Introdução: O câncer de pulmão é uma doença grave, sendo a segunda maior causa de morte em todo o mundo, entretanto, em alguns países desenvolvidos, tornou-se já a primeira causa de morte. Cerca de 90% dos casos de neoplasia pulmonares são causados pela inalação da fumaça do cigarro. Objetivo: Correlacionar a prevalência de tabagismo e morbimortalidade por câncer de pulmão nos estados brasileiros, além de demonstrar a associação destes com sexo e faixa etária. Métodos: Estudo de caráter ecológico acerca da prevalência de tabagismo e morbimortalidade por câncer de pulmão nos estados brasileiros, nos períodos de 2013 e 2019, dividida por sexo e faixa etária. Foram utilizados bancos de coleta de dados como o Tabnet e Pesquisa Nacional de Saúde. Resultados: As maiores taxas de mortalidade e internações hospitalares foram do público masculino, em 2013, com taxa de 2,7 e 10, respectivamente, e em 2019 com 3,3 e 11,9, respectivamente. Ademais, a maior prevalência de tabagismo foi encontrada nos homens; entretanto seu índice tem caído, enquanto a quantidade de mulheres tabagistas tem aumentado. A Região Sul demonstrou maiores números de mortalidade em ambos os períodos estudados, com taxas de 4,9 e 5,8 por 100 mil habitantes, e morbidade hospitalar com 19,9 e 23,5 por 100 mil habitantes. Já a Região Norte se configurou com as menores prevalências: em 2013 apresentou taxa de óbito por câncer de pulmão de 1,0 e morbidade hospitalar de 3,5/100 mil habitantes, em 2019 apresentou taxa de mortalidade de 4,6 e internações de 1,6/100 mil habitantes. Os coeficientes de correlação de morbidade hospitalar e prevalência de tabagismo foram R2=0,0628, r=0,251 e p=0,042, enquanto os de mortalidade e prevalência de tabagismo foram R2=0,0337, r=0,183 e p=0,140. Conclusões: Na presente pesquisa, pode-se inferir que houve associação positiva na comparação entre taxa de morbidade hospitalar e prevalência de tabagismo; em contrapartida, não foi possível observar associação positiva na correlação da taxa de mortalidade por câncer de pulmão e prevalência de tabagismo.
Introduction: Lung cancer is a serious disease, being the second leading cause of death worldwide. Moreover, in some developed countries, it has already become the leading cause of death. About 90% of lung cancer cases are caused by cigarette smoking. Objective: To correlate the prevalence of smoking and lung cancer morbidity and mortality in Brazilian states, and to demonstrate their association with sex and age group as well. Methods: An ecological study on the prevalence of smoking and lung cancer morbidity and mortality in Brazilian states between 2013 and 2019, divided by sex and age group. The data collection databases Tabnet and National Health Survey were used. Results: The highest rates of mortality and hospital admissions were among men, in 2013 with a rate of 2.7 and 10, respectively, and in 2019 with 3.3 and 11.9, respectively. In addition, the highest prevalence of smoking was found in men, but this rate has fallen, while the number of women smokers has increased. The South region showed higher mortality rates in both periods studied, with rates of 4.9 and 5.8 per 100,000 inhabitants, and hospital morbidity with 19.9 and 23.5 per 100,000 inhabitants. The North region had the lowest prevalence, where in 2013, it had a death rate from lung cancer of 1.0 and hospital morbidity of 3.5/100 thousand inhabitants, and where in 2019, it had a mortality rate of 4.6 and hospitalizations of 1.6/100 thousand inhabitants. The correlation coefficients for hospital morbidity and smoking prevalence were R2=0.0628, r=0.251 and p=0.042, while for mortality and smoking prevalence, these were R2=0.0337, r=0.183 and p=0.140. Conclusions: In the present study, it can be inferred that there was a positive association between hospital morbidity rate and prevalence of smoking, while it was not possible to observe a correlation between lung cancer mortality rate and prevalence of smoking.

