RESUMEN

Chinese diesel trucks are the main contributors to NOx and particulate matter (PM) vehicle emissions. An increase in diesel trucks could aggravate air pollution and damage human health. The Chinese government has recently implemented a series of emission control technologies and measures for air quality improvement. This paper summarizes recent control technologies and measures for diesel truck emissions in China and introduces the comprehensive application of control technologies and measures in Beijing-Tianjin-Hebei and surrounding regions. Remote online monitoring technology has been adopted according to the China VI standard for heavy-duty diesel trucks, and control measures such as transportation structure adjustment and heavy pollution enterprise classification control continue to support the battle action plan for pollution control. Perspectives and suggestions are provided for promoting pollution control and supervision of diesel truck emissions: adhere to the concept of overall management and control, vigorously promote the application of systematic and technological means in emission monitoring, continuously facilitate cargo transportation structure adjustment and promote new energy freight vehicles. This paper aims to accelerate the implementation of control technologies and measures throughout China. China is endeavouring to control diesel truck exhaust pollution. China is willing to cooperate with the world to protect the global ecological environment.

RESUMEN

As an interferon-stimulating factor protein, STING plays a role in the response and downstream liaison in antiviral natural immunity. Upon viral invasion, the immediate response of STING protein leads to a series of changes in downstream proteins, which ultimately leads to an antiviral immune response in the form of proinflammatory cytokines and type I interferons, thus triggering an innate immune response, an adaptive immune response in vivo, and long-term protection of the host. In the field of antiviral natural immunity, it is particularly important to rigorously and sequentially probe the dynamic changes in the antiviral natural immunity connector protein STING caused by the entire anti-inflammatory and anti-pathway mechanism and the differences in upstream and downstream proteins. Traditionally, proteomics technology has been validated by detecting proteins in a 2D platform, for which it is difficult to sensitively identify changes in the nature and abundance of target proteins. With the development of mass spectrometry (MS) technology, MS-based proteomics has made important contributions to characterizing the dynamic changes in the natural immune proteome induced by viral infections. MS analytical techniques have several advantages, such as high throughput, rapidity, sensitivity, accuracy, and automation. The most common techniques for detecting complex proteomes are liquid chromatography (LC) and mass spectrometry (MS). LC-MS (Liquid Chromatography-Mass Spectrometry), which combines the physical separation capability of LC and the mass analysis capability of MS, is a powerful technique mainly used for analyzing the proteome of cells, tissues, and body fluids. To explore the combination of traditional proteomics techniques such as Western blotting, Co-IP (co-Immunoprecipitation), and the latest LC-MS methods to probe the anti-inflammatory pathway and the differential changes in upstream and downstream proteins induced by the antiviral natural immune junction protein STING.

(AU)

The lack of information on the use of technology in children with autism spectrum disorder (ASD) of different profiles can make it difficult for teachers and students to benefit from the most effective technology support tailored to their needs. The aim of this review was to analyze and synthesize scientific evidence on the effectiveness of technological resources in improving the emotional understanding of students with high and low functioning ASD profiles. A systematic review of the scientific publications indexed in some of the most relevant databases was carried out following the criteria established in the PRISMA declaration. A total of 38 articles that met the pre-established inclusion criteria were analyzed. The results show the importance of designing versatile systems that can be customized and adapted in real time and in natural contexts with a clearly inclusive approach. But they also suggest that technology may not be an appropriate complementary intervention tool for all children with ASD. This underlines the need for additional well-controlled tests on the characteristics that would allow identifying which students might or might not benefit from different technology modalities. RESUMEN

