Resumen El síndrome de ataxia cerebelosa, neuropatía y arre flexia vestibular (CANVAS) es un trastorno neurodege nerativo progresivo que se manifiesta en etapas tardías de la vida. Su base genética ha sido recientemente identificada en el gen que codifica la subunidad 1 del factor C de replicación (RFC1). Presentamos el caso de una mujer de 62 años con una historial de desequilibrio y deterioro de la marcha de presentación bifásica, con un inicio rápido de los sínto mas seguido de un deterioro neurológico lento y progre sivo. El proceso diagnóstico fue complejo y se realizaron numerosas pruebas para descartar causas adquiridas y genéticas de la ataxia, arribando al diagnóstico de ataxia cerebelosa de inicio tardío idiopática. Ulteriormente, las pruebas de función vestibular identificaron una grave vestibulopatía bilateral. Esto llevó a considerar el CANVAS entre los diagnósticos, que finalmente fue confirmado mediante pruebas genéticas (expansión bialélica del penta-nucleótido AAGGG en el gen RFC1). Este caso subraya la importancia de esta nueva enfer medad genética y su variante de presentación subaguda y enfatiza la relevancia de las pruebas objetivas de fun ción vestibular en las ataxias consideradas idiopáticas para lograr un diagnóstico adecuado y un eventual asesoramiento genético a la descendencia.
Abstract Cerebellar ataxia, neuropathy, and vestibular areflexia syndrome (CANVAS) is a late onset neurodegenerative disorder. Its genetic basis has recently been identified in the gene encoding a subunit of the Replication Fac tor C (RFC1). We present the case of a 62-year-old woman who expe rienced a history of a biphasic presentation of imbalance and gait disorders, with rapid onset of symptoms followed by slow and progressive neurological deterioration. The diagnostic process was challenging, and numerous tests were conducted to rule out acquired and genetic causes of ataxia, leading to a diagnosis of late-onset idiopathic cer ebellar ataxia. Subsequently, vestibular function tests iden tified severe bilateral vestibulopathy. This led to consider ing CANVAS among the diagnoses, which was ultimately confirmed through genetic testing (biallelic expansion of the pentanucleotide AAGGG in the RFC1 gene). This case highlights the importance of this new de scribed genetic disease and its subacute presentation variant, emphasizing the relevance of objective vestibu lar function tests in idiopathic ataxias to achieve proper diagnosis and eventual genetic counseling for offspring.

BACKGROUND: The cochlear implant (CI) is effective for rehabilitating patients with severe to profound sensorineural hearing loss. However, its placement and use have been associated with various complications, such as those affecting the vestibular system. The objective of this study was to compare vestibular function using the video head impulse test (vHIT) in pediatric patients before and after CI placement. METHODS: A descriptive and retrospective study was conducted. The outcomes of 11 pediatric patients of both sexes with a history of profound hearing loss were evaluated. The results of vestibular-ocular reflex (VOR) gain, saccades, asymmetry, Pérez Rey (PR) index, and VOR/saccade ratio for both ears obtained by the vHIT test before and after CI placement were compared. RESULTS: Of the 11 patients evaluated, the VOR gain showed that 81.8% had normal function, 18.2% had hypofunction, and no patients had hyperfunction before implantation. No statistically significant differences were found when compared with post-implant off and post-implant on conditions (p > 0.05). The extracted variables, asymmetry, PR index, and the VOR/saccades ratio also showed no statistically significant differences between the pre- and post-implant conditions, whether off or on. CONCLUSIONS: The vestibular function of pediatric patients did not show significant changes before and after CI placement. The vHIT test is a valuable tool for assessing vestibular function and could be considered a criterion for surgical and rehabilitation decisions in patients undergoing CI placement.
INTRODUCCIÓN: El implante coclear es un dispositivo eficaz para la rehabilitación de pacientes con hipoacusia neurosensorial severa a profunda. Sin embargo, su colocación y uso se ha asociado a diversas complicaciones, entre ellas a nivel del sistema vestibular. El objetivo del presente estudio fue comparar la función vestibular mediante la prueba de videoimpulso cefálico (vHIT) de pacientes pediátricos antes y después de la colocación del implante coclear. MÉTODOS: Se llevó a cabo un estudio descriptivo y retrospectivo. Se evaluaron los resultados de 11 pacientes pediátricos de ambos sexos con antecedente de hipoacusia profunda. Se compararon los resultados de ganancia del VOR, sacadas, asimetría, índice PR así como la relación VOR/sacadas para ambos oídos obtenidos mediante la prueba vHIT antes y después de la colocación del implante coclear. RESULTADOS: De los 11 pacientes evaluados, la ganancia del VOR mostró que el 81.8% tenía normofunción, 18.2% hipofunción y ningún paciente hiperfunción antes del implante. Al compararlo con la ganancia post implante apagado y post implante encendido no se encontraron diferencias estadísticamente significativas (p > 0.05). Las variables sacadas, asimetría, índice PR así como la relación VOR/sacadas tampoco mostraron diferencias estadísticamente significativas entre las condiciones pre y pos implante ya sea apagado o encendido. CONCLUSIONES: La función vestibular de pacientes pediátricos no mostró cambios significativos previo y posterior a la colocación del implante coclear. La prueba vHIT es una herramienta útil que permite evaluar la función vestibular y que podría considerarse como criterio para tomar decisiones quirúrgicas en pacientes que se encuentran en protocolo para implante coclear.