Objetivo: Correlacionar la prevalencia de tabaquismo y la morbimortalidad por cáncer de pulmón en los estados brasileños, además de demostrar la asociación de estos con el género y el grupo de edad. Métodos: estudio ecológico sobre la prevalencia de tabaquismo y morbimortalidad por cáncer de pulmón en los estados brasileños, dentro de los períodos 2013 y 2019, divididos por sexo y grupo de edad. Se utilizaron bancos de recogida de datos como Tabnet y la Encuesta Nacional de Salud. Resultados: las mayores tasas de mortalidad e ingresos hospitalarios se dieron en el público masculino, en 2013 con una tasa de 2,7 y 10, respectivamente, y en 2019 con 3,3 y 11,9, respectivamente. Además, la mayor prevalencia del tabaquismo se encontró en los hombres, sin embargo, su tasa ha disminuido, mientras que la cantidad de mujeres fumadoras ha aumentado. La región Sur presentó cifras más altas de mortalidad en ambos periodos estudiados, con tasas de 4,9 y 5,8 por 100.000 habitantes, y de morbilidad hospitalaria con 19,9 y 23,5 por 100.000 habitantes. INTRODUCTION: Heated tobacco products (HTPs) are promoted as less harmful than cigarettes; nonetheless, whether HTPs help smokers quit is uncertain. METHODS: Data from 4067 Mexican adult smokers surveyed longitudinally every four months (November 2019-March 2021) were analyzed. Mixed-effects multinomial models regressed HTP use frequency (no use=reference; monthly; weekly; and daily use) on sociodemographics and tobacco/nicotine-related variables. Among participants who completed at least two surveys (n=2900) over four months, the duration of their longest smoking quit attempt (SQA) between surveys (SQAs: <30 days; ≥30 days; no SQA=reference) was regressed on HTP use frequency, and changes in the number of cigarettes smoked per day were regressed on HTP initiation between surveys, adjusting for covariates. RESULTS: Consistent predictors of all HTP use frequencies (monthly, weekly, or daily vs no use) were daily smoking >5 cigarettes (ARRR=1.69 [95% CI: 1.12-2.55], 1.88 [95% CI: 1.26-2.81] and 6.46 [95% CI: 3.33-12.52], respectively); e-cigarette use (ARRR =5.68 [95% CI: 3.38-9.53], 6.54 [95% CI: 4.06-10.55] and 2.59 [95% CI: 1.26-5.30]); lower HTP risk perceptions (ARRR=2.12 [95% CI: 1.50-30.00], 2.25 [95% CI: 1.63-3.10] and 2.00 [95% CI: 1.25-3.22]); exposure to HTP information inside (ARRR=2.13 [95% CI: 1.44-3.15], 2.13 [95% CI: 1.49-3.05] and 3.72 [95% CI: 2.28-6.09]) and outside stores (ARRR=2.36 [95% CI: 1.56-3.57], 2.32 [95% CI: 1.65-3.25] and 2.44 [95% CI: 1.41-4.24]) where tobacco is sold; having family (ARRR=2.46 [95% CI: 1.54-3.91], 2.90 [95% CI: 1.93-4.37] and 2.96 [95% CI: 1.52-5.77]) and friends (ARRR=5.78 [95% CI: 3.60-9.30], 4.98 [95% CI: 3.22-7.72] and 6.61 [95% CI: 2.91-15.01]) who use HTPs. HTP use frequency was not associated with quit attempts, except for monthly HTP use predicting SQAs lasting ≥30 days (ARRR=2.12 [95% CI: 1.17-3.85]). Initiation of HTP use was not associated with changes in smoking frequency. Limiting analysis to those who intend to quit smoking also yielded null results. CONCLUSIONS: Among Mexican adult smokers, frequency of HTP use was mostly not associated with either cessation behaviors or changes in cigarette consumption, suggesting that HTPs have limited to no effectiveness for smoking cessation.

Resumo O estudo objetiva comparar o consumo de diferentes produtos do tabaco entre os escolares adolescentes no Brasil em 2015 e 2019 e identificar os fatores associados ao seu uso. Estudo transversal com dados da Pesquisa Nacional de Saúde do Escolar (PeNSE) de 2015 e 2019. Variáveis: uso atual de cigarro, uso de outros produtos do tabaco e uso de qualquer produto do tabaco. Foi usado o teste do Qui-quadrado de Pearson para verificar associação entre as variáveis, realizada análise bivariada e a multivariada por meio da regressão logística. O uso de cigarros se manteve estável entre 2015 (6,6%) e 2019 (6,8%). Mas houve aumento do uso de qualquer produto do tabaco (de 10,6% em 2015 para 14,8% em 2019), sendo o narguilé o mais frequente (7,8%) seguido do cigarro eletrônico (2,8%). O uso de cigarro foi mais elevado entre os adolescentes de 16 e 17 anos, com cor da pele preta e parda, que faltaram as aulas sem autorização, entre aqueles que relataram não ter amigos, que apresentavam outros fatores de risco como consumir bebidas alcoólicas e que eram fumantes passivos. A prevalência de tabagismo aumentou ao longo dos anos e foi associada com aspectos sociodemográficos e a outros comportamentos de risco à saúde, o que alerta para a necessidade de ações de promoção da saúde ao longo do ciclo de vida.
