RESUMEN

The incidence of ulcerative colitis (UC) is on the rise globally. Shen-Zhu-Lian-Bai decoction (SZLBD) can relieve the clinical symptoms of UC. This study aimed to investigate the underlying molecular mechanism of SZLBD in the treatment of UC. The key treatment targets of SZLBD for UC were obtained based on the online database, and combined with the STRING database and Cytoscape 3.7.2 software, PPI network was constructed and visualized. The GEO database was utilized to validate the expression levels of core targets in UC. Metascape database GO functional annotation and KEGG pathway enrichment analysis. Molecular docking technology was used to verify the docking of core compounds with key targets. RT-qPCR and Western Blot were used to detect the expression of key targets in HCoEpiC cells for verification. After screening, 67 targets shared by SZLBD and UC were obtained. It is predicted that IL-6, IL-1B, and AKT1 might be the key targets of SZLBD in the treatment of UC. Quercetin was the main active ingredient. GEO results showed that the expression levels of IL-6, IL-1B and AKT1 were higher in the UC group compared to the control group. GO and KEGG analyses showed that these targets were related to apoptosis and inflammation. The results of molecular docking demonstrated that the AKT1 gene, a key target of quercetin, had the highest affinity of -9.2 kcal/mol. Cell experiments found that quercetin could affect the expression of IL-6, IL-1B, and AKT1. This study preliminarily explored and verified the mechanism of action of SZLBD in the treatment of UC, which provides a theoretical basis for subsequent in vivo mechanism studies.

Objective:To observe the effects of Zhulian stimulant type Ⅰ acupuncture on the expression of brain-derived neurotrophic factor (BDNF) and its receptor TrkB and tissue homogenate cyclic adenosine phosphate (cAMP) in rats with diabetic bladder (DCP); To explore the mechanism of Zhulian stimulant type Ⅰacupuncture on DCP.Methods:Totally 50 SD rats were divided into control group, model group, Western medicine group, ordinary acupuncture group, Zhulian stimulant type Ⅰ acupuncture treatment group (acupuncture treatment group) according to random number table method, with 10 rats in each group. DCP rat model was prepared by intraperitoneal injection of streptozotocin (STZ), except for the control group. The Western medicine group was given mecobalamine for gavage; acupoints of "Zhongji", "Sanyinjiao", "Liechou" and "Taichong" were selected. The ordinary acupuncture group was treated with ordinary acupuncture technique, and the acupuncture treatment group was treated with Zhulian stimulant type Ⅰ acupuncture, 1 time/d, 30 minutes/time. Samples were taken after 4 weeks of treatment. The maximum bladder volume, residual urine volume and wet weight of the bladder were detected. The morphology of rat bladder was observed by HE staining. The expression level of BDNF was detected by immunohistochemistry. The expression of cAMP was detected by Western blot. The level of TrkB was determined by ELISA. Real-time fluorescence quantitative PCR (RT-PCR) was used to detect the mRNA expressions of BDNF and cAMP.Results:Compared with model group, maximum bladder volume, residual urine volume and wet weight of bladder in Western medicine group, ordinary acupuncture group and acupuncture treatment group decreased ( P<0.01), and those in Western medicine group and acupuncture treatment group were lower than those in ordinary acupuncture group ( P<0.01). The expressions of BDNF mRNA and protein, cAMP mRNA and protein in Western medicine group, ordinary acupuncture group and acupuncture treatment group increased ( P<0.05), and the level of TrkB increased, and the Western medicine group and acupuncture treatment group were higher than that in ordinary acupuncture group ( P<0.05). Conclusions:Zhuliping stimulant type Ⅰ acupuncture has a protective effect on the bladder function of diabetic rats. The mechanism may be related to the up-regulation of BDNF and mRNA, TrkB, cAMP and mRNA expressions.

In 1951, ZHU Lian set up Acupuncture Experimental Institute, the predecessor of Acupuncture and Moxibustion Hospital (AMH) of China Academy of Chinese Medical Sciences. Being the founder of AMH, ZHU Lian had paved a road to ensure the correct academic growth and hospital development of AMH because of her outstanding clinical medical thinking and achievements in acupuncture-moxibustion. ZHU Lian's medical practice mode is reviewed and analyzed from 5 aspects, i.e. emphasis on clinical practice, inheritance and development, medical capacity, research orientation and self-growth. Based on this medical practice mode, the development orientation of AMH has been explored.

