Introduction: Although the cause of childhood obesity is multifactorial, it is known that breastfeeding (BF) could be a protective element. Objective: To determine the association of obesity in children with the duration of BF, parental educational levels and types of employment and family formation in a group of children from Encarnación, Paraguay. Methodology: This was a longitudinal, retrospective and descriptive study with an analytical component carried out in 200 children of both sexes. Nutritional status was evaluated at 2, 4 and 6 years, duration of BF. Families were grouped according to parental education levels and types of employment (Group A: up to 6th grade of Elementary Education [EE] and informal economy work; Group B: up to 12th grade of EE and public/commercial employment; Group C: tertiary education and managerial employment positions or business ownership. A value of p <0.05 was considered significant. Results: 50% (n=100) were men, 96.5% (n=193) from urban areas, 70% (n=140) came from nuclear families, 73% (n=146) were families from group B. BF duration was ≤4 months 5.5% (n=11), up to 6 months 9% (n=18), 7 to 11 months 20.5% (n=41), continued BF (≥1 year) 63 .5%(n=127). 14.5%, 18% and 18.5% were obese at 2, 4 and 6 years respectively. There was greater obesity at 4 years with BF <6 months (42.9% vs 16.1% Chi2 p=0.02), but not at 2 or 6 years. No significant association of obesity with parental employment or educational level or with the presence of a partner (Chi2 p≥0.05). Conclusion: Breastfeeding duration of less than 6 months is associated with a higher frequency of obesity at 4 years of age.

RESUMEN

BACKGROUND: Self-perception of oral health is a recognized indicator, based on a single question. Association among oral health and socioeconomic status has been well established, however in Brazil, no results were found on the relation with type of insertion in labor market. OBJECTIVE: The aim of this study was to describe the prevalence of negative self-perception of oral health and associated factors, according to gender among Brazilian workers population, in 2019. METHODS: The National Health Survey, nationwide household-based survey, carried out in 2019, was used as source of data. Absolute and relative frequencies were calculated to understand the distribution of variables of interest and characterize the sample (n = 54,343). Binary logistic regression was used in the analyses. Odds ratio (OR) was estimated using a 95% confidence interval in bivariate and multivariate analyses. RESULTS: The prevalence of negative self-perception of oral health was 29.3%; in that, 31.0% among men and 27.2% among women. Compared to formal employees, the chance of negative self-perception of oral health among the informal workers was almost 20% higher among men and 13% among women after adjusting for all confounding factors. Unemployed women were 33% more likely to have negative self-perception of oral health. CONCLUSIONS: Self-reported unemployment increased the chance of a negative self-perception of oral health among women; however, the same effect was not observed among men. Why is there a difference? The association between informal work and negative SEOH is relevant in the Brazilian context of work deregulation and growth in the number of people without access to formal employment. The results suggested addressing employment conditions in adult oral health promotion actions, with emphasis on gender differences.

RESUMEN

Prevalence studies about family meals, including large and representative samples of children and adolescents on this topic, are scarce. Therefore, the aim of this study was twofold: first, to determine the prevalence of daily family meals in large and representative samples of school-going children and adolescents from 43 countries, and second, to identify the sex, age, socioeconomic status (SES), family structure, immigrant status and parental labour market status inequalities associated with this prevalence. Using data from the 2017/2018 wave of the Health Behaviour in School-aged Children study, a total of 179,991 participants from 43 countries were involved in this cross-sectional study. Family meals were assessed by the following question: 'How often do you and your family usually have meals together?'. Participants had five different response options: 'every day', 'most days', 'about once a week', 'less often', and 'never'. The meta package was utilized for conducting a meta-analysis of single proportions, specifically applying the metaprop function. The analysis involved pooling the data using a random-effects model and presenting the outcomes through a forest plot generated using the inverse variance method. Moreover, we applied generalized linear mixed models to explore the relationships between the studied sociodemographic factors as fixed effects, country as a random effect and the status of daily family meals as an outcome. Overall, the prevalence of daily family meals was 49.12% (95% confidence interval [CI]: 45.00-53.25). A greater probability of having daily family meals was identified for children aged 10-12 years (61.55%; 95% CI: 57.44%-65.49%), boys (61.55%, 95% CI: 57.44%-65.49%), participants with high SES (64.66%, 95% CI: 60.65%-68.48%), participants with both parents at home (65.05%, 95% CI: 61.16%-68.74%) and those with both unemployed parents (61.55%, 95% CI: 57.44%-65.49%). In the present study, which included large representative samples of school-going children and adolescents from 43 countries, more than half of the participants did not have daily family meals.

