Aim: T o validate the content of a self-report questionnaire in Spanish to explore attitudes, practices and barriers of dentists non-specialized in periodontics when facing the diagnosis and treatment of periodontal problems and criteria for referral. Materials and methods: Six specialists in periodontics from five Latin American countries constructed version#1 of a questionnaire. Twenty-one experts from 3 disciplinary areas from 6 countries rated each item from 1 to 5 (1: totally disagree, 5: totally agree) regarding its appropriateness, relevance, clarity, and coherence. According to the degree of agreement, the item was reformulated, and version #2 was built, which was tested on 60 dentists non-specialized in periodontics. Then, 3 of them were interviewed to assess comprehension and semantics. After analysis, the final version was prepared (version#3). Results: The items formulated in version#1 had an average score of 4.6 (range 3.7-5). Questions and options were reformulated, avoiding confusing words and having transversal acceptance in the participating countries. The items with the lowest degree of expert agreement were eliminated. The application of version#2 had a response time of 12 minutes, options were reformulated, and 4 items were added to obtain the final version with 44 questions. Conclusions: A self-report questionnaire with content validity was constructed to assess attitudes and practices in periodontal health and disease in Spanish-speaking general dentists or non-specialists in periodontics.

RESUMEN

Objective: To determine the participation of female dentists in the evolution of dental specialties in Peru and establish the gender distribution in each specialty. Materials and Methods: A descriptive and retrospective cross-sectional study was conducted using data from the National Registry of Specialists of the Peruvian College of Dentists in August 2023. The relative frequency of female gender and the femininity index were calculated in the total population of specialists and in each specialty to assess their respective evolution over the last 25 years. Results: Currently, 51.74% of the 3,827 specialty registrations correspond to female dentists, with a femininity index of 1.072. Nine specialties with a female predominance were identified, including Pediatric Dentistry, Family and Community Health in Dentistry, and Restorative and Esthetic Dentistry. On the other hand, areas with a higher male presence are Orthodontics and Maxillary Orthopedics, and Oral and Maxillofacial Surgery. Upon analyzing the temporal evolution, a shift from the historical male majority to the current female predominance in the last fifteen years is observed, and gender parity was reached in 2019. Conclusion: There is evidence of a change in the gender composition of Peruvian dental specialties, currently with a female predominance.

RESUMEN

INTRODUCTION: Home care provided by dentists is crucial for ensuring adequate oral care. However, oral health professionals face challenges in delivering treatment at patients' residences due to a lack of resources. Our objective was to explore dentists' perspectives and experiences of dental home care and potential challenges for its implementation. METHODS: The study took a qualitative approach. Guided by a semi-structured interview schedule, data were gathered using recorded interviews with 22 dental professionals. After transcription, data were analysed thematically using the Discourse of the Collective Subject (DCS) method, using Qualiquantisoft. RESULTS: The majority of participants were female (n = 20), aged between 30 and 40 years old, and predominantly specialising in primary care (n = 6) or endodontics (n = 6). All participants provided home care, performing general dental procedures, normally responding to requests from the work team (n = 13) or family (n = 7). Six main categories on to the topic emerged: importance and access to home care, procedures performed during home visits, discussions about post-home care, professional competence and patient-centred care, positive aspects of home care, and negative aspects and challenges faced in this type of care. CONCLUSION: This study highlights the fundamental, yet complex, role of home care in dentistry. Continuity of treatment through adaptability and a patient-centred approach are important.

RESUMEN

Oral health interferes with the general condition of the individual, because the oral cavity is a gateway and habitat for pathogens, especially in hospitalized people. There are several diseases that directly interfere in the oral cavity, such as Covid 19. Studies report oral changes in patients infected by SARS-CoV2 and point to the need for satisfactory oral hygiene during hospitalization in order to avoid the occurrence of oral lesions that increase the risk for the individual. The objective of this research is to emphasize the importance of the dentist in hospitals, pointing out the oral diseases that can be caused by poor oral hygiene during hospitalization. For the execution of the work, a questionnaire was carried out with patients who were hospitalized in Covid-19, clarifying if, during the hospitalization period, these patients received help in oral hygiene and if they noticed oral changes. The results achieved are in accordance with the literature, taking into consideration the age range of those affected by covid, over 50 years old, and the length of hospitalization, ran-ging from 10 days. Many hospitals still neglect the dental service and the patient himself performs the oral hygiene. The most common lesions reported by those infected by the virus were taste alteration, xerostomia and thrush, besides caries and dental calculus. We concluded the relevance of the odontologist far beyond the prevention of lesions, but in contributing to the recovery of the individual and the importance of adopting measures for the oral adequacy of hospitalized patients.