The ecological footprint (EF) has become one of the leading indicators for environmental assessments. The EF is an indicator that is at the center of researchers' interest in empirical analysis, as it simultaneously reflects air, water, and soil pollution. Analyzing the six sub-components of the EF is essential for assessing the environmental pressures from forestry, construction, fisheries, agriculture, and livestock, as well as for remediating these pressures. In this context, this study examines the impact of income, globalization, and technological progress (represented by patents) on the EF and its subcomponents for the BRICS countries over the period 1992-2020. The BRICS countries are of critical importance to this study as major countries in the global economic and environmental landscape. The study employs the panel LM cointegration test and the common correlated effects estimator. The results show that economic expansion augments ecological, carbon, and built-up land footprints and that patents have no significant impact on the EF indicators. On the contrary, globalization is a factor that reduces five of the seven EF indicators. A robustness check performed with a half-panel jackknife confirms the analysis findings. These results suggest that BRICS policymakers should harmonize economic development and ecology while making greater use of the environmental benefits of globalization.

RESUMEN

The objective of explainable artificial intelligence systems designed for clinical decision support (XAI-CDSS) is to enhance physicians' diagnostic performance, confidence and trust through the implementation of interpretable methods, thus providing for a superior epistemic positioning, a robust foundation for critical reflection and trustworthiness in times of heightened technological dependence. However, recent studies have revealed shortcomings in achieving these goals, questioning the widespread endorsement of XAI by medical professionals, ethicists and policy-makers alike. Based on a surgical use case, this article challenges generalising calls for XAI-CDSS and emphasises the significance of time-sensitive clinical environments which frequently preclude adequate consideration of system explanations. Therefore, XAI-CDSS may not be able to meet expectations of augmenting clinical decision-making in specific circumstances where time is of the essence. This article, by employing a principled ethical balancing methodology, highlights several fallacies associated with XAI deployment in time-sensitive clinical situations and recommends XAI endorsement only where scientific evidence or stakeholder assessments do not contradict such deployment in specific target settings.

RESUMEN

BACKGROUND: Pregnancy-induced hypertension remains one of the important types of diseases that affect maternal and infant outcomes; prenatal and perinatal ultrasound examination is an important tool for evaluating fetal development. So, this study aimed to explore the clinical value of applying fetal heart quantification (fetal HQ) measuring left ventricular global longitudinal strain (LVGLS) and left ventricular ejection fraction (LVEF) in mid-to-late fetuses to predict neonatal complications in patients with gestational hypertension. METHODS: A retrospective summary of 146 pregnant women with gestational hypertension diagnosed from August 2020 to October 2023 into JinHua Maternal and Child Health Care Hospital was performed. Fetal HQ measured the fetal global spherical index (GSI), left and right ventricular spherical index (SI), left and right ventricular fractional shortening (FS), LVGLS and RVGLS, LVEF, and fractional area change (FAC) of the left and right ventricles. They were divided into complication group and non-complication group based on whether fetal complications occurred 28 days after birth. Multivariate logistic regression was used to screen risk factors to neonatal complications. RESULTS: The 146 neonates were divided into 39 of the complication group and 107 of the non-complication group. Compared with the latter group, pregnant women in the former group had a higher incidence of preeclampsia and eclampsia, increased mean systolic and diastolic blood pressure, significantly lower estimated fetal weight (EFW), left ventricular 24-segment SI, LVGLS, LVEF, and left ventricular FAC values (p < .05). Logistic regression showed higher of LVGLS (adjusted OR = 2.281, p < .001) was risk factors for neonatal complications, while higher LVEF (adjusted OR = 0.600, p < .001) and left ventricular FAC (adjusted OR = 0.784, p = .035) were protective factors. Spearman's correlation analysis showed a significant negative correlation between LVGLS and LVEF (r = -0.368, p < .001). Receiver operating curves (ROCs) showed the area under the curve (AUC) for predicting overall neonatal complications was 0.880 for LVGLS and 0.878 for LVEF (p < .001). CONCLUSIONS: Fetal HQ for fetal LVGLS and LVEF in mid-to-late pregnancy with gestational hypertension helps to assess the overall neonatal complications risk.