RESUMEN

This research addresses the impact of illicit drug use on labour market outcomes of men in Mexico. We leverage statistical information from three waves of a comparable national survey and make use of the Lewbel's heteroskedasticity-based instrumental variable strategy to deal with the endogeneity of the drug consumption. Our results suggests that drug consumption has fairly negative effects in the Mexican context: it reduces employment, occupational attainment and formality and raises unemployment of local males. These effects seem larger than those estimated for high-income economies.

RESUMEN

BACKGROUND: Rates of exclusive breastfeeding fall below recommended levels, particularly among women in paid employment. In Mexico, more than half of women are in informal employment, meaning they lack many of the protections that may support breastfeeding. METHODS: In-depth interviews with 15 key informants representing government agencies (n = 6 organizations), NGOs (n = 4), international organizations (n = 2), and academia (n = 2) in Mexico. Interviews were conducted between March and June 2023. To understand and describe barriers to breastfeeding among informally employed women in Mexico according to key informants and the current and potential policies to address these barriers, we conducted a qualitative thematic analysis. RESULTS: Current policies to promote, protect, and support breastfeeding predominantly apply to all employed women, but respondents expressed concern that they did not provide adequate protection for women in informal employment. Additional themes concerned the need for relevant programs to be institutionalized and coordinated, discussions of breastfeeding as a right, and the legal equivalence (whether true in practice or not) of formal and informal workers. CONCLUSIONS: Women employed in Mexico's informal sector face a dearth of maternity protections. According to key informants, few policies exist to promote, protect, and support breastfeeding among employed women, in general, but the economic vulnerability and challenging working conditions of women in informal employment exacerbates their situation. The lack of access to formal labor protections, such as paid maternity leave, creates a significant barrier to breastfeeding for women in the informal sector. Recommendations include short-term policies to fill gaps in social protection for informally employed women, as well as longer-term solutions such as the development of universal social protection programs and supporting formalization.

RESUMEN

BACKGROUND: The prevalence of cognitive impairment no dementia (CIND) and dementia appears to be higher in low- and middle-income countries (LMICs) compared to high-income economies. Yet few nationally representative studies from Latin American LMICs have investigated life-course socioeconomic factors associated with the susceptibility to these two cognitive conditions. Hence, the present study aimed to examine the associations of early- (education and food insecurity), mid- (employment stability), and late-life (personal income and household per capita income) socioeconomic determinants of CIND and dementia among older adults from Brazil, while simultaneously exploring whether sex plays an effect-modifier role on these associations. METHODS: This population-based study comprised a nationally representative sample of older adults (N = 5,249) aged 60 years and over from the Brazilian Longitudinal Study of Aging (ELSI-Brazil). We fitted multinomial regressions and estimated odds ratios with the respective 95% confidence intervals (CIs). RESULTS: In multivariate analyses, participants with more years of early-life education (0.89, 95% CI [0.81, 0.97]) and mid-life employment stability (0.97, 95% CI [0.96, 0.99]) and higher late-life household per capita income (0.70, 95% CI [0.51, 0.95) were less likely to have dementia. Regarding CIND, more years of mid-life employment stability (0.97, 95% CI [0.96, 0.98]) was the only determinant to confer protection. Notably, secondary sex-based analyses showed the higher the early-life educational attainment, the lower the odds of dementia in women (0.81, 95% CI [0.75, 0.87]) but not in men (1.00, 95% CI [0.86, 1.16]). CONCLUSIONS: These findings may have implications for population health and health policy by advancing our understanding of socioeconomic determinants of CIND and dementia, especially in Latin America.