ABSTRACT This study aimed to characterize the work of professionals from oral health teams in the public service, in the COVID-19 pandemic, from the point of view of managers. This is a cross-sectional, quantitative-qualitative study, with 163 municipal health managers in Bahia, who answered a virtual questionnaire, from June to October 2020. The descriptive analysis of the quantitative data was performed using the Statistical Package for the Social Science, and Thematic Analysis was applied to the subjective responses, using IRaMuTeQ. The scope of action of oral health teams, which operate at all levels of health care, was recognized as broader than the traditional in this period. The pandemic emphasized the chronic problems in the working conditions of dentists, technicians, and oral health assistants in the public service, such as the lack of dental instruments and supplies, the precarious maintenance of dental equipment, the fragility of employment relationships, and the inadequate physical structure in the health units, compromising biosecurity. Thus, there is an opportunity to identify and implement necessary changes not only in the context of a health crisis, but in the post-pandemic period.

RESUMEN

Burnout syndrome (BS) is a highly prevalent occupational disease among dentists who, during the COVID-19 pandemic, have been at greater risk of contracting the disease, generating stress and distancing. The aim of this study was to assess the association of social conditions, professional factors and perceptions of COVID-19 with Burnout Syndrome. This was a cross-sectional study of 302 Brazilian dentists working in the clinical and private sectors. The professionals completed the Oldenburg Burnout Inventory online and answered sociodemographic and professional questions and questions related to their perception of the pandemic. Poisson regression with unadjusted and adjusted robust variance was used to estimate the association between burnout syndrome (dependent variable) and the independent variables. The presence of BS was strongly associated with age, training in a private institution, professionals who claimed to have sufficient protective knowledge and fear of being contaminated by SARS-CoV-2 during patient care (p < 0.05). The findings of this study show that there has been an impact of the COVID-19 pandemic on the occurrence of Burnout syndrome in dentists, especially, those who worked in the public sector and those who were afraid to work with other health professionals.

RESUMEN

AIM: To assess the knowledge of Brazilian dentists about Minimum Intervention (MI) using a Minimum Intervention Dentistry to Dental Caries-Knowledge Scale (MIDDeC-KS) and evaluate its psychometric properties. MATERIALS AND METHODS: An online scale was developed to evaluate dentists' knowledge about MI. The higher the score, the greater the knowledge and vice versa. A preliminary study was carried out with 143 dentists to validate the MIDDeC-KS. For psychometric properties analysis, convergent and discriminant validity, test-retest reliability, and internal consistency of the instrument were assessed. After validation, MIDDeC-KS was applied to a representative sample of Brazilian dentists. T test for independent samples, Chi-square, and/or ANOVA one-way tests were applied. RESULTS: The MIDDeC-KS convergent (Spearman's = 0.48) and discriminant (P = 0.001) validity and reliability (ICC = 0.85, α = 0.72 and ω = 0.74) were proved. The participants (n = 637) obtained an overall mean score of 7.4 ± 2.5. The sample was mainly composed of females (P < 0.01), who graduated over 10 years prior (P = 0.02), from public colleges (P < 0.01), and most were pediatric dentists (38.2%). The highest and lowest scores were reached by pediatric dentists (9.2 ± 1.6) and bucomaxillofacial professionals (3.1 ± 2.1), respectively. Brazilian dentists demonstrated more knowledge about diet, biofilm, and topical fluoride control (84.3%), while the Hall technique (31.9%), resin infiltrant (47.6%), and chemical mechanical caries removal (48.4%) were the least known MI techniques. CONCLUSION: Brazilian dentists demonstrated more knowledge about preventive measures and less about more recent protocols, such as the Hall technique, resin infiltrant, and chemical mechanical caries tissue removal. Sufficient psychometric evidence of the MIDDeC-KS was found. CLINICAL RELEVANCE: Identify dentist's knowledge about MI.