Abstract This cross-sectional study used data from Brazil's National Student Health Survey (PeNSE), from 2015 and 2019, to compare consumption of tobacco products among adolescent students in Brazil and identify associated factors. The study variables were current cigarette smoking, use of other tobacco products and use of any tobacco product. Pearson's Chi-square test was used to ascertain associations between the variables; bivariate and multivariate analyses were performed using logistic regression. Cigarette smoking remained stable between 2015 (6.6%) and 2019 (6.8%), but use of any tobacco product increased (from 10.6% in 2015 to 14.8% in 2019), involving particularly hookahs (7.8%) and e-cigarettes (2.8%). Cigarette smoking was greater among adolescents aged 16 and 17, whose skin colour was black or brown, who missed classes without permission, who reported having no friends, displayed other risk factors, such as drinking alcoholic beverages, or who were passive smokers. The prevalence of smoking has increased over the years and is associated with sociodemographic aspects and other health risk behaviour, highlighting the need for lifelong health promotion actions.

Abstract Objective: To determine whether being a smoker and the years of smoking correlate with the presence and degree of meniscal injury. Materials and Methods: Individuals who underwent magnetic resonance imaging of the knee were divided into two groups: smokers and nonsmokers. For each smoker, the total smoking history was calculated by multiplying the daily consumption (packs/day) by the years of smoking, and the result is expressed as pack-years. In the evaluation of meniscal injury, the grade of injury was recorded. The thickness of the subcutaneous adipose tissue, as an indicator of obesity, was measured at the medial knee on axial plane images. The relationships that smoking and obesity had with meniscal injury were analyzed statistically. Results: A total of 156 individuals were included in the study. The smoker group consisted of 48 individuals (30.8%), and the nonsmoker group consisted of 108 (69.2%). The meniscus was normal in one (2.1%) of the smokers and in 32 (29.6%) of the nonsmokers (p < 0.0001). The median subcutaneous adipose tissue thickness was 23 mm and 24 mm in the smokers and nonsmokers, respectively (p = 0.900). A moderate but statistically significant correlation was observed between packs/day and injury grade, as well as between pack-years and injury grade (r = 0.462, p = 0.001 and r = 0.523, p = 0.001, respectively). Smoking and age significantly increased the risk of meniscal injury, by 31.221 times (p = 0.001) and 1.076 times (p < 0.001), respectively. Conclusion: Our findings indicate that current smoking and smoking history correlate significantly with meniscal injury grade.

O consumo total de cigarros dos pacientes foi calculado multiplicando-se a quantidade diária de cigarros (maços/dia) pelo tempo de tabagismo (anos). O grau mais alto de lesão foi registrado na avaliação da lesão meniscal. A espessura do tecido adiposo subcutâneo foi medida com base em imagem do joelho em plano axial, como indicador de obesidade. A relação entre tabagismo, obesidade e lesão meniscal foi analisada estatisticamente. Resultados: Foram incluídos no estudo 156 indivíduos. O grupo de fumantes consistiu de 48 (30,8%) indivíduos e o grupo de não fumantes, de 108 (69,2%) indivíduos. O menisco estava normal em um (2,1%) fumante e em 32 (29,6%) não fumantes (p < 0,0001). We provide an extensive review of 17 independent and industry-funded studies targeting carbonyls in aerosol emissions of Heated Tobacco Products (HTPs), focusing on quality criteria based on the reproducibility of experiments, appropriate analytic methods, and puffing regimes. Most revised studies complied with these requirements, but some were unreproducible, while others failed to consider analytical variables that may have affected the results and/or produced unrealistic comparisons. We also provide a review of the literature on the physicochemical properties of heated tobacco and HTP aerosols, as well as the evaluation of HTPs by regulatory agencies, addressing various critiques of their relative safety profile. The outcomes from the revised studies and regulatory evaluations tend to agree with and converge to a general consensus that HTP aerosols expose users to significantly lower levels of toxicity than tobacco smoke.

OBJECTIVE: To explore new evidence on illicit practices, such as selling legal brands below the minimum legal price (MLP), and smugglers selling illicit brands at or above the MLP. METHODS: For the first time in Brazil, self-reported information on cigarette brand name and price paid per pack in smokers' last purchase from a nationally representative survey conducted in 2019 was used to distinguish the illicit and the legal markets. We estimated the proportion of illicit cigarette consumption, using the combination of brand and price. RESULTS: The proportion of smuggled illicit cigarette consumption based on brands not approved on the Brazilian market was estimated at 38.6% (95% CI: 35.8% to 41.5%). When we added legal brands not paying taxes, it increased to 47.1% (95% CI: 44.2% to 49.9%). Around 25% of illicit brand cigarettes were sold at or above MLP. CONCLUSIONS: In Brazil, since 2017 there is a lack of adjustment in tobacco taxes and the MLP for inflation and income growth. The increase in cigarette affordability and the presence on the market of a segment of 'higher-priced' illicit brands suggest patterns of illicit brand loyalty and/or perceived 'brand quality' among smokers of illicit cigarettes. The evidence also shows that a sizeable proportion of legal brand cigarettes were sold below the MLP. This study offers insight into what happened in circumstances in which a government failed to keep current with tax policies and the monitoring of domestic manufacturing. Brazil has been a world leader in the monitoring of the tobacco epidemic, and this study also offers an innovative use of data that an increasing number of countries are collecting.

