RESUMEN

Approximately 40% of Japanese physicians report working more than 960 hr of overtime annually, with 10% exceeding 1860 hr. To protect their health, annual overtime limits went into effect in 2024. The objective of this study was to investigate associations of self-reported sleep duration with psychological health and objective alertness. This was a cross-sectional National Survey for The Work Style Reform of Long Working Physicians. Physicians self-reported daily sleep duration, burnout (Abbreviated Maslach Burnout Inventory), depression (Center for Epidemiologic Studies Depression Scale) and traffic accidents. Alertness was then evaluated using the brief Psychomotor Vigilance Test. Of 20,382 physicians invited, 1226 completed the survey and brief Psychomotor Vigilance Test. Daily sleep duration was inversely associated with weekly work hours (ß = -5.4; 95% confidence interval -6.8 to -4.0, p < 0.0001). Sleep duration < 6 hr and ≥ 8 hr per day was associated with slower responses on the brief Psychomotor Vigilance Test (adjusted p < 0.05). An additional 10 hr worked per week was associated with a 0.40 point (95% confidence interval 0.08-0.72) increase in burnout severity and a 1.7% (95% confidence interval 0.1-3.3%) increase in odds of reporting a traffic accident. Increased brief Psychomotor Vigilance Test lapses, indicating lower alertness, were associated with worse symptoms of depression (ß = 0.23 points; 95% confidence interval 0.14-0.31, p < 0.0001) and burnout (ß = 0.25 points; 95% confidence interval 0.13-0.36, p < 0.0001). This study emphasizes the importance of sufficient sleep to maintain alertness, and supports limiting work hours for Japanese physicians to protect psychological health. Performance on the brief Psychomotor Vigilance Test may be a useful indicator of psychological health.

BACKGROUND: Although longer working hours are associated with lower sleep quality, it is still necessary to work a certain number of hours to make a living. In this study, we investigated the relationship between working hours and sleep quality in a community setting. We then explored how to manage work style while maintaining the sleep quality of workers without markedly reducing working hours. METHODS: 4388 day-time workers in various occupations living in Ota ward in Tokyo were included in the analysis. The relationship between working hours and sleep quality measured by the Athens Insomnia Scale was examined by ANOVA and linear regression models. Effect modification by work style (work end time, shift in working start and end time, current work from home status, change in work place) on the relationship between working hours and sleep quality was investigated by multivariate linear regression models. RESULTS: Longer working hours were significantly associated with lower sleep quality. The magnitude of the relationship between long working hours and low sleep quality was significantly larger when work end time was later (p for trend of interaction < 0.01) and when working start and end time were shifted later (vs no change, p for interaction = 0.03). The relationship was marginally greater when the proportion of work from home was increased (vs no change, p for interaction = 0.07). CONCLUSIONS: A relationship between longer working hours and lower sleep quality was observed among workers. Leaving work earlier or optimizing the work environment at home may diminish the adverse effect of long working hours on sleep quality.

Objective: Long working hours have been suggested to affect fathers' parenting behavior, but previously reported findings have been inconsistent. This study examined the association between the working hours and parenting behavior of fathers while accounting for other factors related to their parenting behavior, using data from the Japan Environment and Children Study (JECS), a large cohort study in Japan. Methods: Data from 43,159 father-mother pairs were analyzed. The mother assessed the father's frequency of seven parenting behaviors at 6 months after delivery. Then, each behavior was classified into a high-engagement group (always and sometimes) or a low-engagement group (rarely and never). The father's weekly working hours was obtained from his responses and was classified into six levels. Results: Logistic regression analysis showed that after adjustment for covariates, fathers' weekly working hours was inversely associated with the frequency of all parenting behaviors examined in this study (p for trend <0.0001). Compared with fathers working ≥0 to ≤40 h per week, those working >65 h per week showed the following adjusted odds ratios (95% confidence intervals) for low engagement in parenting behaviors: playing at home, 2.38 (2.08-2.72); changing diapers, 2.04 (1.89-2.20); and bathing the child, 2.01 (1.84-2.18). Conclusion: This study suggests that the greater time constraints imposed by longer working hours constitute a major factor that discourages fathers from engaging in childrearing behavior. Intervention targeting long working hours could contribute to measures aimed at promoting high-engagement parenting behaviors among fathers.

