RESUMEN

INTRODUCTION AND IMPORTANCE: Local skin flap techniques, including rhomboid flap, have been developed to reconstruct cutaneous defects. Rhomboid flap advantages include easy design, excellent contour, texture, thickness, color match, and long-term good cosmesis. There is no patient reported outcome study in literature detailing patients' perception of appearance and satisfaction of scar appearance. METHODS: We studied 100 consecutive rhomboid flaps performed in 73 patients by a single surgeon. After at least one year from flap completion, a validated published Patient Scar Assessment Questionnaire (PSAQ) by Durani et al. was completed for each flap. RESULTS: The median age was 73 years, with a range of 21 years-94 years. Gender breakdown was 65 flaps in males and 35 flaps in females. For appearance, there were significantly greater frequency of "very well-matched scar" patients compared to "well-matched scar" patients (p < 0.001). There were no "a little matched" or "poorly matched" patients. For satisfaction of appearance scores, there were significantly greater frequency of "very satisfied" patients compared to "satisfied" patients (p < 0.001). There were no "dissatisfied" or "very dissatisfied" patients. Although size of primary defect was not statistically significant (p 0.071), there was a trend towards better appearance scar match and scar satisfaction for smaller defects. Age was not statistically significant (p 0.086), there was a trend towards better appearance scar match and scar satisfaction among older patients. There was no significant difference in appearance match and satisfaction scores based on the gender (p 0.733). There were no post-operative complications. CONCLUSIONS: Our study confirms, in experienced hands, the long-term patient perception of well-matched scar appearance and scar satisfaction with rhomboid flap technique. A significantly greater frequency of patients were "very satisfied" than "satisfied", with no "dissatisfied" patients. There was no statistically significant difference based on age, gender, and size of the primary defect.

OBJECTIVE: With the trend towards accelerated rehabilitation, the method of skin closure has become increasingly important in orthopedic surgery. It is imperative to evaluate a technique that provides superior clinical and cosmetic results specifically for TKA, due to relatively thin skin anterior to the knee making poor wound healing an issue of concern. We conducted a prospective, single-arm audit evaluating the patient assessments of incision cosmesis for a novel technique in TKA wound closure called Concealed Cosmetic Closure (CCC). METHODS: 570 knees were included in the study whose wound was closed with concealed cosmetic subcuticular interrupted sutures (CCCIS) between January 2014 and May 2016. A validated, ordinal questionnaire, Stony Brooks Scar Evaluation Scale (SBSES) designed to elicit a score evaluating scar satisfaction was interviewed by the investigators to patients 6 months to 3 years postoperatively. RESULTS: CCC is a simple and relatively rapid single-operator technique that takes about 7-10 min to close 11-17 cm knee incision. In our study, the mean score for Stony Brook Scar Evaluation Scale (SBSES) was 4.4 (SD of 0.73) (range 3-5). The scar was rated highly in terms of cosmesis, patient satisfaction and appearance of the wound. CONCLUSION: CCC is an effective modality for skin closure in total knee arthroplasty providing superior cosmetic healing with minimal complications leading to improved long term patient satisfaction.

The aim of the present study was to evaluate the overall scar satisfaction and quality of life in the conservative surgical treatment of penis cancer with reconstruction according to the Bracka technique. In order to do this, we passed out the questionnaire «Patient Scar Assessment Questionnaire¼ and the IIEF-15 questionnaire. From 2015 to 2017, 5 conventional glandectomy and reconstruction according to the Bracka technique were performed. 100% of the patients were very satisfied with the result, were slightly aware of the presence of the wound and lacked discomfort in relation to the scar. Regarding the quality of sexual life, an average improvement of 18 points was observed in the IIEF-15. We observed improvement in satisfaction of the sexual relationship (P6-8), orgasmic function (P9-10), overall satisfaction (P13-14) and confidence to maintain sexual intercourse (P15). Lesions at the level of the penis represent an important alteration in quality of sexual life. By performing a minimally invasive surgery with subsequent reconstruction, the patient presents a significant improvement in the quality of sexual life.