Abstract Objective To evaluate otolith function by comparing the findings of the Subjective Visual Vertical, the cervical Vestibular Evoked Myogenic Potential and the ocular Vestibular Evoked Myogenic Potential tests in patients in the inter-crisis period of unilateral definite Meniere's disease. Methods The sample consisted of an experimental group (n = 22; 10 men and 12 women, mean age 47.32 ± 12.82 years) with definite unilateral Meniere's disease and a control group (n = 14; 5 men and 9 women, with a mean age of 41.64 ± 13.45 years). They all underwent vestibular evaluation by means of Subjective Visual Vertical with the bucket method and, cervical and ocular Vestibular Evoked Myogenic Potential tests. The data were collected and subjected to statistical analysis. Results The results of the comparison of the Subjective Visual Vertical, the cervical Vestibular Evoked Myogenic Potential, the ocular Vestibular Evoked Myogenic Potential and, the association of cervical Vestibular Evoked Myogenic Potential with the ocular Vestibular Evoked Myogenic Potential showed no significant difference, indicating concordance among the tests. Conclusion The identified abnormalities and the concordance between the combined proportion of the Subjective Visual Vertical, the cervical Vestibular Evoked Myogenic Potential and the ocular Vestibular Evoked Myogenic Potential findings indicate that the association of these three tests contributes to the identification of sustained and transient otolith dysfunction in the inter-crisis of unilateral definite Meniere's disease. RESUMO A Esclerose Múltipla (EM) é uma doença autoimune crônica e inflamatória que afeta o sistema nervoso central (SNC). A disfunção do equilíbrio corporal também é um sintoma comum e pode estar relacionada às lesões neurológicas decorrentes desta doença. O objetivo deste estudo foi caracterizar os achados neurológicos e vestibulares de três casos clínicos com diagnóstico de EM. Dados sobre a avaliação neurológica e do exame de ressonância magnética de crânio foram coletados dos prontuários. Os pacientes responderam a uma entrevista inicial e foram submetidos à avaliação clínica do equilíbrio corporal e ao Video Head Impulse Test (vHIT). Observou-se presença de sintomas vestibulares e alterações em pelo menos uma das provas clínicas do equilíbrio corporal e função cerebelar. No vHIT, obteve-se alterações em testes oculomotores, como presença de nistagmo semi-espontâneo e em parâmetros do teste sacádico, e ganho reduzido em um ou mais canais verticais. Foram verificadas lesões, na ressonância magnética de crânio, em áreas centrais que processam as informações vestibulares, como cerebelo e tronco encefálico. A associação destes achados sugere a presença de disfunção vestibular central, compatível com as lesões detectadas nos exames de imagem.
ABSTRACT Multiple sclerosis (MS) is a chronic and inflammatory autoimmune disease that affects the central nervous system (CNS). Dysfunction of body balance is also a common symptom and may be related to neurological injuries resulting from this disease. The aim of this study was to characterize the neurological and vestibular findings of three clinical cases diagnosed with MS. Data on the neurological evaluation and the magnetic resonance imaging of the skull were collected from the medical records. The patients responded to an initial interview and underwent clinical assessment of body balance and Video Head Impulse Test (vHIT). Vestibular symptoms and alterations were observed in at least one of the clinical tests of body balance and cerebellar function. In vHIT, changes were obtained in oculomotor tests, such as the presence of semi-spontaneous nystagmus and in parameters of the saccade test, and reduced gain in one or more vertical channels. Lesions were found on MRI of the skull in central areas that process vestibular information, such as the cerebellum and brainstem. The association of these findings suggests the presence of central vestibular dysfunction, compatible with the lesions detected in imaging exams.

RESUMO Objetivo Avaliar o controle postural na doença de Menière. Métodos 34 pacientes com doença de Menière definida (grupo experimental) e 34 indivíduos hígidos (grupo controle), homogêneos quanto à idade e ao gênero, foram submetidos à posturografia do Tetrax Interactive Balance System (Tetrax IBS TM) em oito condições sensoriais. Índice de estabilidade, índice de distribuição de peso, índice de sincronização da oscilação postural direita/esquerda e dedos/calcanhar, frequência de oscilação postural e índice de risco de queda foram analisados. Resultados O índice de estabilidade foi maior no grupo experimental, com diferença significativa entre os grupos, em todas as condições sensoriais testadas. O risco de queda foi maior no grupo experimental do que no grupo controle. A oscilação postural foi maior no grupo experimental em todas as faixas de frequência, com diferença significativa em algumas delas. Não houve diferença significativa entre os grupos nos índices de distribuição de peso e de sincronização, nas oito condições sensoriais avaliadas. Conclusão Pacientes com doença de Menière apresentam comprometimento do controle postural, caracterizado por alterações do índice de estabilidade, em frequências de oscilação postural e no índice de risco de queda.
ABSTRACT Purpose To evaluate postural control in Menière's disease. Methods 34 patients with Menière's disease (experimental group) and 34 healthy individuals (control group) were submitted to Tetrax Interactive Balance System posturography under eight sensory conditions. Stability, weight distribution, synchronization, risk of falling and postural oscillation frequency were analyzed. Results Stability index was higher in the experimental group with significant difference between the groups in all sensory conditions. Risk of falling was higher in the experimental group than in the control. Postural oscillation was higher in the experimental group in all frequency ranges, with significant difference in some of them. There was no significant difference between the groups in the weight distribution and synchronization indexes. Conclusion In this study, Menière's disease patients presented impaired postural control, characterized by postural instability and oscillation and risk of falling.