RESUMEN

OBJECTIVES: To evaluate the knowledge of senior dental students and newly graduated dentists about the prevention, diagnosis and treatment of oral manifestations of syphilis. MATERIALS AND METHODS: A 32-question questionnaire was designed with three domains: (I) demographic, academic and professional data of the participants, (II) attitudes, practices and self-perception regarding training about syphilis and (III) knowledge about syphilis. All knowledge responses were classified as correct or incorrect. Scores ranging from 1 to 14 were calculated, and grades were assigned to each participant according to their level of knowledge. RESULTS: The sample comprised 408 dental students and 339 newly graduated dentists. The mean score was 7.70 ± 3.35 for undergraduates and 9.09 ± 3265 for dentists. The highest frequency of correct answers (>70%) was attributed to questions about the aetiology, transmission and treatment of syphilis. The questions with the lowest frequency of correct answers (<50%) were about the identification of oral manifestations and stages of syphilis. CONCLUSIONS: The knowledge of dental practitioners and academics about the oral manifestations of syphilis was unsatisfactory. The lack of understanding of these aspects can delay the diagnosis and treatment of patients with this disease, which is concerning given the steady increase in cases in recent years.

Abstract The scope of this article is to analyze the evolution of the distribution of dentists and undergraduate courses in Dentistry in Brazil between 1960 and 2022, based on institutional data and its correlation with economic and demographic data. The proportions between number of dentists and population were calculated for the federative units in different periods, and the Herfindahl-Hirschman Index (HHI) was used. to determine the concentration of dentists, population and undergraduate courses in Dentistry. The Pearson test was used, with a significance level of 95%, to test the correlation between variables. In the period from 1960 to 2022, the ratio of dentists per 10,000 inhabitants increased from 3.3 to 16.9. Moreover in 2022, the federative units with the highest average household income per capita were closely correlated with the highest concentration of dentists (R2= 0.90; p < 0.00). However, from 1975 to 2022, the concentration of dentists measured by the HHI dropped from 45.1 to 33.4. The decrease in the geographic concentration of Dentistry courses was even more pronounced, with the HHI rising from 39.3 in 1991, to 25.6 in 2022.

RESUMEN

ABSTRACT Objective: To assess dentists' knowledge from Minas Gerais, Brazil, about dentoalveolar trauma (DT) and their experiences during the COVID-19 pandemic. Material and Methods: An online questionnaire with 34 questions was applied to collect personal data, professional training, self-assessment of experience/knowledge about DT, experience in care provided during the social distancing, and knowledge/conduct. The specific responses were evaluated based on the guidelines of the International Association of Dental Traumatology (IADT). Descriptive analysis and Pearson's chi-square test were performed. The level of significance was set at p≤0.05. Results: Most professionals (97.7%) had received information on DT, and only 4.6% of the participants considered their knowledge poor or very poor. However, 92.7% felt the need for more information on the subject. Regarding experiences during the pandemic, 55.7% provided trauma care during that period. Forty percent of the consultations were performed in person, and 33.3% of the professionals noted an increase in cases during the pandemic; 56.6% reported that the frequency did not change. The overall mean number of correct answers about DT was 5.29±2.11, indicating an acceptable level of knowledge. The mean percentage of hits for the specific questions was 44.1%. Conclusion: Although the level of knowledge of the dentists evaluated was acceptable, some aspects were deficient, with the need for more information about the IADT guidelines.

RESUMEN

ABSTRACT Objective: To analyze oral health services for pregnant women in the primary health care of a Brazilian metropolis, based on the performance of dentists who integrate these services. Material and Methods: Exploratory, descriptive and cross-sectional study was conducted through qualitative and quantitative approaches. A semi-structured questionnaire was used for data collection. This study was conducted in a Brazilian metropolis and included all dentists working in primary health care. The compliance analysis was performed considering access, adherence, group activity and individual dental care dimensions. Open questions were analyzed using the content analysis method and the closed questions were performed using frequency estimates. Bivariate statistics (Chi-square, Fisher's Exact Test, α<0.05) were analyzed through STATA. Results: 260 dentists agreed to participate in this study. Strategies to facilitate the scheduling of pregnant women to dental treatment occur in 93.9% of oral health teams, the main one being the scheduling and/or referral performed by the family health team. In terms of access and adherence to treatment, all variables in these dimensions showed relevant compliance results. The group activity dimension presented moderate compliance percentages, whereas individual care obtained high results, except for referral to radiography. Conclusion: The oral health service was compliant regarding access and adherence to treatment, but showed limitations in individual care and collective activities. Recommendations include improving oral health team coverage and permanent education programs.

