BACKGROUND: Few large-scale studies have evaluated the effectiveness of percutaneous coronary intervention (PCI) technological advances in the treatment of patients with unprotected left main coronary artery disease (LM-CAD). We aim to identify independent factors that affect the prognosis of PCI in patients with unprotected LM-CAD and to assess the impact of PCI technological advances on long-term clinical outcomes. METHODS AND RESULTS: A total of 4512 consecutive patients who underwent unprotected LM-CAD PCI at Fuwai Hospital from 2004 to 2016 were enrolled. Multivariable Cox proportional hazards model was used to identify which techniques can independently affect the incidence of major adverse cardiac events (MACEs; a composite of cardiac death, myocardial infarction, or target vessel revascularization). The incidence of 3-year MACEs was 9.0% (406/4512). Four new PCI techniques were identified as the independent protective factors of MACEs, including second-generation drug-eluting stents (hazard ratio [HR], 0.61 [95% CI, 0.37-0.99]), postdilatation (HR, 0.75 [95% CI, 0.59-0.94]), final kissing balloon inflation (HR, 0.78 [95% CI, 0.62-0.99]), and using intravascular ultrasound (HR, 0.78 [95% CI, 0.63-0.97]). The relative hazard of 3-year MACEs was reduced by ≈50% with use of all 4 techniques compared with no technique use (HR, 0.53 [95% CI, 0.32-0.87]). CONCLUSIONS: PCI technological advances including postdilatation, second-generation drug-eluting stent, final kissing balloon inflation, and intravascular ultrasound guidance were associated with improved clinical outcomes in patients who underwent unprotected LM-CAD PCI.

BACKGROUND: Bromhidrosis, characterized by foul-smelling sweat, is a prevalent condition that significantly affects patients' social and psychological well-being. METHODS: This review presents novel treatment approaches and discusses the pros and cons of various treatment options for axillary bromhidrosis. RESULTS: Extensive research has explored numerous treatment modalities for bromhidrosis. This article systematically reviews both surgical and nonsurgical interventions utilized in clinical practice. CONCLUSION: By synthesizing available evidence, this review aims to offer evidence-based recommendations for effectively managing bromhidrosis, considering factors such as treatment efficacy, safety profiles, patient preferences, and clinical outcomes.

RESUMEN

Technological advances in radiation therapy impact on the role and scope of practice of the radiation therapist. The European Society of Radiotherapy and Oncology (ESTRO) recently held two workshops on this topic and this position paper reflects the outcome of this workshop, which included radiation therapists from all global regions. Workflows, quality assurance, research, IGRT and ART as well as clinical decision making are the areas of radiation therapist practice that will be highly influenced by advancing technology in the near future. This position paper captures the opportunities that this will bring to the radiation therapist profession, to the practice of radiation therapy and ultimately to patient care.

RESUMEN

Pathogenic microorganisms pose significant threats to human health, food safety and environmental integrity. Rapid and accurate detection of these pathogens is essential to mitigate their impact. Fast, sensitive detection methods such as biosensors also play a critical role in preventing outbreaks and controlling their spread. In recent years, biosensors have emerged as a revolutionary technology for pathogen detection. This review aims to present the current developments in biosensor technology, investigate the methods by which these developments are used in the detection of pathogenic bacteria and highlight future perspectives on the subject.


Microorganisms that cause disease are important not only to human health but also to food safety and environmental health. Therefore, it is important to detect these microorganisms quickly, easily, at a low cost and with accuracy. Biosensors have all of these features and offer a unique way to detect microorganisms. They are already being used in similar fields and are becoming increasingly portable and ergonomic devices. This paper looks at the uses of biosensors in detecting disease-causing bacteria and how their use will develop with greater technology.