In Japan, the "Act on the Arrangement of Related Acts to Promote Work Style Reform" was enforced in 2019 to reduce long working hours. However, work style reforms largely depend on workers' and companies' awareness and abilities. This study aimed to develop the Work Style Reform scale to assess the competencies required for work style reform and to examine its validity and reliability. This study was conducted with a diverse sample of 1,641 Japanese workers, and 17 questions across three subscales-Work Style, Personal Style, and Work Environment-were developed. After a response bias analysis, some items were discarded based on an exploratory factor analysis; the reliability coefficients were calculated; and a confirmatory factor analysis was performed. The convergent and discriminant validity were evaluated through a multi-trait multi-method analysis. Finally, nine questions remained. Construct validity, reliability and convergent validity were sufficient for all Subscales, and discriminant validity was sufficient only for Work Style. In conclusion, while Work Style was sufficient, issues remained in other subscales. To improve the accuracy of them in the future, it is necessary to examine the validation of discriminant validity using different indicates and the addition of new items to the smallest subscale; Personal Style.

The home has become a new physical workplace, and can therefore influence the work, health, and life of workers. This cross-sectional study aimed to evaluate the chronology of the effects of work hazards at home on factors such as workers' health, productivity, and well-being (WB). Information on novice working-from-home (WFH) workers was derived from the "Occupational health of WFH" project. The selected variables in the hypothesis model comprised problems such as perceived indoor environmental quality (IEQ), working conditions (WC), sick house syndrome (SHS), occupational stress (OS), work productivity (WP), and WB. The relationship between these variables was analyzed using a structural equation model. The group analysis results showed the following significant indirect path effects from work environment through WP: IEQ-> SHS->OS->WP. A non-significant direct effect was observed between IEQ and WP. While WC problems could also have a significant direct effect on WP, or be mediated by OS, WP is a significant consequence and a direct effect of WB. In conclusion, the WFH model's causal impact between home environment, WP, and WB is a physiopsychological pathway. Therefore, creating a healthy home environment and WC, along with OS management, comprise important issues for improving productivity and WB for this new work style.

Teleworking has been adopted as a response to COVID-19 to reduce the spread of the infection, while continuing business operations. Saudi Arabia was among the countries that adopted stringent teleworking policies accompanying the first documented COVID-19 case. A descriptive, cross-sectional study was conducted to explore experiences of employees towards teleworking as a mandated work setting during COVID-19 in Saudi Arabia. We collected data using a questionnaire that was disseminated via different social media platforms during the partial curfew in the Country. The study sample included 471 participants. Most participants (78%) were satisfied with teleworking and reported higher job efficiency (44%) and work concentration (48%), and better work-life balance (56%) and stress management (55%). The positive experience was dependent on factors, such as gender, age, marital status, educational level, working sector and teleworking mode. Men, bachelor's degree holders, nonmarried, and employees working in health and education sectors as well as working flexible hours had less positive experience than their respective counterparts. The research contributes to literature about teleworking as a legitimate alternative work arrangement, while approaching teleworking as a means to reduce risks. As the first study conducted within Saudi Arabia, the study's findings have implications for policy, practice and research.

Background: The “work style reform of physicians” is due to come into effect in April 2024. Cardiovascular surgery involves many life-saving surgeries after hours, and it is expected to be difficult to achieve the upper limit (level A) of 960 h per year and less than 100 h per month for overtime work. In 2021, there were five full-time cardiovascular surgeons, four of whom were responsible for performing emergency surgery for acute aortic dissection in our facility. The ability to provide emergency surgical care with any two-person combination increases the flexibility of staffing for routine surgery or after-hours on-call. The working environment and surgical outcomes of acute aortic dissection under this system are reported, and changes in work style in cardiovascular surgery are discussed. Methods: The surgical outcomes of 39 cases of acute aortic dissection requiring emergency open heart surgery at this hospital during the one-year period from January to December 2021 were investigated. The number of cases (and first assistants) performed by five full-time surgeons were 7(13), 9(6), 12(3), 11(7) and 0(10), respectively. In addition, there were 8 cases of acute aortic dissection requiring urgent stent graft treatment during the same period. The emergency response rate for emergency patients (including those other than acute aortic dissection) was 100% during the same period. Results: The age was 69 years (median), 48.7% were female, 92.3% were Stanford type A, of which 22.2% were DeBakey type II. Shock vital 20.5%, malperfusion 30.8%. The surgical procedures included TAR in 19 cases, PAR in 8 cases, HAR in 12 cases (including 2 Bentall). Concomitant operations were AVR in 5 cases, CABG in 2 cases, TEVAR in 1 case, lower limb arterioplasty in 2 cases and right hemispherectomy in 1 case. Operating time 400 min (median), extracorporeal circulation time 194 min (median), cardiac arrest 108 min (median), selective cerebral perfusion time 125 min (median), lower body circulation arrest 46 min (median). Hospital mortality 7.7%, stroke 12.8%, delayed paraparesis 2.6%. Ventilation time was 1 day (median), hospital stay 23 days (median), 64.1% were discharged at home. Working Environments: 12-13 on-calls per month. Maximum yearly overtime work is 480.5 h with full overtime pay. Exemptions from working after night shift were also possible. Conclusions: The surgical outcomes of acute aortic dissection at our hospital were acceptable. Not having a fixed surgeon enabled a flexible emergency response, and increased the flexibility of staffing for routine surgery and on-call, and was considered to enable both a change in working style and surgical safety while meeting the needs of the community.