RESUMEN Una completa evaluación vestibular y oculomotora es la llave para diferenciar las causas periféricas o centrales de vértigo agudo. El/la kinesiólogo/a debe estar capacitado para realizar una evaluación especializada del paciente. El objetivo de esta guía paso a paso es detallar al lector el procedimiento e interpretación de las herramientas de evaluación más relevantes del examen físico, dentro de la evaluación con y sin fijación visual, para realizar un diagnóstico diferencial de un paciente que ingresa a la consulta kinésica.
ABSTRACT A complete vestibular and oculomotor assessment is key to differentiate between peripheral and central causes of acute vertigo. Physical therapists must be trained to perform a proper assessment. The objective of this study is to describe the procedure and interpretation of the most relevant physical assessment tools, with and without visual fixation, to reach a differential diagnosis in patients attending for physiotherapy consultation.

To assess the interactions between individual cofactors and multisensory inputs on the postural sway of adults with type 2 diabetes and healthy subjects, 69 adults accepted to participate in the study (48 with/ 21 without diabetes). Assessments included neuro-otology (sinusoidal-rotation and unilateral-centrifugation), ophthalmology and physiatry evaluations, body mass index (BMI), physical activity, quadriceps strength, the ankle/brachial index and polypharmacy. Postural sway was recorded on hard/soft surface, either with eyes open/closed, or without/with 30° neck extension. The proportional differences from the baseline of each condition were analyzed using Multivariate and Multivariable analyses. Patients with polyneuropathy and no retinopathy showed visual dependence, while those with polyneuropathy and retinopathy showed adaptation. Across sensory challenges, the vestibulo-ocular gain at 1.28 Hz and the BMI were mainly related to changes in sway area, while the dynamic visual vertical was mainly related to changes in sway length. The ankle/brachial index was related to the effect of neck extension, with contributions from quadriceps strength/physical activity, polyneuropathy and polypharmacy. Across conditions, men showed less sway than women did. In conclusion, in adults with diabetes, sensory inputs and individual cofactors differently contribute to postural stability according to context. Rehabilitation programs for adults with diabetes may require an individualized approach.

Abstract Introduction In recent years, scientific evidence has shown that chronic otitis media may cause balance and vestibular dysfunction. Objective To compare the results of the video head impulse test (gain and symmetry of the vestibulo-ocular reflex and presence of covert and overt saccades) in patients with chronic otitis media and controls. Methods Cross-sectional study of patients with chronic otitis media (study group), aged between 18 and 60 years. The patients in the study group were further divided according to the chronic otitis media type as (1) non-suppurative, (2) suppurative, and (3) cholesteatomatous. For the comparative analysis, we selected volunteers with no history of ear and vestibular diseases (control group), who met the same inclusion and exclusion criteria as the study group. Patients in both groups underwent a video head impulse test. Results The study group consisted of 96 volunteers, and the control group of 61 individuals. The prevalence of vestibular symptoms was 66% in the study group and 3.2% in the control group (p< 0.001). The results show a higher prevalence of changes in the vestibulo-ocular reflex gain (22.9%) and corrective saccades (12.6%) in the chronic otitis media group compared to the control group (p< 0.001). Despite the higher prevalence of changes in gain, the average vestibulo-ocular reflex gains in the chronic otitis media groups were within the pre-defined values of normality; however, the mean vestibulo-ocular reflex gain in the anterior semicircular canal was statistically worse in the cholesteatomatous chronic otitis media group compared to controls (p< 0.001). Regarding the corrective saccades, the prevalence of saccades was statistically higher in the suppurative and cholesteatomatous chronic otitis media subgroups compared to the non-suppurative and control groups (p= 0.004). Conclusion The present study demonstrated that chronic otitis media is associated with a higher prevalence of vestibular symptoms and also a higher prevalence of changes in gain and corrective saccades when compared to controls.
Resumo Introdução Nos últimos anos, evidências científicas demonstraram que a otite média crônica se associa a alterações de equilíbrio e disfunção vestibular. Objetivo Comparar os resultados do teste do impulso cefálico por vídeo (ganho e simetria do reflexo vestíbulo-ocular e presença de sacadas cobertas e descobertas) em pacientes com otite média crônica e controles. Método Estudo transversal que envolveu pacientes com otite média crônica (grupo de estudo), entre 18 a 60 anos. Os pacientes no grupo estudo ainda foram divididos de acordo com o tipo de otite média crônica em (1) não supurativa, (2) supurativa e (3) colesteatomatosa. Para análise comparativa, selecionamos voluntários sem história de doenças otológicas e vestibulares (grupo controle), que obedeceram aos mesmos critérios de inclusão e exclusão do grupo de estudo. Os pacientes de ambos grupos foram submetidos ao teste de impulso cefálico por vídeo. Resultados O grupo estudo foi composto por 96 voluntários e o grupo controle por 61 indivíduos. A prevalência de sintomas vestibulares foi de 66% no grupo de estudo e 3,2% no grupo controle (p < 0,001). Os resultados mostram maior prevalência de alterações do ganho do reflexo vestíbulo-ocular (22,9%) e de sacadas corretivas (12,6%) no grupo otite média crônica em comparação ao grupo controle (p < 0,001). Apesar da maior prevalência de alterações de ganho, a média dos ganhos do reflexo vestíbulo-ocular dos grupos de otite média crônica estava dentro dos valores pré-definidos de normalidade; porém, a média do ganho do reflexo vestíbulo-ocular no canal semicircular anterior foi estatisticamente pior no grupo otite média crônica colesteatomatosa em comparação aos controles (p< 0,001). Em relação às sacadas corretivas, a prevalência de sacadas foi estatisticamente maior nos subgrupos otite média crônica supurativa e colesteatomatosa em comparação aos grupos não supurativa e controle (p = 0,004). Conclusão A otite média crônica se associa à maior prevalência de sintomas vestibulares e também maior prevalência de alterações no ganho e de sacadas corretivas em comparação a controles.