RESUMEN

UNASSIGNED: Continuous renal replacement therapy (CRRT) is the main extracorporeal kidney support therapy used in critical ill patients in the intensive care unit (ICU). Since its conceptualization ~50 years ago, there have been major improvements in its technology and utilization. The last decade, and particularly since the COVID-19 pandemic, has been marked by a growing interest and demand of CRRT in worldwide ICUs. This has underpinned the need for improvements in nomenclature and process standardization, optimization of CRRT deliverables, and the development and validation of key performance indicators. Further, how to leverage digital health technologies to build clinical decision support for CRRT and improve personalized bedside decisions is a subject of intense investigation. RESUMEN

I investigate how computerization increases access to schedule control and widens the class disparity in access. I combine time-varying measurements of occupational-level computerization with individual-level data from the Current Population Survey (1991-2004) and the American Time Use Survey (2018). Results confirm that computerization is positively associated with schedule control, but this association is not robust to the inclusion of other aspects of occupations. The positive association between educational attainment and schedule control is greater among employees in highly computerized occupations. The results shed light on how computerization is related to inequality in access to schedule control, and in turn, work-family conflict and well-being.

Crime scene investigation (CSI) is the complex act of reconstructing the dynamics that led to a crime and the circumstances of its perpetration. Crystallizing the CSI is a difficult task for the forensic pathologist; however, it is often requested by the public prosecutor and many judicial cases remain unsolved precisely for this reason. Recent years have seen an improvement in the ability of 3D scanning technology to obtain dense surface scans of large-scale spaces, for surveying, engineering, archaeology, and medical purposes such as forensics. The applications of this new technology are growing every day: forensic measurement of wounds in clinical reports, for example, reconstruction of traffic accidents, bullet trajectory studies in gunshot wounds, and 3D bloodstain pattern analysis. A retrospective analysis was conducted across all crime scene investigations performed by the forensic staff of the Department of Forensic Pathology of the University of Catania from January 2019 to June 2022. Inclusion criteria were the use of a laser scanner (LS), the use of a camera, a full investigative scene, and collection of circumstantial data thanks to the help of the judicial police. Cases in which the LS was not used were excluded. Out of 200 CSIs, 5 were included in the present study. In case number 1, the use of the LS made it possible to create a complete scale plan of the crime scene in a few hours, allowing a ship to be quickly returned to the judicial police officer. In case 2 (fall from a height), the LS clarified the suicidal intent of the deceased. In case number 3 it was possible to reconstruct a crime scene after many years. In case 4, the LS provided a great contribution in making a differential diagnosis between suicide and homicide. In case 5, the LS was fundamental for the COVID team in planning the study of COVID-19 pathways and areas within a hospital with the aim of reduction of nosocomial transmission. In conclusion, the use of the LS allowed the forensic staff to crystallize the investigative scene, making it a useful tool.

Increasing carbon productivity is regarded as one of the significant ways to strike a balance between national economic growth and environmental protection. As the proportion of China's service sector in the national economy rises and the severity of environmental pollution increases, the matter of carbon productivity in service sector required to be explored in depth. This paper focuses on the biased technological advances index of China's service sector and its impact on carbon productivity by constructing a two-layer nested CES production function, proposing policy countermeasures to raise service sector's carbon productivity in China, which is of great practical significance in reducing carbon emissions of China's service sector, improve carbon productivity of China's service sector and promote the green transformation and sustainable development of China's service sector. The results are as follows. (1) The average value of the biased technological advances index of services in China is negative, indicating that technological advances of services in China are biased towards non-energy elements. The biased technological advances indexes of China's service sector in the western, middle and eastern regions are also negative, and the index of the eastern region is the smallest, indicating that the technological advances of the service sector in the eastern region are biased towards non-energy elements to the highest extent. (2) There is a negative correlation between the biased technological advances index and carbon productivity of services in China. In the western, middle and eastern regions of China, the bias of technological advances in eastern China has the greatest effect on China's productivity of carbon in service sector. The policy implication is that in order to increase China's services' productivity of carbon, it is essential to reduce the biased technological advances index of China's service sector.