RESUMEN

Ovarian cancer is considered the most fatal and costly gynecologic cancer. Although personalized therapies have improved ovarian cancer prognosis, they have resulted in increased financial toxicity concerns among this population. This study evaluated financial toxicity in patients with advanced ovarian cancer. Using secondary data from a study of barriers to palliative care, financial toxicity (FT) was measured through the Comprehensive Score for Financial Toxicity scale. Univariate and bivariate analyses were used to assess the relationship between selected demographic (i.e., age, race, ethnicity, education, place of birth, insurance type, yearly household income, employment status) and treatment-specific variables (i.e., years since diagnosis, surgery, chemotherapy, radiation, hormonal and targeted therapy) with clinically relevant financial toxicity. Characteristics were compared using Fisher's exact or chi squared tests. A total of 38 participants with advanced ovarian cancer were included in this study; 24% (n = 9) reported clinically significant FT. Income (p = .001), place of birth (p = .048) and employment status (p = .001) were related to FT. Study findings highlight that advanced ovarian cancer patients experience high FT, particularly those with low income, who are not able to work and were born outside the US. Further research using larger datasets and more representative samples is needed to inform intervention development and implementation.

Informal employment has been identified as an important social determinant of health. This article addresses the processes through which informal employment affects workers' health in Chile. The study's methodological approach was based on qualitative interviews with 34 formal and informal workers. The findings show how workers perceive informal employment as negatively affecting their mental and physical health through different dimensions of their living and working conditions. Incorporating a gender perspective proves to be integral to the analysis of these processes. The article concludes by discussing how neoliberalism underlies such vulnerability processes and negatively impacts on the population's health.

RESUMEN

BACKGROUND: Multiple Sclerosis (MS) can impact performance of daily occupations in both relapsing-remitting (RRMS) and secondary-progressive (SPMS) clinical courses. Work force participation decreases with advancing physical disability but the influence of non-motor factors, neuroimaging, and reserve have been scarcely investigated. We aimed to evaluate MRI, clinical, and cognitive (social and general) factors associated with impairment in different daily occupations and address whether cognitive and brain reserve have a positive impact on the ability to maintain these activities. METHODS: We prospectively enrolled persons with MS (PwMS) who underwent clinical examination (Expanded Disability Status Scale - EDSS; Timed 25-Foot Walk Test - T25FW; and the Nine Hole Peg Test - 9HPT), general neuropsychological assessment (Brief Repeatable Battery of Neuropsychological Tests - BRBN, including the Symbol Digit Modalities Test - SDMT), social cognition evaluation (Reading the Mind in the Eyes Test), cognitive reserve questionnaire, and MRI (FreeSurfer). We also enrolled healthy subjects for comparison as a control group. Daily occupations (employment, money management, and driving abilities) were assessed in all individuals with questionnaires. RESULTS: We included 62 PwMS (32 RRMS and 30 SPMS; mean age 42.8 years; median educational time 12.75 years) and 67 controls (mean age 39.7; median educational time 12.0 years) which were similar regarding demographics, education, and socioeconomic status (p > 0.1). Most PwMS (67.7%) had work-restrictions. They also reported fewer money management and driving abilities than controls (p < 0.001). Work-restriction was associated with physical disability (p = 0.006), SDMT and BRBN performance (p = 0.035 and p = 0.031, respectively), and T2-lesion volume (p = 0.022), with large effect sizes (d > 0.75). After hierarchical linear regression, money management was associated with hand dexterity, general and social cognition, and cognitive reserve (p < 0.03). Variables associated with driving abilities included fatigue, verbal fluency, striatum volume, and brain reserve (p < 0.05). CONCLUSIONS: PwMS have more frequent work-restrictions and impairment in money management and driving abilities compared to controls. Cognitive function, physical disability, and MS-lesion burden are strongly associated with work-restriction. Social cognition can also influence financial capacity. Cognitive and brain reserve can help retain some of these daily occupations.