Issue: The findings in literature indicate inconsistency in the complications caused by the implant of electrodes in the cochlea; vestibular alterations and balance disorders are mentioned as the most likely. Purpose: To evaluate, in literature, through the results of multiple vestibular function tests, the effects of cochlear implant surgery on postural stability in adult patients and to analyze. Hypothesis: From the PICO strategy, where the Population focuses on adults, Intervention is cochlear implant surgery, Comparisons are between implanted patients, and Outcomes are the results of the assessment of cochlear function, the research question was formulated: Are there deficits in vestibular function in adults undergoing cochlear implant placement? Method: Systematic review based on cohort, case-control, and cross-sectional observational studies. Information sources: Databases between 1980 and 2021, namely, PubMed, Cinahl, Web Of Science, Cochrane, and Scopus. Search strategy using Mesh terms: "Adult," "Cochlear Implant," "Postural Balance," "Posturography," "Cochlear Implant," "Dizziness," "Vertigo," "Vestibular Functional Tests,"and "Caloric Tests." Populational inclusion criteria: studies with adult patients; intervention: cochlear implant placement surgery; comparison: analysis of a vestibular function with vestibular test results and pre- and postoperative symptoms; outcome: studies with at least one of the vestibular function tests, such as computerized vectoelectronystagmography (VENG), vestibular-evoked myogenic potentials (VEMPs), caloric test, video head impulse test (VHIT), head impulse test (HIT), videonystagmography, (VNG) and static and dynamic posturography. Exclusion criteria: studies without records of pre- and postoperative data collection and studies with populations under 18 years of age. Screening based on the reading of abstracts and titles was performed independently by two reviewers. In the end, with the intermediation of a third reviewer, manuscripts were included. Risk of bias analysis, performed by two other authors, occurred using the JBI "Critical Appraisal Checklist." Results: Of the 757 studies, 38 articles met the inclusion criteria. VEMP was the most commonly used test by the studies (44.7%), followed by the caloric test (36.8%) and vHIT (23.6%). Most studies performed more than one test to assess vestibular function. Conclusion: Among all vestibular tests investigated, the deleterious effects on vestibular function after cochlear implant surgery were detected with statistical significance (P < 0.05) using VEMP and caloric test. Comparing abnormal and normal results after implant surgery, the vestibular apparatus was evaluated as having abnormal results after cochlear implant surgery only in the VEMP test. Objectives The aim of this article was to evaluate the relationship between signal intensity of the labyrinth and vestibulocochlear function and morphologic features of vestibular schwannoma (VS). Design Cross-sectional Study. Setting Tertiary referral center. Participants Fifty-four patients with sporadic, untreated VS. Main Outcome Measure Signal intensity of the cochlea and vestibule (SIRc and SIRv: signal intensity of cochlea/vestibule compared with cerebellar signal intensity; AURc and AURv: SIRc/SIRv of the affected side compared with the unaffected side) in 1.5T T2-weighted images was correlated with size (Hannover classification), cystic status, distance from the fundus of the internal auditory canal, video head impulse test (vHIT), and audiometry. Results Signal intensity of the vestibule was higher than that of the cochlea ( p < 0.01). Large tumors had lower SIRc than smaller tumors ( p = 0.03); Hannover T1 tumors had higher SIRc ( p < 0.01), SIRv ( p < 0.01), AURc ( p < 0.01) and AURv ( p < 0.01) than the rest; heterogenous and cystic tumors had higher SIRv than solid large tumors ( p = 0.02); superior vestibular nerve pattern on vHIT had higher SIRv and AURv than inferior vestibular nerve and mixed patterns ( p = 0.03 and 0.004, respectively); and there was a weak correlation between AURv and speech discrimination ( r = 0.33, p = 0.04). Conclusion A more abnormal signal intensity of the labyrinth is associated with larger size and solid status of VS. There was a positive relationship between signal intensity of the labyrinth and speech discrimination scores on audiogram.