BACKGROUND: Straw cigarettes are hand-rolled tobacco in a corn straw, common in Brazil and believed to be less harmful by consumers. Traditionally consumed in rural areas and by men, a recent increase in use among young people in urban areas was observed. This study assessed marketing appeals of straw cigarette packaging in Brazil. METHODS: Following a systematic protocol, unique straw cigarette packs were purchased in 2013, 2016 and 2019 from three cities: Manaus, Salvador and São Paulo. Packs were double-coded for the absence/presence of marketing appeals (ie, luxury/quality, masculinity, classic/timeless, flavours and less harm). RESULTS: The number of unique straw cigarette packs purchased as part of the overall sample increased in the most recent collection: 2.3% (n=3), 1.3% (n=2) and 15.5% (n=27) in 2013, 2016 and 2019, respectively. Across the years, all packs presented luxury/quality elements (eg, images of crests); 93.8% (n=30) included the word 'artisanal'. Masculine appeal (eg, images of men) was present on 56.3% (n=18) of packs. Almost half had classic/timeless elements, including the words 'tradition'/'original'. In 2019, 59.3% (n=16) of the packs were flavoured compared with none in previous years. Over a quarter of all packs presented a less harm appeal (eg, 'smooth'). CONCLUSION: We identified an increase in availability of unique straw cigarette packs, strong branding and use of misleading descriptors on the packs. The adoption of plain packaging and the implementation of Brazil's additive ban could help to reduce the appeal of straw cigarettes and curb the current increase in use among youth.

O objetivo deste trabalho foi investigar as alterações nas propriedades ópticas de cor e de luz transmitida e na rugosidade de superfície do polímero constituinte de alinhadores transparentes após a exposição à fumaça de cigarro. A amostra foi constituída de 20 superfícies das faces vestibulares de incisivos centrais superiores obtidas através de 10 alinhadores de alinhadores ortodônticos (Invisalign®). As amostras foram divididas igual (n=10) e aleatoriamente em 2 grupos: grupo controle (GC) e grupo experimental (GE). Para a exposição do GE à fumaça de cigarro foi utilizado um dispositivo de acrílico hermeticamente fechado, onde as amostras foram expostas a 21 ciclos de fumaça e posteriormente armazenadas em saliva artificial à 37ºC por 15 dias. O GC foi exposto somente à saliva artificial nas mesmas condições, pelo mesmo período. Para avaliar as mudanças de cor e transmitância foi utilizado um espectrofotômetro calibrado pelo sistema cores CIE (L*a*b*) e utilizado o National Bureal of Standards (NBS) para classificação clínica da alteração de cor. A diferença total de cor (∆E) e o NBS entre os grupos foi calculado. Foram feitas medidas de cor e transmitância inicial (T0), ou seja, antes da exposição à fumaça de cigarro e à saliva artificial e final (T1), registradas após a exposição ao cigarro e à saliva. A rugosidade foi avaliada através do rugosímetro digital e foram medidas a rugosidade em T0 e T1. Foram utilizados os parâmetros de rugosidade Ra (rugosidade média) e Rz (rugosidade de profundidade média). Para comparar os valores dos parâmetros de cor, transmitância e rugosidade entre GC e GE foi utilizado o teste t para amostras independentes. Para comparar os valores dos parâmetros analisados intragrupo ao longo do tempo (T0 e T1) foi utilizado o teste t para amostras pareadas. Em todas as análises foi estabelecido um nível de significância de 0,05. Para todos os parâmetros de cor em T1 houve diferença (p<0,05) quando comparados GC (L: 87,400 ± 0,681; a: 0,746 ± 0,094; b: 3,393 ± 0,171) com GE (L: 77,208 ± 3,337; a: 1,576 ± 0,025; b: 6,747 ± 0,557). O ΔE do GC (2,39) e GE (14,27) apresentaramse diferentes (p=0,002), ao converter tais valores para a escala NBS, resultaram em um valor de 2,20 e 13,13. Ao comparar os grupos houve diminuição da transmitância (p=0,014) sendo de maior magnitude no GE. Em T1 entre GC e GE houve diferença (p=0,046) para Ra (GC: 0,122 ± 0,002; GE: 0,139 ± 0,007), sendo maiores os valores no GE. Para Rz, quando comparados GC (0,616 ± 0,021) e GE (0,741 ± 0,026), em T1, também houve diferença (p=0,002), sendo também maiores os valores encontrados no GE. No GC não houve diferença estatisticamente significativa de Ra (p=0,807) e Rz (p=0,231) quando comparadas as amostras ao longo do tempo T0 e T1. No GE ocorreu tanto aumento de Ra (p=0,003) e de Rz (p=0,014). Conclui-se, portanto, que os alinhadores se tornaram mais escuros, amarelados, diminuíram sua transmissão de luz, tornando-se opacos e tiveram sua rugosidade superficial aumentada, tanto no parâmetro Ra