RESUMEN

Introduction: Returning to work is an important cancer recovery milestone. Permanent colostomy can be required for rectal cancer treatment and can significantly impact well-being. We aimed to evaluate the impact of permanent colostomy on health-related quality of life and return to work in patients with rectal cancer. Methods: This was a retrospective cohort study on 23 employed patients receiving curative surgery for rectal cancer requiring permanent colostomy. Demographic and health-related quality-of-life questionnaires (the Colostomy Impact Score (CIS), the EORTC Quality of Life Questionnaire (QLQ)-C30, and the EORTC QLQ-CR29) were posted to eligible patients. Results: On average, patients (10 female, 13 male, mean age 61.8 years) were 5.0 ± 3.5 years post-surgery. At the time of questioning, 73.9% had returned to work (21.7% changed their type of work), while 17.4% never returned to work. Of those that returned to work, 11.8% returned within 1 month of surgery, while 23.5% had not returned after 12 months. Comparison of CIS between patients that returned to the same work (14.6 ± 0.93), changed their work (13.0 ± 0.74), and did not return to work (14.3 ± 2.3) revealed no significant differences (p = 0.36). CIS did not correlate with days worked on return, or time to return to work (p > 0.05). Conclusion: Returning to work following rectal cancer treatment with permanent colostomy is challenging, with 17.4% never returning to work. Of those who returned to work, 23.5% required more than 12 months. This was not associated with CIS in our study. (AU)

RESUMEN

Objective: to interpret young nursing professionals' perceptions about the relationship between working, employment and health conditions. Method: a qualitative study with an interpretive approach regarding the work-related experiences of 15 young nurses, who took part in the research through voluntary snowball sampling. The data from the interviews and the focus group were analyzed to reach an approximation to the realities inherent to the nurses' work life. Results: the relevant study findings pointed out that work precariousness is a characteristic feature in the population group, mainly related to hiring modalities and to wages. According to the participants, the psychosocial working conditions (which were intensified during the COVID-19 pandemic) were the ones that caused the most effects on their mental and physical health. Conclusion: from the young nurses' perspective, this study reports how the macro- and micro-structural working conditions and their relationship with health are perceived, pointing out the key elements to devise interventions focused on life paths that foster decent and healthy work postulates in their work environments, as well as actions to prevent injuries or harms to nurses' health.

RESUMEN

As an essential way to enhance farmers' self-development ability, off-farm employment plays an indispensable role in farmers' multidimensional poverty reduction in many countries. Employing a survey of 1926 farmers in five provinces of the Yellow River Basin in China, this paper examined the multi-dimensional poverty reduction effect of off-farm employment and the heterogeneous influence of different dimensions of off-farm employment (modes, levels, distances and frequency). The results showed that (1) although absolute poverty in the income dimension was largely eliminated in the Yellow River Basin, the poverty in social resources, transportation facilities, employment security were the key bottlenecks restricting farmers' self-development. (2) The province with the best multidimensional poverty reduction effect for non-farm employment was Shaanxi, with the largest contribution to employment security. (3) Improving off-farm employment level, distance and time can significantly alleviate the multi-dimensional poverty of farmers. Therefore, to lessen the multi-dimensional poverty of farmers in the Yellow River Basin, it is necessary to focus on the governance of multi-dimensional key poverty-stricken areas, such as the middle and upper courses of the Yellow River, adopting multidimensional poverty alleviation strategy of off-farm employment according to local conditions, working on the farmers' deficiencies in social resources, mobility, employment security, and deepening the effect of off-farm employment on benefiting farmers and helping the poor.

The search for employment using social contacts, despite being a common practice, sometimes does not generate the expected results. In order to verify the above, this paper describes the social contact networks of a group of young people living in contexts of marginality and violence in Cali's communes 1, 14, 16, 18 and 20 who participated in the program called Integral Treatment for Gangs - Youth Without Borders (TIP-JSF), a process of psychosocial intervention from an integral perspective that promoted social inclusion. The social networks found through a survey are characterized by being small and closed, increasing the likelihood that those who make up these networks have the same employment status: unemployment or informal employment. It is essential for young people to expand their contacts and link them with other contacts that can provide them with information on better job vacancies.

With the declaration of a health emergency in the face of the global Covid-19 pandemic, the government began to implement a series of measures to mitigate the risk of contagion, with significant consequences in the economic sector especially in labor matters. This article describes the labor regulations issued during the year 2020, characterizing their scope and affected actors. For this, a qualitative methodology was used through documentary analysis, with national regulations as the main unit of analysis, and research on labor measures at the international level as comparative sources. The results of the analysis allow us to arrive at a conceptual map represented by the regulations in the categories of labor and fiscal protection, from which classification by actors/beneficiaries and a comparison with the main measures in the central regions of the world are derived, identifying that the measures did not reach the expected effectiveness given the high rates of unemployment and informality that have been maintained since 2020 to date.