Estratégia de pesquisa: trata-se de uma revisão integrativa de literatura realizada entre julho e outubro de 2020, os artigos foram selecionados por meio das principais bases de dados da saúde MEDLINE via PubMed, LILACS via Portal Regional da BVS; Cochrane, Scopus, Web of Science, e CINAHL acesso via Portal CAPES, utilizando os descritores "Cognition" OR "Cognitive Dysfunction" AND "Vestibular Function Tests" AND "Vertigo" AND "Vestibular Rehabilitation" e seus correlatos em português e espanhol. Critérios de seleção: Foram incluídos artigos publicados até 2020 que investigaram a cognição com avaliação e reabilitação vestibular (tradicional e/ou com tecnologias) em indivíduos acima de 18 anos. Foram excluídos artigos que não possuíam texto completo disponível ou que utilizaram outra forma de tratamento. Resultados: dos 6965 artigos resultantes da busca inicial, 16 foram incluídos na presente revisão por satisfazerem os critérios de inclusão. Destes, 12 são estudos transversais, e quatro, estudos longitudinais. Conclusão: observou-se relação entre disfunção vestibular uni e bilateral com a memória de trabalho, funções executivas, navegação espacial e atenção. Nos estudos que realizaram a reabilitação vestibular encontrou-se melhora das habilidades cognitivas em geral, capacidade visuoespacial, atenção, funções executivas, memória de trabalho espacial, aumento do ganho do reflexo vestíbulo-ocular, do controle postural e uma diminuição do sofrimento psicológico.
Introduction: The literature reports an association between imbalance and cognitive impairment. However, it is not yet clear which cognitive skills are involved with the vestibular system. Objective: To evaluate which cognitive skills are involved in vestibular assessment and rehabilitation in young and older adults. Research strategy: This is an integrative review of the literature, conducted between July and October 2020. The articles were selected through search in the main health databases - MEDLINE via PubMed, LILACS via Regional Portal of VHL, Cochrane, Scopus, Web of Science, and CINAHL, accessed via Portal CAPES, using the following descriptors "Cognition" OR "Cognitive Dysfunction" AND "Vestibular Function Tests" AND "Vertigo" AND "Vestibular Rehabilitation", and their equivalent terms in Portuguese and Spanish. Selection criteria: Articles published until 2020, investigating cognition with vestibular assessment or traditional and/or technology rehabilitation in subjects aged 18 years or older were included. Articles not available in full text or that used other types of treatment were excluded. Results: 16 out of the 6,965 articles initially retrieved met the inclusion criteria and were included in this review; 12 of them are cross-sectional, and four longitudinal studies. Conclusion: There was a relationship between uni- and bilateral vestibular dysfunction and working memory, executive functions, spatial navigation, and attention. The studies that conducted vestibular rehabilitation found improved overall cognitive skills, visuospatial capacity, attention, executive functions, spatial working memory, increased vestibulo-ocular reflex, postural control gains, and diminished psychological suffering.

Conclusión: hubo relación entre la disfunción vestibular uni y bilateral con la memoria de trabajo, funciones ejecutivas, navegación espacial y atención. En estudios que realizaron rehabilitación vestibular se encontró una mejora en las habilidades cognitivas en general, capacidad visuoespacial, atención, funciones ejecutivas, memoria de trabajo espacial, aumento de ganancia en el reflejo vestibular-ocular, control postural y una disminución del malestar psicológico.

OBJECTIVE: To assess the balance sensory organization among patients with migraine, considering the influence of migraine subdiagnosis, otoneurological function, falls, and psychosocial factors. BACKGROUND: Migraine has been associated with vestibular symptoms and balance dysfunction; however, neither comprehensive balance assessment nor associated factors for greater impairment have been addressed thus far. METHODS: Patients from a tertiary headache clinic with a diagnosis of episodic migraine with aura (MWA), without aura (MWoA), and chronic migraine (CM) were included for this cross-sectional study (30 patients per group). Thirty headache-free controls (CG) were recruited. Participants underwent a comprehensive evaluation protocol, including the Sensory Organization Test (SOT) and otoneurological examination. Questionnaires about fear of falls, dizziness disability, and kinesiophobia were administered. RESULTS: All migraine groups presented lower composite SOT scores than controls (CG: 82.4 [95% confidence interval (CI): 79.5-85.3], MWoA: 76.5 [95% CI: 73.6-79.3], MWA: 66.5 [95% CI: 63.6-69.3], CM: 69.1 [95% CI: 66.3-72.0]; p < 0.0001). Compared to controls and to MWoA, MWA and CM groups exhibited greater vestibular (CG: 75.9 [95% CI: 71.3-80.4], MWoA: 67.3 [95% CI: 62.7-71.8], MWA: 55.7 [95% CI: 51.2-60.3], CM: 58.4 [95% CI: 53.8-63.0]; p < 0.0001) and visual functional impairment (CG: 89.6 [95% CI: 84.2-94.9], MWoA: 83.2 [95% CI: 77.9-88.6], MWA: 68.6 [95% CI: 63.3-74.0], CM: 71.9 [95% CI: 66.5-77.2], p < 0.0001). Fall events during the assessment were documented more often among patients with migraine (CG: 0.0, interquartile range [IQR], 0.0, 0.0); MWoA: 1.0 [IQR: 1.0, 1.0], MWA: 2.0 [IQR: 1.8, 4.3], CM: 1.0 [IQR: 1.0, 2.0]; p = 0.001). The SOT scores correlated with fear of falls (r = -0.44), dizziness disability (r = -0.37), kinesiophobia (r = -0.38), and migraine frequency (r = -0.38). There was no significant influence of the vestibular migraine diagnosis in the study outcomes when used as a covariate in the analysis (composite score [F = 3.33, p = 0.070], visual score [F = 2.11, p = 0.149], vestibular score [F = 1.88, p = 0.172], somatosensory score [F = 0.00, p = 0.993]). CONCLUSIONS: Aura and greater migraine frequency were related to falls and balance impairment with sensory input manipulation, although no otoneurological alterations were detected. The diagnosis of vestibular migraine does not influence the balance performance. The vestibular/visual systems should be considered in the clinical examination and treatment of patients with migraine.