Rumination and its related mental phenomena share associated impairments in cognition, such as executive functions and attentional processes across different clinical conditions (e.g., in psychotic disorders). In recent decades, however, the notion of rumination has been increasingly narrowed to the "self-focused" type in depressive disorders. A closer review of the literature shows that rumination may be construed as a broader process characterized by repetitive thoughts about certain mental contents that interfere with one's daily activities, not only limited to those related to "self". A further examination of the construct of rumination beyond the narrowly focused depressive rumination would help expand intervention opportunities for mental disorders in today's context. We first review the development of the clinical construct of rumination with regard to its historical roots and its roles in psychopathology. This builds the foundation for the introduction of the "Flow Model of Rumination (FMR)", which conceptualizes rumination as a disruption of a smooth flow of mental contents in conscious experience that depends on the coordinated interactions between intention, memory, affect, and external events. The conceptual review concludes with a discussion of the impact of rapid technological advances (such as smartphones) on rumination. Particularly in contemporary societies today, a broader consideration of rumination not only from a cognition viewpoint, but also incorporating a human-device interaction perspective, is necessitated. The implications of the FMR in contemporary mental health practice are discussed.

This study examines the causal impact of very important person (VIP) medical services on hospital total factor productivity in Deyang, a prefectural-level city in western China, spanning the years 2015-2020. This aims to offer empirical evidence and policy recommendations for the implementation of VIP practices in the medical field. A secondary unbalanced panel dataset of 416 observations was collected from the annual reports of the Health Commission and 92 eligible medical institutions were included. This study utilized a two-stage strategy. First, the Global Malmquist index was used to calculate the total factor productivity and its decomposition terms for hospitals from 2015 to 2020. In the second stage, two-way fixed effects models and Tobit models were used to identify the relationship between VIP medical services and hospital efficiency; instrumental variables were used to solve potential endogeneity problems in the model. The results showed that VIP medical services had a significantly negative impact on medical institutions' efficiency. The technological advances and pure technical efficiency related to VIP medical care may help explain these negative impacts, which were heterogeneous across groups divided by the nature of the hospital and the outside environment. It is imperative to prioritize the standardized provision of VIP medical services for medical institutions, optimize management and service process, enhance the training of clinical and scientific research capabilities of medical personnel, and scientifically allocate resources for both VIP and general medical services. This will help mitigate health inequality while improving the overall quality of medical services.

It could be claimed that without any doubt the Internet has revolutionized the educational system to a great extent. Even though some are still interested in traditional ways of teaching and learning and also face-to-face classes, technological advances, in particular, social media have changed the English as a foreign language (EFL) contexts in a way that they will not be compatible with any other methods that have long been utilized before. Despite the fact that some studies have been conducted in different learning contexts, to the researcher's best knowledge, attention has not been focused on the importance of social media inside and outside the classroom on EFL students' indulgence. Therefore, the aim of this review is to synthesize the findings of the previous studies to highlight the role of social media in EFL students' engagement throughout class and outside the class in both face-to-face and online classes. To reach this goal, this review first discusses the impact of social media on engagement, and then engagement and its classifications are dealt with. Next, the relationship between using social media and students' engagement is discussed. And finally, some implications are proposed.

PURPOSE OF REVIEW: Deep brain stimulation (DBS) is an established treatment in several movement disorders, including Parkinson's disease, dystonia, tremor, and Tourette syndrome. In this review, we will review and discuss the most recent findings including but not limited to clinical evidence. RECENT FINDINGS: New DBS technologies include novel hardware design (electrodes, cables, implanted pulse generators) enabling new stimulation patterns and adaptive DBS which delivers potential stimulation tailored to moment-to-moment changes in the patient's condition. Better understanding of movement disorders pathophysiology and functional anatomy has been pivotal for studying the effects of DBS on the mesencephalic locomotor region, the nucleus basalis of Meynert, the substantia nigra, and the spinal cord. Eventually, neurosurgical practice has improved with more accurate target visualization or combined targeting. A rising research domain emphasizes bridging neuromodulation and neuroprotection. Recent advances in DBS therapy bring more possibilities to effectively treat people with movement disorders. Future research would focus on improving adaptive DBS, leading more clinical trials on novel targets, and exploring neuromodulation effects on neuroprotection.