quanto Rz. The aim of this study was to investigate the changes in the optical properties of color and transmitted light and in the surface roughness of the constituent polymer of clear aligners after exposure to cigarette smoke. The sample consisted of 20 surfaces of the buccal surfaces of the upper central incisors of aligners (Invisalign®). The samples were divided equally (n=10) and randomly into 2 groups: control group (CG) and experimental group (EG). For the exposure of the EG to cigarette smoke, a hermetically closed acrylic device was used, where the samples were exposed to 21 smoke cycles and later stored in artificial saliva at 37ºC for 15 days. The CG was only exposed to artificial saliva under the same conditions, for the same period. To evaluate changes in color and transmittance, a spectrophotometer calibrated by the CIE color system (L*a*b*) was used and the National Bureau of Standards (NBS) was used for clinical classification of the color change. The total difference in color (∆E) and NBS between groups was calculated. Initial (T0) color and transmittance measurements were taken, that is, before exposure to cigarette smoke and artificial saliva, and final (T1), recorded after exposure to cigarettes and saliva. Roughness was evaluated using a digital roughness meter and roughness was measured at T0 and T1. The roughness parameters Ra (mean roughness) and Rz (mean depth roughness) were used. To compare the values of color, transmittance and roughness parameters between GC and EG, the t test for independent samples was used. To compare the values of the parameters analyzed within the group over time (T0 and T1), the t test for paired samples was used. In all analyses, a significance level of 0.05 was established. There was a difference (p<0.05) for all color parameters in T1 when comparing GC (L: 87.400 ± 0.681; a: 0.746 ± 0.094; b: 3.393 ± 0.171) with GE (L: 77.208 ± 3.337; a: -1.576 ± 0.025; b: 6.747 ± 0.557). The ΔE of the GC (2.39) and GE (14.27) were different (p=0.002), when converting these values to the NBS scale, resulted in a value of 2.20 and 13.13. Comparing the groups there was a decrease in transmittance (p=0.014), with greater magnitude in the GE. At T1, there was a difference (p=0.046) between GC and GE for Ra (GC: 0.122 ± 0.002; GE: 0.139 ± 0.007), with higher values in EG. For Rz, when comparing GC (0.616 ± 0.021) and GE (0.741 ± 0.026), at T1, there was also a difference (p=0.002), with higher values found in GE. In the CG, there was no statistically significant difference in Ra (p=0.807) and Rz (p=0.231) when comparing samples over time T0 and T1. In the GE, there was both an increase in Ra (p=0.003) and Rz (p=0.014). It is concluded, therefore, that the aligners became darker, yellowish, reduced their light transmission, becoming opaque and had their surface roughness increased, both in the Ra and Rz parameters. ABSTRACT Objective: To estimate the prevalence of current commercial cigarette smoking, as well as those of e-cigarette and hookah experimentation and current use among adults (≥ 18 years of age) in Brazil. Methods: This study was based on a countrywide cross-sectional telephone-based survey conducted in 2022. The sample was designed to be representative of the five macroregions in Brazil and included 1,800 individuals from each of the regions. Telephone numbers, using a random digit dialing procedure, were proportionally selected for each direct distance dialing code in each region and then electronically validated (i.e., 900 cell and 900 landline phone numbers per region). Information on current commercial cigarette smoking (regardless of frequency/amount), as well as lifetime history of or current e-cigarette and hookah use (regardless of amount), were collected. Results: The prevalence of lifetime history of e-cigarette and hookah use was identical (7.3%; 95% CI: 6.0-8.9), whereas the prevalence of current commercial cigarette smoking was 12.2% (95% CI: 10.4-14.1). Young adults (18-24 years) had the highest prevalence of e-cigarette experimentation (19.7%; 95% CI: 15.1-17.0) and hookah experimentation (17%; 95% CI: 12.2-23.2). E-cigarette and hookah use was more common in the Central-West region and among those with a high level of education, whereas current commercial cigarette smoking was more common among those with a lower level of education. Individuals who used the three forms of nicotine delivery corresponded to 1.5% of the sample (nearly 2 million individuals based on the estimated size of the Brazilian adult population). Conclusions: Surveillance is essential for the monitoring and prevention of these new forms of nicotine consumption.
