As the most visible and vulnerable organ of the human organism, the skin can provide an impression of its state of health. Rare forms of diabetes and endocrinopathies are often diagnosed late or primarily misinterpreted due to their rarity. Skin peculiarities associated with these rare diseases may be indicative of the underlying endocrinopathy or form of diabetes. At the same time, rare skin changes in diabetes or endocrinopathies can also be a major challenge for dermatologists, diabetologists and endocrinologists in optimal patient and therapy management. Active collaboration between these different specialist groups can therefore lead to increased patient safety, better therapeutic success and more targeted diagnostics.
RESUMEN
Balneotherapy experiences a sharp increase in popularity during recent years. The present paper gives a perspective on the therapeutic use of mineral baths in the town of Solec-Zdroj in southeastern Poland, an old time spa endowed with rich natural sulfurous water resources. Historical aspects of the formation and development of the spa are presented, along with the contemporary insights into the plausible mechanisms and benefits of sulfide treatment, not only in otherwise hardly treatable chronic skin disorders but also in a variety of disease processes in organ systems. Sulfurous balneotherapy is not without potential risks, particularly for the skin, a tissue it is considered the most viable treatment for. The healing effect of sulfide waters does not increase in proportion to the sulfur content. As in every stimulus-based treatment, the stimulant strength should not go beyond the favorable hormetic boundaries of safety. Although the exact bioproperties of a high content of hydrogen sulfide in natural mineral springs are yet to be fully unraveled and understood in the context of healing capability, sulfide bath treatment is capable of growing and developing. It remains a cost-effective alternative to pharmaceutical products in a variety of disorders.

Clinical manifestations of drug-induced skin reactions include a wide range of symptoms, from mild drug-induced exanthemas to dangerous and life-threatening generalized systematic reactions. Drug-induced skin reactions to psychotropic medication are usually associated with antiepileptic drugs. However, a significant role can be assigned to selective serotonin reuptake inhibitors. We report a case of a female patient, who after approximately one month therapy with escitalopram developed a bilateral ankle edema, which resolved completely within the first week following its discontinuation. Although serious complications are rare, clinicians should be aware of severe skin complications in patients treated with antidepressants, which necessitate careful clinical monitoring and management. Individualization of pharmacotherapy is crucial, together with regular evaluation of safety and tolerance of the treatment.