RESUMEN

This article synthesizes recent findings on antecedents of healthy eating. We discuss consumer-related and environment-related forces that influence consumers' healthy food choices and emphasize the duality of these forces so that they can facilitate but also prevent healthy eating. Specifically, our review documents how consumer lay beliefs, goals, and habits shape eating patterns. We further document the impact of environment-related forces on healthy consumption-focusing on intervention strategies and environmental changes (i.e., the trend towards online retail channels). Finally, we discuss three salient tensions (i.e., an innate craving for unhealthy food, a focus on single decisions, and a selective focus on self-control dilemmas) that emerge when taking a holistic view on existing research.

Population growth, even if coupled to economic growth, and resources, were already on a collision course, especially in Africa. The 2019 United Nations World Water Development Report provided a dramatic status of world water, however without questioning the main drivers of an imminent water crisis, that were unbounded, unequal, economic, and population growth, within the context of reducing resources in a finite world. Despite the report was a small step forward in awareness, still, it was not proposing satisfactory remedies. With business-as-usual, without acting on the drivers of water scarcity, regional water crises were inevitable in the next 3 decades, starting from Africa. Constrained by political, financial, and energy burdens, the technological improvements that have helped humanity to deal with the increased demand for water, food, and energy over the last 70 years, were likely not enough to avoid the water crisis. On top of forecast is the Covid19 pandemic. Coronavirus cases are (August 4, 2020) 18,446,065 and fatalities are 697,202 worldwide, and still growing. The containment measures enforced for Covid19 infection following the examples in the United Kingdom have already produced significant damage to the world economy. This will limit social expenditures in general, and the expenditures for the water issue in particular. The water crisis will consequently become worse in the next months, with consequences still difficult to predict. This will be true especially for Africa, where the main problem has always been poverty. There is the opportunity of significant health, food, and water crisis, especially in Africa. While the concepts of washing hands and social distancing that are difficult to apply haven't produce so far major issues with the Covid19 outbreak in Africa, borders closure, restrictions on movement, and more poverty will translate in a lack of food and water potentially much more worrying than the virus spreading.

Glycans are expressed on the surface of nearly all host and bacterial cells. Not surprisingly, glycan-mediated molecular interactions play a vital role in bacterial pathogenesis and host responses against pathogens. Glycan-mediated host-pathogen interactions can benefit the pathogen, host, or both. Here, we discuss (i) bacterial glycans that play a critical role in bacterial colonization and/or immune evasion, (ii) host glycans that are utilized by bacteria for pathogenesis, and (iii) bacterial and host glycans involved in immune responses against pathogens. We further discuss (iv) opportunities and challenges for transforming these research findings into more effective antibacterial strategies, and (v) technological advances in glycoscience that have helped to accelerate progress in research. These studies collectively offer valuable insights into new perspectives on antibacterial strategies that may effectively tackle the drug-resistant pathogens that are rapidly spreading globally.

BACKGROUND & PURPOSE: Activity-based neuroplasticity and re-organization leads to motor learning via replicating real-life movements. Increased repetition of such movements has growing evidence over last few decades. In particular, computer-game-based rehabilitation is found to be effective, feasible and acceptable for post-stroke upper limb deficits. Our study aims to evaluate the feasibility and effectiveness of 12 weeks of computer-game-based rehabilitation platform (GRP) on fine and gross motor skills post-stroke in India. METHODS: Through this trial we will study the effect of adjunctive in-hospital GRP (using a motion-sensing airmouse with off-the-shelf computer games) in 80 persons with subacute stroke, for reduction of post-stroke upper limb deficits in a single-centre prospective Randomized Open, Blinded End- point trial when compared to conventional therapy alone. RESULTS: We intend to evaluate between-group differences using Wolf Motor Function test, Stroke Specific Quality of Life, and GRP assessment tool. Feasibility will be assessed via recruitment rates, adherence to intervention periods, drop-out rate and qualitative findings of patient experience with the intervention. CONCLUSION: The CARE FOR U trial is designed to test the feasibility and effectiveness of a computer-game based rehabilitation platform in treating upper limb deficits after stroke. In case of positive findings GRP can be widely applicable for stroke populations needing intensive and regular therapy with supervision.