RESUMO Objetivo: Estimar a prevalência do consumo atual de cigarros industrializados, bem como da experimentação e uso atual de cigarro eletrônico e narguilé entre adultos (≥ 18 anos) no Brasil. Métodos: Este estudo baseou-se em uma inquérito telefônico nacional realizada em 2022. A amostra foi projetada para ser representativa das cinco macrorregiões do Brasil e foi composta por 1.800 indivíduos de cada uma das regiões. Por meio de um procedimento de discagem aleatória, os números de telefone foram selecionados proporcionalmente para cada código de discagem direta à distância em cada região e, em seguida, validados eletronicamente (isto é, 900 telefones celulares e 900 telefones fixos por região). Foram coletadas informações sobre o consumo atual de cigarros industrializados (independentemente da frequência/quantidade), bem como sobre a história de uso ou uso atual de cigarro eletrônico e narguilé (independentemente da quantidade). Resultados: As prevalências de história de uso de cigarro eletrônico e narguilé foram idênticas (7,3%; IC95%: 6,0-8,9), ao passo que a prevalência de consumo atual de cigarros industrializados foi de 12,2% (IC95%: 10,4-14,1). Adultos jovens (18-24 anos) apresentaram as maiores prevalências de experimentação de cigarro eletrônico (19,7%; IC95%: 15,1-17,0) e de narguilé (17%; IC95%: 12,2-23,2). O uso de cigarro eletrônico e narguilé foi mais comum na região Centro-Oeste e entre aqueles com maior grau de escolaridade, ao passo que o consumo atual de cigarros industrializados foi mais comum entre aqueles com menor grau de escolaridade. Os indivíduos que usavam as três formas de liberação de nicotina corresponderam a 1,5% da amostra (quase 2 milhões de indivíduos com base na estimativa do tamanho da população adulta brasileira). Conclusões: A vigilância é essencial para o monitoramento e prevenção dessas novas formas de consumo de nicotina.

Abstract Brazil has advanced in tobacco control actions, with a significant decline in the prevalence of tobacco use. However, it is essential to identify more vulnerable populations, such as LGBT persons. This study aimed to analyze the discursive production sustaining the search for support and penetration of the tobacco industry among the LGBT population, taking the sponsorship of Philip Morris Brasil to the LGBT parade in São Paulo in 2019. We employed the critical discourse analysis of a report published on a blog. The analysis of the piece points to the use of vocabularies such as diversity, inclusion, modernity, and innovation, which concern the LGBT cause and the launch of its new product. The text conveys a high commitment and a courteous tone, using discursive resources that associate the company with technical and behavioral innovation ideas. It communicates intertextually with regulatory bodies about introducing its new product in the Brazilian market (prohibited in Brazil). It uses different ideological operators, such as the euphemism of the smokeless or smoke-free future. The work shows a tobacco industry strategy to promote its heated product while supporting LGBT cause and promoting a positive corporate image.
Resumo O Brasil tem avançado em ações de controle do tabaco, resultando em grande declínio da prevalência de tabagismo. Entretanto, é fundamental identificar populações mais vulneráveis, como as pessoas LGBT. Este estudo teve como objetivo analisar a produção discursiva que sustenta a busca de apoio e penetração da indústria tabageira junto à população LGBT, tomando o caso do patrocínio da Philip Morris Brasil à parada LGBT em São Paulo em 2019. A metodologia utilizada foi análise de discurso Crítica de notícia publicada em um blog. A análise da peça aponta para utilização de vocabulários como diversidade, inclusão, modernidade e inovação, que dizem respeito à causa LGBT e ao lançamento de novo produto. O texto traz alto grau de comprometimento e tom cortês, fazendo uso de recursos discursivos que associam a empresa às ideias de inovação técnica e comportamental. Comunica intertextualmente com órgãos regulatórios acerca da entrada no mercado brasileiro de seu novo produto (proibido no Brasil). Faz uso de diferentes operadores ideológicos, como o eufemismo do futuro sem fumaça ou futuro livre de fumaça. O trabalho apresenta uma estratégia da indústria do tabaco para promover seu produto de tabaco aquecido enquanto apoia a causa LGBT e promove imagem corporativa positiva.