Background: Disparities in survival after pediatric out-of-hospital cardiac arrest (OHCA) between on-duty hours and off-duty hours have previously been reported. However, little is known about whether these disparities have remained in recent years. Objectives: This study aimed to examine the association of outcomes after pediatric OHCA with time of day and day of week. Methods: This observational study analyzed the Japanese government-led nationwide population-based registry data of OHCA patients. Pediatric (<18 years) patients who experienced OHCA between 2012 and 2017 were included. A multivariable logistic regression model was used to examine the association of both time of day (day/evening vs night) and day of week (weekday vs weekend) with outcomes after OHCA. The primary outcome was 1-month survival. Results: A total of 7,106 patients (mean age, 5.7 ± 6.5 years; 60.9% male) were included. 1,897 events (26.7%) occurred during night hours, and 2,096 events (29.5%) occurred on weekends. Overall, 1,192 (16.8%) survived 1 month after OHCA. After adjusting for potential confounders, 1-month survival during day/evening (1,047/5,209 [20.1%]) was significantly higher than that at night (145/1,897 [7.6%]) (adjusted odds ratio: 2.31 [95% CI: 1.87-2.86]), whereas there was no significant difference in 1-month survival between weekdays (845/5,010 [16.9%]) and weekends (347/2,096 [16.6%]) (adjusted odds ratio: 1.04 [95% CI: 0.88-1.23]). Conclusions: One-month survival after pediatric OHCA remained significantly lower during night than during day/evening, although disparities in 1-month survival between weekdays and weekends have been eliminated over time. Further studies are warranted to investigate the mechanisms underlying decreased survival at night.

OBJECTIVES: The Japanese government partially enacted the "Work Style Reform Bill" in 2019. The National Center for Child Health and Development (NCCHD) introduced an Overnight Call Shift (OCS) system for pediatrician training. We conducted a follow-up survey in 2019 to investigate the long-term effectiveness of the OCS system to improve the pediatric residents' mental wellness at NCCHD. METHODS: We conducted a questionnaire-based cross-sectional survey for pediatric residents in 2019 to compare the data with those of the previous survey in 2012. The questionnaire includes demographic data, working conditions data, and mental wellness assessment by the Center for Epidemiologic Studies Depression scale (CES-D) and the Maslach Burnout Inventory (MBI). RESULTS: The collection rate for the 2019 survey was 94.5% (37 participants/39 eligible). Compared to 2012, there were no significant changes in demographic data and working hours, a significant increase by about 30% in residents who took daytime off after night work, about 10% decrease in residents who scored 16 and above on the CES-D, and a significant decrease in the mean score for depersonalization (DP) in the MBI. Multiple regression analyses showed that daytime off after night work was the decreasing factor for CES-D and Emotional exhaustion (EE). CONCLUSIONS: The overnight shiftwork system shortened the pediatric residents' duty hours somewhat, and imposed an impact on the pediatric residents' mental wellness.