INTRODUCTION: In recent years, scientific evidence has shown that chronic otitis media may cause balance and vestibular dysfunction. OBJECTIVE: To compare the results of the video head impulse test (gain and symmetry of the vestibulo-ocular reflex and presence of covert and overt saccades) in patients with chronic otitis media and controls. METHODS: Cross-sectional study of patients with chronic otitis media (study group), aged between 18 and 60 years. The patients in the study group were further divided according to the chronic otitis media type as (1) non-suppurative, (2) suppurative, and (3) cholesteatomatous. For the comparative analysis, we selected volunteers with no history of ear and vestibular diseases (control group), who met the same inclusion and exclusion criteria as the study group. Patients in both groups underwent a video head impulse test. RESULTS: The study group consisted of 96 volunteers, and the control group of 61 individuals. The prevalence of vestibular symptoms was 66% in the study group and 3.2% in the control group (pâ¯<⯠0.001). The results show a higher prevalence of changes in the vestibulo-ocular reflex gain (22.9%) and corrective saccades (12.6%) in the chronic otitis media group compared to the control group (pâ¯<⯠0.001). Despite the higher prevalence of changes in gain, the average vestibulo-ocular reflex gains in the chronic otitis media groups were within the pre-defined values ââof normality; however, the mean vestibulo-ocular reflex gain in the anterior semicircular canal was statistically worse in the cholesteatomatous chronic otitis media group compared to controls (pâ¯<⯠0.001). Regarding the corrective saccades, the prevalence of saccades was statistically higher in the suppurative and cholesteatomatous chronic otitis media subgroups compared to the non-suppurative and control groups (pâ¯=⯠0.004). CONCLUSION: The present study demonstrated that chronic otitis media is associated with a higher prevalence of vestibular symptoms and also a higher prevalence of changes in gain and corrective saccades when compared to controls.

Contexto: Las pérdidas auditivas en adolescentes se acompañan de desórdenes vestibulares desapercibidos que impactan el desarrollo motor e influyen en el aprendizaje. La identificación temprana y el conocimiento sobre la neuromaduración vestibular arroja alternativas para mejorar el rendimiento académico. Objetivos: caracterizar los resultados de la posturografía en adolescentes con pérdida auditiva severa a profunda. Método: estudio cuantitativo, con técnica descriptiva, buscando relaciones entre variables estudiadas. Población: 38 adolescentes con pérdida auditiva neurosensorial severa a profunda, en grupos de 12 15 y 16 - 19 años. Resultados: los adolescentes mayores presentaron un menor movimiento oscilatorio. Con antecedentes del desarrollo evidenciaron desempeño inferior en la posturografía y mayores dificultades en pensamiento cuantitativo, lenguaje y comunicación. Discusión: Los antecedentes neuromadurativos son importantes para el desarrollo del balance y el equilibrio. No se registraron relaciones directas entre la posturografía con bajo rendimiento académico, sin embargo, fue de utilidad clínica aportando datos del desempeño sensorial, motor y vestibular, para la intervención. Conclusiones: Los resultados sugieren efecto indirecto sobre el rendimiento académico verbal. La integración de los sistemas sensoriomotor, visual, vestibular y cognitivo, es clave para el desarrollo de habilidades de aprendizaje. La maduración es un proceso activo en el paso de la adolescencia a la edad adulta. La relación entre el déficit vestibular y los antecedentes del desarrollo, como nacimiento prematuro o edad de inicio de marcha, amerita identificar factores de riesgo del sistema vestibular en la primera infancia. El entorno socioafectivo, la familia y la convivencia con compañeros son importantes para el desempeño académico.
Context: Hearing loss in adolescents is accompanied by unnoticed vestibular disorders that impact motor development and influence learning. Early identification and knowledge about vestibular neuromaturation provides alternatives to improve academic performance. Objectives: to characterize the results of posturography in adolescents with severe to profound hearing loss. Method: quantitative study, with descriptive technique, searching for relationships between studied variables. Population: 38 adolescents with severe to profound sensorineural hearing loss, in groups of 12 - 15 and 16 - 19 years old. Results: older adolescents presented less oscillatory movement. With a developmental history, they showed lower performance in posturography and greater difficulties in quantitative thinking, language and communication. Discussion: neuromadurative antecedents are important for the development of balance and equilibrium. No direct relationships were recorded between posturography with low academic performance, however, it was clinically useful providing sensory, motor and vestibular performance data, for intervention. Conclusions: The results suggest indirect effect on verbal academic performance. The integration of the sensorimotor, visual, vestibular and cognitive systems is key to the development of learning skills. Maturation is an active process in the transition from adolescence to adulthood. The relationship between vestibular deficit and developmental history, such as premature birth or age of onset of walking, merits identifying risk factors of the vestibular system in early childhood. The socio-affective environment, the family and living with peers are important for academic performance.