RESUMEN

As global environmental challenges intensify, manufacturing firms face increasing pressure to innovate sustainably. Green innovation, characterized by the development of environmentally friendly products, processes, and technologies, has become essential for firms striving to remain competitive. This study aims to investigate the influence of key factors-green logistics, green finance, and green technology-on green innovation within manufacturing firms, while exploring the mediating role of green technology in these relationships. A multi-method approach was employed, combining partial least squares structural equation modeling, fuzzy-set qualitative comparative analysis, and necessity condition analysis. 447 responses were collected from manufacturing companies in Dhaka city, Bangladesh, using structured questionnaires. The analysis revealed that green logistics and green finance have a significant positive impact on green innovation, while the influence of the green work environment was found to be positive but statistically insignificant. Additionally, green technology was identified as a significant mediator in the relationships between green finance, green logistics, and green innovation. This study offers a comprehensive green innovation model while green technology is a mediator. Furthermore, this study advances the resource-based view theory by integrating green technology as a pivotal resource that enhances a firm's competitive advantage in sustainable markets. By adopting a multi-method approach, this research provides a rigorous examination of the research questions, offering a comprehensive understanding of the dynamic interactions between green finance, green logistics, and green technology in driving innovation. Thus, this research has thought provoking implications to prioritize investments in green finance, logistics, and technology, manufacturing firms can enhance their competitiveness, improve operational efficiency, and meet evolving environmental regulations and consumer preferences.

RESUMEN

BACKGROUND: Rural populations experience ongoing health inequities with disproportionately high morbidity and mortality rates, but digital health in rural settings is poorly studied. Our research question was: How does digital health influence healthcare outcomes in rural settings? The objective was to identify how digital health capability enables the delivery of outcomes in rural settings according to the quadruple aims of healthcare: population health, patient experience, healthcare costs and provider experience. METHODS: A multi-site qualitative case study was conducted with interviews and focus groups performed with healthcare staff (n = 93) employed in rural healthcare systems (n = 10) in the state of Queensland, Australia. An evidence-based digital health capability framework and the quadruple aims of healthcare served as classification frameworks for deductive analysis. Theoretical analysis identified the interrelationships among the capability dimensions, and relationships between the capability dimensions and healthcare outcomes. RESULTS: Seven highly interrelated digital health capability dimensions were identified from the interviews: governance and management; information technology capability; people, skills, and behaviours; interoperability; strategy; data analytics; consumer centred care. Outcomes were directly influenced by all dimensions except strategy. The interrelationship analysis demonstrated the influence of strategy on all digital health capability dimensions apart from data analytics, where the outcomes of data analytics shaped ongoing strategic efforts. CONCLUSIONS: The study indicates the need to coordinate improvement efforts targeted across the dimensions of digital capability, optimise data analytics in rural settings to further support strategic decision making, and consider how consumer-centred care could influence digital health capability in rural healthcare services. Digital transformation in rural healthcare settings is likely to contribute to the achievement of the quadruple aims of healthcare if transformation efforts are supported by a clear, resourced digital strategy that is fit-for-purpose to the nuances of rural healthcare delivery.

RESUMEN

BACKGROUND: Gaze behavior can serve as an objective tool in undergraduate pre-clinical dental education, helping to identify key areas of interest and common pitfalls in the routine evaluation of tooth preparations. Therefore, this study aimed to investigate the gaze behavior of undergraduate dental students and dental educators while evaluating a single crown tooth preparation. METHODS: Thirty-five participants volunteered to participate in the study and were divided into a novice group (dental students, n = 18) and an expert group (dental educators, n = 17). Each participant wore a binocular eye-tracking device, and the total duration of fixation was evaluated as a metric to study the gaze behavior. Sixty photographs of twenty different tooth preparations in three different views (buccal, lingual, and occlusal) were prepared and displayed during the experimental session. The participants were asked to rate the tooth preparations on a 100 mm visual analog rating scale and were also asked to determine whether each tooth preparation was ready to make an impression. Each view was divided into different areas of interest. Statistical analysis was performed with a three-way analysis of the variance model with repeated measures. RESULTS: Based on the participants' mean rates, the "best" and the "worst" tooth preparations were selected for analysis. The results showed a significantly longer time to decision in the novices compared to the experts (P = 0.003) and a significantly longer time to decision for both the groups in the best tooth preparation compared to the worst tooth preparation (P = 0.002). Statistical analysis also showed a significantly longer total duration of fixations in the margin compared to all other conditions for both the buccal (P < 0.012) and lingual (P < 0.001) views. CONCLUSIONS: The current study showed distinct differences in gaze behavior between the novices and the experts during the evaluation of single crown tooth preparation. Understanding differences in gaze behavior between undergraduate dental students and dental educators could help improve tooth preparation skills and provide constructive customized feedback.