Objective: The concept of self-endangering work behavior (SEWB) was recently proposed to describe problematic behaviors to cope with heavy workloads and self-management. Although SEWB may enable workers to achieve immediate goals, it risks health and long-term work capacity. In this study, we developed a Japanese version of the SEWB (J-SEWB) scale, which was originally in German, and verified its validity and reliability. Materials: The original SEWB scale consisted of 21 items, constituting five subscales: "Intensification of working hours," "Prolongation/extension of working hours," "Refraining from recovery/leisure activities," "Working despite illness," and "Use of stimulating substances." We translated the scale into Japanese, then checked the wording using back-translation. Methods: The J-SEWB scale and questions for working conditions and sociodemographic variables was administered via an online survey with 600 participants registered with an internet survey company in Japan. Cronbach's α coefficients were calculated for each subscale to assess internal consistency. Construct validity was examined using principal factor analysis with equamax rotation. An analysis of variance evaluated the relationships of J-SEWB scores with working conditions and sociodemographic variables. Results: Cronbach's α coefficients ranged from 0.846 to 0.964 for five subscales, and 0.957 for all 21 items (total J-SEWB score) in 600 participants. The factor analysis identified five factors, classifying 21 items into corresponding subscales. Total J-SEWB scores were significantly higher for flexible work as well as longer working hours. Conclusions: The J-SEWB scale appears to be an effective tool for assessing SEWB in Japanese employees, with satisfactory reliability and construct validity.

BACKGROUND: The process of care for traumatic out-of-hospital cardiac arrest (OHCA) may be different at night and on the weekend. However, little is known about whether the rate of survival after OHCA is affected by the time of day and day of the week. METHODS: This observational study analyzed the Japanese government-led nationwide population-based registry data of OHCA patients. Patients who experienced traumatic OHCA following traffic collisions from 2013 to 2017 were included in the study. A multivariable logistic regression model was used to examine the association of both time of day (day/evening vs. night) and day of the week (weekday vs. weekend) with outcomes after traumatic OHCA. Night was defined as 23:00 p.m. to 6:59 a.m., and weekends were defined as Saturday and Sunday. The primary outcome was one-month survival. RESULTS: A total of 8500 patients (mean [SD] age, 57.7 [22.3] years; 68.6% male) were included. 2267 events (26.7%) occurred at night, and 2482 events (29.2%) occurred on weekends. Overall, 173 patients (2.0%) survived one month after OHCA. After adjusting for potential confounders, one-month survival during the day/evening (148/6233 [2.4%]) was significantly higher than during the night (25/2267 [1.1%]) (adjusted OR, 1.95 [95%CI, 1.24-3.07]), whereas there was no significant difference in one-month survival between weekdays (121/6018 [2.0%]) and weekends (52/2482 [2.1%]) (adjusted OR, 0.97 [95%CI, 0.69-1.38]). CONCLUSIONS: One-month survival after traumatic OHCA was significantly lower during the night than during the day/evening, although there was no difference in one-month survival between weekdays and weekends. Further studies are warranted to investigate the underlying mechanisms of decreased survival at night.

OBJECTIVES: The coronavirus disease 2019 (COVID-19) outbreak and the resulting state of emergency have restricted work environments, which may contribute to increased duration of sedentary behaviors. This study investigated the self-reported sedentary time of Japanese workers during and after the first state of emergency (April 7 to May 25, 2020) and examined differences in sedentary time after starting work from home and according to job type. METHODS: We used cross-sectional data from the Japan COVID-19 and Society Internet Survey, a web-based questionnaire survey conducted from August to September 2020 (n = 11,623; age range 15-79 years; 63.6% male). Prolonged sedentary time was calculated by subtracting the sedentary time after the state of emergency (defined as the normal sedentary time) from that during the emergency, with adjustments using inverse probability weighting for being a respondent in an internet survey. RESULTS: An increase in sedentary time of at least 2 hours was reported by 12.8% of respondents who started working from home during the state of emergency, including 9.7% of salespersons and 7.7% of desk workers. After adjusting for potential confounders, the multivariate-adjusted odds ratios (ORs) for a prolonged sedentary time ≥2 hours was significantly higher in respondents who started to work from home (OR: 2.14, 95% confidence interval: 1.78-2.57), and certain job types (desk workers; OR: 1.56, 95% CI: 1.27-1.91, salespersons; OR: 2.03, 95% CI: 1.64-2.51). CONCLUSIONS: Working from home and non-physical work environments might be important predictors of prolonged sedentary time.