RESUMEN

Through checking the archives from China Academy of Chinese Medical Sciences (CACMS), the event that the experts from the former Soviet Union visiting China and learning acupuncture-moxibustion in 1956 was sorted out. In 1956, under the framework of Sino-Sovien Agreement on Scientific and Technology Cooperation, 3 medical experts assigned by the former Soviet Union came to China for the investigation and study of acupuncture-moxibustion. Institute of Acupuncture and Moxibustion, CACMS was in charge of this activity at that time. The Institute had developed a detailed plan, in which, the teaching schedule was designed on the base of New Acupuncture and Moxibustion written by ZHU Lian and ZHU Lian herself gave the lecture. After 3-month study, the experts had mastered the basic theory and clinical techniques of acupuncture-moxibustion and had launched the relevant clinical and scientific work of acupuncture-moxibustion successfully after returning to their country. This event opens up the foreign exchange of acupuncture-moxibustion in New China and provides the foundation for international training and higher education of acupuncture-moxibustion thereafter.

OBJECTIVE: To observe the efficacy of ZHU Lian inhibition type Ⅰ acupuncture for acne with spleen-stomach dampness-heat, and to explore its possible action mechanism. METHODS: A total of 82 patients of acne with spleen-stomach dampness-heat were randomly divided into an observation group and a control group, 41 cases in each group. The patients in the control group were treated with danshentong capsules (1 g, 3 times per day) and 0.1% adapalene gel smear (once every night) for 4 weeks. The patients in the observation group were treated with ZHU Lian inhibition type Ⅰ acupuncture at Hegu (LI 4), Neiting (ST 44), Quchi (LI 11), Yangbai (GB 14), Sibai (ST 2), Qihai (CV 6), Xuehai (SP 10), Yinlingquan (SP 9) and skin lesions, once every other day, 7 times as a course of treatment, totaling for 2 courses of treatment. The skin lesion score of the global acne grading system (GAGS) and quality of life-acne (Qol-Acne) score as well as the serum levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) were compared between the two groups before and after treatment; the clinical efficacy were compared between the two groups, and the recurrence rate was evaluated 4 weeks after treatment. RESULTS: After treatment, the skin lesion scores of GAGS in the two groups were reduced (P<0.05), and the score in the observation group was lower than that in the control group (P<0.05). After treatment, the Qol-Acne scores were increased in the two groups (P<0.05), and the score in the observation group was higher than that in the control group (P<0.05). The serum levels of TNF-αand IL-6 in the two groups were reduced after treatment (P<0.05), and those in the observation group were lower than the control group (P<0.05). The total effective rate was 95.1% (39/41) in the observation group, which was higher than 82.9% (34/41) in the control group (P<0.05). Four weeks after treatment, the recurrence rate of acne lesions was 10.3% (4/39) in the observation group, which was lower than 32.4% (11/34) in the control group (P<0.05). CONCLUSION: ZHU Lian inhibition type Ⅰ acupuncture is effective for acne with spleen-stomach dampness-heat, and the recurrence rate is low. Its mechanism may be related to the reduction of serum inflammatory factor TNF-α and IL-6.

OBJECTIVE: To observe the clinical efficacy of ZHU Lian's type Ⅱ inhibition acupuncture for chronic migraine, and explore the possible mechanism. METHODS: A total of 120 patients with chronic migraine were randomized into an observation group (60 cases, 3 cases dropped off) and a control group (60 cases, 2 cases dropped off). In the control group, flunarizine hydrochloride capsule was taken orally, 5 mg each time, once a day. In the observation group, ZHU Lian's typeⅡ inhibition acupuncture was applied at Erheliao (TE 22), Shousanli (LI 10), Hegu (LI 4), Yangbai (GB 14), Tongziliao (GB 1), Zusanli (ST 36) ect., once every other day. The treatment was given 4 weeks in the two groups. Before and after treatment, the migraine clinical symptom score, cerebral hemodynamics indexes (blood flow velocity of arterior cerebral artery [ACA], posterior cerebral artery [PCA], bilateral middle cerebral artery [MCA] and basilar artery [BA]), serum related indexes (levels of 5-hydroxytryptamine [5-HT], vascular endothelial growth factor [VEGF] and calcitonin gene-related peptide [CGRP]) and migraine specific quality of life questionnaire (MSQ) score were observed in the two groups, and the clinical effect was evaluated. RESULTS: The total effective rate in the observation group was 93.0% (53/57), which was higher than 79.3% (46/58) in the control group (P<0.05). After treatment, the number of headache attack was reduced, duration time was shortened, and the scores of pain intensity and concomitant symptom, cerebral hemodynamics indexes (blood flow velocity of ACA, PCA, MCA and BA) and serum levels of VEGF and CGRP were lower than before treatment in the two groups (P<0.05, P<0.01), and those in the observation group were lower than the control group (P<0.05). After treatment, the serum levels of 5-HT and MSQ scores of functional limitation, dysfunction and emotion were higher than before treatment in the two groups (P<0.05), and those in the observation group were higher than the control group (P<0.05). CONCLUSION: ZHU Lian's type Ⅱ inhibition acupuncture could reduce frequency of migraine attack and duration time, improve pain intensity, cerebral blood flow velocity and quality of life for patients with chronic migraine, its mechanism may be related to regulating serum levels of 5-HT, CGRP and VEGF.