Objective: To assess progress in and barriers to implementing bans on tobacco advertising, promotion and sponsorship (TAPS) in Uruguay, which has a complete ban, and Argentina, with a partial ban. Methods: Legislation on TAPS bans in Uruguay and Argentina was reviewed and relevant published literature, news stories, civil society reports and tobacco industry reports retrieved to analyze progress in implementing TAPS bans. Results: In Uruguay, the complete TAPS ban, which includes standardized tobacco packaging, maintains high compliance and severely limits exposure of TAPS, despite a few problems with corporate social responsibility, social media, and transnational advertising. In Argentina, the partial TAPS ban has more problems with compliance and exposure to TAPS. The most important barriers to implementing TAPS bans in both countries are the tobacco companies. In Uruguay, tobacco companies do not comply in a few areas but the complete ban greatly minimizes this. In Argentina, however, tobacco companies can more easily exploit gaps in the partial TAPS ban, such as advertising at the points of sale, promoting contests, and using influencers on social media. Conclusions: The partial TAPS ban in Argentina illustrates the problems with enforcement and the tobacco industry's ability to exploit loopholes and continue to market their products, especially to young people. A complete TAPS ban, including standardized tobacco packaging, as in Uruguay, is easier to implement and enforce and is effective in reducing exposure to tobacco advertising. Nevertheless, governments should prioritize implementing TAPS bans on social media, which remains a difficult sphere to monitor and allows tobacco companies to continue recruiting and targeting young people.

BACKGROUND: Paraguay is a major supplier of illicit cigarettes for the Latin American region and beyond. In July 2022, Paraguay ratified the FCTC Protocol. This is an opportunity and a challenge for neighboring countries to implement coordinated actions. This is the first analysis of the foreign trade data for cigarettes and their inputs using Paraguayan data to contextualise the illicit trade problem in Latin America and globally. METHODS: Combining publicly available Paraguayan databases, this research constructs a database to analyse imported cigarette inputs, particularly by identifying the companies and their national origins. RESULTS: A complex multinational supply chain perpetuates a flow of inputs into Paraguay that contributes to the production and export of illicit tobacco products. Brazil is a relevant legal supplier of intermediate goods for the Paraguayan tobacco industry yet is not a significant destination of the legal exports of cigarettes produced in Paraguay. Yet, Paraguayan cigarettes are widely available in the Brazilian market, almost all of them illicit. Trade data also show the role of other countries in the region as major cigarette input exporters to Paraguay. Evidence also supports that high volumes of legal exports from Paraguay to third countries (including Bolivia, Suriname, Aruba and Curacao) may be fuelling illicit trade through triangulation to other countries. CONCLUSIONS: The oversupply-that is, more supply than necessary for domestic consumption and legal exports-of cigarette inputs likely divert illegally back to the countries exporting these inputs and others. Thus, the responsibility for illicit trade in cigarettes falls not only on Paraguayan companies but also on companies exporting inputs to Paraguay to producing these illicit goods. Furthermore, Paraguay is not only exporting illicitly directly to Brazil and Argentina, but also appears to oversupply other countries in South America and the Caribbean that cannot legally absorb this trade through domestic consumption and/or legal re-export.

Objetivo: Determinar el grado dependencia al tabaquismo y la motivación a la cesación del consumo de tabaco en estudiantes fumadores de una institución universitaria en la ciudad de Tunja (Boyacá). Materiales y métodos: Estudio transversal analítico con fase correlacional, con muestreo probabilístico por conglomerados. La identificación de la conducta tabáquica se realizó a través de los instrumentos de Fagerström y Richmond, en relación con la medición de los niveles de concentración de monóxido de carbono a través de cooximetría de pulso. Resultados: En los 79 estudiantes se observó una edad promedio de 21 años, siendo en la mayoría del sexo masculino (86%), con un consumo de cigarrillo en promedio de 4.2 años. Se pudo establecer que el 82.3% [IC : 73.4 - 91.1] de los estudiantes fumaron menos de 10 cigarrillos al día, con un grado leve correspondiente al 89 % [IC : 81.0-94.9], quienes se caracterizaron por fumar 5 o menos paquetes de cigarrillos al año. De igual manera el 82 % [IC : 73.4 - 89.9] de los fumadores se clasificaron como moderados y el 94 % [IC : 87.3 - 98.7] de ellos son poco dependientes a la nicotina. De igual forma, se estableció que solo el 32.91 % [IC : 24.1 - 43.0] presentó alto grado de motivación para dejar de fumar. En relación con el nivel de porcentaje de saturación de monóxido de carbono, en promedio fue de 6 % (DE 2.2), existiendo relación directamente proporcional con el grado y la clasificación del fumador determinada a través del método de Wilcoxon (P=0.00). Conclusión: Se identificó que la mayoría de los participantes presenta moderada motivación para la cesación del consumo; por lo tanto, es necesario incentivar el inicio de programas de deshabituación tabáquica que impacten la calidad de vida en adultos jóvenes, así mismo, en la reducción de la morbi/mortalidad por enfermedades respiratorias crónicas a futuro.