RESUMEN

Passive daytime radiative cooling (PDRC) textiles hold substantial potential for localized outdoor cooling of the human body without additional energy consumption, but their limited multifunctional integration severely hinders their practical application. Herein, aluminum-doped zinc oxide (AZO) nanoparticles were purposefully introduced into poly(vinylidene fluoride) (PVDF) nanofibers via a facile electrospinning process, forming a large-scale and flexible PDRC textile with the desired antibacterial, UV-shielding, and self-cleaning capabilities. These prepared PDRC textiles present a weighted sunlight reflection rate of 92.3% and a weighted emissivity of 89.5% in the mid-infrared region. Furthermore, outdoor tests with an average solar intensity of ∼715 W/m2 demonstrated that a skin simulator temperature could be cooled by ∼16.1 °C below the ambient temperature, outperforming cotton fabric by ∼6.3 °C. Owing to the outstanding photocatalytic properties of the AZO nanoparticles, these prepared PVDF textiles exhibit antibacterial properties (Escherichia coli: 99.99%), UV-shielding performance (UPF > 50+), and superior self-cleaning capabilities, providing a cost-effective and eco-friendly avenue for daytime personal thermal management.

RESUMEN

This research examined values of individuals with neurodegenerative conditions about features of speed and accuracy as they consider potential use of augmentative and alternative communication brain-computer interface systems (AAC-BCI). Sixty-six individuals with neurodegenerative disease responded to prompts about six hypothetical ethical vignettes. Data were analyzed with qualitative content analysis. The following themes emerged. (1) Disease progression may contribute to the trade-off between speed and accuracy with AAC-BCI systems. (2) Individual experiences with technology use inform their views about the speed-accuracy trade-off. (3) There is a range of views about how slow or inaccurate communication may impact relationships, the integrity of a message, and quality of life. (4) Design solutions are proposed to address trade-offs in AAC-BCI systems. With the rapid development of AAC-BCI systems, user-centered design must integrate values of potential end-users illustrating that context, partner, message, and environment impact the prioritization of speed or accuracy in any communication exchange.

RESUMEN

Background: Dialysis modalities and their various treatment schedules result from complex compromises ('trade-offs') between medical, financial, technological, ergonomic, and ecological factors. This study targets summarizing the mutual influence of these trade-offs on (trans)portable, wearable, or even (partially) implantable haemodialysis (HD) systems, identify what systems are in development, and how they might improve quality of life (QoL) for patients with kidney failure. Methods: HD as defined by international standard IEC 60601-2-16 was applied on a PUBMED database query regarding (trans)portable, wearable, and (partly) implantable HD systems. Out of 159 search results, 24 were included and scanned for specific HD devices and/or HD systems in development. Additional information about weight, size, and development status was collected by the internet and/or contacting manufacturers. International airplane hand baggage criteria formed the boundary between transportable and portable. Technology readiness levels (TRLs) were assigned by combining TRL scales from the European Union and NATO medical staff. Results: The query revealed 13 devices/projects: seven transportable (six TRL9, one TRL5); two portable (one TRL6-7, one TRL4); two wearable (one TRL6, one frozen); and two partly implantable (one TRL4-5, one TRL2-3). Discussion: Three main categories of technical approaches were distinguished: single-pass, dialysate regenerating, and implantable HD filter with extracorporeal dialysate regeneration (in climbing order of mobility). Conclusions: Kidneys facilitate mobility by excreting strongly concentrated waste solutes with minimal water loss. Mimicking this kidney function can increase HD system mobility. Dialysate-regenerating HD systems are enablers for portability/wearability and, combined with durable implantable HD filters (once available), they may enable HD without needles or intravascular catheters. However, lack of funding severely hampers progress.