RESUMEN

OBJECTIVES: The present study aimed to investigate the prevalence of work-related musculoskeletal disorders (WRMDs) and to determine whether differences regarding the employment status (standard and non-standard), according to gender, increased the probability of WRMDs. STUDY DESIGN: Cross-sectional study. METHODS: Data from the 2019 National Health Survey (NHS) in Brazil were used. In total, 50,066 participants, both women and men, aged 18-65 years were included. Data were analysed for self-reported WRMDs. The binary logistic regression model, adjusting for possible covariables, was used to evaluate the association between working in the labour market and the occurrence of WRMDs. For covariates in the multivariate analysis, a P-value of <0.20 was considered in the univariate analysis. The variables with a P-value of <0.05 remained in the model. The odds ratio (OR), together with their confidence intervals at 95% (95% CI), were estimates both in the univariate and multivariate models. RESULTS: The prevalence of WRMDs was higher in women (4.2%) than men (1.9%). The likelihood of WRMDs was associated with the employment status only among men and proved to be lower for non-standard employment than standard employment (OR = 0.61; 95% CI = 0.43-0.87). Only among women, an increase in the likelihood of WRMDs associated with exposure to occupational noise was observed (OR = 1.77; 95% CI = 1.31-2.38) and a less favourable self-reported health (OR = 1.66; 95% CI = 1.08-2.54). Results showed higher change of WRMD for men and women associated with rheumatoid arthritis (men: OR = 3.94; 95% CI = 2.10-7.38; women: OR = 2.39; 95% CI = 1.68-3.39), depression (men: OR = 2.62; 95% CI = 1.54-4.45; women: OR = 2.39; 95% CI = 1.68-3.39) and occupational accidents (men: OR = 4.36; 95% CI = 2.34-8.14; women: OR = 2.20; 95% CI = 1.19-4.07). CONCLUSIONS: It is possible that the "double presence" that involves both the productive and reproductive spheres explains the higher prevalence of WRMDs among women Indicators that are sensitive to the effects of gender norms are necessary to understand the context of inequalities in the prevalence of WRMDs. The results of this study should be considered when developing strategies in the workplace and public health policies to reduce the prevalence of WRMDs.

RESUMEN

BACKGROUND: The regulation of working hours is governed by legal standards in formal employment. While the association between long working hours and various health outcomes has been extensively studied, there is limited evidence regarding Brazil. The objective of this study was to investigate the association among working hours, employment status, and self-rated health (SRH), taking into account differences between men and women in a national representative sample of the working population in Brazil. METHODS: A cross-sectional study was conducted among a representative sample of 33,713 workers in Brazil to assess self-rated health (SRH). We examined the associations between working hours and employment status, categorizing working hours as standard (40-44 h per week) or long (> 44 h per week), and employment status as formal or informal. Logistic regression models were employed, adjusting for sociodemographic, occupational characteristics, and health behaviors. Probabilities of negative SRH were calculated for men and women in different exposure profiles. Results were stratified by gender, and 95% confidence intervals (CIs) were applied to the findings. RESULTS: The prevalence of long working hours was higher among informal workers for both men and women. Adjusted odds ratio (AOR) results revealed that informal employment (AORwomen = 1.53; 95% CI: 1.13-2.07 and AORmen = 1.55; 95% CI: 1.22-1.96) and long working hours (AORwomen = 1.23; 95% CI: 1.06-1.43 and AORmen = 1.14; 95% CI: 1.00-1.30) were independently associated with negative SRH. Significant interactions between long working hours and informal employment were observed. Among individuals with the same exposure profile, women who engaged in long working hours had a higher probability of reporting negative SRH compared to men. CONCLUSIONS: The results of this study are in line with the literature, as differences between men and women in the likelihood of negative self-rated health were observed. The adverse health effects underscore the importance of implementing intersectoral actions to inform the revision of regulations concerning weekly working hours and the expansion of informal employment in low- and middle-income countries.