RESUMO Objetivos verificar a aplicabilidade do Video Head Impulse Test (vHIT) em doenças do sistema nervoso central (SNC), bem como os resultados encontrados e as doenças descritas. Estratégia de pesquisa revisão integrativa da literatura, em que foi realizada a busca em nove bases eletrônicas de dados, a partir da palavra-chave "video head impulse test". Critérios de seleção foram incluídos estudos que utilizaram o vHIT no diagnóstico de doenças do SNC e excluídos os estudos publicados antes de 2009 e estudos que realizaram outros procedimentos de investigação clínica, ou que aplicaram o teste no diagnóstico de doenças vestibulares periféricas. Resultados a amostra final foi composta por 18 estudos. Os resultados verificados mostraram que o reflexo vestíbulo-ocular (RVO) tem apresentado alterações na população investigada. Foram observados achados sugestivos de acometimento central, tais como ganho ou média de ganho do RVO nos canais semicirculares verticais, inferior aos laterais, ganho aumentado, correlação negativa do ganho com a gravidade da doença na ataxia espinocerebelar tipo 3, ponto de corte de 0,70 e assimetria de ganho menor de 20% para diferenciar neurite vestibular de derrame no ramo medial da artéria cerebelar posteroinferior, ganho normal com provas oculomotoras alteradas, presença de nistagmo espontâneo vertical, além de alterações no RVO com e sem otimização visual, na perseguição sacádica e no teste de desvio de inclinação. Conclusão verificou-se que o vHIT é aplicável quanto a avaliação do RVO de alta frequência em indivíduos com doenças do SNC, uma vez que trouxe evidências clínicas sobre alterações da função vestibular periférica e central nos diferentes quadros neurológicos.
ABSTRACT Purpose To verify the applicability of the Video Head Impulse Test (vHIT) in central nervous system (CNS) diseases, as well as the results found and the diseases described. Research strategy Integrative literature review, in which nine electronic databases were searched using the keyword "video head impulse test". Selection criteria Studies that used the vHIT in the diagnosis of CNS diseases were included, and studies published before 2009, studies that performed other clinical investigation procedures or that concerned the diagnosis of peripheral vestibular diseases were excluded. Results The final sample consisted of 18 studies. The verified results show that the vestibulo-ocular reflex (VOR) has shown alteration in this population. Suggestive findings of central involvement were observed, such as lower gain or average VOR in the vertical semicircular canals than in the lateral ones, increased gain, the negative correlation of gain with disease severity in Spinocerebellar Ataxia Type 3, cutoff point of 0.70, and gain asymmetry of less than 20% to differentiate vestibular neuritis from a stroke in the medial branch of the posteroinferior cerebellar artery, normal gain with altered oculomotor tests, presence of spontaneous vertical nystagmus, as well as alterations in the VOR with and without visual enhancement, in saccadic pursuit, and the tilt deviation test. Conclusion We found that the vHIT applies to the assessment of high-frequency VOR in individuals with CNS diseases since it provided clinical evidence of changes in peripheral and central vestibular function in different neurological conditions