Objective: To determine the degree of dependence on smoking and the motivation to stop smoking in students of a university institution in the city of Tunja (Boyacá). Materials and methods: Analytical cross-sectional study with a correlational phase, with probabilistic cluster sampling. The identification of smoking behavior was carried out using the Fagerström and Richmond instruments, in relation to the measurement of carbon monoxide concentration levels through pulse co-oximetry. Results: An average age of 21 years was observed in the 79 students, with the majority being male (86%), with an average cigarette consumption of 4.2 years. It was established that 82.3% [95% CI: 73.4 - 91.1] of the students smoked less than 10 cigarettes a day, with a slight degree corresponding to 89% [95% CI: 81.0-94.9] who were characterized by smoking 5 or less cigarette packs per year. Similarly, 82% [95% CI: 73.4 - 89.9] of smokers were classified as moderate and 94% [95% CI: 87.3 - 98.7] of them were little dependent on nicotine. Similarly, it was established that only 32.91% [95% CI: 24.1 - 43.0] presented a high degree of motivation to quit smoking. In relation to the level of percentage of carbon monoxide saturation, on average it was 6% (SD 2.2), with a directly proportional relationship with the degree and classification of the smoker determined through the Wilcoxon method (P = 0.00). Conclusion: Most of the participants were found to present moderate motivation to stop smoking, therefore, it is necessary to encourage the start of smoking cessation programs that impact the quality of life in young adults, as well as reducing morbidity/mortality from chronic respiratory diseases in the future.

BACKGROUND: Illicit trade in tobacco products is a menace to the goal of eliminating tobacco consumption. Although tax policy is very effective in reducing consumption, illicit trade can reduce (though not eliminate) its effectiveness. METHODS: This article discusses the recent evolution of illicit trade and the context in which it occurred; the new methods that have been developed to measure it and, finally, the challenges in the next phase in the control of illicit trade. RESULTS: There has been a remarkable stability in the penetration of cigarette illicit trade in the past decade. Such a stability, however, occurred in a world of shrinking tobacco consumption, implying a decreasing absolute illicit trade. Most countries have progressed in increasing tobacco taxes and changing tax structures. Prices of illicit cigarettes follow legal cigarette prices. Concomitantly, many new studies, independent from the tobacco industry, have been conducted allowing for better understanding of the illicit trade and providing inputs to its solution. The entry into force of the WHO FCTC Protocol to Eliminate Illicit Trade in Tobacco Products provides both a global and a national policy framework to further curb illicit trade. Instruments such as track-and-trace systems must be promoted and adopted to maximise reductions in illicit trade. CONCLUSIONS: Global efforts to curb the illicit trade in tobacco products are gaining momentum and progress has been made in many parts of the world. The next decade can witness a decisive decrease in tobacco consumption, both licit and illicit, if countries further engage in international collaboration.

BACKGROUND: Little is known about the use of novel heated tobacco products (HTPs) in low- and middle-income countries. We examined among smokers in Mexico the prevalence and correlates of HTP use, as well as reasons for using HTPs. METHODS: We analyzed data from five surveys (November 2019-March 2021) of an open cohort of adult smokers (n = 6500), including an oversample of those who also use e-cigarettes. Mixed-effects multinomial logistic models were used to estimate associations between study variables and current HTP use or prior HTP trial relative to never trying HTPs. RESULTS: The weighted prevalence of current HTP use was 1.1%. Independent correlates of current HTP use included smoking frequency, intention to quit, e-cigarette use, having partners/family-members who use e-cigarettes, friends who use HTPs, and exposure to HTP information inside/outside tobacco shops. Having partners/family members who smoke and not knowing about the harm of HTPs relative to cigarettes were associated with lower likelihood of current HTP use. Having tried HTPs was more likely among light daily smokers, those with family who use HTPs and exposure to HTP information outside shops and on newspapers/magazines. Among current users, the top reasons for using HTPs were greater social acceptability (50.6%) and lower perceived harm (34.9%) relative to cigarettes. CONCLUSIONS: Uptake of HTPs appears relatively low among Mexican smokers, and correlates of use are similar to those for e-cigarette use. Further research is needed to determine if HTPs use promotes or impedes smoking cessation, given current HTP users are also likely to use various nicotine products.
