RESUMEN
Medication iatrogenia is a real public health problem. Elderly people are particularly at risk, due to their multiple pathologies, including heart failure; residents of residential care facilities for the dependent elderly (Ehpad) are no exception. Studies show that this risk is avoidable in 60% of cases, and that advanced practice nurses (APNs) can play a pivotal role in preventive measures. How would the role of the APN be perceived by other healthcare professionals working with these heart failure patients institutionalized in Ehpad?

The institutionalization of the nursing discipline within French universities can take a number of different paths, but must not ignore a fundamentally diverse practical reality. Heterogeneity is also evident in the abundance of theories available and mobilized to address objects of nursing interest. This text emphasizes this mobilization and presents a rhizomic nursing discipline that departs from a classical, vertical and hierarchical academic discipline. It draws on multiple theoretical contributions to address the encounter with cared-for bodies, and on critical perspectives to question the future of advanced practice nurses as a caring corps.

Many elderly people live well and at home, but some require somatic and psychiatric care. Mobile psychiatric teams for the elderly are called upon to intervene with people aged 65-70 and over who are experiencing psychological distress. Among senior citizens, addictive disorders are very common, and are associated with psychiatric and geriatric co-morbidities. Alcohol, tobacco and benzodiazepine use disorders require holistic, individualized care, based on identification, prevention and referral, as well as training and coordination of professionals and caregivers.

The prevalence of haematological malignancies increases in elderly patients. The complexity of therapeutic management makes it necessary to evaluate them using a global approach, namely bio-psycho-environmental. Identifying and assessing their weaknesses are part of the roles of advanced practice nurse (APN). Various tools are used to do this, including the G8 questionnaire, which directs patients towards a comprehensive geriatric assessment, or the Geriatric core dataset, designed to be used during clinical trials and which is more detailed and faster. Could an APN use the latter?

Transition is a complex process that enables a young person living with a chronic illness to become an adult. It begins early in adolescence and ends once the transfer to the adult service is complete. The high risk of disruption during this transition calls for coordinated care focused on the patient and his or her life course. Many programs are dedicated to this delicate stage, including Jump, developed within the neurology department of a Paris hospital.

The introduction of an advanced practice nurse in psychiatry and mental health (APN) at the University Hospital of Nîmes has made it possible to improve the quality and safety of care while controlling costs and ensuring the satisfaction of patients, partners and care teams. A favorable institutional policy, the involvement of the management, psychiatrists and the IPA PSM facilitated the acceptance of this new profession by the care teams and other professionals, despite the existence of statutory and logistical obstacles.

Elderly people are the main victims of discontinuities in their treatment because they require appropriate care. The current system is no longer able to satisfactorily cover all these complex and increasingly important demands due to the lengthening of life expectancy and the profound crisis affecting health professionals. Feedback from the experience of nurses with a Master's degree in Advanced Practice in Gerontology since 2012 shows that in this new and critical context, they are a serious solution to be adopted by the legislator to meet these public health challenges.

In many countries, health systems are facing major difficulties in maintaining the quality, continuity and safety of the care provision. That is why healthcare systems are being reformed across the world. These reforms are redefining the boundaries between medical and paramedical professions, and have resulted in the emergence of advanced practices, notably in nursing. The field of oncology is not immune from these issues. It must deal with the evolution of techniques and technologies, the increase in knowledge as well as an evolution in the needs of patients with cancer. In France, advanced practice nurses appear to have found their place in oncology as they can take an active part in the priorities fixed by the different national cancer plans, notably through the prevention role which is one of the objectives.