RESUMEN

INTRODUCTION: As artificial intelligence (AI) transforms healthcare, medical education must adapt to equip future physicians with the necessary competencies. However, little is known about the differences in AI knowledge, attitudes, and practices between undergraduate and postgraduate medical students. This study aims to assess and compare AI knowledge, attitudes, and practices among undergraduate and postgraduate medical students, and to explore the associated factors and qualitative themes. METHODS: A mixed-methods study was conducted, involving 605 medical students (404 undergraduates, 201 postgraduates) from a tertiary care center. Participants completed a survey assessing AI knowledge, attitudes, and practices. Semi-structured interviews and focus group discussions were conducted to explore qualitative themes. Quantitative data were analyzed using descriptive statistics, t-tests, chi-square tests, and regression analyses. Qualitative data underwent thematic analysis. RESULTS: Postgraduate students demonstrated significantly higher AI knowledge scores than undergraduates (38.9±4.9 vs. 29.6±6.8, p<0.001). Both groups held positive attitudes, but postgraduates showed greater confidence in AI's potential (p<0.001). Postgraduates reported more extensive AI-related practices (p<0.001). Key qualitative themes included excitement about AI's potential, concerns about job security, and the need for AI education. AI knowledge, attitudes, and practices were positively correlated (p<0.01). CONCLUSIONS: This study reveals a significant AI knowledge gap between undergraduate and postgraduate medical students, highlighting the need for targeted AI education. The findings can inform curriculum development and policies to prepare medical students for the AI-driven future of healthcare. Further research should explore the long-term impact of AI education on clinical practice.

RESUMEN

Background and Objective: The current AMS 800 artificial urinary sphincter (AUS) device is designed to simulate the function of the biological urinary sphincter to prevent urinary flow through mucosal coaptation, compression, and pressure transmission. The challenges in designing the AMS 800 device involve not only the mechanical operation of the artificial sphincter device but also producing a device that is effective, safe, and durable for patients in the long term. The following article provides a narrative review regarding the evolution and development of the AMS 800 devices over the years and evaluates the advances in surgical techniques relating to AMS 800 implantation. Methods: Available literature pertaining to the AMS 800 device was reviewed from the MEDLINE and EMBASE databases between 1 January 2000 to 31 December 2022. Emphasis is placed on key scientific publications including previous reviews and clinical guidelines relevant to AMS 800 device(s) and surgical techniques. Key Content and Findings: From the engineering point of view, the current AMS 800 device is ingenious and has stood the test of time. The basic design of this modern AUS consists of 3 separate components namely a pressure regulating balloon (PRB), an inflatable cuff, and a control pump. Continued innovations in device design and technology, coupled with refinements in surgical techniques over the past 5 decades have ensured that the AMS 800 device is and remains the standard of care in male stress urinary incontinence. While the long-term AMS 800 efficacy, safety, and durability are well documented, it is not without its limitations and complications. Mechanical and non-mechanical complications can occur especially in high-risk populations (such as in radiated patients) despite strict adherence to surgical principles and manufacturer's guidelines. Conclusions: Continued innovations in device design, technology, and surgical techniques have ensured that the AMS 800 device is an effective and safe treatment for male stress urinary incontinence (SUI). Future directions in the treatment of male SUI likely reside in cellular regenerative therapy and nanotechnology to restore, replace, or simulate the damaged native urinary sphincter.