Aim: The aim of this study was to evaluate the acceptability and knowledge of undergraduate dental students and dentists on selective caries tissue removal (SCTR). Materials and Methods: Third- and fourth-year Dentistry undergraduates (Group 1) and graduates working in a Hospital Dentistry Center (Group 2) were included in the study. Participants anonymously and confidentially answered a validated questionnaire containing eleven questions on the diagnosis and management of deep caries lesions. Pearson's Chi-square test and multivariate logistic regression compared the answers (p<0.05). Results: Total sample comprised 146 participants. Of these, 81.5% were female; 52.05% were graduates and the most prevalent age group was 18-29 years old (85.62%). Sixty-nine participants chose stepwise caries removal (p=0.027). The logistic regression analysis showed statistically significant differences. The participants who considered pulp wall moist have approximately eight times more likelihood to choose an invasive treatment (p=0.028). Those who chose endodontic treatment as an option for two-year survival have three times more likelihood to choose an invasive treatment (p=0.032). Those who affirmed that the carious dentin close to the pulp should not be removed had almost three times more likelihood to choose minimally invasive treatments (p=0.031). Discussion: Studies with questionnaires can be useful tools to detect whether the students and dentists are following the most current evidences to treat deep carious lesions. Conclusion: The participants had certain level of knowledge on SCTR, but the technique acceptability lacked consensus.

RESUMEN

In the present case report, a patient with esophageal squamous cell carcinoma (SCC) developed a second primary tumor in the tongue at the same time. Stage III middle esophageal SCC was discovered in a 60-year-old Caucasian male patient who was institutionalized, a smoker, and had previously struggled with alcoholism. Following treatment, the patient was admitted to the hospital for 15 days due to dysphagia and malnutrition. According to him, he had mouth pain while in the hospital. An intraoral examination revealed tooth mobility, bacterial plaque, dental calculus, and a 1.3-cm ulcerated lesion on the right lateral edge of the tongue. During the extraoral examination, no palpable lymph nodes were found in the submandibular or cervical regions. The tongue lesion was excisionally biopsied, and the histological evaluation agreed. The patient was advised to seek treatment for tongue SCC. Given that patients with esophageal SCC have a 9.3%-11.4% risk of developing head-and-neck SCC, multiprofessional follow-up of patients with esophageal SCC is critical for early detection of this cancer.

RESUMEN

Better understanding of dentists' decision-making about defective restorations is needed to close the evidence-practice gap (EPG). Aim: this study aimed to quantify the EPG about defective restorations and identify dentist factors associated with this EPG. Methods: 216 dentists from São Paulo State, Brazil, completed a questionnaire about three clinical case scenarios involving defective composite restorations with cementum-dentin margins (case 1) and enamel margins (case 2), and an amalgam (case 3) restoration. Dentists were asked what treatment, if any, they would recommend, including preventive treatment, polishing, re-surfacing, or repairing the restoration, or replacing the entire restoration. Replacing the entire restoration in any of these three scenarios was classified as inconsistent with the evidence, comprising an EPG. Bivariate analyses using Chi-square, ANOVA, or multiple comparison tests were performed (p<.05). Results: for defective composite restorations, 49% and 55% of dentists chose to replace the entire restoration for cases 1 and 2, respectively. Twenty-nine percent of dentists chose to replace the entire amalgam restoration. Dentists were significantly more likely to choose to replace the defective amalgam restoration than the composite restoration with a defect at the cementum-dentin margins or the enamel margins (both at p < .001). Female dentists were more likely to choose a conservative treatment than male dentists for cases 1 (p=.034) and 2 (p=.009). Dentists with a higher percentage of patients interested in individualized caries prevention were also more conservative in case 1 (p=.045). Conclusion: a substantial EPG regarding treatment decisions for defective restorations exists, especially for composite restorations. This study adds to the international evidence that an EPG exists in this clinical area and that global strategies need to be developed to close the gap.