OBJECTIVES: To address ongoing problems concerning population aging and labor shortages in Japan, employers have sought to improve work efficiency and labor productivity. However, it is unclear how presenteeism is affected by working styles in line with current corporate initiatives, such as reduced working hours, varied employment status, and flexible work arrangements. The purpose of this article was to investigate the association between work style and presenteeism. METHODS: This cross-sectional study extracted data from employee profiles, employee attendance records, and a questionnaire in a large service sector company. Multiple linear regression was conducted to estimate the contributions of work style variables to the Work Limitations Questionnaire (WLQ) index score. RESULTS: In total, 21 500 participants were eligible for analysis. The WLQ index was lower for those working < 35 h/week (adjusted regression coefficient [ARC]:-0.35%; 95% CI: -0.48 to - 0.21) and higher for those working 40-44 h/week or ≥ 45 h/week, compared with those working 35-39 h/week. The position of team manager was positively associated with the WLQ index, whereas senior manager (ARC: -1.44%; 95% CI: -1.71 to - 1.17) and part-time staff (ARC: -1.75%; 95% CI: -1.98 to - 1.52) positions were negatively associated with the WLQ index, compared with non-managers. Those who worked remotely had significantly lower WLQ index scores (ARC: -0.61%; 95% CI: -0.95 to - 0.27). CONCLUSIONS: Reduced working hours and flexible work arrangements were associated with lower work limitations, which imply presenteeism, although additional research is necessary to verify these results.

Introduction: Work style reform of medical doctors is now an actively discussed matter in Japan. There is also the problem of how to balance the management of legal working hours and the value of medical education in junior residency programs. In particular night and holiday shifts in emergency departments are one of the most important causes of working hour elongation for junior residents. We tried to construct a legally appropriate management method to control their working hours including night and holiday shifts. Method: One-Month Variable Working Hours System and two-shift system were applied to labor management in the medical residency program. Night and holiday working hours were included in regular working hours. Day-time working hours were adjusted to the weekly schedule of each clinical department to cover most of its daily work. Result: Overtime working hours of all residents, including the night and holyday shifts, were managed within the legal limits of 45 hours a month, unless additional overtime work was required. Discussion: Managing worktime through the use of the One-Month Variable Working Hours System is thought to be feasible. In the junior residency program, night and holiday shifts in the emergency department could also be balanced.

Gastroenterologists working in hospitals that have adopted the chief physician system are often required to work overtime during the night and on holidays treating critically ill patients and ordering emergency tests. To help alleviate the attending physician's duties, our hospital initiated an on-call system in October 2019 to replace the existing system. Changes in overtime hours worked and business stress before and after the introduction of the on-call system were verified. After the introduction of the on-call system, both overtime hours and the number of holidays worked decreased and work stress was reduced. We report that the on-call system is a suitable alternative to the attending physician system because it increases the work efficiency and satisfaction of attending physicians.

PURPOSE: The purpose of this study was to investigate recent trends in work-style reform and the use of information and communication technology (ICT) among board-certified diagnostic radiologists in Japan. MATERIALS AND METHODS: We conducted online questionnaire surveys of board-certified radiologists of the Japan Radiological Society (JRS) and registered training institutions. Completed surveys were obtained from 1192 radiologists and 275 training institutions (response rates of 25.5% and 38.1%, respectively). Respondents were assured of confidentiality. RESULTS: 13.5% (134/991) of full-time radiologists and 56.7% (89/157) of part-time radiologists had shifted some of their work to teleradiology at home. In addition, 52.9% (83/157) of part-time radiologists and 27.3% (12/44) of board-certified individuals who had stopped working as radiologists responded that they would consider starting full-time work in hospitals, if teleradiology at home was permitted as part of full-time work. Furthermore, 16.7% of training institutions (46/275) had introduced teleradiology systems for radiologists, and 47.2% (108/229) of the remaining training institutions wanted to introduce teleradiology systems in the future. CONCLUSION: Teleradiology using ICT is already a part of Japanese radiologists' workload. Work-style reform may progress with the use of ICT, such as part-time radiologists, and board-certified individuals who stop working as radiologists, becoming full-time radiologists.

RESUMEN

The work style of doctors gets attention within the Work Style Reforms that have been fully implemented since 2019. Now, we conducted a questionnaire survey at 10 institutions in Chugoku and Shikoku region and reviewed the latest work style of cardiovascular surgeons in comparison with other departments.