OBJECTIVE: To observe the clinical efficacy of ZHU Lian's type-Ⅰ inhibition acupuncture combined with Zhuang medicinal thread moxibustion for acute herpes zoster (HZ), and explore the possible analgesic mechanism. METHODS: Sixty-six patients with acute HZ were randomly divided into an observation group (33 cases, 1 case dropped off) and a control group (33 cases, 1 case dropped off). The patients in the control group were treated with valaciclovir hydrochloride tablets, mecobalamin tablets and vitamin B1 tablets for 14 days. The patients in the observation group were treated with ZHU Lian's type-Ⅰ inhibition acupuncture and Zhuang medicinal thread moxibustion. ZHU Lian's type-Ⅰ inhibition acupuncture was given at ashi points (around herpes site), Qimen (LR 14), Geshu (BL 17), Quchi (LI 11), Xuehai (SP 10) and Neiguan (PC 6); Zhuang medicinal thread moxibustion was given at Changzi point, Kuihua point, Xionglongji point, Zhuang medicine Jiaji points; both the treatment was given once every other day for 14 days. The symptom improvement (blister stopping time, scab forming time, scab removing time) in the two groups were observed. The pain visual analogue scale (VAS) score and serum levels of interleukin (IL)-6, IL-8, substance P (SP) in the two groups before and after treatment were compared. The clinical efficacy of the two groups and the recurrence rate of neuralgia 1, 2, 3 months after treatment were evaluated. RESULTS: The blister stopping time, scab forming time and scab removing time in the observation group were earlier than those in the control group (P<0.05). Compared before treatment, VAS scores and serum levels of IL-6, IL-8 and SP in the two groups were decreased after treatment (P<0.05), and those in the observation group were lower than those in the control group (P<0.05). The total effective rate in the observation group was 96.9% (31/32), which was higher than 84.4% (27/32) in the control group (P<0.05). The recurrence rates of neuralgia 1, 2, 3 months after treatment in the observation group were 9.7%, 6.5%, 3.2%, which were lower than 29.6%, 22.2%, 14.8% in the control group (P<0.05). CONCLUSION: The ZHU Lian's type-Ⅰ inhibition acupuncture combined with Zhuang medicinal thread moxibustion have better analgesic effect and lower recurrence rate of neuralgia in patients with acute HZ, and this effect might be related to the reduction of serum IL-6, IL-8 and SP levels.

The thinking and experience of professor WEI Li-fu in the treatment of spasmodic torticollis under the guidance of ZHU Lian's academic thoughts are introduced. In pathogenesis, spasmodic torticollis is related to the obstruction of yang qi circulation and malnutrition of tendons and vessels. "Promoting yang and benefiting qi" is considered as the main treating principle. In the affected local area, the acupoints are selected from yangming meridians, shaoyang meridians and taiyang meridians of hand and foot. The acupoints of the governor vessel, the he-sea points of the involved yang meridians on the four limbs as well as "four-gate" points are selected as the distal acupoints. During treatment, the "restricting" needling technique invited by ZHU Lian is combined, deqi (qi arrival) is emphasized particularly. Besides, the ironing moxibustion technique is commonly adopted in combination in the affected area.

The control group was given ovulation triggering medications; the observation group was intervened by ZHU Lian's acupuncture plusWen Ren Tiao Shu method, with Tianshu (ST25), Zigong (EX-CA 1), Guanyuan (CV4), Xuehai (SP10), Zusanli (ST36), Sanyinjiao (SP6), Ganshu (BL18), Pishu (BL20), Sanjiaoshu (BL22) and Shenshu (BL23) selected, once per day, 10 sessions as a course of treatment. The two groups were observed for 3 menstrual cycles. The symptoms score of traditional Chinese medicine (TCM), menstruation score, basal body temperature (BBT), and ovulation rate by gynecological scanning were compared before and after the treatment, and the clinical efficacies were compared between the two groups.Result The TCM symptoms score changed significantly after the intervention in the observation group (P<0.05); the TCM symptoms score didn't show significant change after the treatment in the control group (P>0.05); after the treatment, there was a significant difference in comparing the symptoms score between the two groups (P<0.01); there was a significant difference in comparing the BBT between the two groups after the intervention (P<0.05); according to Chi-square test, after the treatment, the difference in ovulation rate between the two groups was statistically significant (P<0.05); the total effective rate was 94.3% in the observation group, higher than 82.9% in control group.ConclusionZHU Lian's acupuncture plusWen Ren Tiao Shu method can improve the symptoms of anovulatory fertility, increase the conception rate, and reduce adverse reactions; this method can produce a more significant efficacy than oral administration of Western medications in treating anovulatory fertility.