RESUMEN

Background: This article identifies the factors associated with burnout among dentists in the state of Bahia, Brazil, during the COVID-19 pandemic. Methods: A cross-sectional study, conducted between November 2020 and February 2021, of 251 dentists, selected by snowball sampling. Sociodemographic, occupational, and epidemiological information was collected in an electronic form. Burnout was evaluated through the Maslach Burnout Inventory; anxiety through the Beck Anxiety Inventory; and quality of life through the WHOQol-BREF questionnaire. Multiple linear regression was used to identify factors associated with variations in levels of the three burnout dimensions. Results: The mean levels of the burnout dimensions were: Emotional Exhaustion: 28.9 ± 8.9 (SD); Depersonalization = 8.2 ± 6.3; and Personal Accomplishment: 28.0 ± 6.2. The proportion of professionals with high Emotional Exhaustion (≥ 30 cut-off score), high Depersonalization (≥ 12 cut-off score), and low Personal Accomplishment (≤ 33 cut-off score) were 43.4%, 26.3% and 81.3%, respectively. Burnout components were associated with high anxiety; lower levels of the physical, psychological, social relations, and environmental dimensions of quality of life; less time working as a dentist; increased alcohol consumption during the pandemic, feeling safe when using personal protective equipment, and support from work fellows. Conclusion: The dentists investigated showed moderate levels of Emotional Exhaustion and Depersonalization, and very low levels of Personal Accomplishment. The prevention and mitigation of burnout syndrome among dentists during a pandemic should consider several factors, such as quality of life, amount of time in the profession, use of personal protective equipment, support from fellow dentists, and alcohol consumption.

This study aimed to analyze the factors associated with the work ability of dentists working in the city of Cuiabá (MT). Quantitative and cross-sectional research carried out in August 2020 with 64 dentists. The assessment of work ability was performed using the Work Ability Index (WAI), validated in Brazilian Portuguese. Data analysis was performed using Kolmogorov-Smirnov and chi-square tests or Fisher's exact test (p < 0.05). The overall average ICT score was 36.89 points. Among the evaluated characteristics, only the last level of education completed and the time working in the current location showed a significant association with the WAI classification (p < 0.001 and p= 0.007). Dental surgeons who work in Cuiabá (MT) had a satisfactory ICT, and an association was found between the best ICT and the highest level of education and longer time working in the current job.

Abstract This cross-sectional study examined factors associated with occupational stress among 384 dentists working in the private sector in the first year of the COVID-19 pandemic in Brazil. Data were collected from August to October 2020 through an online form. Two outcomes - (1) anxiety and worry and (2) preparation and safety for work during the pandemic - constituted a proxy for occupational stress. The independent variables were grouped according to the explanatory theoretical model into individual, organisational and extra-organisational factors. Associations were tested by bivariate and multivariate logistic regression. Although prepared and confident, dentists felt anxious and worried about providing clinical care. Women, younger respondents and those who received no guidance on safety measures were more likely to report insecurity and unpreparedness. Younger professionals, those who did not participate in decision-making and who only sometimes had an assistant for four-hand work, were more likely to feel anxious and worried. In addition to individual factors, work organisation factors were associated with occupational stress among dentists in the first year of the pandemic.

Chagas disease is a systemic parasitic infection caused by the protozoan Trypanosoma cruzi (T. cruzi), discovered in 1909 by the Brazilian physician and researcher, Carlos Chagas. The aim of this study is to discuss the clinical manifestations of Chagas disease and its important implications in Dentistry. This article is based on a narrative review of the literature, with a qualitative approach. The survey of articles was carried out in the PubMed, LILACS and SciELO databases, totaling 13 studies used to complement the research. Chagas disease is transmitted by contact with the feces of the vector insect, called "kissing bug" in Brazil. When the cell, which the protozoan invaded, ruptures, the new microorganisms look for new cells to inhabit, traveling through the blood and lymph, reaching the heart, digestive tract and nervous plexuses. By affecting the coronary tissue, the microorganisms settle in the cardiac cells, mainly in the phagosomes. In addition to affecting the cardiac system, Chagas disease can involve two more important systems of the human body: the stomatognathic and digestive systems. When caring for chagasic patients who have cardiac involvement, the dentist needs to carry out a very detailed anamnesis in order to learn more about their systemic condition, stress reduction protocols are essential considering that dental care induces a picture of anxiety in patients. Prior knowledge about Chagas disease is mandatory for the dentist, since the oral manifestations of the disease can directly impact the quality of life of these individuals.
