titamax Ti Cortical, HE, Neodent) (n=10) were submerged in acrylic resin and four tightening techniques for Universal Abutment fixation were evaluated: A - torque with 32 Ncm (control); B - torque with 32 Ncm holding the torque meter for 20 seconds; C - torque with 32 Ncm and retorque after 10 minutes; D - torque (32 Ncm) holding the torque meter for 20 seconds and retorque after 10 minutes as initially. Samples were divided into subgroups according to the screw used: conventional titanium screw or diamond like carbon-coated (DLC) screw. Metallic crowns were fabricated for each abutment. Samples were submitted to cyclic loading at 10(6) cycles and 130 N of force. Data were analyzed by two-way ANOVA and Tukey's test (5%). RESULTS: The tightening technique did not show significant influence on the loosening torque of screws (P=.509). Conventional titanium screws showed significant higher loosening torque values than DLC (P=.000). CONCLUSION: The use of conventional titanium screw is more important than the tightening techniques employed in this study to provide long-term stability to Universal Abutment screws.

The aseptic loosening rate of the Mayo hip prosthesis femoral stem was determined in a 44 patient (52 hips) series, by comparing preoperative and postoperative clinical and radiographic parameters, over a minimal 5-year postoperative period. Loosening was obvious before five years in four hips (7.6%), while the stem was considered stable in the remaining 48 hips over a mean 6.1 year period, therefore accounting for a 92.3% Kaplan-Meier survival rate. We conclude that the aseptic loosening rate is too high for the Mayo hip prosthesis femoral stem, as compared to literature data concerning the non-conservative stems.

BACKGROUND: Many reconstruction techniques have already been developed for treating posterolateral corner (PLC) injuries, with still no consensus regarding what would be the best option. Some techniques use non-bone tunnel fixation, attaching the graft to the femur using a cortical screw with toothed washer. The main objective of the present study is to evaluate complications related to fixation performed by a screw and toothed washer technique. METHODS: A prospective study with surgical reconstruction of the PLC structures of the knee between January 2008 and December 2009 was performed. PLC reconstruction included reconstruction of the lateral collateral ligament, popliteofibular ligament and popliteal muscle tendon. Fixation of the grafts in the femur was achieved by means of a 4.5mm screw with a toothed washer. The assessments were done using the following methods: objective IKDC, subjective IKDC, Lysholm and Tegner. Radiographic evaluations were performed immediately after the operation, at 3, 6 and 12months after surgery, and yearly thereafter. Complications were documented. RESULTS: The mean subjective IKDC score after the operation was 69.2; Lysholm scale was 80.7. Two patients presented failure of reconstructions of the PLC. In the radiographic evaluations, signs of loosening of the screw with toothed washer in the femur were observed in eight cases (66.6%). Six patients (50%) complained of lateral pain after the operation. CONCLUSION: The technique of femoral fixation was shown to be efficient in restoration of stability. However there was a high rate of complications secondary to implant, such as loosening of the screws and iliotibial tract friction. LEVEL OF EVIDENCE: Case series, leve IV.