Objective: To observe the clinical effects of Zhu Lian's type Ⅰ excitation needling technique for postpartum urinary retention. Methods: A total of 60 cases with postpartum urinary retention were recruited and divided randomly into an observation group and a control group, 30 cases in each group. The control group was treated with routine acupuncture, and stimulated with sparse and dense wave of electric acupuncture for 15 min after arrival of needling sensation, and then the needles were taken out. The observation group was treated with Zhu Lian's type Ⅰ excitation needling technique, by inserting the needles with the quick inserting method, swift and temporary lifting and thrusting technique for shallow insertion for 5 times, by an in-and-out technique, without retaining the needles. Results: The total effective rate was 96.7% in the observation group and 83.3% in the control group. The difference in the total effective rate between the two groups was statistically significant (P<0.05). After the treatment, the first urination time was shorter in the observation group than that in the control group, with a statistical significance (P<0.01). After the treatment, the volume of residual urine after the first urination was less in the observation group than that in the control group (P<0.01). Conclusion: Zhu Lian's type Ⅰ excitation needling technique can effectively promote the voluntary urination and bladder emptying in patients with postpartum urinary retention, and it takes effect faster.

Objective To observe the clinical efficacy of ZHU Lian's acupuncture activating method with type 1 manipulations in treating hypotonic cerebral palsy.Method Sixty patients with hypotonic cerebral palsy, aged 9-59 months, were randomized into a treatment group and a control group, 30 cases in each group. The treatment group was intervened by ZHU Lian's acupuncture activating method with type 1 manipulations, while the control group was intervened by acupoint injection of vitamin B1 and B12. The two groups were treated once a day, 5 sessions as a treatment course. The Gross Motor Function Measure-88 (GMFM-88) was evaluated before the treatment and after 10 treatment courses.Result The GMFM-88 score was significantly changed after the treatment in both groups (P<0.05). After the intervention, the GMFM-88 score in the treatment group was significantly different from that in the control group (P<0.05).Conclusion ZHU Lian's acupuncture activating method is an effective approach in treating hypotonic cerebral palsy.

Objective To investigate the effect of different time intervention of Zhu Lian acupuncture exciting method on nerve cell apoptosis and tissue expressions of PI3K (phosphatidylinositol 3-kinase), AKt (serine/threonine kinase) and Caspase-3 (cysteinyl aspartate specific proteinase-3) proteins in young rats with hypoxic-ischemic brain damage.Methods Fifty 7-day-old rats were randomized into groups A (acupuncture exciting methodⅠ), B (acupuncture exciting methodⅡ), C (model), D (sham operation) and E (normal control), 10 rats each. Groups A and E began to receive acupuncture in 24 hours after model making and group B, at 8 days after model making. Groups C and D were not given acupuncture. Every group of animals was sacrificed at 21 days after model making. Nerve cell apoptosis was examined using In situ end labeling (TUNEL) technique. Cerebral expressions of PI3K, AKt and Caspase-3 proteins were determined by immunohistochemical method.Results The number of apoptotic cells was significantly smaller in groups A and B than in group C (P<0.01,P<0.05) and decreased significantly in group A compared with group B (P<0.05). The expressions of PI3K and AKt proteins increased significantly and the expression of Caspase-3 protein decreased significantly in groups A and B compared with group C; there were statistically significant differences (P<0.01,P<0.05). PI3K expression increased significantly and Caspase-3 protein expression decreased significantly in group A compared with group B (P<0.05); there were statistically significant differences (P<0.05).Conclusions Zhu Lian acupuncture exciting method can inhibit nerve cell apoptosis, stimulate the expression of PI3K/AKt signaling pathway, increase PI3K and AKt activities and reduce the expression of Caspase-3 protein in young rats with hypoxic-ischemic brain damage. Early intervention of Zhu Lian acupuncture exciting method is of important significance in producing a protective effect on brain nerves in young rats with hypoxic-ischemic brain damage.