Abstract Introduction: Vestibular migraine as an entity was described in 1999 and its pathophysiology is still not established. Simultaneously with research to better understand vestibular migraine, there has been an improvement in vestibular function assessment. The video-head impulse test is one of the latest tools to evaluate vestibular function, measuring its vestibular-ocular reflex gain. Objective: To evaluate vestibular function of vestibular migraine patients using video-head impulse test. Methods: Cross-sectional case-control study homogeneous by age and gender with vestibular migraine patients according to the 2012-2013 Barany Society/International Headache Society diagnostic criteria submitted to video-head impulse test during intercrisis period. Results: 31 vestibular migraine patients were evaluated with a predominantly female group (90.3%) and mean age of 41 years old. Vestibular function was normal in both patient and control groups. Gain values for horizontal canals were similar between the two groups, but gain values for vertical canals were higher in the group with vestibular migraine (p < 0.05). Patients with vestibular migraine felt more dizziness while performing the video-head impulse test than control subjects (p < 0.001). Conclusions: Patients with vestibular migraine present normal vestibular function during intercrisis period when evaluated by video-head impulse test. Vertical canals, however, have higher gains in patients with vestibular migraine than in control subjects. Vestibular migraine patients feel dizziness more often while conducting video-head impulse test.
Resumo Introdução: A enxaqueca vestibular foi descrita como uma entidade em 1999 e sua fisiopatologia ainda não está estabelecida. Simultaneamente às pesquisas para entender melhor essa condição clínica, houve uma melhoria na avaliação da função vestibular. O teste de impulso cefálico por vídeo, vHIT, é uma das mais recentes ferramentas para avaliar a função vestibular, mede seu ganho do reflexo vestíbulo-ocular. Objetivo: Avaliar com o vHIT a função vestibular de pacientes com enxaqueca vestibular. Método: Estudo transversal de caso-controle, homogêneo em relação a idade e sexo, de pacientes com enxaqueca vestibular, de acordo com os critérios de diagnóstico da Barany Society/International Headache Society de 2012-2013, submetidos ao vHIT durante o período intercrises. Resultados: Foram avaliados 31 pacientes com enxaqueca vestibular com um grupo predominantemente feminino (90,3%) e média de 41 anos. A função vestibular era normal nos grupos paciente e controle. Os valores de ganho para canais horizontais foram semelhantes entre os dois grupos, mas os valores de ganho para canais verticais foram maiores no grupo com enxaqueca vestibular (p < 0,05). Pacientes com enxaqueca vestibular sentiram mais tontura ao fazer o vHIT do que os controles (p < 0,001). Conclusões: Pacientes com enxaqueca vestibular apresentam função vestibular normal durante o período intercrises quando avaliados pelo vHIT. Os canais verticais, no entanto, apresentam ganhos maiores em pacientes com enxaqueca vestibular do que em indivíduos controle. Pacientes com enxaqueca vestibular sentem tontura com maior frequência enquanto fazem o vHIT.

Introdução: os potenciais evocados miogênicos vestibulares (VEMP) são respostas eletrofisiológicas que conseguem fornecer informações dos órgãos otolíticos sáculo, utrículo e do nervo vestibular. O VEMP é um exame complementar à avaliação vestibular, consistindo num exame rápido, de fácil aplicação e objetivo. Objetivo: analisar os parâmetros de latência, amplitude, limiar e índice de assimetria das respostas do VEMP cervical (cVEMP) e ocular (oVEMP) de indivíduos sem queixas vestibulares. Métodos: estudo transversal realizado com 53 indivíduos de ambos os sexos, sem queixas auditivas e vestibulares. Resultados: encontrou-se simetria de respostas nas latências, amplitudes e limiares de respostas do exame cVEMP. Entretanto, verificou-se diferença entre orelhas da latência P15 do exame oVEMP, sendo maior à direita no sexo feminino. Conclusão: Encontrou-se simetria nas respostas de todos os parâmetros avaliados do cVEMP. Houve assimetria apenas na latência de P15 do oVEMP no sexo feminino. Os limiares de resposta encontrados nos exames cVEMP e oVEMP foram iguais ou maiores que 75 dBNA.
Introduction: Vestibular evoked myogenic potentials (VEMP) are electrophysiological responses that can provide information on the otolithic organs saccule, utricle and of the vestibular nerve. VEMP is a complementary exam to the vestibular assessment; it is a quick exam, easy to apply and objective. Purpose: to analyze the parameters of latency, amplitude, threshold and asymmetry index of the cervical (cVEMP) and ocular VEMP (oVEMP) responses of individuals without vestibular complaints. Methods: cross-sectional study carried out with 53 individuals of both genders without hearing and vestibular complaints. Results: response symmetry was found in the latencies, amplitudes and thresholds of cVEMP test responses. However, there was a difference between the ears of the P15 latency of the oVEMP exam, and this was greater on the right ear in females. Conclusion: symmetry was found in the responses of all cVEMP evaluated parameters. There was asymmetry in oVEMP P15 latency only in female patients. The response thresholds found in the cVEMP and oVEMP tests were equal or greater than 75 dBHL.
Introducción: los potenciales miogénicos evocados vestibulares (VEMP) son respuestas electrofisiológicas que pueden proporcionar información sobre los órganos otolíticos el sáculo, el utrículo y el nervio vestibular. El VEMP es un examen complementario a la evaluación vestibular; es un examen rápido, fácil de aplicar y objetivo. Objetivo: analizar los parámetros de latencia, amplitud, umbral e índice de asimetría de las respuestas VEMP cervical (cVEMP) y ocular (oVEMP) de individuos sin quejas vestibulares. Métodos: estudio transversal realizado con 53 individuos de ambos sexos, sin quejas auditivas y vestibulares. Resultados: Se encontró simetría de respuestas en las latencias, amplitudes y umbrales de respuestas en el examen cVEMP. Sin embargo, hubo una diferencia entre los oídos de la latencia P15 del examen oVEMP, siendo mayor a la derecha en el sexo femenino . Conclusión: se encontró simetría en las respuestas de todos los parámetros evaluados de cVEMP. Hubo asimetría solo en la latencia P15 de oVEMP en el sexo femenino. Los umbrales de respuesta encontrados en las pruebas cVEMP y oVEMP fueron iguales o superiores a 75 dBHL.
